Fractures of the lesser tuberosity of the humerus are typically met in combination with other injuries of the shoulder. Case reports of isolated lesser tuberosity fractures are particularly rare and, consequently,therapeutic protocols have not yet been completely clarified. Conservative as well as surgical treatment has been recommended, while several operative techniques have been applied. We present a case of a 39-yearold man with an isolated lesser tuberosity fracture who was treated surgically in our institution. Due to fracture comminution, a two-level reconstruction technique with headless screws and buttress plate was applied. As far as we know, this method of fixation of this type of fracture has not been previously described in the *** patient tolerated the procedure well and excellent results were obtained at the latest follow-up.

Background:To evaluate the safety and efficacy of an approach that combines corneal customized transepithelial therapeutic ablation to treat irregular corneal optics and accelerated corneal collagen cross-linking(CXL)to strengthen the corneal tissue and stop the progression of *** transepithelial therapeutic ablation applied a novel concept named central corneal regularization(CCR)which could correct the corneal morphological irregularities and the eye’s spherocylindrical refractive error with minimal stromal tissue ***:Retrospective *** that underwent CCR combined with CXL were evaluated preoperatively and up to 12 months postoperatively for visual acuity,subjective refraction,corneal haze,pachymetry and maximum keratometry(kmax).Results:Twenty four eyes of 24 patients with a mean age of 28.92±9.88 years were *** mean spherical equivalent(SE)refractive error changed from−0.74±1.17 D preoperatively to−1.05±1.52 D at 12 months *** mean uncorrected distance visual acuity(UDVA)and corrected distance visual acuity(CDVA)*** eye lost lines of CDVA,21 had a mean improvement of 3.21 *** mean cylinder error and kmax value dropped from−3.06±1.83 D and 51.38±3.29 D to−1.04±0.80 D and 48.70±2.58 D,*** mean haze score at 3,6 and 12 months was 0.56,0.19 and 0.06,***:CCR combined with CXL offers promising results as a safe and effective treatment in keratoconic patients.

Total knee replacement(TkR) is a widely used operation that has radically improved the quality of life of millions of people during the last few decades. However, some technical details, concerning the surgical procedure and the rehabilitation following total knee arthroplasty, are still a matter of a strong debate. In this review of the literature, we have included the best evidence available of the last decade, in an effort to shed light on some of the most controversial subjects related to TkR surgery. Posterior-stabilized or cruciate-retaining prosthesis? To use a tourniquet during operation or not? Do patients need continuous passive motion for their post-surgery rehabilitation? To resurface patella or not? These are some of the most controversial topics that until now have been persistent dilemmas for the orthopedic surgeon. Results of this systematic review of the literature are highly controversial. These conflicting results are an indication that larger and more well conducted high quality trials are needed in order to gain more secure answers. At the same time, it is becoming apparent that a meticulous operative technique, respecting the soft tissue envelope and knowing the principles of alignment and soft tissue balancing, aresome of the parameters that might contribute more to achieving the optimal results for the patients.

Successful endovascular treatment of transplant intrarenal artery stenosis in renal transplant recipients: Two case reports

Transplant renal artery stenosis(TRAS) is a relatively rare complication after renal transplantation. The site of the surgical anastomosis is most commonly involved, but sites both proximal and distal to the anastomosis may occur, as well. Angioplasty is the gold standard for the treatment of the stenosis, especially for intrarenal lesions. We report two cases of intrarenal TRAS and successful management with angioplasty without stent placement. Both patients were male, 44 and 55 years old respectively, and they presented with elevated blood pressure or serum creatinine within three months after transplantation. Subsequently, they have undergone angioplasty balloon dilatation with normalization of blood pressure and serum creatinine returning to baseline level. Percutaneous transluminal balloon renal angioplasty is a safe and effective method for the treatment of the intrarenal TRAS.

