Cemented Müller straight stem total hip replacement:18 year survival,clinical and radiological outcomes

AIM: To present the 18 year survival and the clinical and radiological outcomes of the Müller straight stem, cemented, total hip arthroplasty(THA).METHODS: Between 1989 and 2007, 176 primary total hip arthroplasties in 164 consecutive patients were performed in our institution by the senior author. All patients received a Müller cemented straight stem and a cemented polyethylene liner. The mean age of the patients was 62 years(45-78). The diagnosis was primary osteoarthritis in 151 hips, dysplasia of the hip in 12 and subcapital fracture of the femur in 13. Following discharge, serial follow-up consisted of clinical evaluation based on the Harris Hip Score and radiological assessment. The survival of the prosthesis using revision for any reason as an end-point was calculated by kaplanMeier ***: Twenty-four(15%) patients died during the follow-up study, 6(4%) patients were lost, while the remaining 134 patients(141 hips) were followedup for a mean of 10 years(3-18 years). HSS score at the latest follow-up revealed that 84 hips(59.5%) had excellent results, 30(22.2%) good, 11(7.8%) fair and 9(6.3%) poor. There were 3 acetabular revisions due to aseptic loosening. Six(4.2%) stems were diagnosed as having radiographic definitive loosening; however, only 1 was revised. 30% of the surviving stems showed no radiological changes of radiolucency, while 70% showed some changes. Survival of the prosthesis for any reason was 96% at 10 years and 81% at 18 years. CONCLUSION: The 18 year survival of the Müller straight stem, cemented THA is comparable to those of other successful cemented systems.

The present study deals with the determination of Mount Olympus summit(Mytikas),exploiting modern observations such as Global Navigation Satellite Systems(GNSS)and existing geodetic *** last official Olympus height determination goes back 102 years by the Swiss surveyor *** then,only unofficial measurement campaigns have taken *** is a variety of released heights,which range from 2917 to 2919 *** September and October of 2022,we conducted a GNSS campaign,focusing on the area of highest Olympus *** precise GNSS measurements(accuracy of 1-3 cm),in conjunction with height information from Greece’s National Triangulation Network(5-10 cm accuracy)and an appropriate,recent geoid model(5-6 cm accuracy),we estimate through the so-called GNSS-levelling,the height of Olympus to 2917.727 m with respect to Greece’s officially accepted mean sea level and 2918.390 m with respect to the global vertical *** estimation of Olympus highest peak shows remarkable consistency at the level of 12.8 cm to that of *** in 1921.

Measuring the Accuracy and Precision of the Garmin GPS Positioning in Forested Areas： A Case Study in Taxiarchis-Vrastama University Forest

In this paper, it is attempted to examine and compare different orientation, one recreational and another more precise, in the performance of two （global positioning system） receivers of forested areas. In doing this, a field test on horizontal and vertical positional errors of GPS positioning at different points in the forested area of Taxiarchis-Vrastama University forest was conducted. The two GPS receivers were used to determine the positional accuracy of a selected number of points under tree canopies. Specifically, the precision and accuracy of Garmin＇s GPS positioning at different points were calculated and compared with the corresponding positioning and accuracy of another GPS system, namely the TOPCON GPS. By the calculation of various measures of accuracy and precision suitable for GPS receivers and the use of statistical methods, accuracy between the different receivers differed significantly is shown. Also, regression analysis revealed that the basal area and the number of available satellites are the most important factors for predicting position error.

Coronavirus disease 2019 is an infectious disease caused by the severe acute respiratory syndrome coronavirus 2 that manifests as a variety of clinical manifestations,including liver damage commonly detected by a hepatocellular pattern from liver function *** injury is associated with a worse prognosis *** associated with the severity of the disease include obesity and cardiometabolic comorbidities,which are also associated with nonalcoholic fatty liver disease(NAFLD).The presence of NAFLD,similarly to obesity,is associated with an unfavourable impact on the coronavirus disease 2019 *** with these conditions could present with liver damage and elevated liver function tests due to direct viral cytotoxicity,systemic inflammation,ischemic or hypoxic liver damage or drug side ***,liver damage in the setting of NAFLD could also be attributed to a pre-existing chronic low-grade inflammation associated with surplus and dysfunctional adipose tissue in these *** we investigate the hypothesis that a pre-existing inflammatory status is exacerbated after severe acute respiratory syndrome coronavirus 2 infection,which embodies a second hit to the underestimated liver damage.

