India has approximately 73 million people living with diabetes and another 37 million with prediabetes while nearly 47% of the diabetes cases are undiagnosed. The high burden of poor glycemic control and early onset of complications with associated economic costs indicates a high prevalence of poor self-management practices. It is well-established that achieving patient-centered primary care consistent with a chronic care model ensures optimum diabetes self-management support and improves long-term clinical and health outcomes in diabetes patients. The public sector primary care system in India provides services free of cost to beneficiaries but lacks patient-centered care that undermines diabetes selfmanagement education and support. Furthermore, factors like poor patient knowledge of diabetes, suboptimal medication adherence, persistent clinical inertia, lack of data for monitoring and evaluation through clinical audit worsens the standards of diabetes care in primary care settings of India. There is a need for government initiatives to be directed towards the provision of comprehensive outpatient care that is inclusive of uninterrupted supply of drugs, provision of essential laboratory investigators, training and availability of qualified diabetes educators and availability of specialist support when required. Furthermore, the integration of depression screening and smoking cessation services at the primary care level is warranted.

A Discrete Cosine Adaptive Harmonic Wavelet Packet and Its Application to Signal Compression

A new adaptive Packet algorithm based on Discrete Cosine harmonic wavelet transform (DCHWT), (DCAHWP) has been proposed. This is realized by the Discrete Cosine Harmonic Wavelet transform (DCHTWT) which exploits the good properties of DCT viz., energy compaction (low leakage), frequency resolution and computational simplicity due its real nature, compared to those of DFT and its harmonic wavelet version. Hence the proposed wavelet packet is advantageous both in terms of performance and computational efficiency compared to those of existing DFT harmonic wavelet packet. Further, the new DCAHWP also enjoys the desirable properties of a Harmonic wavelet transform over the time domain WT, viz., built in decimation without any explicit antialiasing filtering and easy interpolation by mere concatenation of different scales in frequency (DCT) domain with out any image rejection filter and with out laborious delay compensation required. Further, the compression by the proposed DCAHWP is much better compared to that by adaptive WP based on Daubechies-2 wavelet (DBAWP). For a compression factor (CF) of 1/8, the ratio of the percentage error energy by proposed DCAHWP to that by DBAWP is about 1/8 and 1/5 for considered 1-D signal and speech signal, respectively. Its compression performance is better than that of DCHWT, both for 1-D and 2-D signals. The improvement is more significant for signals with abrupt changes or images with rapid variations (textures). For compression factor of 1/8, the ratio of the percentage error energy by DCAHWP to that by DCHWT, is about 1/3 and 1/2, for the considered 1-D signal and speech signal, respectively. This factor for an image considered is 2/3 and in particular for a textural image it is 1/5.

AIM:To perform a systematic review of literature on use of cardiovascular imaging in assessment of donor ***: A systematic search of current literature from January 1965 to August 2015 was performed using Pub Med and Google Scholar to investigate the different imaging modalities used to assess donor ***: Recent literature still estimates only a 32% utilization of available donor hearts in the United States. Most common imaging modality used is transthoracic echocardiography. Use of advanced imaging modalities such as 3D echocardiography, cardiac computer tomography and cardiac magnetic resonance to evaluate donor hearts is not reported in literature. This review attempts to highlight the relevant imaging modalities that can be used to assess cardiac function in a time-efficient manner. The algorithm suggested in this review would hopefully pave the way to standardized protocols that can be adopted by organ procuring organizations to increase the donor ***: Use of advanced imaging techniques for a thorough assessment of organs will likely increase the donor pool.
