The Inter- and Intraindividual Anatomical Relationship of the Femoral Anteversion and Distal Femoral Rotation. A Cadaveric Study on the Femoral Anteversion Angle, Posterior and Inferior Condylar Angle Using Computed Tomography

malrotation following Total Knee Arthroplasty (TKA) is directly related to poor outcome. Knowledge of the rotational axes (torsion) and angles is therefore important. The aim of the study was to determine whether an association existed between the Femoral Anteversion Angle (FAA) Posterior Condylar Angle (PCA) and the Inferior Condylar Angle (ICA) in individuals. A CT scan of 50 (25 paired) cadaver femora was made. The FAA, PCA and ICA were measured. Statistical analysis of comparative relationships between these different angles was examined by calculating Pearson correlation coefficients and a paired t-test. The mean FAA, PCA and ICA for the whole group were respectively 11.7° (range 0 - 32, SD 8.2), 5.18° (range 0 - 12, SD 2.4) and 4.4° (range 0 - 10, SD 2.1). A correlation of 0.82 (p = 0.01) of the FAA was found between left versus right. For the overall group a correlation coefficient between the PCA of the left and right femur was r = 0.59, p = 0.01. The Pearson correlation between the FAA and PCA in the whole group was r = 0.27, p = 0.06. In females this was r = 0.54 (p = 0.03). Although the difference of the mean ICA and PCA was very small (0.7°), there was no correlation between these angles (r = 0.14, p = 0.23). In conclusion, one should be aware that, considering the weak correlation of the FAA and PCA, an individual rotational variation exists. Furthermore, no correlation was found between the PCA and ICA. Therefore, for now, this angle cannot be assumed to be helpful in TKA. A more individual approach in total knee arthroplasty seems essential for future TKA.

Background Pediatric post coronavirus disease 2019(COVID-19)condition(PPCC)is a heterogeneous syndrome,which can significantly affect the daily lives of *** study aimed to identify clinically meaningful phenotypes in children with PPCC,to better characterize and treat this *** Participants were children with physician-diagnosed PPCC,referred to the academic hospital Amsterdam UmC in the Netherlands between November 2021 and march *** factors and information on post-COVID symptoms,comorbidities,and impact on daily life were *** clusters were identified using an unsupervised and unbiased approach for mixed data *** Analysis of 111 patients(aged 3–18 years)revealed three distinct clusters within *** 1(n=62,median age=15 years)predominantly consisted of girls(74.2%).These patients suffered relatively more from exercise intolerance,dyspnea,and smell *** 2(n=33,median age=13 years)contained patients with an even gender distribution(51.5%girls).They suffered from relatively more sleep problems,memory loss,gastrointestinal symptoms,and *** 3(n=16,median age=11 years)had a higher proportion of boys(75.0%),suffered relatively more from fever,had significantly fewer symptoms(median of 5 symptoms compared to 8 and 10 for clusters 1 and 2 respectively),and experienced a lower impact on daily *** This study identified three distinct clinical PPCC phenotypes,with variations in sex,age,symptom patterns,and impact on daily *** findings highlight the need for further research to understand the potentially diverse underlying mechanisms contributing to post-COVID symptoms in children.

market Knowledge and Stakeholder Considerations for the Biopharmaceutics Sector—Incorporating User Value and Societal Needs in Therapeutic Interventions

Purpose: many health systems are considered ill-equipped to adequately meet future demand. Although academic entrepreneurs and small and medium-sized enterprises (SmEs) drive health technology innovation, they often suboptimally consider important market and stakeholder requirements. This can result in missed opportunities to improve health system performance, address demand-supply gaps, and achieve financial rewards. market intelligence can provide crucial information in this regard, but scientifically sourced guidance on how to perform market research for biopharmaceutics is scarce. Our objective was, therefore, to create and provide an easily accessible biopharmaceutics market research toolbox that can be perused by academic entrepreneurs and SmEs to improve the alignment of their developments with stakeholder needs. Design: Guided by market orientation theory, we interviewed senior managers and commercialisation experts from the biopharmaceutics sector. We asked them which information they consider most relevant, and how they collect, analyse and respond to new information to align product features, positioning and stakeholder-relationship management with market needs. Findings: Our innovator toolbox consists of a high-level overview of market intelligence considerations mapped against a novel innovation framework, a stakeholder reasoning and value model, and detailed considerations (checklists) per stakeholder of biopharmaceutics interventions. Originality: We provide here the first market orientation-based overview of market intelligence considerations for the biopharmaceutics sector. Although primarily collated for academic entrepreneurs and SmEs, our innovator toolbox for the biopharmaceutics sector can help all stakeholders better understand their orientation towards markets.

