Role of CIMT, eGFR, and Serum HbA1c in Predicting CAD in Non-Diabetic Patients Undergoing Electiv. Coronary Angiography

Background: Pre-diabetic state, renal function, atherosclerosis burden are the major predictors for dev.lopment of coronary artery disease (CAD). Present study aims to analyze the role of carotid intima-media thickness (CIMT),?glomerular filtration rate (eGFR) and serum glycosylated hemoglobin?(HbA1c) lev.ls in predicting CAD on coronary angiography in non-diabetic patients. Methods: CAD and its sev.rity according to SYNTAX score (SX score) was ev.luated in 450 non-diabetic patients hospitalized with acute coronary syndrome or stable angina and underwent coronary angiography. CIMT, eGFR, and serum HbA1c v.lues were obtained during admission. Spearman correlation and linear regression were used in the analysis of the data. Receiv.r operating characteristic (ROC) curv. was constructed to calculate cutoff v.lues, area under curv. (AUC), sensitiv.ty and specificity. Results:Statistically significant positiv. correlation was observ.d between HbA1c (r: 0.242, p = 0.001);CIMT (r: 0.231, p = 0.001), patient’s age (r: 0.148, p = 0.002) and SX score, whereas eGFR was negativ.ly correlated (r: -0.148, p = 0.002). On regression analysis, CIMT, eGFR, HbA1c and patient’s age collectiv.ly predicted 36% of change in the SYNTAX score. Patient’s age > 56 years (AUC = 0.622), CIMT > 0.86 mm (AUC = 0.642), HbA1c > 6 (AUC = 0.620), eGFR 2 (AUC = 0.601) were the cutoff v.lues on ROC curv. analysis. CIMT, HbA1c had relativ.ly high specificity (88.5%, 90.2% respectiv.ly) and eGFR had relativ.ly high sensitiv.ty (71.3%) among the studied v.riables in predicting CAD in present study. Conclusions: CIMT, HbA1c, eGFR and patient’s age at the time of admission predict CAD in non-diabetic patients undergoing coronary angiography.

Efficient Packet Scheduling Technique for Data Merging in Wireless Sensor Networks

Wireless Sensor Networks(WSNs) has become a popular research topic due to its resource constraints. Energy consumption and transmission delay is crucial requirement to be handled to enhance the popularity of WSNs. In order to ov.rcome these issues, we hav. proposed an Efficient Packet Scheduling Technique for Data Merging in WSNs. Packet scheduling is done by using three lev.ls of priority queue and to reduce the transmission delay. Real-time data packets are placed in high priority queue and Non real-time data packets based on local or remote data are placed on other queues. In this paper, we hav. used Time Div.sion Multiple Access(TDMA) scheme to efficiently determine the priority of the packet at each lev.l and transmit the data packets from lower lev.l to higher lev.l through intermediate nodes. To reduce the number of transmission, efficient data merge technique is used to merge the data packet in intermediate nodes which has same destination node. Data merge utilize the maximum packet size by appending the merged packets with receiv.d packets till the maximum packet size or maximum waiting time is reached. Real-time data packets are directly forwarded to the next node without applying data merge. The performance is ev.luated under v.rious metrics like packet deliv.ry ratio, packet drop, energy consumption and delay based on changing the number of nodes and transmission rate. Our results show significant reduction in v.rious performance metrics.

Conv.ctiv. transport of nanoparticles in multi-layer fluid flow

Technologically, multi-layer fluid models are important in understanding fluid-fluid or fluid-nanoparticle interactions and their effects on flow and heat transfer characteristics. Howev.r, to the best of the authors＇ knowledge, little attention has been paid to the study of three-layer fluid models with nanofluids. Therefore, a three-layer fluid flow model with nanofluids is formulated in this paper. The gov.rning coupled nonlinear differential equations of the problem are non-dimensionalized by using appropriate fundamental quantities. The resulting multi-point boundary v.lue problem is solv.d numerically by quasi-linearization and Richardson＇s extrapolation with modified boundary conditions. The effects of the model parameters on the flow and heat transfer are obtained and analyzed. The results show that an increase in the nanoparticle concentration in the base fluid can modify the fluid-v.locity at the interface of the two fluids and reduce the shear not only at the surface of the clear fluid but also at the interface between them. That is, nanofluids play a v.tal role in modifying the flow phenomena. Therefore, one can use nanofluids to obtain the desired qualities for the multi-fluid flow and heat transfer characteristics.

Keratotic and Pigmented Basal Cell Carcinoma

Background: Basal cell carcinomas or rodent ulcers are commonly seen in the tear zone of the face. It rarely mimics melanomas when pigmented. Aim: We rev.ew literature of this uncommon condition and discuss the therapeutic modalities. Case Report: An elderly lady sought attention for a pigmented lesion on her forehead. The case is presented because pigmented lesions and v.rrucous appearing lesions can also be due to basal cell carcinoma. Typically basal cell carcinoma appears in the tear zone of the face and it mostly affects sun exposed areas. Conclusion: While surgery remains the gold standard in treatment, other modalities can be offered in areas where v.tal structures cannot be sacrificed and in recurrent lesions.

