目的在重症监护病房(Intensive care Unit,IcU),护士难免会遇到需要限制治疗努力(limiting therapeutic effort,LTE)的患者,尽管多数护十还缺乏为这些临终患者及其家庭提供支持的准备。本综述旨在描述IcU护士在LTE中需要具备的能力。方法对2010年—2023年发表的文献进行综述。系统检索了5个数据库:Medline、Wiley Online Library SciELO、ScienceDirect和Web of Science。使用关键评估项目工具西班牙语版(critical Appraisal Skills Programme in Spanish,cASPe)作为评判性阅读工具。使用牛津循证医学中心(center for Evidence-Based Medicine,cEBM)评估工具确定证据级别和推荐强度。结果共纳入25篇文献。IcU护士LTE护理的能力包括对患者的直接护理、以家庭为中心的护理以及护士在团队中的作用。该综述确定了3个类别的能力:作为患者、家庭与跨学科团队之间维护者的能力,在LTE方面的决策能力,在患者生命末期治疗和护理干预的能力。结论 IcU护十在LTE方面需要具备相应的能力,这对患者的生活质量、死亡尊严以及患者家庭应对悲伤的全面支持至关重要。

In recent years,the interest in the development of highly concentrated electrolyte solutions for battery applications has increased *** electrolyte solutions are typically characterized by a low flammability,a high thermal and electrochemical stability and by the formation of a stable solid electrolyte interphase(SEI)in contact to electrode ***,the classification of concentrated electrolyte solutions in terms of the classical scheme"strong"or"weak"has been controversially discussed in the *** this paper,a comprehensive theoretical framework is presented for a more general classification,which is based on a comparison of charge transport and mass *** combining the Onsager transport formalism with linear response theory,center-of-mass fluctuations and collective translational dipole fluctuations of the ions in equilibrium are related to transport properties in a lithium-ion battery cell,namely mass transport,charge transport and Li^(+)transport under anion-blocking *** relevance of the classification approach is substantiated by showing that i)it is straightforward to classify highly concentrated electrolytes and that ii)both fast charge transport and fast mass transport are indispensable for achieving fast Li^(+)transport under anion-blocking conditions.

Huntington’s disease is a neurodegenerative disease caused by the expansion mutation of a cytosine-adenine-guanine triplet in the exon 1 of the HTT gene which is responsible for the production of the huntingtin (Htt) protein. In physiological conditions, Htt is involved in many cellular processes such as cell signaling, transcriptional regulation, energy metabolism regulation, DNA maintenance, axonal trafficking, and antiapoptotic activity. When the genetic alteration is present, the production of a mutant version of Htt (mHtt) occurs, which is characterized by a plethora of pathogenic activities that, finally, lead to cell death. Among all the cells in which mHtt exerts its dangerous activity, the GABAergic Medium Spiny Neurons seem to be the most affected by the mHtt-induced excitotoxicity both in the cortex and in the striatum. However, as the neurodegeneration proceeds ahead the neuronal loss grows also in other brain areas such as the cerebellum, hypothalamus, thalamus, subthalamic nucleus, globus pallidus, and substantia nigra, determining the variety of symptoms that characterize Huntington’s disease. From a clinical point of view, Huntington’s disease is characterized by a wide spectrum of symptoms spanning from motor impairment to cognitive disorders and dementia. Huntington’s disease shows a prevalence of around 3.92 cases every 100,000 worldwide and an incidence of 0.48 new cases every 100,000/year. To date, there is no available cure for Huntington’s disease. Several treatments have been developed so far, aiming to reduce the severity of one or more symptoms to slow down the inexorable decline caused by the disease. In this context, the search for reliable strategies to target the different aspects of Huntington’s disease become of the utmost interest. convection-permitting modeling allows us to understand mechanisms that influence rainfall in specific ***,microphysics parameterization(MP) and planetary boundary layer(PBL) schemes remain an important source of uncertainty,affecting rainfall intensity,occurrence,duration,and ***,we study the sensitivity of rainfall to three MP [Weather Research and Forecasting(WRF) Single-Moment 6-class(WSM6),Thompson,and Morrison] and two PBL [the Yonsei University(YSU) and Mellor–Yamada Nakanishi Niino(MYNN)] schemes with a convection-permitting resolution(4 km) over northwestern South America(NWSA).Simulations were performed by using the WRF model and the results were evaluated against soundings,rain gauges,and satellite data,considering the spatio-temporal variability of rainfall over diverse regions prone to deep convection in *** and PBL schemes largely influenced simulated rainfall,with better results for the less computationally expensive WSM6 MP and YSU PBL *** rain gauges and satellite estimates,simulations with Morrison MP overestimated rainfall,especially westward of the Andes,whereas the MYNN PBL underestimated precipitation in the Amazon–Savannas *** found that the uncertainty in the rainfall representation is highly dependent on the region,with a higher influence of MP in the colombian Pacific and PBL in the Amazon–Savannas *** analyzing rainfall-related processes,the selection of both MP and PBL parameterizations exerted a large influence on the simulated lower tropospheric moisture flux and moisture *** schemes significantly influenced the downward shortwave radiation,with MYNN simulating a greater amount of low clouds,which decreased the radiation ***,latent heat fluxes were greater for YSU,favoring moist convection and *** schemes had a marked impact on vertical ***,Morrison MP showed stronger convection and higher precipitation rates,which is assoc

