AIM:To evaluate refractive outcomes and corneal astigmatism changes after Toric implantable collamer lens with a central port(V4c T-ICL)implantation over 1y of follow-up.METHODS:A retrospective study was performed including 50 eyes of 50 patients that underwent V4c T-ICL implantation.Uncorrected(UDVA)and corrected(CDVA)distance visual acuities,refraction,refractive and corneal astigmatism changes and corneal coupling correction were evaluated preoperatively,1 and 12mo postoperatively.Vector analysis was used for astigmatism changes.Coefficient of adjustment(CAdj)was calculated for corneal coupling analysis.RESULTS:The mean UDVA achieved was 0.03 logMAR at 1mo and remained unchanged throughout the whole follow-up(P=0.193).At the last visit,84%of the eyes achieved a CDVA of 0.00 logMAR or better.Regarding spherical equivalent refraction(SEQ),96%of eyes were ranges of±1.00 D and 84%of them within±0.50 D.Also,94%of eyes had a remaining refractive cylinder within±1.00 D and 78%of them within±0.50 D.Both,SEQ and refractive cylinder,remain stable over the postoperative follow-up(P=1.000 and P=0.660,respectively).In terms of surgically induced astigmatism(SIA),no statistically significant differences were found over the follow-up(P=0.102)and under correction was found with a correction index lower than the unit at each visit.A keratometric astigmatism induced of 0.59±0.53(vector mean:0.26×73º)D was reached at the last visit.No significant changes in terms of corneal astigmatism orientation were reported over post-surgery visits(P=0.129 and P=0.097 at 1 and 12mo respectively).No clinical significance was found for CAdj on with-the-rule astigmatism.No postoperative complications resulting from the surgery were found.CONCLUSION:Refractive outcomes suggest that the V4c T-ICL implantation for correction of myopic astigmatism was satisfactory in terms of effectiveness,safety,and stability during 1y of follow-up.Corneal astigmatism induced by the incision around 0.5 D is achieved according to the remaining refractive cylinder f

We present an unusual case of portal vein thrombosis with a splanchnic hemangioma secondary to acute biliary pancreatitis. We report a 45-year-old patient, who has systemic arterial hypertension in treatment, was admitted for abdominal pain in the epigastrium, with irradiation to the right hypochondrium, accompanied by nausea and vomiting of 10 occasions of bile content, physical examination with pain in the right hypochondrium, Murphy positive. We have laboratory studies with a lipase of 788, so a diagnosis of pancreatitis is made with an etiology to be determined. The laboratories suggestive of acute biliary pancreatitis (lipase 788.71);an imaging study was subsequently performed (ultrasonography) with the result of stone in the common bile duct. A laparoscopy was performed with relative improvement, so he was discharged and returned 20 days after surgery due to abdominal pain of the same intensity in the left hypochondrium. Ending his hospitalization with a splenectomy for splenic hemangioma with portal vein thrombosis.

Serological profiling of Crohn’s disease and ulcerative colitis patients reveals anti-microbial antibody signatures

BACKGROUND The healthcare burden of inflammatory bowel disease(IBD)is rising globally and there are limited non-invasive biomarkers for accurate and early diagnosis.AIM To understand the important role that intestinal microbiota play in IBD pathogenesis and identify anti-microbial antibody signatures that benefit clinical management of IBD patients.METHODS We performed serological profiling of 100 Crohn’s disease(CD)patients,100 ulcerative colitis(UC)patients and 100 healthy controls against 1173 bacterial and 397 viral proteins from 50 bacteria and 33 viruses on protein microarrays.The study subjects were randomly divided into discovery(n=150)and validation(n=150)sets.Statistical analysis was performed using R packages.RESULTS Anti-bacterial antibody responses showed greater differential prevalence among the three subject groups than anti-viral antibody responses.We identified novel antibodies against the antigens of Bacteroidetes vulgatus(BVU_0562)and Streptococcus pneumoniae(SP_1992)showing higher prevalence in CD patients relative to healthy controls.We also identified antibodies against the antigen of Streptococcus pyogenes(SPy_2009)showing higher prevalence in healthy controls relative to UC patients.Using these novel antibodies,we built biomarker panels with area under the curve(AUC)of 0.81,0.87,and 0.82 distinguishing CD vs control,UC vs control,and CD vs UC,respectively.Subgroup analysis revealed that penetrating CD behavior,colonic CD location,CD patients with a history of surgery,and extensive UC exhibited highest antibody prevalence among all patients.We demonstrated that autoantibodies and anti-microbial antibodies in CD patients had minimal correlation.CONCLUSION We have identified antibody signatures for CD and UC using a comprehensive analysis of antimicrobial antibody response in IBD.These antibodies and the source microorganisms of their target antigens improve our understanding of the role of specific microorganisms in IBD pathogenesis and,after future validation,should aid early and accurate d

The current trend of replacing a percentage of gasoline with ethanol has promoted the development of new processes for its production from lignocellulosic biomass. This work reports the production of ethanol from the Camalote grass (Paspalum fasciculatum Willd). The lignocellulosic biomass was subjected to acid hydrolysis at 125C and 15 psi with H2SO4 concentrations at 5%, 10%, and 20%, obtaining an average of reducing sugars (pentoses and hexoses) from the hydrolyzed juice with 12.3%, 10%, and 17% Brix, respectively. The sugars were fermented using yeast of the Saccharomyces cerevisiae at 30C for 48 hours. Finally, the ethanol was distilled at 78C, and the average yields were obtained through analysis of variance with a 95% confidence level. The values indicate that there is a significant difference (p > 0.05), the Tukey study shows that all the % v/v averages are different from each other. For H2SO4 concentration at 5% (10.33 ± 2), H2SO4 at 10% (9.33 ± 1.8), and H2SO4 at 20% (6.33 ± 2). The acidity analysis for the ethanol obtained from each treatment gave a value of 1.8 mg/L of acetic acid in all cases.