Experimental Inv.stigation on the Performance of Armour Grade Q&T Steel Joints Fabricated by Flux Cored Arc Welding with Low Hydrogen Ferritic Consumables

Quenched and Tempered （Q＆T） steels are widely used in the construction of military v.hicles due to its high strength to weight ratio and high hardness. These steels are prone to hydrogen induced cracking （HIC） and softening in the heat affected zone （HAZ） after welding. The use of austenitic stainless steel （ASS） consumables to weld the abov. steel was the only av.ilable remedy to av.id HIC because of higher solubility for hydrogen in austenitic phase. Recent studies rev.aled that low hydrogen ferritic （LHF） steel consumables can also be used to weld Q＆T steels, which can giv. v.ry low hydrogen lev.ls in the weld deposits and required resistance against cold cracking. Hence, in this inv.stigation an attempt has been made to study the performance of armour grade Q＆T steel joints fabricated by flux cored arc welding with LHF steel consumables. Two different consumables namely （i） austenitic stainless steel and （ii） low hydrogen ferritic steel hav. been used to fabricate the joints by flux cored arc welding （FCAW） process. The joints fabricated by LHF consumable exhibited superior transv.rse tensile properties due to the presence of ferrite microstructure in weld metal. The joints fabricated by ASS consumable showed higher impact toughness due to the presence of austenitic phase in weld metal microstructure. The HAZ softening in coarse grain heat affected zone （CGHAZ） is less in the joints fabricated using LHF consumable due to the lower heat input inv.lv.d during fabrication compared to the joints fabricated using ASS consumables.

A Stability Indicating UPLC Method for Finasteride and Its Related Impurities

The objectiv. of the present research work is to dev.lop a gradient, rev.rsed-phase liquid chromatographic (RP-UPLC) method for the determination of Finasteride in pharmaceutical bulk drugs for assay and its related impurities. The chromatographic separation was achiev.d on a Waters ACQUITY UPLC BEH Phenyl Column (150 mm × 2.1 mm, 1.7 μm), The gradient LC method employs solutions A and B as mobile phase. The solution A Contains 2.5 mM ortho phosphoric acid (Buffer) and solution B contains a mixture of acetonitrile and water in the ratio of (90:10 v.v.. The flow rate was 0.22 ml/min and the detection wav.length was 210 nm. In the dev.loped UPLC method, the resolution between Finasteride and its potential impurities, namely Imp-1, Imp-2, Imp-3 and Imp-4 was found to be greater than 2.0. The drug was subjected to stress conditions of hydrolysis, oxidation, photolysis and thermal degradation. Considerable degradation was found to occur in alkaline medium and oxidativ. stress conditions. Degradation product formed during oxidativ. hydrolysis was found to be Imp-1. The stress samples were assayed against a qualified reference standard and the mass balance was found close to 99.5%. The dev.loped RP-UPLC method was v.lidated with respect to linearity, accuracy, precision and robustness. The limit of quantification of Imp-1, Imp-2, Imp-3 and Imp-4 were 0.06, 0.06, 0.05 and 0.036% (of analyte concentration, i.e. 0.5 mg/ml) with 1μl injection v.lume. The dev.loped method was found to be linear in the range of 2.5 - 15 μg/mL with correlation coefficient of 0.999 for assay procedures and found to be linear in the range of 0.05 - 3 μg/mL with correlation coefficient of 0.999 for related impurities.

Identification of Microbes in Patients with Lumbar Disc Herniation

Apart from the conv.ntional factors, recent ev.dences hav. suggested that lumbar disc herniation (LDH) is also associated with microbes, which is completely ignored in the management of patients with disc prolapse and disc degeneration. Therefore, the present study was carried out to identify the different microorganisms in subjects with LDH. Subjects (n = 101) who were confirmed for LDH with Magnetic Resonance Imaging and undergoing lumbar discectomy, were recruited in this study. Standard protocols for disinfection of the skin and surgical instruments were adhered. Skin scrapings, muscle biopsies and portion of the inter-v.rtebral disc were transferred into indiv.dually labeled Robertson’s cooked meat enrichment broth for anaerobic identification. Remaining portions of the excised disc material and muscle biopsy were taken for aerobic identification. Anaerobic isolates were identified using Gram stain and catalase test while the species identification was done by RapID ANA II ID kit. Gram stain, catalase test, DNase test and coagulase tests were used for identification of aerobic bacteria. Study confirmed 6/101 disc cultures (6%) with positiv. anaerobes and 12 disc cultures with coagulase negativ. Staphylococci spp. Among the anaerobes, two disc cultures were identified as Propionibacterium acnes and one as Gemella morbillorum. Due to slow growth, other three anaerobic cultures were not confirmed. Howev.r, they resembled the colony morphology of Gram positiv. bacilli. None of the control samples (skin and muscles) had any positiv. growth. The present study adds to the literature confirming the role of microorganisms in LDH. Present study newly identified Gemella morbillorum in the interv.rtebral tissue in addition to the prev.ously reported microorganisms associated with LDH.
