Ventriculo-atrial shunt with occlusion of the internal jugular vein: operative experience and surgical technique

Background Ventricular shunts are one of the most frequent techniques used for the management of hydrocephalus. The ventriculoperitoneal shunt (VPS) is the most commonly performed procedure, and the ventriculoatrial shunt (VAS) is the second option in most medical centers. The main objective of this study is to introduce and describe a surgical approach for VAS outlining our experience and comparing it with traditional shunting *** In this retrospective cohort comparison study, we included patients with hydrocephalus treated with a surgical procedure between January 2010 and February 2021 at a single academic institution. We categorized the procedures into two groups: patients with VPS and conventional VAS grouped together into the conventional technique (CT) group, and the second group was patients with whom we performed VAS with complete internal jugular vein occlusion (IJVOT). We compared the surgical time, postoperative complications, and occurrence of shunt failure among the groups by performing univariate analysis using the Fisher exact *** Out of the 106 patients included in the analysis, IJVOT was performed in 66 patients, and CT in 40 patients. The median surgical time was 60 min (IQR 60-90) for IJVOT versus 100 min (IQR 60-120) for CT (p < 0.01). In the follow-up a month after the procedure, 83.3% of patients with IJVOT and 62.5% of patients with CT did not require shunt removal or shunt revision (p < 0.01). Shunt revision rates were 12.5% and 1.5% for CT while 1.5% and 2.5% for IJVOT at 1 and 6 months after the *** Our findings demonstrate that VAS with IJVOT is a safe method that exhibited shorter surgical times and outcomes comparable to CT. However, since the present study represents the first cohort evaluating IJVOT, it is imperative to conduct larger prospective studies, along with clinical trials, to fully explore and establish efficacy, long-term outcomes, and an in-depth comparison among shunting techniques

Spoon-assisted autologous particulate bone graft harvesting technique

Particulate bone plays a crucial role in various oral and maxillofacial surgical procedures, including reconstruction, implantation, and craniofacial surgery. Autologous bone and deproteinized bovine bone xenografts are the two primary resources used for such procedures, with the former demonstrating superior cost-effectiveness and reduced comorbidity rates, particularly in cases involving donor zones. However, a significant challenge lies in acquiring uniformly sized autologous particulate bone specimens, with existing tools often yielding coarse particles at a high cost. Consequently, commercial bone xenograft solutions are frequently favored despite lower standards. This technical note introduces a novel technique for swiftly, safely, and efficiently obtaining autologous particulate bone specimens. The procedure involves the use of a motor handpiece fitted with a micro drill and surgical spoon to collect the bone particles. The continuous irrigation with saline maintains a clear surgical field during the milling process. The collected bone particles are then transferred to a metal capsule for further use. This technique offers a promising solution to the challenge of inconsistent particle size associated with harvesting using traditional methods, providing surgeons with a reliable and efficient method of obtaining autologous particulate bone samples.

Effects of coals microscale structural features on their mechanical properties,propensity to crushing and fine dust formation

The work is dedicated to revealing the structural features of coals with different ranks,such as anthracites,metaanthracite and graphite,that determine their ability to crush and form fine *** this purpose,a combination of various nanoin-dentation techniques and Raman spectroscopy was *** mechanical behavior of the selected coals was investigated by cyclic nanoindentation with increasing peak load and quasi-static *** alteration of the mechanical properties was studied by analysis of elastic moduli and damage indices *** groups of coals were identified based on their propensity to crushing during cyclic *** assigned to the first and second groups are characterized by local destruction in the contact zone with the indenter and the formation of a core of crushed *** assigned to the third group are characterized by bulk destruction(outside the zone of contact with the indenter).In general,the ability of coals to fracture under mechanical loading decreases in the series of metamorphism due to microscale compaction of vitrinite *** the series of anthracite,metaanthracite and graphite,it is established that the coal matter compaction takes place for the anthracite and metaanthracite,whereas graphite reveals rather different behavior due to abrupt change of its *** ratios between the amorphous and crystalline phases of carbon(S)were determined by deconvolution of coals Raman *** propensity of coals to crushing(a damage index Rw)increases with growth of the proportion of amorphous carbon in the coal *** the considered coals and metaanthracite,it is established that the proneness to destruction outside the contact zone with the indenter is determined by the ratio of amorphous and crystalline carbon of 1 and *** S parameter is lower than 1,the coals are being crushed only in the zone of contact with the indenter.
