Dear Editor,We report the case of severe mixed-mechanism glaucoma observed in a monocular Axenfeld-Rieger syndrome(ARS) patient who was successfully managed with an ahmed glaucoma implant(AGI) followed by a novel technique described as augmented Micro Pulse transscleral cyclophotocoagulation(MP-TSCPC)[1]. ARS encompasses a heterogeneous spectrum of diseases involving anterior segment dysgenesis and systemic developmental anomalies[2].

We applied a multi-taxa approach integrating the co-occurrence of plants,ground beetles,spiders and springtails with soil parameters(temperatures and chemical characteristics)in order to describe the primary succession along two glacier forelands in the Maritime Alps(Italy),a hotspot of Mediterranean *** compared these successions to those from Central Alps:Maritime glacier forelands markedly differ for their higher values of species richness and species *** to our expectation,Maritime glacier forelands follow a‘replacement change model’,like continental succession of Inner Alps and differently from other peripheral *** propose that the temperatures along these Mediterranean glacier forelands are warmer than those along other Alpine glacier forelands,which promote the faster species ***,we found that early and mid successional stages of the investigated glaciers are richer in cold-adapted and endemic species than the later ones:we confirmed that the‘replacement change’model disadvantages pioneer,cold-adapted *** the overall correspondence among coldadapted and endemic species,the most threatened in this climate phase,our results raise new concerns about the extinction risk of these *** also describe supraglacial habitat of Maritime glaciers demonstrating that supraglacial debris represents an environment decoupled from the regional climate and may have an important role as refugium for coldadapted and hygrophilous plant and animal species,whose survival can be threatened by climate change and by a rapid ecological succession in the adjacent forelands.

The engineering of self-organized plasmonic metasurfaces is demonstrated using a maskless technique with defocused ion-beam sputtering and kinetically controlled deposition. The proposed reliable, cost-effective, and controllable approach enables large-area （order of square centimeter） sub-wavelength periodic patterning with close-packed gold nanostrips. A multi-level variant of the method leads to high-resolution manufacturing of vertically stacked nanostrip dimer arrays, without resorting to lithographic approaches. The design of these self-organized metasurfaces is optimized by employing plasmon hybridization methods. In particular, preliminary results on the so-called gap-plasmon configuration of the nanostrip dimers, implementing magnetic dipole resonance in the near-infrared range, are reported. This resonance offers a superior sensitivity and field enhancement, compared with the more conventional electric dipole resonance. The translational invariance of the nanostrip configuration leads to a high filling factor of the hot spots. These advanced features make the large-area metasurface based on gap-plasmon nanostrip dimers very attractive for surface-enhanced linear and nonlinear spectroscopy （e.g., surface-enhanced Raman scattering） and plasmon-enhanced photon harvesting in solar and photovoltaic cells.

We introduce a technique for increasing density in optical data storage systems. This technique is based on the use of a superresolving filter at the pupil of a confocal readout system. The main characteristic of this confocal readout system is that the light beam traverses twice through the pupil filter. We describe how to analyze the system performance for general filters, but we focus the study on filters with no focus displacement. Although the storage density attainable depends on the filter characteristics, we show that the storage density can be easily duplicated.

Optimal length of triple therapy for H pylori eradication in a population with high prevalence of infection in Chile

AIM:To compare the efficacy of 7-d versus 14-d triple therapy for the treatment of H pylori infection in Chile,with a prevalence of 73% in general population. METHODS:H pylori-infected patients diagnosed by rapid urease test,with non-ulcer dyspepsia or peptic ulcer disease were randomized to receive omeprazole 20 mg bid,amoxicillin 1 g bid,and clarithromycin 500 mg bid for 7 (OAC7) or 14 (OAC14) d. Primary outcome was eradication rate 6 wk after the treatment. Subgroup analysis was carried out considering the eradication rate among patients with or without peptic ulcer disease and eradication rate among smokers or non-smokers. RESULTS:One hundred and thirty-one patients were randomized to OAC7 (n = 69) or OAC14 (n = 62). The overall eradication rate (intention-to-treat) was 78.3% in OAC7 and 85.5% in OAC14 groups,without a significant difference (P =0.37). No significant difference in the eradication rate was found among the patients with peptic ulcer disease (n = 31) between the OAC7 group (85.7%) and OAC14 group (87.5%). However,smokers had an obviously lower eradication rate compared to non-smokers,particularly in the OAC7 group (57.1% in smokers vs 83.6% in non-smokers; P = 0.06). Adverse effects rate were similar between both groups. CONCLUSION:Short-term efficacy of triple therapy with OAC for 7 d is comparable to 14 d in this high-prevalence population. Longer follow-up,and studies focused to some subgroups of patients (smokers and non-ulcerpatients) are necessary to support widespread use of 7-d instead of 10-14-d triple therapy in a developing country like Chile.

