Transitioning patients with inflammatory bowel disease from hospital-based to rapid home-based infliximab: A stepwise, safety and patient-orientated process towards sustainability

BACKGROUND Infliximab and other intravenous biologic infusions are increasingly used for chronic disorders like inflammatory bowel disease(IBD).Rapid infliximab and home-based infusions are attractive solutions to address resource and capacity issues for infusion centres,yet infliximab infusion reactions reportedly occur in up to 25%of patients with IBD,even at the manufacturers’recommended infusion duration of 2 *** To evaluate the safety,cost and patient satisfaction of transitioning from hospitalbased,standard 2 h to rapid home-based,30-min infliximab *** All patients receiving rapid infliximab infusions for IBD between 2014 to 2017(39 mo)were compared with those who received standard two-hour IFX infusions between 2005-2013(96 mo)at a single IBD ***(per-infusion and perindividual)including adverse drug reactions(ADR),duration(based on needledeparture time)and other clinical data were extracted from electronic medical *** logistical regression analysis assessed factors potentially associated with increased risk of ADRs to rapid *** primary outcome was the safety[as per relative risk(RR)of (ADR)of(1)rapid 30 m infusions(both hospital-and home-based)vs standard 2 h infliximab ***,relative cost per infusion and patient satisfaction and productivity were evaluated in rapid infusion recipients who transitioned to home-based *** Of 129 patients who received 1461 rapid IFX infusions(2014-2017)were compared with 169 patients who received 2214 standard IFX infusions(2005-2013).Within the rapid cohort,55(42.6%)were males,median age 42 years(range 18,86),114(84%)had Crohn’s disease(CD)with a median disease duration 5 years(0,36).Median needle to departure time was higher in the standard than the rapid protocol group,108(70,253)vs 50(33,90)min,(P<0.001),with a per infusion cost of$AUD 107.50 vs$49.77,respectively(both P<0.001).There was no difference in median infusion duration or costs between rapi

Hybrid Control Strategy for BCD Topology Based Modular Multilevel Inverter

In this paper, a Binary Coded Decimal (BCD) topology of modular multilevel inverter with reduced component count is proposed. For the control of this inverter, hybrid control strategy is used. The proposed modular multilevel inverter uses asymmetrical dc sources and reduced number of switches topology. This hybrid modulation technique uses the multicarrier based Pulse Width Modulation (PWM) and the fundamental frequency modulation strategy. The hybrid modulation algorithm is implemented with “NUC140” micro-controller. In comparison with the conventional and some of the recently reported inverter topologies, the proposed inverter topology is able to generate high number of voltage levels in the output by using minimum number of components such as dc sources, power switches and driver circuits. This inverter offers significant performance with less number of components. The feasibility of the proposed topology is confirmed by simulation and experimental results.

Deep neural networks for understanding noisy data applied to physical property extraction in scanning probe microscopy

The rapid development of spectral-imaging methods in scanning probe,electron,and optical microscopy in the last decade have given rise for large multidimensional *** many cases,the reduction of hyperspectral data to the lower-dimension materialsspecific parameters is based on functional fitting,where an approximate form of the fitting function is known,but the parameters of the function need to be ***,functional fits of noisy data realized via iterative methods,such as least-square gradient descent,often yield spurious results and are very sensitive to initial ***,we demonstrate an approach for the reduction of the hyperspectral data using a deep neural network approach.A combined deep neural network/least-square approach is shown to improve the effective signal-to-noise ratio of band-excitation piezoresponse force microscopy by more than an order of magnitude,allowing characterization when very small driving signals are used or when a material’s response is weak.

Gait refers to a person’s particular movements and stance while moving *** each person’s gait is unique and made up of a variety of tiny limb orientations and body positions,they all have common characteristics that help to define *** identifying such characteristics that are difficult to spot by the naked eye,can help in monitoring the elderly who require constant care and *** silhouettes is the easiest way to assess and make any necessary adjustments for a smooth *** also becomes an important aspect of decision-making while analyzing and monitoring the progress of a patient during medical *** images made publicly available by the Chinese Academy of Sciences(CASIA)Gait Database was used in this *** evaluating using the CASIA B and C datasets,this paper proposes a Convolutional Neural Network(CNN)and a CNN Long Short-TermMemory Network(CNN-LSTM)model for classifying the gait silhouette *** learningmodels such as MobileNetV2,InceptionV3,Visual Geometry Group(VGG)networks such as VGG16 and VGG19,Residual Networks(ResNet)like the ResNet9 and ResNet50,were used to compare the efficacy of the proposed *** proved to be the best by achieving the highest accuracy of 94.29%.This was followed by ResNet9 and CNN-LSTM,which arrived at 93.30%and 87.25%accuracy,respectively.

