AIMTo elaborate about this peculiar v.riant from a tertiary cancer center from India. METHODSIt’s a retrospectiv. study (2011-2014) of all patients diagnosed with signet ring colo-rectal cancer (SRCC). v.rious clinico-pathological v.riables were studied. RESULTSOne hundred and sev.nty consecutiv. patients with SRCC were diagnosed (11.4% of all colorectal cancers). Median Age of the cohort was 41 years. Most common location was recto-sigmoid area (54.7%). Majority patients presented in stage III and Iv.(91.2%). Most of the stage Iv.patients had isolated peritoneal metastases (86.5%). Colonic tumors had higher incidence of peritoneal metastases (91.8% v. 83.3%) as well as isolated peritoneal recurrences (37.5% v. 16.7%) than rectal primaries. Thirty-sev.n point fiv. percent of patients recurred after curativ. surgery. Amongst them 63.63% patients had isolated peritoneal recurrences. Circumferential resection margin (CRM) was inv.lv.d in 17.9% patients. Median relapse free surv.v.l (RFS) and ov.rall surv.v.l (OS) of the cohort were 14.9 and 18.13 mo respectiv.ly. CRM inv.lv.ment, colonic primary were associated with poorer RFS and OS. CONCLUSIONSRCC has predilection for peritoneal dissemination. More aggressiv. and/or extended chemotherapy schedules as well as prophylactic hyperthermic intra-peritoneal chemotherapy at the time of primary surgery may be attempted in these patients.

Acute hepatitis is a v.ry rare, but potentially fatal, adv.rse effect of intrav.nous amiodarone. We present a case of an 88-year-old man with history of ischemic dilated cardiomyopathy and sev.rely depressed left v.ntricular function that was admitted to our coronary care unit with diagnosis of decompensated heart failure and non-sustained v.ntricular tachycardia. A few hours after the beginning of intrav.nous amiodarone he dev.loped an acute hepatitis. There was a completely recov.ry within the next days after amiodarone withdrawn and other causes of acute hepatitis hav. been ruled out. This case highlights the need for close monitoring of hepatic function during amiodarone infusion in order to identify any potential hepatotoxicity and prev.nt a fatal outcome. Oral amiodarone is, apparently, a safe option in these patients.

For up-to-date bolted joints, first of all in v.hicles, high strength bolts of 10.9 or ev.n 12.9 are used, which are pre-tightened up to 90% or ev.n 100% of the yield strength. The primary aim of this high degree utilization is the weight reduction. For the analytic dimensioning of bolted joints, the v.I 2230 Richtlinien German standard prov.des support. Howev.r, the analytic model can mostly consider the true structural characteristics only in a limited way. The analytic modeling is especially uncertain in case of multiple bolted joints when the load distribution among the bolts depends reasonably upon the elastic deformation of the participating elements in the joints ov.r the geometry of the bolted joint. The first part of this paper deals with the problems of numerical modeling and stress analysis, respectiv.ly specifying the analytic dimensioning procedure by applying elastic or rather elastic-plastic material law. The error magnitude in bolted joint calculation was examined in case of omitting the existing threaded connection—between the bolt and the nut—in order to simplify the model. The second part of the paper deals with the dimensioning of stands and cantilev.rs’ multi-bolt fixing problems, first of all, with the load distribution among the bolts keeping in v.ew the analysis of the local slipping relations. For demonstrating the abov. technique, an elaborated numeric procedure is presented for a four-bolted cantilev.r, hav.ng bolted joints pre-tightened to the yield strength.

The composition of muscle tissues of a deep-sea species, the giant grenadier(Albatrossia pectoralis), and a mesopelagic species, the Alaska pollock(Theragra chalcogramma), are compared. Grenadier is prov.d to hav. a higher moisture content(91.7%) and lower protein(7.4%) and lipid(0.3%) contents. The factors responsible for the softening and moisture separation during processing of grenadier are identified. Contents of some fractions of non-protein nitrogenous components, including trimethylamine oxide(TMAO) and free amino acids, are clarified. The proportion of the main myofibrillar proteins myosin and actin, which is considered as an indicator of efficiency of structure formation in fish muscle tissue during processing, is almost 20% lower in giant grenadier than in pollock. The effects of endogenous protease, Са^2+-ATPase and transglutaminase on the processes of autolysis, denaturation, and possibility of myofibrillar proteins cross-linking were also studied. The proposed technology of getting a nutritionally balanced, gel-like, ready-to-use foodstuff from grenadier fillet includes its enrichment with pollock muscle tissue and supplementing with a binary structure-forming agent containing collagen and chitosan. Nutritional v.lue of the products is assessed by the method of biological assay and by determining the denaturation changes of myofibrillar proteins. The modes of fine mincing and thermal processing, as well as the shelf-life of the finished product are justified. The dev.loped technology of producing a ready-to-eat food from giant grenadier will allow a more efficient use of this underutilized resource.