To validate prognostic scores for acute decompensation of cirrhosis and acute-on-chronic liver failure in Brazilian *** is a prospective cohort study designed to assess the prognostic performance of the chronic liver failure-consortium (CLIF-C) acute decompensation score (CLIF-C AD) and CLIF-C acute-on-chronic liver failure score (CLIF-C ACLF), regarding 28-d and 90-d mortality, as well as to compare them to other prognostic models, such as Model for End-Stage Liver Disease (MELD), MELD Sodium (MELD-Na), Child-Pugh (CP) score, and the CLIF-C Organ Failure score (CLIF-C OF). All participants were adults with acute decompensation of cirrhosis admitted to the Emergency Department of a tertiary hospital in southern Brazil. Prognostic performances were evaluated by means of the receiver operating characteristic (ROC) curves, area under the curves (AUC) and 95%*** hundred and thirteen cirrhotic patients were included. At admission, 18 patients had acute-on-chronic liver failure (ACLF) and 95 individuals had acute decompensation (AD) without ACLF, of which 24 eventually developed ACLF during the course of hospitalization (AD evolving to ACLF group). The AD group had significantly lower 28-d (9.0%) and 90-d (18.3%) mortality as compared to the AD evolving to ACLF group and to the ACLF group (both P < 0.001). On the other hand, 28-d and 90-d mortalities were not significantly different between AD evolving to ACLF group and ACLF group (P = 0.542 and P = 0.708, respectively). Among patients with ACLF, at 28 d from the diagnosis, CLIF-C ACLF was the only score able to predict mortality significantly better than the reference line, with an AUC (95%CI) of 0.71 (95%CI: 0.54-0.88, P = 0.021). Among patients with AD, all prognostic scores performed significantly better than the reference line regarding 28-d mortality, presenting with similar AUCs: CLIF-C AD score 0.75 (95%CI: 0.63-0.88), CP score 0.72 (95%CI: 0.59-0.85), MELD score 0.75 (95%CI: 0.61-0.90),

AIM: To determine the impact (morbidity/mortality) of biliary stent-related events (SRE) (cholangitis or stent obstruction) in chemotherapy-treated pancreatico-biliary ***: All consecutive patients with advanced pancreatobiliary cancer and a biliary stent in-situ prior to starting palliative chemotherapy were identified retrospectively from local electronic case-note records (Jan 13 to Jan 15). The primary end-point was SRE rate and the time-to-SRE (defined as time from first stenting before chemotherapy to date of SRE). Progression-free survival and overall survival were measured from the time of starting chemotherapy. Kaplan-Meier, Cox and Fine-Gray regression (univariate and multivariable) analyses were employed, as appropriate. For the analysis of time-to-SRE, death was considered as a competing ***: Ninety-six out of 693 screened patients were eligible; 89% had a metal stent (the remainder were plastic). The median time of follow-up was 9.6 mo (range 2.2 to 26.4). Forty-one patients (43%) developed a SRE during follow-up [cholangitis (39%), stent obstruction (29%), both (32%)]. There were no significant differences in baseline characteristics between the SRE group and no-SRE groups. Recorded SRE-consequences were: none (37%), chemotherapy delay (24%), discontinuation (17%) and death (22%). The median time-to-SRE was 4.4 mo (95%CI: 3.6-5.5). Patients with severe comorbidities (P < 0.001) and patients with ≥ 2 baseline stents/biliary procedures [HR = 2.3 (95%CI: 1.2-4.44), P = 0.010] had a shorter time-to-SRE on multivariable analysis. Stage was an independent prognostic factor for overall survival (P = 0.029) in the multivariable analysis adjusted for primary tumour site, performance status and development of SRE (SRE group vs no-SRE group).CONCLUSION: SREs are common and impact on patient’s morbidity. Spontaneous bacterial peritonitis(SBP) is the most typical infection observed in cirrhosis patients. SBP is responsible for an in-hospital mortality rate of approximately 32%. Recently, pattern changes in the bacterial flora of cirrhosis patients have been observed, and an increase in the prevalence of infections caused by multi-resistant bacteria has been noted. The wide-scale use of quinolones in the prophylaxis of SBP has promoted flora modifications and resulted in the development of bacterial resistance. The efficacy of traditionally recommended therapy has been low in nosocomial infections(up to 40%), and multi-resistance has been observed in up to 22% of isolated germs in nosocomial SBP. For this reason, the use of a broad empirical spectrum antibiotic has been suggested in these situations. The distinction between community-acquired infectious episodes, healthcare-associated infections, or nosocomial infections, and the identification of risk factors for multi-resistant germs can aid in the decision-making process regarding the empirical choice of antibiotic therapy. Broad-spectrum antimicrobial agents, such as carbapenems with or without glycopeptides or piperacillin-tazobactam, should be considered for the initial treatment not only of nosocomial infections but also of healthcare-associated infections when the risk factors or severity signs for multi-resistant bacteria are apparent. The use of cephalosporins should be restricted to community-acquired infections.

Evaluating the potential of(epi)genotype‑by‑low pass nanopore sequencing in dairy cattle:a study on direct genomic value and methylation analysis

Background Genotype-by-sequencing has been proposed as an alternative to SNP genotyping arrays in genomic selection to obtain a high density of markers along the *** requires a low sequencing depth to be cost effective,which may increase the error at the genotype *** generation nanopore sequencing technology offers low cost sequencing and the possibility to detect genome methylation,which provides added value to *** aim of this study was to evaluate the performance of genotype-by-low pass nanopore sequencing for estimating the direct genomic value in dairy cattle,and the possibility to obtain methylation marks *** Latest nanopore chemistry(LSK14 and Q20)achieved a modal base calling accuracy of 99.55%,whereas previous kit(LSK109)achieved slightly lower accuracy(99.1%).The direct genomic value accuracy from genotype-by-low pass sequencing ranged between 0.79 and 0.99,depending on the trait(milk,fat or protein yield),with a sequencing depth as low as 2×and using the latest chemistry(LSK114).Lower sequencing depth led to biased estimates,yet with high rank *** LSK109 and Q20 achieved lower accuracies(0.57-0.93).More than one million high reliable methylated sites were obtained,even at low sequencing depth,located mainly in distal intergenic(87%)and promoter(5%)*** This study showed that the latest nanopore technology in useful in a LowPass sequencing framework to estimate direct genomic values with high *** may provide advantages in populations with no available SNP chip,or when a large density of markers with a wide range of allele frequencies is *** addition,low pass sequencing provided nucleotide methylation status of>1 million nucleotides at≥10×,which is an added value for epigenetic studies.
