Peyronie's disease:a silent consequence of diabetes mellitus

Aim： To investigate the clinical characteristics of patients with Peyronie＇s disease （PD） and diabetes mellitus （DM）. Methods： During an 8-year period, a total of 307 men seen at our outpatient clinic were diagnosed with PD. Clinical characteristics, penile deformities and the erectile status of patients with PD and DM together （n = 102） were retrospectively analyzed and compared to patients with PD alone with no risk factors for systemic vascular diseases （n = 97）. Results：The prevalence of PD among men with DM and sexual dysfunction was 10.7 %. The mean age of diabetic patients with PD was （55.9 ± 8.9） years; in the no risk factor group it was （48.5 ± 9.0） years （P 〈 0.05）. The median duration of DM was 5 years. The majority of diabetic patients with PD （56.0 %） presented in the chronic phase （P 〈 0.05）, and they were more likely to have a severe penile deformity （〉 60°） than the no risk factor group （P 〈 0.05）. In the diabetic group, the most common presenting symptom was penile curvature （81.4%）, followed by a palpable nodule on the shaft of the penis （22.5%） and penile pain with erection （14.7%）. A total of 19.6% of patients were not aware of their penile deformities in the diabetic group. Erectile function, provided by history and in response to intracavernosal injection and a stimulation test, was significantly diminished in patients with PD and DM （P 〈 0.05）. Conclusion： DM probably exaggerates the fibrotic process in PD. Diabetic patients with PD have a higher risk of severe deformity and erectile dysfunction （ED）. Background： A few studies have been conducted to determine the relationship between road motor vehicle crashes （MVC） and serious injuries related to tiredness, fatigues and sleeping. Aim： To determine the effects of aggressive behaviour, driver sleepiness and fatigue on MVC and related injuries among Turkish population. Design and setting： Population-based case and control study conducted at the accident emergency departments of hospitals and roads. Subjects： 515 car drivers involved in crashes with injury were admitted to hospital and 1030 car drivers involved while driving on public roads as control group during the study period. Methods： The Manchester driver behaviour questionnaire （DBQ） measured the aberrant driving behaviours leading to accidents. Participants completed a fatigue severity scale （FSS） and Stanford Sleeping questionnaire an epworth scale with items related to socio-demographic information, driving experiences, adherence to traffic laws （such as speed limits and seat belt）, and drivers＇ driving records. Results： In a representative sampling, participant＇s age ranged from 25 to 65 and the mean and standard deviation were 36.5 _＋ 7.8 for cases and 37.0 4- 8.0 for controls. There was a significant difference in both group of drivers regarding BMI, level of education, marital status, driving experience, seat belt use, excessive speed limits, physical activity number of sleeping hours, mobile phone use, and cigarette smoking habit （p = 0.017）. Also, there was a significant higher mean score on all the DBQ violation questions among case group in comparison with the control group （p 〈 0.001）. Further, cases had higher prevalence of Epworth sleeping disorders （p 〈 0.001） and fatigue severity （p - 0.003） compared to control drivers. Multivariate logistic regression revealed that excessive speed, fatigue, lapses, errors, Stanford sleepiness score, violations, mobile phone use and Epworth sleepiness scale were significantly associated with injury involvemen

AIM： To investigate the effects of granulocyte-colony stimulating factor （G-CSF） on peritoneal defense mechanisms and bacterial translocation after systemic 5-Fluorouracil （5-FU） administration. METHODS： Thirty Wistar albino rats were divided into three groups; the control, 5-FU and 5-FU ＋ G-CSF groups. We measured bactericidal activity of the peritoneal fluid, phagocytic activity of polymorphonuclear leucocytes in the peritoneal fluid, total peritoneal cell counts and cell types of peritoneal washing fluid. Bacterial translocation was quantified by mesenteric lymph node, liver and spleen tissue cultures. RESULTS： Systemic 5-FU reduced total peritoneal cell counts, neutrophUs and macrophage numbers. It also altered bactericidal activity of the peritoneal fluid and phagocytic activity of polymorphonuclear leucocytes in the peritoneal fluid. 5-FU also caused significant increase in frequencies of bacterial translocation at the liver and mesenteric lymph nodes. G-CSF decreased bacterial translocation, it significantly enhanced bactericidal activity of the peritoneal fluid and phagocytic activity of polymorphonuclear leucocytes in the peritoneal fluid. It also increased total peritoneal cell counts, neutrophils and macrophage numbers. CONCLUSION： Systemic 5-FU administration caused bacterial translocation, decreased the bactericidal activity of peritoneal fluid and phagocytic activity of polymorphonuclear leucocytes in the peritoneal fluid. G-CSF increased both bactericidal activity of the peritoneal fluid and phagocytic activity of polymorphonuclear leucocytes in the peritoneal fluid, and prevented the bacterial translocation. We conclude that intraperitoneal GCSF administration protects the effects of systemic 5-FU on peritoneal defense mechanisms.

