Rates and risk factors for suicidal ideation,suicide attempts and suicide deaths in persons with HIV:a systematic review and meta-analysis

Background People living with HIV/AIDS(PLWHA)must contend with a significant burden of ***,current studies of this demographic have yielded wide variations in the incidence of suicidality(defined as suicidal ideation,suicide attempt and suicide deaths).Aims This systematic review and meta-analysis aimed to assess the lifetime incidence and prevalence of suicidality in *** Publications were identified from Pubmed(mEDLINE),SCOPUS,OVID(mEDLINE),Joanna Briggs Institute EBP and Cochrane Library databases(from inception to before 1 February 2020).The search strategy included a combination of medical Subject Headings associated with suicide and *** independently screened records,extracted outcome measures and assessed study *** were pooled using a random-effects *** and meta-regression analyses were conducted to explore the associated risk factors and to identify the sources of *** outcomes were lifetime incidence of suicide completion and lifetime incidence and prevalence of suicidal ideation and suicide *** A total of 185199 PLWHA were identified from 40 studies(12 cohorts,27 cross-sectional and 1 nested case-control).The overall incidence of suicide completion in PLWHA was 10.2/1000 persons(95%CI:4.5 to 23.1),translating to 100-fold higher suicide deaths than the global general population rate of 0.11/1000 *** lifetime prevalence of suicide attempts was 158.3/1000 persons(95%CI:106.9 to 228.2)and of suicidal ideation was 228.3/1000 persons(95%CI:150.8 to 330.1).meta-regression revealed that for every 10-percentage point increase in the proportion of people living with HIV with advanced disease(AIDS),the risk of suicide completion increased by 34 per 1000 *** quality of evidence by Grading of Recommendations,Assessment,Development and Evaluations for the suicide deaths was graded as‘moderate’*** The risk of suicide death is 100-fold higher in people living wit

Heuristic Estimation of the Vacuum Energy Density of the Universe: Part II-Analysis Based on Frequency Domain Electromagnetic Radiation

In Part I of this paper, an inequality satisfied by the vacuum energy density of the universe was derived using an indirect and heuristic procedure. The derivation is based on a proposed thought experiment, according to which an electron is accelerated to a constant and relativistic speed at a distance L from a perfectly conducting plane. The charge of the electron was represented by a spherical charge distribution located within the Compton wavelength of the electron. Subsequently, the electron is incident on the perfect conductor giving rise to transition radiation. The energy associated with the transition radiation depends on the parameter L. It was shown that an inequality satisfied by the vacuum energy density will emerge when the length L is pushed to cosmological dimensions and the product of the radiated energy, and the time duration of emission is constrained by Heisenberg’s uncertainty principle. In this paper, a similar analysis is conducted with a chain of electrons oscillating sinusoidally and located above a conducting plane. In the thought experiment presented in this paper, the behavior of the energy radiated by the chain of oscillating electrons is studied in the frequency domain as a function of the length L of the chain. It is shown that when the length L is pushed to cosmological dimensions and the energy radiated within a single burst of duration of half a period of oscillation is constrained by the fact that electromagnetic energy consists of photons, an inequality satisfied by the vacuum energy density emerges as a result. The derived inequality is given by where is the vacuum energy density. This result is consistent with the measured value of the vacuum energy density, which is 5.38 × 10-10 J/m. The result obtained here is in better agreement with experimental data than the one obtained in Part I of this paper with time domain radiation.

Background:Varicella outbreaks occur frequently in closed environments such as those of *** paper compares the economic outcomes of varicella vaccination in enlisted servicemen without prior reported varicella infection or ***:We analyzed the economic outcomes of a varicella vaccination program on all enlisted servicemen without prior reported varicella infection or vaccination in the Singapore Armed Forces(SAF) between December 11,2010–December 20,2013,compared with the previous program of varicella vaccination only for selected personnel between December 1,2007–December 10,***:In the at-risk population of all active SAF servicemen,the program of varicella vaccination for all servicemen without prior reported varicella infection or vaccination upon enlistment would save 72.0 work days per 1000(95% CI:61.2-82.9),valued at SG$6,544 per 1000(95% CI:6,524-6,564),i.e.,costing SG$91.5 per work day saved(95% CI:78.7-107.3).This also results in a reduction of 2.7 varicella cases per 1000 and 5.43 outbreaks per 10000,or a total savings of SG$1,695 per 1000(95% CI:–2,730-6,834),taking into account the cost of work days lost over a three-year period,compared with the previous regime of vaccinations only for selected ***:The varicella vaccination strategy targeting all enlisted servicemen without prior reported varicella infection or vaccination is able to prevent varicella infections and outbreaks,thus reducing absenteeism and days lost.

Remarkable Predictions of Classical Electrodynamics on Elementary Charge and the Energy Density of Vacuum

In a recent paper, we have studied the nature of the electromagnetic energy radiated over a single period of oscillation by an antenna working in frequency domain under ideal conditions and without losses when the oscillating charge in the antenna is reduced to the elementary charge. Here we extend and expand that study. The energy radiated by an oscillating current in an antenna occurs in bursts of duration T/2, where T is the period of oscillation. The results obtained here, based purely on classical electrodynamics, can be summarized by the inequality U ≥hv→q0 ≥e where U is the energy radiated in a single burst of duration T/2, h is the Planck constant, ν is the frequency of oscillation and q0 is the magnitude of the oscillating charge associated with the current. The condition U=hv→q0=e is obtained when the length of the antenna is equal to the ultimate Hubble radius of the universe (i.e. the maximum value of the antenna length allowed by nature) and the wavelength is equal to the Bohr radius (resulting from the smallest possible radius of the conductor allowed by nature). The ultimate Hubble radius is directly related to the vacuum energy density. The inequality obtained here is in general agreement with the one obtained in the previous study. One novel feature of this extended analysis is the discovery of an expression, in terms of the elementary charge and other atomic constants, for the vacuum energy density of the universe. This expression predicts the vacuum energy density to be about 4×10-10?J/m3 which is in reasonable agreement with the measured value of 6×10-10 J/m3.