Defective recovery of QT dispersion following transcatheter aortic valve implantation： frequency, predictors and prognosis

Background Corrected QT dispersion （cQTD） has been correlated with non-uniform ventricular repolarisation and increased mortality. In patients with aortic stenosis, cQTD has been shown improved after surgical valve replacement, but the effects of transcatheter aortic valve implantation （TAVI） are unknown. Therefore, we sought to explore the frequency, predictors and prognostic effects of defective cQTD recovery at 6 months after TAVI. methods A total of 222 patients underwent TAVI with the medtronic-CoreValve System between November 2005 and January 2012. Patients who were on class Ⅰ or Ⅲ antiarrhythmics or on chronic haemodialysis or who developed atrial fibrillation, a new bundle branch block or became pacemaker dependent after TAVI were excluded. As a result, pre-, post- and follow-up ECG （median： 6 months） analysis was available in 45 eligible patients. Defective cQTD recovery was defined as any progression beyond the baseline cQTD at 6 months. Results In the 45 patients, the mean cQTD was 47 ± 23 ms at baseline, 45 ±17 ms immediately after TAVI and 40 ± 16 ms at 6 months （15% reduction, P = 0.049）. Compared to baseline, cQTD at 6 months was improved in 60% of the patients whereas defective cQTD recovery was present in 40%. cQTD increase immediately after TAVI was an independent predictor of defective cQTD recovery at 6 months （per 10 ms increase; OR： 1.89, 95% CI： 1.15-3.12）. By univariable analysis, defective cQTD recovery was associated with late mortality （HR： 1.52, 95% CI： 1.05-2.17）. Conclusions Despite a gradual reduction ofcQTD after TAVI, 40% of the patients had defective recovery at 6 months which was associated with late mortality. more detailed ECG analysis after TAVI may help to avoid late death.

Waternet is the first water cycle company in the Netherlands, responsible for drinking water treatment and distribution, wastewater collection and treatment, and water system management and control in and around Amsterdam. Waternet started on 1 January 2006 to overcome the disadvantages of the rather fragmented organisation of the Dutch public water sector. The water cycle concept offers better opportunities to meet the future challenges in the water sector. The first four years of operation of Waternet show very promising results with respect to efficiency, customer orientation and quality improvement. To further develop the water cycle concept and to find sustainable solutions for the challenges the water sector is confronted with, Waternet focuses on six strategic innovation areas the coming years： water resources and water system; closing the water cycle; sustainability; new sanitation concepts; new products and services; management innovations.

Does magnesium sulfate increase the incidence of postpartum hemorrhage? A systematic review

The incidence of Postpartum Hemorrhage (PPH) is increasing in the western world. We hypothesize that magnesium sulfate (mgSO4) could be a contributing factor. mgSO4 might increase the incidence of PPH by induction of vasodilation, tocolytic effects, and effects on the blood like red cell deformity, platelet activity inhibition and a prolonged bleeding time. Based on these effects of mgSO4 a correlation with PPH is suspected. mgSO4 is widely used in the prevention of eclampsia. However, the working mechanism of this effective drug is largely unknown. We performed a systematic search to find all Randomized Controlled trials (RCTs) containing mgSO4 in preeclamsia as well as all mgSO4 studies with information on PPH. Titles, abstracts and references of publications were evaluated for appropriateness and whether they met the inclusion criteria. RCTs about mgSO4 with original data on PPH prevalence were included in our systematic review. We calculated the relative risk of PPH in every study as well as an overall relative risk. Four relevant and valid RCTs were found, totalling 11,621 relevant patients. The relative risk of PPH in women treated with mgSO4 is 0.964 (95% CI 0.886 - 1.050) In this systematic review we found no significant increase in PPH in women treated with mgSO4. However, there is still room for discussion due to the heterogeneity in methods (dosage and duration of treatment), results, and tertiary outcomes, as well as the small number of studies found with respect to this important issue.

分别以TiH2,ZrH2为原料,结合原位反应与脉冲电流辅助烧结制备了TiB2-SiC及ZrB2-SiC复相陶瓷。研究发现,所制备的复相陶瓷表现出一定的织构化现象,TiB2及ZrB2晶粒在反应烧结过程中其(001)面沿垂直压力和电流方向生长。金属氢化物粉体的粒径大小对复相陶瓷的致密化及微结构有较大影响:粒径越小越有利于陶瓷的致密化和硼化物晶粒的定向生长。由于金属硼化物的定向,复相陶瓷的机械性能表现出各向异性。TiB2-SiC复相陶瓷具有较高的断裂韧性,最高可达7.3 mPa.m1/2,而ZrB2-SiC复相陶瓷具有更高的抗弯强度(937 mPa)。
