BACKGROUND Pituitary apoplexy represents one of the most serious,life threatening endocrine emergencies that requires immediate ***-releasing hormone agonist(GnRHa)can induce pituitary apoplexy in those patients who have insidious pituitary adenoma *** SUMMARY A 46-year-old woman,with a history of hypertension and menorrhagia was transferred to our hospital from a secondary care hospital after complaints of headache and vomiting,with loss of consciousness 5 min after an injection of *** drug was prescribed by her gynecologist due to the presence of uterine *** clinical neurological examination revealed right cranial nerve III palsy,ptosis and movement limitation of the right *** first clinical consideration was a pituitary *** hormonal analysis revealed mild hyperprolactinemia and high follicle stimulating hormone level;PTH and calcium was high with glomerular filtration rate mildly to moderately decrease.A computed tomography scan,revealed an enlarged pituitary gland(3.5 cm)impinging upon the optic chiasm with bone involvement of the *** contrast media administration,the lesion showed homogeneous enhancement with high-density focus that suggests hemorrhagic infarction of the *** endoscopic surgery was perfomed and adenomatous tissue was *** was positive for luteinizing hormone(LH)and follicular-stimulating hormone(FSH).A solid hypoechoic nodule(14 mm x 13 mm x 16 mm)was found in the caudal portion of the right thyroid lobe after a parathyroid ultrasound.A genetic test of Multiple Endocrine Neoplasia type 1(MEN1)was negative.A right lower parathyroidectomy was performed and the pathologic study showed the presence of an encapsulated parathyroid carcinoma of 1.5 cm.A MEN type 4 genetic test was performed result was *** This case demonstrates an uncommon complication of GnRH agonist therapy in the setting of a pituitary macroadenoma an

Recent technical advances in electrophysiological measurements, organelle-targeted fluorescence imaging, and organelle proteomics have pushed the research of ion transport a step forward in the case of the plant bioenergetic organelles, chloroplasts and mitochondria, leading to the molecular identification and functional characterization of several ion transport systems in recent years. Here we focus on channels that mediate relatively high-rate ion and water flux and summarize the current knowledge in this field, focusing on targeting mechanisms, proteomics, electrophysiology, and physiological function. In addition, since chloroplasts evolved from a cyanobacterial ancestor, we give an overview of the information available about cyanobacterial ion channels and discuss the evolutionary origin of chloroplast channels. The recent molecular identification of some of these ion channels allowed their physiological functions to be studied using genetically modified Arabidopsis plants and cyanobacteria. The view is emerging that alteration of chloroplast and mitochondrial ion homeostasis leads to organelle dysfunction, which in turn significantly affects the energy metabolism of the whole organism. Clear-cut identification of genes encoding for chan- nels in these organelles, however, remains a major challenge in this rapidly developing field. Multiple stra- tegies including bioinformatics, cell biology, electrophysiology, use of organelle-targeted ion-sensitive probes, genetics, and identification of signals eliciting specific ion fluxes across organelle membranes should provide a better understanding of the physiological role of organellar channels and their contribution to signaling pathways in plants in the future.

Effect of viscosity on the avalanche dynamics and flow transition of wet granular matter

The dynamic behaviour of granular flows is important in geo-mechanics and industrial applications,yet poorly *** studied the effects of liquid viscosity and particle size on the dynamics of wet granular material flowing in a slowly rotating drum,in order to detect the transition from the avalanching to the continuous flow regime.A discrete element method(DEM)model,in which contact forces and cohesive forces were considered,was employed to simulate this flow *** model was validated experimentally,using glass beads in a wooden drum and water–glycerol mixtures to tune the liquid *** DEM simulations showed comparable results to the experiments in terms of average slope angle and avalanche *** observed that the avalanche amplitude,flow layer velocity and granular temperature decrease as the liquid viscosity *** effect is more pronounced for smaller sized *** increase in viscous forces causes the flowing particles to behave as a bulk,pushing the free surface towards a convex *** addition,avalanches become less pronounced and the granular flow transitions from the avalanching regime to the continuous *** avalanching flow regime is marked by intermittent rigid body movement of the particulate bed and near-zero drops in the granular temperature,while no rigid body movement of the bed occurs in the continuous flow *** identified the avalanching-continuous flow transition region as a function of a dimensionless granular Galileo number.