AIM:To analyze gastric polypoid lesions in our patient- population with respect to histopathologic features and demographic,clinical,and endoscopic characteristics of patients. METHODS:Clinical records and histopathologic reports of patients with gastric polypoid lesions were analyzed *** lesions had been totally removed by either endoscopic polypectomy or hot biopsy *** histopathologic slides were re-evaluated by the same histopathologist. RESULTS:One-hundred and fifty gastric polypoid lesions were identified in 91 *** were 53(58 %)women and 38(42 %)men with a median age of 53(range,31 to 82)*** most frequent presenting symptom was dyspepsia that was observed in 35(38.5 %)patients. Symptoms were mostly related to various associated gastric abnormalities such as chronic gastritis or H pylori infection rather than polypoid lesion *** lesions were commonly located in the antrum followed by *** of 150 lesions,80(53 %)had the largest dimensions less than or equal to 5 mm and only 7 were *** frequencies of hyperplastic polyps,foveolar hyperplasia,and fundic gland polyps were 46 %,18 %,and 14 % respectively. We also detected gastritis varioliformis in 12 specimens, lymphoid follicles in 9,4 adenomatous polyps in 4,polypoid lesions with edematous mucosa in 4,inflammatory polyps in 3,and carcinoid tumor in *** changes were observed within two hyperplastic polyps and low grade dysplasia in one *** evaluation of the surrounding gastric mucosa demonstrated chronic gastritis in 72(79 %)patients and H pylori infection in 45(49 %). CONCLUSION:Hyperplastic polyps are the mbst frequently encountered subtype of gastric polypoid *** are usually associated with chronic gastritis or H pylori *** to the previous belief,they may harbour adenomatous changes or dysplastic foci. A 21-year-old man presented with an enlarged giant hemangioma on glans penis which also causes an erectile dysfunction （ED） that partially responded to the intracavernous injection stimulation test. Although the findings in magnetic resonance imaging （MRI） indicated a glandular hemangioma, penile colored Doppler ultrasound revealed an invaded cavernausal hemangioma to the glans. Surgical excision was avoided according to the broad extension of the gland lesion. Holmium laser coagulation was applied to the lesion due to the cosmetically concerns. However, the cosmetic results after holmium laser application was not impressive as expected without an improvement in intracavernous injection stimulation test. In conclusion, holmium laser application should not be used to the hemangiomas of glans penis related to the corpus cavernosum, but further studies are needed to reveal the effects of holmium laser application in small hemangiomas restricted to the glans penis.

1 Introduction Nigella sativa, known as black seed, has analgesic, anti-inflammatory, antioxidant and anticancer effects. It has been shown to reduce the development of kidney failure when given prior to the use of nephrotoxic drugs particularly due to its antioxidant action. However, as far as the authors could ascertain, there is no human study in literature showing these effects. Here we present a case of acute renal failure after the use of N. sativa, rather than exhibiting antioxidant or antidiabetic effects.

为了限制地震与风对建筑物造成的损坏,估算建筑物的侧向变形已成为建筑结构设计的重要内容.尽管有些国家的建筑规范中已给出了估算木结构剪力墙侧向变形的方法,但这些方法并不适用于无抗倾覆锚固件木结构剪力墙.在Forintek钉节点试验结果的基础上,建议一个新的钉节点——侧向荷载—变形公式,并将美国《统一建筑规范》(Uniform Building Code)所采用的四项式变形公式进行了扩展,给出能进行无抗倾覆锚固件木结构剪力墙侧向变形计算的公式.计算预测值与试验结果对比较好.

Objective The present study was aimed to determine the effect of penile prosthesis implantation(PPI)surgery on penile sensation by evaluating the penile electromyography(EMG)*** The research was designed as a prospective *** patients who were diagnosed with organic erectile dysfunction and had underwent PPI surgery between January 2017 and January 2018 in the Urology Clinic of Antalya Training and Research Hospital were included in this *** sensory EMG was performed on each patient 1 day prior to the surgery by the Physical Therapy and Rehabilitation ***,the control EMG study was also performed in the 3rd and 6th postoperative *** We included 27 patients in this study who attended regular follow-ups and had complete EMG *** of the 27 patients,23(85.2%)patients had received malleable(ProMedon)penile prosthesis,one(3.7%)patient had received a two-piece inflatable(Ambicor)penile prosthesis,and three(11.1%)patients had received a three-piece inflatable(AMS 700 CXR)penile *** mean nerve conduction value(NCV)of the patients was 29.85(standard derivation:22.54;range:0-78.4)m/s during the preoperative period,whereas it was 27.64(standard derivation:24.72;range:0-83.3)m/s in the 3rd postoperative month and 24.80(standard derivation:22.31;range:0-88.4)m/s in the 6th postoperative *** was no significant difference between preoperative NCV and 3rd postoperative month NCV(p=0.607).Similarly,no significant difference was observed between preoperative NCV and 6th postoperative month NCV(p=0.276).Additionally,the change between NCV values at postoperative 3rd and 6th months was not statistically significant(p=0.553).Conclusion Significant loss of penile sensation does not occur in patients who undergo PPI surgery.

Time evaluation of wave functions for any quantum mechanical system/particle is essential nevertheless quantum mechanical counterpart of the time dependant classical wave equation does simply not appear. Epistemologically and ontologically considered time dependant momentum operator is initially defined and an Alternative Time Dependant Schrodinger Wave Equation (ATDSWE) is plainly derived. Consequent equation is primarily solved for the free particles, in a closed system, signifying a good agreement with the outcomes of the ordinary TDSWE. Free particle solution interestingly goes further possibly tracing some signs of new pathways to resolve the mysterious quantum world.