The deacetylase SIRT2 contributes to autoimmune disease pathogenesis by modulating IL-17A and IL-2 transcription

Aberrant IL-17A expression together with reduced IL-2 production by effector CD4^(+)T cells contributes to the pathogenesis of systemic lupus erythematosus(SLE).Here,we report that Sirtuin 2(SIRT2),a member of the family of NAD^(+)-dependent histone deacetylases,suppresses IL-2 production by CD4^(+)T cells while promoting their differentiation into Th17 ***,we show that SIRT2 is responsible for the deacetylation of p70S6k,activation of the mTORC1/HIF-1α/RORγt pathway and induction of Th17-cell ***,SIRT2 was shown to be responsible for the deacetylation of c-Jun and histones at the Il-2 gene,resulting in decreased IL-2 *** found that the transcription factor inducible cAMP early repressor(ICER),which is overexpressed in T cells from people with SLE and lupus-prone mice,bound directly to the Sirt2 promoter and promoted its ***-7,a SIRT2 inhibitor,limited the ability of adoptively transferred antigen-specific CD4^(+)T cells to cause autoimmune encephalomyelitis in mice and limited disease in lupus-prone MRL/lpr ***,CD4^(+)T cells from SLE patients exhibited increased expression of SIRT2,and pharmacological inhibition of SIRT2 in primary CD4^(+)T cells from patients with SLE attenuated the ability of these cells to differentiate into Th17 cells and promoted the generation of IL-2–producing T ***,these results suggest that SIRT2-mediated deacetylation is essential in the aberrant expression of IL-17A and IL-2 and that SIRT2 may be a promising molecular target for new SLE therapies.

The median arcuate ligament syndrome(MALS)is recognized as a rare clinical entity,characterized by chronic post-prandial abdominal pain,nausea,vomiting,and unintentional weight *** to its vague symptomatology,it is mainly regarded as a diagnosis of *** can often be misdiagnosed for several years before a correct diagnosis is established,also due to a medical team’s clinical *** present a case series of two patients who suffered from MALS and were treated *** first patient is a 32-year-old woman,presenting with post-prandial abdominal pain and weight loss that have lasted for the past ten *** second patient,a 50-year-old woman,presented with similar symptomatology,with the symptoms lasting for the last five *** cases were treated by laparoscopic division of the median arcuate ligament fibers,which alleviated extrinsic pressure from the celiac *** cases of MALS were retrieved from PubMed,to assemble a better diagnostic algorithm and propose a treatment method of *** literature review suggests an angiography with a respiratory variation protocol as the diagnostic modality of choice,along with the laparoscopic division of the median arcuate ligament fibers as the proposed treatment of choice.

Hepatocellular carcinoma occurrence in DAA-treated hepatitis C virus patients:Correlated or incidental? A brief review

Hepatitis C virus(HCV) chronic infection induces liver fibrosis and cirrhosis but is also responsible for a significant portion of hepatocellular carcinoma(HCC) occurrence. Since it was recognized as a causative factor of chronic hepatitis,there have been multiple efforts towards viral eradication,leading to the first-generation HCV treatment that was based on interferon(IFN)-α and its analogs,mainly PEGylated interferon-α(PEG IFNα). Sustained virological response(SVR),defined as the absence of detectable RNA of HCV in blood serum for at least 24 wk after discontinuing the treatment,was accepted as a marker of viral clearance and was achieved in approximately one-half of patients treated with PEG IFNα regimens. Further research on the molecular biology of HCV gave rise to a new generation of drugs,the so-called direct antiviral agents(DAAs). DAA regimens,as implied by their name,interfere with the HCV genome or its products and have high SVR rates,over 90%,after just 12 wk of per os treatment. Although there are no questions about their efficacy or their universality,as they lack the contraindication for advanced liver disease that marks PEG IFNα,some reports of undesired oncologic outcomes after DAA treatment raised suspicions about possible interference of this treatment in HCC development. The purpose of the present review is to investigate the validity of these concerns based on recent clinical studies,summarize the mechanisms of action of DAAs and survey the updated data on HCV-induced liver carcinogenesis.