Non-cycloplegic spherical equivalent refraction in adults:comparison of the double-pass system,retinoscopy,subjective refraction and a table-mounted autorefractor

AIM:To evaluate the accuracy of spherical equivalent(SE) estimates of a double-pass system and to compare it with retinoscopy,subjective refraction and a table mounted ***:Non-cycloplegic refraction was performed on 125 eyes of 65 healthy adults(age 23.5±3.0 years) from October 2010 to January 2011 using retinoscopy,subjective refraction,autorefraction(Auto kerato refractometer TOPCON KR-8100,Japan) and a double pass system(Optical Quality Analysis System,OQAS,Visiometrics S.L.,Spain).Nine consecutive measurements with the double-pass system were performed on a subgroup of 22 eyes to assess *** evaluate the trueness of the OQAS instrument,the SE laboratory bias between the double pass system and the other techniques was ***:The SE mean coefficient of repeatability obtained was *** correlations could be established between the OQAS and the SE obtained with retinoscopy(r=0.956,P<0.001),subjective refraction(r=0.955,P<0.001) and autorefraction(r=0.957,P<0.001).The differences in SE between the double-pass system and the other techniques were significant(P<0.001),but lacked clinical relevance except for retinoscopy;Retinoscopy gave more hyperopic values than the double-pass system-0.51±0.50D as well as the subjective refraction-0.23±0.50D;More myopic values were achieved by means of autorefraction 0.24±0.49D. CONCLUSION:The double-pass system provides accurate and reliable estimates of the SE that can be used for clinical *** technique can determine the correct focus position to assess the ocular optical ***,it has a relatively small measuring range in comparison with autorefractors(-8.00 to +5.00D),and requires prior information on the refractive state of the patient.

Human viral pathogens are pervasive in wastewater treatment center aerosols

Wastewater treatment center（WTC） workers may be vulnerable to diseases caused by viruses, such as the common cold, influenza and gastro-intestinal infections. Although there is a substantial body of literature characterizing the microbial community found in wastewater, only a few studies have characterized the viral component of WTC aerosols,despite the fact that most diseases affecting WTC workers are of viral origin and that some of these viruses are transmitted through the air. In this study, we evaluated in four WTCs the presence of 11 viral pathogens of particular concern in this milieu and used a metagenomic approach to characterize the total viral community in the air of one of those WTCs. The presence of viruses in aerosols in different locations of individual WTCs was evaluated and the results obtained with four commonly used air samplers were *** detected four of the eleven viruses tested, including human adenovirus（h Ad V）,rotavirus, hepatitis A virus（HAV） and Herpes Simplex virus type 1（HSV1）. The results of the metagenomic assay uncovered very few viral RNA sequences in WTC aerosols, however sequences from human DNA viruses were in much greater relative abundance.

An unusual case of renal calculi leading to myocardial infarction and cardiogenic shock

INTRODUCTION The presentation of cardiogenic shock （CS） is usually straightforward, and includes hypotension, absence of hypovolemia, and clinical signs of poor tissue perfusion such as oliguria, cyanosis, cool extremities and altered mentation. The most common etiology of CS is acute myocardial infarction, Here, we report a case of a 42-year-old male who presented with right flank pain, nausea and vomiting initially thought to be nephrolithiasis, which he had a history of but was ultimately diagnosed with CS due to a type II myocardial infarction. This case illustrates the importance of having a broad differential diagnosis especially when a patient＇s vital signs take a drastic turn since this patient initially resembled someone with nephrolithiasis.

The“genetic erosion”of the soil ecosystem

This paper takes into consideration the influence of human activities on the loss of pedodiversity in a Mediterranean area due to large scale *** particular it examines the quantitative and qualitative soil changes in a period of 53 years(from 1955 to 2008)evaluating the loss of soil diversity at soil subgroups level of the USDA Soil Taxonomy *** following indices were used:richness;Shannon’s diversity index;Simpson diversity index;Shannon’s evenness index;Simpson’s evenness *** this case study,considering what we observed in time,the human intervention in soil transformation could increase the diversity in the landscape in an initial phase,but forwarding by large scale farming the result is a huge loss of pedodiversity in time,as diversity indices remarkably have *** analysis enabled identification of disappeared soil types,with their unique history of *** our opinion this strongly reflects a sort of“genetic erosion”of the soil types,resulting in a substantial weakening of the whole pedo-ecosystem.

Temporal-spatial segregation among hummingbirds foraging on honeydew in a temperate forest in Mexico

Spatial and temporal variation in interactions between hummingbirds and plants have often been examined, and hummingbirds and insects are known to indirectly interact in networks of nectar plants. In a highland temperate forest in Hidalgo, Mexico some oak trees were heavily infested by honeydew-producing insects （family Margarodidae, tribe Xylococcini, genus Strigmacoccus） and the honeydew was consumed by hummingbirds. Here using survival analysis we investigate how the honey- dew produced by dense populations of these margarodids is temporally and spatially partitioned by hummingbirds. We also measured the availability and quality of honeydew exudates, and then we recorded the time until a bird visited and used such re- sources. Four hummingbird species consumed this resource （Atthis eloisa, Hylocharis leucotis, Colibri thalassinus and Eugenes fulgens）. Data from 294 hours of observation on seven focal trees suggested temporal and spatial segregation among visiting birds according to body size and territorial behavior during the most honeydew-limited time. Hummingbird species differed in the dally times they foraged, as well as in the location where honeydew-producing insects were visited on the trees. Temporal and spatial segregation among hummingbird species is interpreted as an adaptation to reduce the risk of aggressive encounters. This may facilitate multispecies coexistence and allow these birds to exploit honeydew more effectively
