Implementing population-based screening programs for colorectal cancer has led to an increase in the detection of large but benign histological ***,endoscopic mucosal resection can be considered the standard technique for the remov.l of benign lesions of the colon due to its excellent safety profile and good clinical ***,sev.ral studies from different geographic areas agree that many benign colon lesions are still referred for ***,the referral rate to surgery is not decreasing ov.r the years,despite the theoretical improv.ment of endoscopic resection *** article will rev.ew the leading causes for benign colorectal lesions to be referred for surgery and the influence of the endoscopist experience on the referral *** will also describe how to categorize a polyp as complex for resection and consider an endoscopist as an expert in endoscopic *** finally,we will propose a framework for the accurate and ev.dence-based treatment of complex benign colorectal lesions.

AIM: To analyze the clinico-pathological spectrum of primary duodenal neoplasms. METHODS: A total of 55 primary duodenal neoplasms reported in the last 10 years after excluding ampullary and periampullary tumors were included in the study. Clinical details were noted and routine hematoxylin and eosin stained paraffin sections were studied for histological subtyping of the tumors. RESULTS: On histopathological examination primary duodenal neoplasms were categorized as: epithelial tumor in 27 cases (49.0%) including 10 cases of adenoma, 15 cases of adenocarcinoma, and 2 cases of Brunner gland adenoma; mesenchymal tumor in 9 cases (16.3%) consisting of 4 cases of gastrointestinal stromal tumor, 4 cases of smooth muscle tumor and I case of neurofibroma; lymphoproliferativ. tumor in 12 cases (21.8%), and neuroendocrine tumor in 7 cases (12.7%). CONCLUSION: Although non-ampullary/periampullary duodenal adenocarcinomas are rare, they constitute the largest group. Histopathological examination of primary duodenal tumors is important for correct histological subtyping.

We hav. inv.stigated the 20 nm p-type double gate junctionless tunnel field effect transistor （P-DGJLTFET） and the impact of v.riation of different dev.ce parameters on the performance parameters of the P-DGJLTFET is discussed. We achiev.d excellent results of different performance parameters by taking the optimized dev.ce parameters of the P-DGJLTFET. Together with a high-k dielectric material （TiO2） of 20 nm gate length, the simulation results of the P-DGJLTFET show excellent characteristics with a high IoN of ～ 0.3 mA/μm, a low/OFF of ～ 30 fA/μm, a high ION/IOFF ratio of ～ 1×10^10, a subthreshold slope （SS） point of ～ 23 mv.decade, and an av.rage SS of ～ 49 mv.decade at a supply v.ltage of -1 v.and at room temperature, which indicates that PDGJLTFET is a promising candidate for sub-22 nm technology nodes in the implementation of integrated circuits.

In the last years, operativ. laparoscopy became a standard approach in gynaecology and general surgery. Ev.n in pregnancy its use is becoming more widely accepted. In fact, it offers adv.ntages similar to those in no pregnant women, associated with good maternal and fetal outcomes. Around 0.2% of pregnant women require abdominal surgery. The most common indications of laparoscopy in pregnancy are cholelithiasis complications, appendicitis, persistent ov.rian cyst and adnexal torsion. Authors describe a v.ry rare case of acute abdomen due to isolated Fallopian tube torsion in a 24 th weeks pregnant woman, managed by laparoscopic salpingectomy.

In this report, we present computed tomographic findings of colonic trichuriasis. The patient was a 75-year-old man who complained of abdominal pain, and weight loss. Diagnosis was achiev.d by colonoscopic biopsy. Abdominal computed tomography showed irregular and nodular thickening of the wall of the cecum and ascending colon. Although these findings are nonspecific, they may be one of the findings of trichuriasis. These findings, confirmed by pathologic analysis of the biopsied tissue and KatoKatz parasitological stool flotation technique, rev.aled adult Trichuris. To our knowledge, this is the first report of colonic trichuriasis indicated by computed tomography.

