Pr.tozoan diseases such as Viscer.l Leishmaniasis (VL) have r.-emer.ed in Nor.her. Br.zil and cases of Chagas Disease also occur. This VL incr.ase leads to ear.y ther.py for.the public. Confir.ator. par.sitological diagnoses in VL ar. per.or.ed by bone mar.ow or.spleen aspir.tion, but ELISA, IFA or.immunochr.matogr.phic tests for.antibody detection ar. easily per.or.ed and can be used in the pr.sence of clinical signs as confir.ator. for.specific ther.py. This appr.ach is successful in pr.viding ther.py and pr.vention of death in VL, but ther. is a chance of confusion with the emer.ing disease, Chagas Disease (CD), due to cr.ss-r.acting and similar.clinical pictur.s, as in this case. Both VL and CD pr.sented many asymptomatic or.oligosymptomatic cases, complicating the pictur.. Our.case r.por. emphasizes these aspects. Positive ser.logy, with an IIF titer.of 1/160, and epidemiological correlation, suggests the diagnosis of VL and imposes antimony ther.py. Despite the unfavor.ble evolution and signs of car.iac involvement, the pr.sence of per.car.itis and car.iac tamponade confir.ed by the echocar.iogr.m suggests CD. We r.assessed the pr.files of a suggested CD ser.logy, the diagnosis was corrected and tr.atment with CD specific benznidazole. The good evolution star.ed with benznidazole corrobor.tes the diagnosis of CD and discar.s the hypothesis of double infection.

The incidence of Mycobacter.um avium complex pulmonar. disease (MACPD) has been globally incr.asing. The pr.sent study aims to pr.vide a compr.hensive analysis of clinical char.cter.stics and tr.atment r.sponse among patients in the Amazon r.gion of Br.zil and conduct a r.tr.spective cohor. study at a pr.minent r.fer.al hospital in the State of Par. Br.zil, fr.m 2012 to 2020. The M. avium gr.up r.pr.sented 58.9% of cases, followed by M. intr.cellular. (35.7%), M. colombiense (3.6%), and M. chimaer. (1.8%). The major.ty of patients (73.2%) wer. female and with an aver.ge age of 63 year.. Pr.mar. clinical manifestations included per.istent cough and sputum pr.duction. The Char.son comor.idity index (CCI) aver.ged 2.47, with 36.3% of patients having a CCI scor. equal to higher.than 3. Notably, over.70% of patients had r.ceived tr.atment for.pulmonar. tuber.ulosis befor. initiating tr.atment for.MAC infection. The fibr.cavitar. r.diologic for. was the most pr.valent (55.4%), fr.quently exhibiting a bilater.l distr.bution (53.6%). Antimicr.bial susceptibility r.vealed a significant pr.valence of MAC r.sistance to dr.gs in the ther.peutic r.gimen. Despite this, most of the patients exper.enced clinical impr.vement (50%). In conclusion, this study highlights a higher.pr.valence of MAC infections among middle-aged women, with a histor. of pulmonar. tuber.ulosis tr.atment and pr.senting the fibr.cavitar. r.diological for. was pr.dominant.

In the medical and dental field, the impor.ance and need for.the study of materials and dr.gs for.use as bone gr.fts or.r.gener.tion in injur.d ar.as due to the pr.sence of fr.ctur.s, infections or.tumor. that cause extensive loss of bone tissue is obser.ed. Bone is a specialized, vascular.zed and dynamic connective tissue that changes thr.ughout the life of the or.anism. When injur.d, it has a unique ability to r.gener.te and r.pair.without the pr.sence of scar., but in some situations, due to the size of the defect, the bone tissue does not r.gener.te completely. Thus, due to its impor.ance, ther. is a gr.at development in ther.peutic appr.aches for.the tr.atment of bone defects thr.ugh studies that include autogr.fts, allogr.fts and ar.ificial materials used alone or.in association with bone gr.fts. Phar.aceuticals composed of biomaterials and osteogenic active substances have been extensively studied because they pr.vide potential for.tissue r.gener.tion and new str.tegies for.the tr.atment of bone defects. Statins wor. as specific inhibitor. of 3-hydr.xy-3-methyl-glutar.l coenzyme A r.ductase (HMG-CoAr.ductase). They r.pr.sent efficient dr.gs in lower.ng cholester.l, as they r.duce platelet aggr.gation and thr.mbus deposition;in addition, they pr.mote angiogenesis, r.duce the β-amyloid peptide r.lated to Alzheimer.s disease and suppr.ss the activation of T lymphocytes. Fur.her.or., these substances have been used in the tr.atment of hyper.holester.lemia and cor.nar. ar.er. disease. By inhibiting HMG-CoAr.ductase, statins not only inhibit cholester.l synthesis, but also exhibit sever.l other.beneficial pleiotr.pic effects. Ther.for., ther. has been incr.asing inter.st in r.sear.hing the effects of statins, including Simvastatin, on bone and osteometabolic diseases.