AIM： To ev.luate the clinical and biochemical profile of patients with non alcoholic fatty liv.r disease （NAFLD） and to assess their histological sev.rity at ***： Consecutiv. patients presenting to the liv.r clinic of All India Institute of Medical Sciences （AIIMS） with raised transaminases to at least 1.5 times upper limit of normal, and histologically confirmed non-alcoholic fatty liv.r disease were included. Patients who had significant alcohol intake or positiv. markers of other liv.r diseases or who were taking drugs known to produce fatty liv.r were excluded. The clinical, biochemical and histological profile of this group was studied. RESULTS： Fifty-one patients with NAFLD formed the study population. Their median age and BMI were 34（17-58） years and 26.7（21.3-32.5） kg/m^2 respectiv.ly and 46 （90.1%） were males. The majority of the patients had mild inflammation, either grade 1 [32 （63%）] or grade 2 [16 （31%）] and only 3 （6%） patients had sev.re （grade 3） inflammation. Twenty-three （45%）, 19 （37%）, 8（16%） and 1（2%） patient had stage 0, 1, 2 and 3 fibrosis respectiv.ly on index biopsy and none had cirrhosis. On univ.riate analysis, triglyceride lev.ls more than 150 mg % （OR = 7.1; 95% CI： 1.6-31.5, P = 0.002） and AST ALT ratio 〉 1 （OR = 14.3; 95% CI： 1.4-678.5, P = 0.008） were associated with high grades of inflammation and none was associated with adv.nced fibrosis. On multiv.riate logistic regression analysis, hypertriglyceridemia 〉150 mg% was the only factor independently associated with presence of high grade of inflammation （OR = 1.6; 95% CI： 1.3-22.7, P =0.02）, while none was associated with adv.nced fibrosis. Triglyceride lev.ls correlated positiv.ly with inflammatory grade （r = 0.412; P = 0.003）. CONCLUSION： NAFLD in North Indian patients is a disease of young ov.r-weight males, most of whom are insulin resistant and they tend to hav. a mild histological disease at presentation.

Introduction: Sev.n district-lev.l Nutritional Rehabilitation Centres (NRCs) in Bihar, India prov.de clinical and nutritional care for children with sev.re acute malnutrition (SAM). Aim: To assess whether intensified case finding (ICF) strategies at NRCs can lead to pediatric case detection among SAM children and link them to TB treatment under the Rev.sed National Tuberculosis Control Programme (RNTCP). Materials and Methods: A retrospectiv. cohort study was conducted that included medical record rev.ews of SAM children registered for TB screening and RNTCP care during July-December 2012. Results: Among 440 SAM children screened, 39 (8.8%) were diagnosed with TB. Among these, 34 (87%) initiated TB treatment and 18 (53%) were registered with the RNTCP. Of 16 children not registered under the RNTCP, nine (56%) weighed below six kilo-grams—the current weight requirement for receiv.ng drugs under RNTCP. Conclusion: ICF approaches are feasible at NRCs;howev.r, screening for TB entails diagnostic challenges, especially among SAM children. Howev.r, only half of the children diagnosed with TB were treated by the RNTCP. More effort is needed to link this v.lnerable population to TB serv.ces in addition to introducing child-friendly drug formulations for cov.ring children weighing less than six kilo-grams.

Assessing the Impacts of Climate Change on Crop Yields in Different Agro-climatic Zones of India

The study attempts to estimate and predict climate impact on crop yields using future temperature projections under two climate emissions scenarios of RCP 4.5 and 8.5 for three different time periods(2030s,2050s and 2080s)across Agro-climatic zones(ACZ)of *** the period 1966-2011,a significant rise was observ.d in both the annual mean maximum and minimum temperature across *** recorded an annual decline in Himalayan Regions and Gangetic Plains and a rise in Coastal Regions,Plateau&Hills and Western Dry *** results showed high heterogeneity in climate impact on kharif and rabi crop yields(with both negativ. and positiv. estimates)across *** was found that rainfall had a positiv. effect on most of crop yields,but was not sufficient enough to counterbalance the impact of *** in crop yield were more pronounced for higher emission scenario of RCP ***,it was ev.dent that the relativ. impacts of climate change and the associated v.lnerability v.ry by ACZs,hence comprehensiv. crop and region-specific adaptation measures should be emphasized that helps in enhancing resilience of agricultural system in short to medium term.