AIM: To determine the predictors of 50 d in-hospital mortality in decompensated cirrhosis patients with spontaneous bacterial peritonitis(SBP).METHODS: Two hundred and eighteen patients admitted to an intensiv. care unit in a tertiary care hospital between June 2013 and June 2014 with the diagnosis of SBP(during hospitalization) and cirrhosis were retrospectiv.ly analysed. SBP was diagnosed by abdominal paracentesis in the presence of polymorphonuclear cell count ≥ 250 cells/mm3 in the peritoneal fluid. Student's t test, multiv.riate logistic regression, cox proportional hazard ratio(HR), receiv.r operating characteristics(ROC) curv.s and Kaplan-Meier surv.v.l analysis were utilized for statistical analysis. Predictiv. abilities of sev.ral v.riables identified by multiv.riate analysis were compared using the area under ROC curv.. P v.v.l duration for those who died was 9 d. In univ.riate analysis acute kidney injury(AKI), hepatic encephalopathy, septic shock, serum bilirubin, international normalized ratio, aspartate transaminase, and model for end-stage liv.r disease- sodium(MELD-Na) were significantly associated with in- hospital mortality in patients with SBP(P ≤ 0.001). Multiv.riate coxproportional regression analysis showed AKI(HR = 2.16, 95%CI: 1.36-3.42, P = 0.001) septic shock(HR = 1.73, 95%CI: 1.05-2.83, P = 0.029) MELD-Na(HR = 1.06, 95%CI: 1.02-1.09, P ≤ 0.001) was significantly associated with 50 d in-hospital mortality. The prognostic accuracy for AKI, MELD-Na and septic shock was 77%, 74% and 71% respectiv.ly associated with 50 d inhospital mortality in SBP ***: AKI, MELD-Na and septic shock were predictors of 50 d in-hospital mortality in decompensated cirrhosis patients with SBP.

Association of ezrin expression in intestinal and diffuse gastric carcinoma with clinicopathological parameters and tumor type

AIM： To inv.stigate the correlation between ezrin expression and types of gastric carcinoma and clinicopathological v.riables.METHODS： We examined ezrin protein expression in 75 gastric carcinoma （53 intestinal types of adenocarcinoma, 22 diffuse types of carcinoma） tissues by immunohistochemistry. The results were compared with clinicopathological parameters such as tumor type, grade of tumor, clinical stage, presence of metastatic lymph node, and depth of inv.sion. RESULTS： Ezrin immunostaining was positiv. in 43 cases （81.1%） of intestinal type and in 9 （40.9%） cases of diffuse type adenocarcinomas （P 〈 0.001）. In gastric carcinomas, the expression of ezrin protein correlated with the status of H py/ori and surv.v.l. There was no correlation between expression of ezrin with TNM stage and histological grade of gastric carcinomas （P 〉 0.05）. CONCLUSION： The low expression of ezrin implicates the loss of adhesion in diffuse carcinomas. Furthermore, ov.rexpression of ezrin in carcinomas with H pylori infection may be a genuine specific pathway in which Hpylori may cause/initiate gastric carcinoma.

AIM: To inv.stigate the electric and contractile mechanisms inv.lv.d in the deranged function of the transposed stomach in relation to the course of the symptoms and the changes in contractile and electrical parameters ov.r ***: Twenty-one patients after subtotal esophagectomy and 18 healthy v.lunteers were *** were compiled by using a questionnaire, and a symptom score was formed. Synchronous electrogastrography and gastric manometry were performed in the fasting state and ***: Eight of the operated patients were symptomfree and 13 had symptoms. The durations of the postoperativ. periods for the symptomatic (9.1±6.5 mo)and the asymptomatic (28.3±8.8 mo) patients were significantly different. The symptom score correlated negativ.ly with the time that had elapsed since the operation. The percentages of the dominant frequency in the normogastric, bradygastric and tachygastric ranges differed significantly between the controls and the patients.A significant difference was detected between the power ratio of the controls and that of the patients. The occurrence of tachygastria in the symptomatic and the symptom-free patients correlated negativ.ly both with the time that had elapsed and with the symptom score. There was a significant increase in motility index after feeding in the controls, but not in the patients. The contractile activ.ty of the stomach increased both in the controls and in the symptom-free patients. In contrast, in the group of symptomatic patients, the contractile activ.ty decreased postprandially as compared with the fasting ***: The patients' post-operativ. complaints and symptoms change during the post-operativ. period and correlate with the parameters of the myoelectric and contractile activ.ties of the stomach. Tachygastria seems to be the major pathogenetic factor inv.lv.d in the contractile dysfunction.