The Recurrence Rate in Meningiomas: Analysis of Tumor Location, Histological Grading, and Extent of Resection

Background: Purpose of this study was to evaluate various parameters of meningioma after surgical treatment and analyze predictive factors for recurrence. Methods: During 1992-2007, 353 patients were operated for intracranial meningioma in our department. They were followed since, and related data were combined with parameters such as tumor histology (WHO system), tumor location and the extent of tumor resection (Simpson’s scale). The results were analyzed with respect to tumor reappearance. This study was approved by the ethics committee of G. Papanikolaou Hospital. Results: The mean follow up period was 6.45 years. The overall percentage of recurrence was 21.52%. Grade 1 meningiomas (benign) recurred at a rate of 19.1%, grade 2 tumors (atypical) showed 41.7% rate of recurrence and grade 3 meningiomas (malignant) recurred at a rate of 75%. Tumor histopathology was not significant to recurrence (p > 0.001). The location of meningiomas was not found to be significant to recurrence (p > 0.001). Complete tumor resection was accomplished in 269 patients (76.2%). Incomplete resection (Simpson grades 2-5) took place in 84 cases (23.8%). The recurrence rate after complete resection was 13.8%, while the rate for cases with incomplete resection was 46.7%. The extent of removal was significantly associated with recurrence (p < 0.001). Conclusions: Tumor recurrence was observed at 21.5% of patients in our series of intracranial meningioma surgery. The rate of recurrence was related primarily to the extent of surgical removal. Neither tumor site, nor tumor histology were predictive factors for recurrence (ma-lignant meningiomas excluded).

Real-Time Urban Traffic State Estimation with A-GPS Mobile Phones as Probes

This paper presents a microscopic traffic simulation-based method for urban traffic state estimation using Assisted Global Positioning System (A-GPS) mobile phones. In this approach, real-time location data are collected by A-GPS mobile phones to track vehicles traveling on urban roads. In addition, tracking data obtained from individual mobile probes are aggregated to provide estimations of average road link speeds along rolling time periods. Moreover, the estimated average speeds are classified to different traffic condition levels, which are prepared for displaying a real-time traffic map on mobile phones. Simulation results demonstrate the effectiveness of the proposed method, which are fundamental for the subsequent development of a system demonstrator.

Impact of age on transcatheter aortic valve implantation outcomes：a comparison of patients aged≤ 80 years versus patients ＆gt; 80 years

ObjectiveTo investigate the procedural outcomes and the long-term survival of patients undergoing transcatheter aortic valve implantation （TAVI） and compare study results of patients≤ 80 years and patients ＆gt; 80 years *** total of 240 patients treated with TAVI were divided into two groups according to age≤ 80 years （n = 105; 43.8%） and ＆gt; 80 years （n= 135; 56.2%）. The baseline characteristics and the pro-cedural outcomes were compared between these two groups of *** the exception of peripheral artery disease and hypercho-lesterolemia, which were more frequently observed in the older age group, baseline characteristics were comparable between groups. Complica-tion rates did not differ significantly between patients≤ 80 years and patients ＆gt; 80 years. There were no differences in 30-day mortality rates be-tween patients aged≤ 80 years and patients ＆gt; 80 years old （9.5%vs. 7.4%, respectively;P = 0.557）. After a median follow-up of 28 months （in-terquartile range： 16-42 months）, 50 （47.6%） patients aged≤ 80 years died compared to 57 （42%） deaths in the group of patients ＆gt; 80 years old （P= 0.404）.ConclusionThe results of the present single center study showed that age did not significantly impact the outcomes of TAVI.
