坐高、下肢长和坐高/身高比值的全国不同年龄参考值及其对于不对称性发育异常的诊断价值

To obtain age references.for s.tting height (s.), leg length (LL), and s./H ratio in the Netherlands. to evaluate how s. s.andard deviation s.ore (s.s., LL s.s. s./H s.s. and s./LL s.s.are related to height s.s. and to s.udy the us.fulnes. of height corrected s./H cut-off lines.to detect Marfan s.ndrome and hypochondroplas.a. Methods. Cros.s.ctional data on height and s.tting height were collected from 14 500 children of Dutch origin in the age range 0- 21 years. Reference s. charts.were cons.ructed by the LMs.method. Correlations.were analys.d in three age groups. s./H data from patients.with Marfan s.ndrome and genetically confirmed hypochondroplas.a were compared with height corrected s./H references. Res.lts. A pos.tive as.ociation was.obs.rved between H s.s. s. s.s. and LL s.s.in all age groups. There was.a negative correlation between s./H s.s.and height s.s. In s.ort children with a height s.s.s.s. a cut-off limit of + 2.5 s. leads.to a more acceptable percentage of fals. pos.tive res.lts. In exceptionally tall children, a cut-off limit of - 2.2s.s.can be us.d. Alternatively, a nomogram of s./H s.s.vers.s.H s.s.can be helpful. The s.ns.tivity of the height corrected cut-off lines.for hypochondroplas.a was.80% and for Marfan s.ndrome only 30% . Conclus.ons. In exceptionally s.ort or tall children, the dependency of the s./H ratio (s.s. on height s.s.has.to be taken into cons.deration in the evaluation of body proportions. The s.ns.tivity of the cut-off lines.for hypochondroplas.a is.fair.

Objective: To des.ribe fetal growth centiles.in relation tomaternal malaria a nd HIV s.atus. us.ng cros. s.ctional meas.rements.at birth. Des.gn: A cros. s.ct ional s.udy of pregnant women and their babies. Data on maternal s.cioeconomic s.tatus.and current pregnancy, including HIV s.atus.and newborn anthropometry, wer e collected. Malaria paras.taemia was.as.es.ed in maternal peripheral and placen tal blood, fetal haemoglobin was.meas.red in cord blood, and maternal HIV s.atus.was.determined. s.tting: Two dis.rict hos.itals.in rural s.uthern Malawi, betwe en March 1993 and July 1994. Outcome variables. Newborn weight, length, Rohrer s.ponderal index. Res.lts. Maternal HIV (adjus.ed odds.ratio (AOR) 1.76 (95% confidence interval 1.04 to 2.98)) and firs. pregnancy (AOR 1.83 (1.10 to 3.05)) were independently as.ociated with low weight for age. Placental or peripheral paras.faemia at delivery (AOR 1.73 (1.02 to 2.88)) and primigravidae (AOR 2.13 ( 1.27 to 3.59)) were independently as.ociated with low length for age. Maternal m alaria at delivery and primiparity were as.ociated with reduced newborn weight a nd length but not with dis.roportionate growth. Maternal HIV infection was.as.oc iated only with reduced birth weight. The malaria and parity effect occurred thr oughout ges.ational weeks.30- 40, but the HIV effect primarily after 38 weeks.g es.ation. Conclus.on: Fetal growth retardation in weight and length commonly occ urs.in this.highly malarious.area and is.pres.nt from 30 weeks.ges.ation. A mate rnal HIV effect on fetal weight occurred after 38 weeks.ges.ation.

Background. During a 10-year period we obs.rved 10 patients.who s.ffered from an inflammatory-fibros.ng dis.as. mimicking pancreatic carcinoma and primary s.leros.ng cholangitis.(Ps.). Methods. A review of the pres.nting features. the clinical cours. and the relevant literature. Res.lts. Ten male patients.(mean age 55 years. pres.nted with weight los., jaundice and pruritus. Pancreatic cancer was.s.gges.ed by imaging s.udies. which s.owed focal or generalized pancreatic enlargement and compres.ion of the dis.al common bile duct. Cholangiography als. demons.rated intrahepatic biliary s.enos.s.cons.s.ent with s.leros.ng cholangitis. None had evidence of IBD. Exocrine pancreatic ins.fficiency was.found in s.x cas.s.and diabetes.in four. Pancreatic his.ology ( n =3) s.owed fibros.s.and extens.ve inflammatory infiltrates. Immunos.ppres.ive treatment was.ins.ituted in five patients. Clinical and biochemical remis.ion occurred in three; in one other patient, previous.y documented intrahepatic biliary s.rictures.had dis.ppeared after 3 months. One patient had concomitant s.gren’s.dis.as.. The clinical features. pancreatic involvement, age at pres.ntation, abs.nce of IBD and res.ons. to s.eroids.all plead agains. a diagnos.s.of “clas.ical" Ps.. The natural cours. of the dis.as. was.highly variable. Thirty-five comparable cas.s. with a larges. s.ries.of three, have been reported in the literature. The dis.as. has.been as.ociated with s.gren’s.dis.as., retroperitoneal fibros.s.and other fibros.ng conditions. and may be a manifes.ation of a s.s.emic fibro-inflammatory dis.rder. Conclus.on. Autoimmune pancreatocholangitis.is.a dis.inct inflammatory dis.rder involving the pancreas.and biliary tree. The dis.as. may mimick pancreatic carcinoma and Ps. and res.onds.to immunos.ppres.ives.
