Use of Medicinal Cannabis for Palliative Care Patients: A Systematic Review

Background: Medical cannabis is a rapidly growing area of medicine. In this sense, due to the numerous benefits associated with its use, it has been increasingly proposed for patients in palliative care, in which the improvement of debilitating symptoms is directly associated with better quality of life. However, due to the complexity of treatments for these individuals, further studies are needed to determine the best possible prescription for them. Objective: Understand the endocannabinoid system and evaluate the feasibility of using medical cannabis in palliative care patients. Methods: Systematic literature review, addressing the following topics: “cancer, cannabis, and palliative care”. The search was carried out using the Virtual Health Library: Lilacs, SciELO, Medline, PubMed, and Cochrane, with articles between 2013 and 2022. Results and discussion: A total of 116 articles were found with the descriptors. After reading the abstracts, 22 showed greater association with the theme and were chosen for the preparation of this review. Conclusion: It is concluded that medical cannabis is an excellent treatment option for most of the symptoms present in patients in palliative care. In this way, the adoption of this drug by the public becomes logical. However, it is the physician’s role to seek to understand the particularities of these individuals and this new possibility of treatment, which, it seems, will significantly improve the end of life of these patients—this being the main role of the medical teams.

Comparative stability analyses of traditional and selective room-and-pillar mining techniques for sub-horizontal tungsten veins

The stability and productivity concerning a modification on the traditional room and pillar for a new selective technique at the Portuguese Panasqueira Mine have been described. The traditional room-and-pillar stoping uses 5.0-m wide rooms with 3.0 m ×3.0 m pillars, while the selective room-and-pillar mining technique consists in stoping with rooms of 4.0 m wide and pillars of 4 m ×4 m with a subsequent selective cutting of the quartz veins at the mid pillar of approximately 0.5 m high, to obtain a pillar section with an area of 3.0 m × 3.0 m. The stability and productivity analyses indicate that the selective technique obtains smaller average pillar safety factor, more rock mass displacement, more extraction and selectivity ratios, and ore grade improvement, compared with the traditional technique. These results show that the selective technique is also more convenient. This proposed selective room-and-pillar mining technique is applicable to any sub-horizontal narrow quartz veins with wolfram, gold, etc. such as the famous La Rinconada gold mine in the Peruvian Andes.

Tracheal Intubation Difficulties: Systematic Literature Review

Background: Tracheal intubation is a common procedure in medical practice, being performed mainly in surgical centers and emergencies in trauma patients. Objective: This systematic literature review aims to understand the main difficulties and complications related to tracheal intubation. Methods: Systematic literature review, conducted in the Virtual Health Library databases between 2016 and 2021, restricted to articles in English and Portuguese with the descriptors: Airway, Difficult intubation, and Complications. After reading the abstracts, 34 articles were found, and, after reading them, 9 articles were selected. Results: This review analyzed 9 scientific studies that rigorously met the previously established characteristics in the sample selection. The synthesis included the following aspects (author/year of publication, article title, objective, type of study, and database. Discussion: Several factors that influence the success of intubation were identified, such as technical, professional, and patient issues. In addition, measures to minimize the risks involving the procedure were highlighted. Conclusion: Tracheal intubation is a common procedure in surgical centers and emergencies. However, its risks, while low, should not be ignored. Unsuccessful procedures can occur and have consequences for the patient, such as trauma to the upper airway.

Therapeutic Approaches to Post-Mastectomy Pain Syndrome

Background: Breast cancer is increasingly common in society and post-mastectomy pain is a striking condition associated with surgery, both intra and postoperatively. The post-mastectomy syndrome is characterized by the persistence of pain for more than 3 months after the procedure. It is mostly of neuropathic origin and is highly refractory to treatments, which vary from clinical to surgical measures and alternative techniques. In this context, it is essential to understand the possible approaches to these patients. Objective: Understand alternatives for pain management in patients undergoing mastectomy, especially in those in whom pain persists for more than 3 months. Methods: Systematic literature review, conducted in the Virtual Health Library databases includes: Lilacs, SciELO, Medline, PubMed and Cochrane between 2018 and 2023, restricted to articles in English with the descriptors: Mastectomy, chronic Pain, Nerve Blocks and Breast Cancer. 317 articles were found involving the descriptors presented, all in English. After reading the abstracts, 28 articles were selected. Results: This review analyzed 28 scientific studies that rigorously met the previously established characteristics in the sample selection. The synthesis included the following aspects: author/year of publication, article title, objective, type of study and database. Conclusion: It was concluded that, according to the analyzed studies, post-mastectomy pain is a problem that has increased its incidence and that needs the attention of health professionals. The use of antineuropathic medications, nerve blocks, fat grafting, lymph node transplantation, therapy, physical activity and acupuncture are some examples of therapeutic approaches for these women. Therefore, the team must assist the patients, seeking to provide a better prognosis, quality of life and comfort.