Introduction: Pediatric Flat Foot (PFF) is a deviation consisting of loss of the Medial Longitudinal Arch in children. Purpose: Aim of this study is to collect information on the recent literature and to investigate through clinical assessment and parental observations the effectiveness of the use of foot orthotics by children with SFFF. Method: 20 children aged 6 - 7 years old with mobility difficulties were evaluated before and after a six-month treatment with orthotics regarding pain, post-game pain, fatigue during game, balance impairment, gait deviations, falls, clumsiness, activity avoidance and worn soles. Results: Study results indicate that foot orthotics (FOs) appears to have a positive impact on the advance of pain, post game pain, gait deviations and worn insoles. It is implied that orthoses have a great potential as a treatment for SFFF. Thorough comprehension of the literature evidence, as well as composition of supplementary studies of larger pediatric populations is essential in order to reach a consensus on the use of foot orthotics (FOs) by children SFFF.

Assessment of the Relationships among Catchments’ Morphometric Parameters and Hydrologic Indices

In Greece the hydrological analysis of ephemeral streams has been especially difficult due to the lack of precipitation and discharge gauges. This study focuses on the investigation of possible relationship between morphometric characteristics of small to medium drainage basins and hydrological indices in order to discover morphometric parameters “predictors” of flash flood potential of ungauged catchments. Twenty-two morphometric parameters of twenty-seven drainage basins (ranging in area between 3.6 km2 and 330.5 km2) located in the northern part of the Peloponnese in southern Greece were calculated utilizing GIS software ArcGIS10. Hydrological modeling was performed using a simplified Matlab implementation of TOPMODEL, a conceptual model based on the principle of variable contributing area to runoff production through saturated overland flow, and LISEM, a physically based hydrologic and soil erosion model. Rainfall-runoff simulations were performed for an extreme precipitation event. The simulations outcomes, which include the peak discharge, time to peak and the percentage runoff, were correlated with the morphometric parameters of the catchments. Results were not consistent between the two models, probably due to their different structure, with the LISEM results being closer to what is anticipated. The results demonstrate that area, length of the basin, perimeter and compactness factor appear better correlated with the peak discharge (Qpeak) of the catchment. The same parameters as well as Melton’s number correlate with percentage runoff (C), while “celerity” of the flood wave (length of the basin/time to peak) is better correlated with relief, indicating that as the relief becomes greater, the response of the basin becomes fastest.

Promoter Methylation, BRAF Mutation Analysis and Topoisomerase IIa Expression for the Detection of Endometrial Carcinoma in Liquid Based Cytology Samples

Cancer of the corpus uteri remains the most common gynecological related cancer in developed countries. Cytology, after the induction of liquid based cytology, has reemerged as a possible first line non-interventional diagnostic procedure with promising results. Apart from slide preparation for cytology diagnosis, LBC allows the application of elaborate molecular tests on the residual material. Samples from 74 symptomatic women were collected in ThinPrep?PreservCyt medium, from witch immunocytochemical and molecular tests were performed. Final diagnosis of 39 endometrioid carcinomas, 20 non-endometrioid carcinomas and 15 non-malignant was set after hysterectomy. Topoisomerase IIa expression was common (42%) in both types of cancer. Promoter methylation analysis revealed that hMLH1 is commonly methylated in cancers (52.7%), CDkN2A and MGMT less often (27.1%) and RARB rarely methylated (8.4%). BRAF activating mutation V600E was a rare event (8.4%) only found in low grade endometrioid carcinomas. Topoisomerase IIa expression correlated with BRAF mutations, hMLH1 and to lesser extent with CDkN2A methylation. Almost none of the biomarkers were positive in cytological negative or hyperplastic without atypia samples. Detection of methylation in any gene displayed sensitivity, specificity, PPV and NPV similar to cytology of cancer. However, inclusion of cytology diagnosis of hyperlasias with atypia increased sensitivity and NPV of cytology outperforming methylation of any gene. Further evaluation of the panel of promoter methylation, especially in cytology diagnoses of hyperplasia with or without atypia should be evaluated since initial results are promising. Even though methylation of MGMT and RARB are rare events, some patients could be benefit from specific chemotherapeutics that target either of them or the more frequently expressed topoisomerase IIa.
