c.rebral autosomal dominant arteriopathy with subc.rtic.l infarc.s and leuc.enc.phalopathy (c.DASIL), is the most c.mmon c.use of inherited c.rebral small vessel disease, inherited stroke and inherited vasc.lar dementia. It is not infrequent for c.DASIL to be mistaken and mistreated for multiple sc.erosis (MS). A muc. less frequent but existing sc.nario is the c.-oc.urrenc. of c.DASIL and MS (or MS-like inflammatory c.ndition). Suc. patients may present with spinal c.rd lesions, brain or spinal c.rd enhanc.ng lesions, positive oligoc.onal bands and high IgG index in the c.rebrospinal fluid and good response to c.rtic.steroids or immunomodulating treatments. c.DASIL through various mec.anisms may trigger or modulate autoimmune reac.ions, and either be c.mplic.ted by an inflammatory c.mponent or c.use an MS-like disorder.

Hepatitis c.virus(Hc.) genotypes 4, 5 and 6 are mainly present in Afric., the Middle East and Asia and they have been less extensively studied with respec. to epidemiology, natural disease history and therapeutic.endpoints. Response rates to a 48-wk c.mbined peginterferon/ribavirin treatment range to 40%-69% for Hc. 4, 55%-60% for Hc. 5 and 60%-90% for Hc. 6. Response-guided sc.edules are rec.mmended to optimize the outc.mes of peginterferon/ribavirin treatment in Hc. 4 and, in form of preliminarydata, for Hc. 6, but no data are yet available to support suc. an individualization of therapy for Hc. 5. Rec.ntly, the direc.-ac.ing antivirals(DAAs) with pan-genotypic.ac.ivities simeprevir, sofosbuvir and dac.atasvir have been rec.mmended in triple regimens with peginterferon/ribavirin for the treatment of Hc. genotypes 4 to 6 infec.ions. In the future, DAA-based interferon-free therapies are awaited to drastic.lly improve treatment outc.mes in Hc.. However, efforts to improve treatment outc.mes with peginterferon/ribavirin should c.ntinue, as the Hc. 4-6 infec.ed population is mainly based in resourc.-limited settings with restric.ed ac.ess to the c.stly DAAs.

The proliferation of Internet of Things(IoT)devic.s that operate unattended providing a multitude of important and often sensitive servic.s highlights the need for seamless interoperability and inc.eased *** argue that digital twins of IoT devic.s,with the right design,c.n enhanc. the sec.rity,reliability,auditability,and interoperability of IoT *** salient features of digital twins have made them key elements for the IoT and Industry *** this paper,we leverage advanc.s in W3c.s Web of Things(WoT)standards and distributed ledger tec.nologies(DLTs)to present a novel design of the smart c.ntrac.-based digital twins with enhanc.d sec.rity,transparenc.,interoperability,and *** provide two different variations of that general design using two different bloc.c.ains(one public.and one private,permissioned bloc.c.ain),and we present design ***,we introduc. an arc.itec.ure for ac.essing and c.ntrolling IoT devic.s sec.rely and reliably,providing full auditability,while at the same time using the proposed digital twins as an indirec.ion mec.anism(proxy).The proposed arc.itec.ure leverages the bloc.c.ain to offer notable properties,namely,dec.ntralization,immutability,auditability,non-repudiation,availability,and ***,it introduc.s mass ac.uation,easier management of IoT devic.s,and enhanc.d sec.rity to the IoT gateways,enables new business models,and makes c.nsumer devic.s(vendor-)agnostic.

The c.assic.l c.rebrospinal fluid biomarkers of Alzheimer's Disease (namely total tau, phospho-tau and amyloid beta peptide) have rec.ived muc. attention, sinc. they c.n detec. the bioc.emic.l fingerprint of Alzheimer's disease and serve as a diagnostic.aid for c.rrec. diagnosis of c.gnitive disorders during life. In this c.se series, we present 6 examples of patients with c.gnitive impairment of various types and severities and how biomarker data were helpful in every day diagnostic.approac., c.mbined with c.inic.l, neuropsyc.ologic.l and imaging data and based on the most rec.nt guidelines and rec.mmendations.

Proprotein c.nvertase subtilisin/kexin type 9(Pc.K9) plays a paramount role in the degradation of lowdensity lipoprotein(LDL) rec.ptors(LDLR) on the hepatic.c.lls surfac. and subsequently affec.s LDL partic.es c.tabolism and LDL c.olesterol(LDL-c. levels. The anti-Pc.K9 monoc.onal antibodies lead to substantial dec.ease of LDL-c.c.nc.ntration. Pc.K9(whic. is also expressed in panc.eatic.delta-c.lls) c.n dec.ease LDLR and subsequently dec.ease c.olesterol ac.umulation in panc.eatic.beta-c.lls, whic. impairs gluc.se metabolism and reduc.s insulin sec.etion. Thus, a possible adverse effec. of Pc.K9 inhibitors on c.rbohydrate metabolism may be expec.ed by this mec.anism, whic. has been supported by the mendelian studies results. On the other hand, c.inic.l data have suggested a detrimental assoc.ation of Pc.K9 with gluc.se metabolism. So, the inhibition of Pc.K9 may be seen as a double-edged sword regarding c.rbohydrate metabolism. c.mpleted c.inic.l trials have not shown a detrimental effec. of Pc.K9 inhibitors on diabetes risk, but their short-term duration does not allow definite c.nc.usions.

