An Early Experience of Stapled Hemorrhoidectomy in a Medical College Setting

Background: Stapled hemorrhoidectomy, popularly known as Longo technique is in use for the treatment of hemorrhoids since its first description to surgical fraternity in the world congress of endoscopic surgeons in 1998. Objectives: To evaluate the feasibility, patient acceptance, recurrence and results of stapled haemorrhoidectomy in our early experience. Methods: Between Jan 2012 and Dec 2013, 42 patients with symptomatic GRADE III and IV hemorrhoids were operated by stapled hemorrhoidectomy by a single surgeon at our surgery department. The evaluation of this technique was done by assessing the feasibility of the surgery;and recording operative time, postoperative pain, complications, hospital stay, return to work and recurrence. Results: All the procedures were completed successfully. The mean (range) operative time was 30 (20 - 45) min. The blood loss was minimal. Mean (range) length of hospitalization for the entire group was 1 (1 - 3) days. Only 3 patients required more than 1 injection of diclofenac (75 mg) while as rest of the patients were quite happy switching over to oral diclofenac (50 mg) just after a single parenteral dose. All the patients returned to their routine work in less than a week’s time. The mean (range) follow-up was 6 (4 - 12) months. There were no major intraoperative or postoperative complications except for retention of urine in 8 patients. The patients are still on regular follow-up and have not had recurrence as yet. Conclusion: Stapled hemorrhoidectomy technique is a safe alternative to the traditional Milligan-Morgan technique. It can be performed as an office procedure, is well tolerated by patients and is cost effective than conventional surgical therapy.

Multimodal medical image fusion has attained immense popularity in recent years due to its robust technology for clinical *** fuses multiple images into a single image to improve the quality of images by retaining significant information and aiding diagnostic practitioners in diagnosing and treating many ***,recent image fusion techniques have encountered several challenges,including fusion artifacts,algorithm complexity,and high computing *** solve these problems,this study presents a novel medical image fusion strategy by combining the benefits of pixel significance with edge-preserving processing to achieve the best fusion ***,the method employs a cross-bilateral filter(CBF)that utilizes one image to determine the kernel and the other for filtering,and vice versa,by considering both geometric closeness and the gray-level similarities of neighboring pixels of the images without smoothing *** outputs of CBF are then subtracted from the original images to obtain detailed *** further proposes to use edge-preserving processing that combines linear lowpass filtering with a non-linear technique that enables the selection of relevant regions in detailed images while maintaining structural *** regions are selected using morphologically processed linear filter residuals to identify the significant regions with high-amplitude edges and adequate *** outputs of low-pass filtering are fused with meaningfully restored regions to reconstruct the original shape of the *** addition,weight computations are performed using these reconstructed images,and these weights are then fused with the original input images to produce a final fusion result by estimating the strength of horizontal and vertical *** standard quality evaluation metrics with complementary properties are used for comparison with existing,well-known algorithms objectively to validate the fusion *** results from th

Efficacy evaluation of standardized Rheum rhaponticum root extract(ERr 731®)on symptoms of menopause:A systematic review and meta-analysis study

Menopause is characterized by various physical,mental and emotional *** 731®is a standardized extract from Rheum rhaponticum root and has been clinically studied for its role in reducing menopausal *** current systematic review and meta-analysis aimed to evaluate the efficacy of ERr 731®supplementation in alleviating the severity of menopausal *** this review,we searched across three online databases up to March 2023,evaluated the quality of the included studies by the Physiotherapy Evidence Database scale,and assessed the risk of bias by the Cochrane Risk of Bias *** then performed a metaanalysis using RevMan software to estimate the pooled mean difference(MD).The study protocol was registered in the Prospective Register of Systematic Reviews(CRD42023416808).After screening and evaluation,we included four high-quality studies(a total of 390 participants;the ERr 731®group:193 participants;the control group:197 participants)in the *** results showed that ERr 731®supplementation significantly reduced the Menopause Rating Scale score(MD:–15.12;P<0.001),compared with control *** analysis revealed no effect of individual studies on the overall pooled estimate or overall observed *** current review provides evidence that ERr 731®supplementation is effective in reducing menopause *** bias and high heterogeneity in the results warrant further clinical studies.

Secure and Efficient Outsourced Computation in Cloud Computing Environments

Secure and efficient outsourced computation in cloud computing environments is crucial for ensuring data confidentiality, integrity, and resource optimization. In this research, we propose novel algorithms and methodologies to address these challenges. Through a series of experiments, we evaluate the performance, security, and efficiency of the proposed algorithms in real-world cloud environments. Our results demonstrate the effectiveness of homomorphic encryption-based secure computation, secure multiparty computation, and trusted execution environment-based approaches in mitigating security threats while ensuring efficient resource utilization. Specifically, our homomorphic encryption-based algorithm exhibits encryption times ranging from 20 to 1000 milliseconds and decryption times ranging from 25 to 1250 milliseconds for payload sizes varying from 100 KB to 5000 KB. Furthermore, our comparative analysis against state-of-the-art solutions reveals the strengths of our proposed algorithms in terms of security guarantees, encryption overhead, and communication latency.

Impact of Social Media Challenges on Pediatric Single-Use Detergent Sacs and Diphenhydramine Ingestions Reported to United States Poison Control Centers

Background: Social media platforms are popular among children and often feature challenges that become viral. Notably, the Tide Pod® and Benadryl® challenges encouraged viewers to ingest these substances for their visual appeal and hallucinogenic effects, respectively. This study aimed to assess the clinical impact and outcomes of single-use detergent sacs (SUDS) and diphenhydramine challenges on pediatric ingestions reported to United States (U.S.) Poison Control Centers (PCCs). Methods: We conducted a retrospective review of pediatric exposures reported to U.S. PCCs using data from the National Poison Data System (NPDS). The study included intentional single-substance ingestions of both brand-name and generic forms of SUDS and diphenhydramine among children ≤ 19 years. We compared the number of calls, clinical effects, disposition, and management strategies for SUDS (pre: 01/01/17 to 12/31/17 vs. post: 01/01/18 to 12/31/18) and diphenhydramine (pre: 08/01/19 to 07/31/20 vs. post: 08/01/20 to 07/31/21) ingestions 12 months before and after the introduction of the respective social media challenges. Differences in proportions were compared using the Chi-square test. Results: During the study period, 469 ingestions of SUDS and 5,702 ingestions of diphenhydramine were reported. Post-challenge periods saw an increase in both SUDS (pre: 82 vs. post: 387;372% increase) and diphenhydramine ingestions (pre: 2,672 vs. post: 3,030;13% increase). While there were no significant changes in moderate or major clinical outcomes, hospitalizations increased post-challenge for both SUDS [pre: 4 (4.9%) vs. post: 33 (8.5%);p = 0.25] and diphenhydramine [pre: n = 904 (33.8%) vs. post: 1,190 (39.3%);p Conclusion: Pediatric ingestions reported to U.S. PCCs and hospitalizations increased coinciding with the introduction of Tide Pod® and Benadryl® challenges.