Approach to Pain in Endometriosis Patients: Systematic Literature Review

Background: Endometriosis is a gynecological, multifactorial condition that affects women’s life quality, especially due to the pain inherent to it. Pain is caused by physical and psychological aspects;therefore, several factors must be analyzed, aiming to treat the whole. Conventional, drug or surgical treatments are the most used;however, the search for alternative therapeutic methods is growing. Objective: To analyze the approach to pain in women with endometriosis, seeking a better understanding of the pathophysiological mechanisms of this disease, its impact on life quality, as well as the means of treatments in use and innovations in this field. Methods: Systematic literature review, conducted in the Virtual Health Library databases between 2010 and 2021, restricted to articles in English and Portuguese with the descriptors: Endometriosis, Pain, and Treatment. After reading the abstracts, 197 articles were found, and, after reading them, 59 articles were selected. Results: This review analyzed 59 scientific studies that rigorously met the previously established characteristics in the sample selection. The synthesis included the following aspects: author/year of publication, article title, objective, type of study and database. Related factors such as acute or chronic pain, late diagnosis, discrediting symptomatic complaints and trivialization by health professionals have a profound impact on the physical and mental health of women with endometriosis. On the other hand, there are several classes of drugs, surgical procedures, and alternative therapies available for pain management. Conclusion: Given the context addressed, a certain need is suggested for health professionals to better understand the disease and adopt new behaviors or improve existing ones, such as reception, active listening, early diagnosis and comprehensive and individualized treatment. Introduction: At the beginning of the COVID-19 pandemic, a drop in the number of patients treated for cardiac emergencies raised concern about cardiovascular mortality in that period. An increase in care delay for patients with ST-segment elevation myocardial infarction (STEMI) may have affected clinical outcomes. Objectives: To analyze delay times and clinical outcomes of patients with STEMI undergoing primary percutaneous coronary intervention (PPCI), before and during the COVID-19 pandemic. Methods: Retrospective observational study that included patients with STEMI undergoing PPCI from December 2018 to July 2021. The COVID-19 pandemic cases were divided into two groups: pandemic I—from March to August 2020;and pandemic II—from September 2020 to July 2021. Patients were compared according to the period of hospitalization. Primary outcomes were delay times in assistance and clinical outcomes (acute kidney injury [AKI], post-procedural vascular complications and in-hospital mortality). Results: 108 patients were included, 39 (36.1%) in the pre-pandemic period, 13 (12.1%) in pandemic I and 56 (51.8%) in pandemic II. Time from onset of symptoms to arrival at the service and door-to-balloon time did not differ significantly among groups. Vascular complications were more frequent during the pandemic (I and II) than in the pre-pandemic period (2.5% pre-pandemic vs 15.4% pandemic vs 12.5% pandemic II;p = 0.03). AKI incidence was similar in all three periods. There was a non-significant increase in in-hospital mortality during the COVID-19 pandemic. Conclusion: In patients with STEMI, there was an increase in vascular complications and a trend toward increased mortality during the COVID-19 pandemic. Delay times to admission and reperfusion did not differ significantly between before and during the pandemic.

Sub-chronic effects of a Phthirusa pyrifolia aqueous extract on reproductive function and comparative hormone levels in male rats

Objective: To explore the effects of aqueous extract from Phthirusa pyrifolia leaves(67 mg/kg body weight for 12 days) on the reproductive function of male Wistar rats through oral ***: Animals(n = 30), aged 13 weeks and weighing(378.5 ± 5.0) g, were housed in a vivarium under controlled environmental conditions [photoperiod of 12 h light/dark,temperature of(23 ± 1)C] and were fed standard rations ad libitum. The experiment ran for 12 days, wherein animals were divided into three groups: negative control(n = 6)received water, positive control(n = 12) with finasteride at a concentration of 1.0 mg/kg;and a test group(n = 12) submitted to aqueous extract. At the end of the experiment, the animals were sacrificed and submitted to ***: The morphological results of the testes showed that the aqueous extract induced significant changes in the diameter and cross-sectional area of the seminiferous tubules as well as the thickness of the seminiferous epithelium. Furthermore, the extract was able to abruptly decrease testosterone concentrations by about 81.88% in the treated group when compared with the negative control,(47.0 ± 4.8) ng/d L and(255.0 ± 2.0) ng/d L,respectively, and 76.8%,(211.0 ± 8.7) ng/d L, when compared with finasteride. However,the extract causes neither liver damage nor impairment of renal ***: These results suggest that the high amounts of flavonoids shown to be in the extract may be responsible for its hepato-protective effects and suggest a possible decrease in the libido and reproduction of rats.