We investigate the ability to reconstruct and derive spatial structure from sparsely sampled 3D piezoresponse force microcopy data,captured using the band-excitation(BE)technique,via Gaussian Process(GP)*** for weakly informative priors,GP methods allow unambiguous determination of the characteristic length scales of the imaging process both in spatial and frequency *** further show that BE data set tends to be oversampled in the spatial domains,with~30% of original data set sufficient for high-quality reconstruction,potentially enabling faster BE *** the same time,reliable reconstruction along the frequency domain requires the resonance peak to be within the measured *** behavior suggests the optimal strategy for the BE imaging on unknown ***,we discuss how GP can be used for automated experimentation in SPM,by combining GP regression with non-rectangular scans.

Exploration of structure-property relationships as a function of dopant concentration is commonly based on mean field theories for solid ***,such theories that work well for semiconductors tend to fail in materials with strong correlations,either in electronic behavior or chemical *** these cases,the details of atomic arrangements are generally not explored and *** knowledge of the generative physics and chemistry of the material can obviate this problem,since defect configuration libraries as stochastic representation of atomic level structures can be generated,or parameters of mesoscopic thermodynamic models can be *** obtain such information for improved predictions,we use data from atomically resolved microscopic images that visualize complex structural correlations within the system and translate them into statistical mechanical models of structure *** the significant uncertainties about the microscopic aspects of the material’s processing history along with the limited number of available images,we combine model optimization techniques with the principles of statistical hypothesis *** demonstrate the approach on data from a series of atomically-resolved scanning transmission electron microscopy images of Mo_(x)Re_(1-x)S_(2) at varying ratios of Mo/Re stoichiometries,for which we propose an effective interaction model that is then used to generate atomic configurations and make testable predictions at a range of concentrations and formation temperatures.

Employee Satisfaction： A Case Study at Bank Muamalat Malaysia Berhad

Employee satisfactions emerge as the central subject in this study because employee satisfaction is one of the most important factors in determining the success of an organization. Leading from such probe, this study formulates a model to test factors that drive towards employee satisfaction. The study considers the most positive and unique factors that most organization has, thus suggests that core factors for employee satisfaction notably are leadership style, working environment, employee rewards and welfare, and organization culture. The analysis involved two phases of qualitative and quantitative techniques. First, the study is based on the management of Bank Muamalat Malaysia Berhad （BMMB） to explore how far the relevant factors opined the literature review synchronized with the issue being examined. In the second phase, the study developed a questionnaire, based on the findings to reach out 1,216 employees of BMMB in 1999. This study found few differences in the factors that affected employee satisfaction. Various independent variables hypothesized were significant. In view of more than one independent variable, the multiple regression analysis is adopted in this study. It is assumed that these independent variables： leadership style, working environment, employee rewards and welfare, and organization culture, explained the employee satisfaction variation. The results show that there is significant relationship between employee satisfaction and these independent variables. The statistical package for the social sciences was used to analyze the data. It is anyhow the result shows that the independent variable： organizational culture-the most important variable that determines employee satisfaction.

Evaluation of Corrosion Stability of Water Soluble Epoxy-Ester Primer through Electrochemical Studies

The corrosion behavior of water borne epoxy-ester primer coatings (10% - 50% PVCs) was evaluated through electrochemical techniques such as polarization and impedance spectroscopic (EIS) measurements. Studies were carried out for a longer duration of exposure extending up to 60 days in aqueous solution of NaCl (5%). Corrosion current (icorr), Corrosion potential (Ecorr), Coating resistance (Rc), Charge transfer resistance (Rct), Coating capacitance (Cc), Double layer capacitance (Cdl), break point frequency (fb), Water uptake (∮), diffusion coefficient (Dw) etc., indicated that 10% - 30% PVC coatings performed well in comparison to higher PVCs. Changes in the electrochemical characteristics were found to occur as a function of exposure time in all cases. The corrosion stability of the coatings were found to be greatly affected by the percentage of PVCs. Studies further indicated that when lower concentration of pigments were available, they remained completely surrounded by the binder;Thus leaving no space for the entry of corrosive agents. From these studies, it was concluded that the water borne paints could replace the conventional coatings, containing organic solvents. Therefore, water soluble epoxy-ester primers have to be employed in paints for developing eco- friendly coatings.