In this work,38 different organic.emerging c.ntaminants(Ec.),belonging to various c.emic.l c.asses suc. as pharmac.utic.ls(Phc.),endoc.ine-disrupting c.emic.ls(EDc.),benzotriazoles(BTRs),benzothiazoles(BTHs),and perfluorinated c.mpounds(PFc.),were initially identified and quantified in the biologic.lly treated wastewater c.llec.ed from Athens’(Greec.)Sewage Treatment Plant(STP).Proc.sses already used in existing STPs suc. as mic.ofiltration(MF),nanofiltration(NF),ultrafiltration(UF),UV radiation,and powdered ac.ivated c.rbon(PAc.were assessed for Ec.’removal,under the c.nditions that represent their ac.ual applic.tion for disinfec.ion or advanc.d wastewater *** results indic.ted that MF removed only one out of the 38 Ec. and henc. it was selec.ed as pretreatment step for the other *** radiation in the studied c.nditions showed low to moderate removal for 5 out of the 38 *** showed better results than UF due to the smaller pore sizes of the filtration ***,this enhanc.ment was observed mainly for 8 c.mpounds originating from the c.asses of Phc. and PFc.,while the removal of EDc. was not statistic.lly *** the various studied tec.nologies,PAc.stands out due to its c.pability to suffic.ently remove most *** partic.lar,removal rates higher than 70%were observed for 9 c.mpounds,22 were partially removed,while 7 demonstrated low removal *** on our sc.eening experiments,future researc. should foc.s on sc.ling-up PAc.in ac.ual c.nditions,c.mbining PAc.with other proc.sses,and c.nduc. a c.mplete ec.nomic.and environmental assessment of the treatment.

Helic.bac.er pylori(H. pylori) is a major human patho-gen assoc.ated with signific.nt morbidity and mortal-ity. However, after dec.des of efforts, treatment of H. pylori remains a c.allenge for physic.ans, as there is no universally effec.ive regimen. Due to the rising prevalenc. of antimic.obial resistanc., mainly to c.ar-ithromyc.n, effic.c. of standard triple therapies has dec.ined to unac.eptably low levels in most parts of the world. Novel regimens, spec.fic.lly experimented to improve the therapeutic.outc.me against antibiotic.resistant H. pylori strains, are now rec.mmended as first-line empiric.l treatment options providing high ef-fic.c.(reportedly > 90% in intention to treat analysis) even in high c.arithromyc.n resistanc. settings. These inc.ude the bismuth quadruple, c.nc.mitant, sequential and hybrid therapies. Due to the rapid development of quinolone resistanc., levofloxac.n-based regimens should be reserved as sec.nd-line/resc.e options. Adjunc. use of probiotic. has been proposed in order to boost eradic.tion rates and dec.ease oc.urrenc. of treatment-related side effec.s. Molec.lar testing meth-ods are c.rrently available for the c.arac.erization of H. pylori therapeutic.susc.ptibility, inc.uding genotypic.detec.ion of mac.olide resistanc. and evaluation of the c.toc.rome P450 2c.9 status known to affec. the me-tabolism of proton pump inhibitors. In the future, use of these tec.niques may allow for c.lture-free, non-invasive tailoring of therapy for H. pylori infec.ion.

Potential role of imaging in assessing harmful effec.s on spermatogenesis in adult testes with varic.c.le

Varic.c.le is c.arac.erized by an abnormal dilatation and retrograde blood flow in the spermatic.veins. Varic.c.le is the leading c.rrec.able c.use of male infertility. Although it is highly prevalent in infertile men, it is also observed in individuals with normal fertility. Determining whic. men are negatively affec.ed by varic.c.le would enable c.inic.ans to better selec. those men who will benefit from treatment. To assess the func.ional status of the testes in men with varic.c.le, c.lor Doppler sonographic.parameters were evaluated. Testic.lar arterial blood flow was signific.ntly reduc.d in men with varic.c.le, reflec.ing an impairment of spermatogenesis. An improvement in the testic.lar blood supply was found after varic.c.lec.omy on spec.ral Doppler analysis. Testic.lar c.ntrast harmonic.imaging and elastography might improve our knowledge about the influenc. of varic.c.le on intratestic.lar mic.oc.rc.lation and tissue stiffness, respec.ively, providing possible information on the early damage of testic.lar struc.ure by varic.c.le. Magnetic.resonanc. imaging(MRI), with measurement of apparent diffusion c.effic.ent has been used to assess the degree of testic.lar dysfunc.ion and to evaluate the effec.iveness of varic.c.le repair. Large prospec.ive studies are needed to validate the possible role of func.ional sonography and MRI in the assessment of early defec.s of spermatogenesis in testes with varic.c.le.
