Iatrogenic perforation is the most feared adverse event in *** the expansion of interventional endoscopy in favor of traditional surgery,it is now more crucial than ever to develop effective defect closure *** has culminated in the dissemination of multiple novel closure technologies,including through-the-scope clips,over-the-scope clips,through-the-scope suturing and over-the-scope suturing *** this editorial,we comment on the recent publication by Wang and colleagues discussing the performance of the double-nylon purse-string suture technique in the closure of large(>3 cm)gastric full-thickness *** technique offers a promising,practical and cost-effective approach to closure of large full-thickness defects that can be readily imple-mented across diverse healthcare settings.

Chronic pancreatitis is a chronic fibro-inflammatory disorder of the pancreas,resulting in recurrent abdominal pain,diabetes mellitus,and *** may lead to various other complications such as pseudocyst formation,benign biliary stricture,gastric outlet obstruction;and vascular complications like venous thrombosis,variceal and pseudoaneurysmal *** of varices is usually due to chronic venous thrombosis with collateral formation and variceal bleeding can easily be tackled by endoscopic *** bleed can be catastrophic and requires radiological interventions including digital subtraction angiography followed by endovascular obliteration,or sometimes with a percutaneous or an endoscopic ultrasound-guided approach in technically difficult ***-related bleed is usually venous and mostly managed ***-related arterial bleed,however,may require radiological interventions.

At present days,object detection and tracking concepts have gained more importance among researchers and business ***,deep learning(DL)approaches have been used for object tracking as it increases the perfor-mance and speed of the tracking *** paper presents a novel robust DL based object detection and tracking algorithm using Automated Image Anno-tation with ResNet based Faster regional convolutional neural network(R-CNN)named(AIA-FRCNN)*** AIA-RFRCNN method performs image anno-tation using a Discriminative Correlation Filter(DCF)with Channel and Spatial Reliability tracker(CSR)called DCF-CSRT *** AIA-RFRCNN model makes use of Faster RCNN as an object detector and tracker,which involves region proposal network(RPN)and Fast *** RPN is a full convolution network that concurrently predicts the bounding box and score of different *** RPN is a trained model used for the generation of the high-quality region proposals,which are utilized by Fast R-CNN for detection ***,Residual Network(ResNet 101)model is used as a shared convolutional neural network(CNN)for the generation of feature *** performance of the ResNet 101 model is further improved by the use of Adam optimizer,which tunes the hyperparameters namely learning rate,batch size,momentum,and weight ***,softmax layer is applied to classify the *** performance of the AIA-RFRCNN method has been assessed using a benchmark dataset and a detailed comparative analysis of the results takes *** outcome of the experiments indicated the superior characteristics of the AIA-RFRCNN model under diverse aspects.

The treatment of choice for patients with severe alcoholic hepatitis (AH) is use of *** randomized well designed studies have been reported from all over the world on the use of corticosteroids in the treatment of ***,the data on the efficacy of corticosteroids in these patients have been *** meta-analyses also failed to show beneficial effects of *** on individual data meta-analysis showing clear benefit of corticosteroids amongst patients with severe AH (modified discriminant function of 32 or more),led American College of Gastroenterology to recommend use of corticosteroids as the first line treatment option amongst patients with severe ***,corticosteroids are relatively contraindicated amongst patients with severe AH and coexistent sepsis,gastrointestinal bleeding,and acute *** patients may be candidates for second line treatment with ***,specific treatment of AH with corticosteroids far from satisfactory with as many as 40%-50% of patients failing to respond to steroids,thus classified as nonresponsive to *** management of these patients is a continuing challenge for *** treatment modalities need to be developed for this group of patients in order to improve the outcome of patients with severe *** article describes at length the available trials on use of corticosteroids and pentoxifylline with their current *** of administration,dosage,adverse effects,and mechanisms of action of these two drugs are also ***,an algorithm with clinical approach to management of patients who present with clinical syndrome of AH is described.

Coronavirus disease 2019(COvID-19)has affected patients with pre-existing chronic liver disease(CLD)in various *** maximum impact was seen on patients with underlying cirrhosis who have shown to have poor clinical outcomes in the form of increased risk of hepatic decompensation,acute-onchronic liver failure,and even *** is of paramount importance to identify various factors which are associated with unfavorable outcomes for prognostication and making informed management *** factors have been evaluated in different studies in patients with underlying *** of these factors include the severity of underlying chronic liver disease,comorbid conditions,age,and severity of ***,the outcomes are not favorable in patients with cirrhosis as evidenced by data from various *** main purpose of this review is to identify the predictors of adverse clinical outcomes including mortality in patients with CLD for risk stratification,prognostication,and appropriate clinical management.