Background Forest ecosystems undergo significant transformations due to harvesting and climate fluctuations, emphasizing the critical role of seeding in natural regeneration and long-term structural preservation. climate change further amplifies these dynamics, affecting phenology across species and regions. In Tierra del Fuego(Argentina), Nothofagus pumilio(lenga) forests represent the most important timber resource, and it is managed through different silvicultural strategies. This species demonstrates notable post-disturbance regeneration, yet seed fall exhibits signifi-cant variability, leading to variations in seed quality(e.g., viability). This study aims to assess fluctuations in N. pumilio seed quality, determine how it varies concerning forest management strategies, annual productivity, and the co-occurrence of climatic phenomena including El Ni?o-Southern Oscillation(ENSO) and the Southern Annular Mode(SAM).Results Viable seeds represented 18.4% of the total, notably higher in unharvested than in managed areas. con-versely, empty seeds were more prevalent in harvested areas(> 75%). Seed quality exhibited significant differences across silvicultural treatments, except for insect-predated seeds, which had similar proportions across all areas, though dispersed retention showed higher predation. When considering years with varying production levels, high-production years favoured full and viable seeds, particularly in unharvested forests and aggregated retention, while low-production years saw reduced viability across all treatments. As the sole obligate symbiotic birds in Africa,oxpeckers offer a unique model for studying symbiotic *** to the multitrophic level they occupy and the context dependent foraging behavior they exhibit,the type of symbiotic relationship can be *** addition to providing a cleaning service to the host by removing ticks,oxpeckers frequently feed on blood,mucus,and saliva,inflicting potential damage on the ***,we used DNA metabarcoding on faecal samples to analyze the taxonomic composition of the trophic interactions of the Yellow-billed Oxpecker(Buphagus africanus)and Red-billed Oxpecker(***)in northeastern *** contrast to conventional methods,DNA metabarcoding allows for a detailed identification of dietary resources encompassing both mammal hosts and consumed arthropods within the same *** this information,we examined differences in the diet composition between oxpecker species and localities,as well as the co-occurrence between host and arthropod *** findings revealed that oxpeckers predominantly source their diet from mammals,ticks,and flies;however,ticks and flies rarely co-occur in the diet of an *** observed variability among individuals in their feeding ecology,which is strongly correlated with locality and,to a lesser extent,with the mammal *** noted a high degree of mobility between hosts within relatively short periods,with 32%of the samples showing traces of at least two mammal *** study illustrates the dynamic foraging behavior of these specialized symbiotic birds,shedding light on their potential role in pest control services and disease transmission.