In this paper,we presented the development of a navigation control system for a sailboat based on spiking neural networks(SNN).Our inspiration for this choice of network lies in their potential to achieve fast and low-energy computing on specialized hardware.To train our system,we use the modulated spike time-dependent plasticity reinforcement learning rule and a simulation environment based on the BindsNET library and USVSim simulator.Our objective was to develop a spiking neural network-based control systems that can learn policies allowing sailboats to navigate between two points by following a straight line or performing tacking and gybing strategies,depending on the sailing scenario conditions.We presented the mathematical definition of the problem,the operation scheme of the simulation environment,the spiking neural network controllers,and the control strategy used.As a result,we obtained 425 SNN-based controllers that completed the proposed navigation task,indicating that the simulation environment and the implemented control strategy work effectively.Finally,we compare the behavior of our best controller with other algorithms and present some possible strategies to improve its performance.

The generation of electricity based on renewable energy sources,parti-cularly Photovoltaic(PV)system has been greatly increased and it is simply insti-gated for both domestic and commercial uses.The power generated from the PV system is erratic and hence there is a need for an efﬁcient converter to perform the extraction of maximum power.An improved interleaved Single-ended Primary Inductor-Converter(SEPIC)converter is employed in proposed work to extricate most of power from renewable source.This proposed converter minimizes ripples,reduces electromagnetic interference due toﬁlter elements and the contin-uous input current improves the power output of PV panel.A Crow Search Algo-rithm(CSA)based Proportional Integral(PI)controller is utilized for controlling the converter switches effectively by optimizing the parameters of PI controller.The optimized PI controller reduces ripples present in Direct Current(DC)vol-tage,maintains constant voltage at proposed converter output and reduces over-shoots with minimum settling and rise time.This voltage is given to single phase grid via 1�Voltage Source Inverter(VSI).The command pulses of 1�VSI are produced by simple PI controller.The response of the proposed converter is thus improved with less input current.After implementing CSA based PI the efﬁciency of proposed converter obtained is 96%and the Total Harmonic Distor-tion(THD)is found to be 2:4%.The dynamics and closed loop operation is designed and modeled using MATLAB Simulink tool and its behavior is performed.

Inflammation is an established etiopathogenesis factor of infantile spasms（IS）, a therapy-resistant epileptic syndrome of infancy. We investigated the IS-associated transcriptomic alterations of neurotransmission in rat hypothalamic arcuate nucleus, how they are corrected by antiinflamatory treatments and whether there are sex differences. IS was triggered by repeated intraperitoneal administration of N-methyl-D-aspartic acid following anti-inflammatory treatment（adreno-cortico-tropic-hormone（ACTH） or PMX53）or normal saline vehicle to prenatally exposed to betamethasone young rats. We found that treatments with both ACTH and PMX53 resulted in substantial recovery of the genomic fabrics of all types of synaptic transmission altered by IS. While ACTH represents the first line of treatment for IS, the even higher efficiency of PMX53（an antagonist of the complement C5 a receptor） in restoring the normal transcriptome was not expected. In addition to the childhood epilepsy, the recovery of the neurotransmission genomic fabrics by PMX53 also gives hope for the autism spectrum disorders that share a high comorbidity with IS. Our results revealed significant sex dichotomy in both IS-associated transcriptomic alterations（males more affected） and in the efficiency of PMX53 anti-inflammatory treatment（better for males）. Our data further suggest that anti-inflammatory treatments correcting alterations in the inflammatory transcriptome may become successful therapies for refractory epilepsies.

Wood is a lignocellulosic material that,because of its organic nature,is a source of nutrients for bacteria,fungi,and insects.Incisitermes marginipennis(Latreille)is an interesting dry wood-boring termite.Because it is an extremely destructive pest,difficult to control due to its cryptic lifestyle,and for its role in the biogeochemical cycle of carbon and other elements;it is a fundamental organism.The use of less toxic and eco-friendly substances is important for protecting domestic,cultural,and historical heritage made of wood or lignocellulosic materials.The aim of this study was to evaluate the biological activity of various naphthoquinones extracted from the heartwood of T.grandis on I.marginipennis.First,heartwood flour was used for Soxhlet-type extractions to isolate and characterize the heartwood compounds by thin-layer chromatography.To identify the heartwood compounds,nuclear magnetic resonance and gas chromatography coupled to a mass spectrometer were used.The information obtained was supported using the Interactive Spectroscopy software.The identified heartwood compounds were lapachol,tectoquinone,deoxylapachol,dehydro-α-lapachone,tectol and tecomaquinone-I.Using nymphal termites of I.marginipennis as a biological model,the isolated and identified molecules were under three different tests(repellent,insecticide and protective)and their biological activity was individually evaluated.The results showed that the molecules obtained have a repellent-protective effect against I.marginipennis,although not in all cases,these molecules have high percentages of mortality and decreased the weight loss of the treated material exposed to I.marginipennis.