Functional and Neural mechanisms of Out-of-Body Experiences: Importance of Retinogeniculo-Cortical Oscillations

Current research on the various forms of autoscopic phenomena addresses the clinical and neurological correlates of out-of-body experiences, autoscopic hallucinations, and heautoscopy. Yet most of this research is based on functional magnetic resonance imaging results and focuses predominantly on abnormal cortical activity. Previously we proposed that visual consciousness resulted from the dynamic retinogeniculo-cortical oscillations, such that the photoreceptors dynamically integrated with visual and other vision-associated cortices, and was theorized to be mapped out by photoreceptor discs and rich retinal networks which synchronized with the retinotopic mapping and the associated cortex. The feedback from neural input that is received from the thalamus and cortex via retinogeniculo-cortical oscillations and sent to the retina is multifold higher than feed-forward input to the cortex. This can effectively translate into out-of-body experiences projected onto the screen formed by the retina as it is perceived via feedback and feed-forward oscillations from the reticular thalamic nucleus, or “internal searchlight”. This article explores the role of the reticular thalamic nucleus and the retinogeniculo-cortical oscillations as pivotal internal components in vision and various autoscopic phenomena.

Classical Electromagnetic Fields of moving Charges as a Vehicle to Probe the Connection between the Elementary Charge and Heisenberg’s Uncertainty Principle

The radiation fields generated when a charged particle is incident on or moving away from a perfectly conducting plane are obtained. These fields are known in the literature as transition radiation. The field equations derived thus are used to evaluate the energy, momentum and the action associated with the radiation. The results show that for a charged particle moving with speed ν, the longitudinal momentum associated with the transition radiation is approximately equal to ΔU/c for values of ?1- ν/c smaller than about 10-3 where ΔU is the total radiated energy dissipated during the interaction and cis the speed of light in free space. The action of the radiation, defined as the product of the total energy dissipated and the duration of the emission, increases as 1- ν/c decreases and, for an electron, it becomes equal to h/4π when ν = c - νm where νm is the speed pertinent to the lowest possible momentum associated with a particle confined inside the universe and?h is the Planck constant. Combining these results with Heisenberg’s uncertainty principle, an expression that predicts the value of the elementary charge is derived.

The Dynamic Role of Breathing and Cellular membrane Potentials in the Experience of Consciousness

Understanding the mechanics of consciousness remains one of the most important challenges in modern cognitive science. One key step toward understanding consciousness is to associate unconscious physiological processes with subjective experiences of sensory, motor, and emotional contents. This article explores the role of various cellular membrane potential differences and how they give rise to the dynamic infrastructure of conscious experience. This article explains that consciousness is a body-wide, biological process not limited to individual organs because the mind and body are unified as one entity;therefore, no single location of consciousness can be pinpointed. Consciousness exists throughout the entire body, and unified consciousness is experienced and maintained through dynamic repolarization during inhalation and expiration. Extant knowledge is reviewed to provide insight into how differences in cellular membrane potential play a vital role in the triggering of neural and non-neural oscillations. The role of dynamic cellular membrane potentials in the activity of the central nervous system, peripheral nervous system, cardiorespiratory system, and various other tissues (such as muscles and sensory organs) in the physiology of consciousness is also explored. Inspiration and expiration are accompanied by oscillating membrane potentials throughout all cells and play a vital role in subconscious human perception of feelings and states of mind. In addition, the role of the brainstem, hypothalamus, and complete nervous system (central, peripheral, and autonomic) within the mind-body space combine to allow consciousness to emerge and to come alive. This concept departs from the notion that the brain is the only organ that gives rise to consciousness.

Sensory Consciousness is Experienced through Amplification of Sensory Stimuli via Lateral Inhibition

At present, researchers are unclear about which activity within the brain is responsible for the emergence of consciousness—the subconscious or unconscious. Current literature suggests that consciousness is isolated in the brain;however, we suggest consciousness emerges from both—subconscious and unconscious activity, in addition to sensory consciousness. This article contends that sensory consciousness arises from neurophysiological brain activity, intrapersonal space, sensory information, and parallel processing of the external and internal environment through vision, olfaction, the integumentary system, gustation, and audition. Traditionally, lateral inhibition is defined as the ability for an excited neuron to laterally inhibit its neighbors, and is an integral part of neurophysiology in all senses. In this article, we are connecting the science behind the well-established physiological observations of gamma wave activity in the interneurons of peripheral receptors with what is currently unknown regarding the functional significance of seemingly unrelated gamma activity in the cortico-thalamic gamma oscillations. We suggest that this allows for instantaneous integration of the brain with sensory receptors. This article uses existing literature on lateral inhibition to investigate its role in sensory organs and various areas of the body. We explain how sensory consciousness is only one component of unified consciousness. We propose that lateral inhibition also plays a vital role in consciousness theory, and understanding this can help illustrate the dynamic interactions between the central and peripheral nervous systems within the body.

The inequality derived is given by ρΛ ≤ 9.9×10-9J/m3 where ρΛ is the vacuum energy density. This result is consistent with the measured value of the vacuum energy density, which is 0.538 × 10-9J/m. Since there is a direct relationship between the vacuum energy density and the Einstein’s cosmological constant, the inequality can be converted directly to that of the cosmological constant.
