Neoadjuvant treatment for advanced esophageal cancer: response assessment before surgery and how to predict response to chemoradiation before starting treatment

Patients with advanced esophageal cancer（T3-4, N） have a poor prognosis. Chemoradiation or chemotherapy before esophagectomy with adequate lymphadenectomy is the standard treatment for patients with resectable advanced esophageal carcinoma. However, only patients with major histopathologic response（regression to less than 10% of the primary tumor） after preoperative treatment will have a prognostic benefit of preoperative chemoradiation. Using current therapy regimens about 40% to 50% of the patients show major histopathological response. The remaining cohort does not benefit from this neoadjuvant approach but might benefit from earlier surgical resection. Therefore, it is an aim to develop tools for response prediction before starting the treatment and for early response assessment identifying responders. The current review discusses the different imaging techniques and the most recent studies about molecular markers for early response prediction. The results show that [^18F]-fluorodeoxyglucose-positron emission tomography（FDGPET） has a good sensitivity but the specificity is not robust enough for routine clinical use. Newer positron emission tomography detector technology, the combination of FDG-PET with computed tomography, additional evaluation criteria and standardization of evaluation may improve the predictive value. There exist a great number of retrospective studies using molecular markers for prediction of response. Until now the clinical use is missing. But the results of first prospective studies are promising. A future perspective may be the combination of imaging technics and special molecular markers for individualized therapy. Another aspect is the response assessment after finishing neoadjuvant treatment protocol. The different clinical methods are discussed. The results show that until now no non-invasive method is valid enough to assess complete histopathological response.

Neoadjuvant chemoradiation changes podoplanin expression in esophageal cancer patients

BACKGROUND Locally advanced adenocarcinoma of the esophagus(EAC) and squamous cell carcinoma(ESCC) result in a worse prognosis. Neoadjuvant treatment improves survival, however, only for responders. The transmembrane glycoprotein podoplanin is overexpressed in squamous cell carcinomas, mi RNA-363 is associated to its regulation in head and neck *** To predict therapy response and prognosis markers, and targets for novel therapies would individualize treatments leading to more favourable *** Expression of podoplanin protein has been visualized by immunohistochemistry in surgical specimens of 195 esophageal cancer patients who underwent transthoracic esophagectomy: 90 ESCC and 105 EAC with clinical T2-3, Nx, *** hundred and six patients received neoadjuvant chemoradiation. RNA was extracted from paraffin-embedded tissue, and mi RNA-363 quantified by realtime Taq Man-real-time-PCR. D2-40 mab staining of > 5% was scored as high podoplanin expression(HPE). We related podoplanin and mi RNA-363 expression to histopathologic response after neoadjuvant treatment and clinicopathological characteristics, such as histological tumor type, survival rate or clinical tumor *** confirmed expression of membrane-bound podoplanin in 90 ESCC patients.26% showed HPE of > 5%. In addition, absence in EAC patients(only 2% with HPE) was shown. Lower podoplanin expression has been detected in resectionspecimen of 58 ESCC patients after neoadjuvant(RTx/CTx) treatment, only 11% with HPE, compared to 50% HPE of 32 non-pretreated primary surgery patients,P = 0.0001. This difference of podoplanin expression was confirmed comparing pre-treatment biopsies with matching post-treatment surgical specimens, P <0.001. Podoplanin has been identified as a prognostic marker in 32 patients that underwent primary surgery without neoadjuvant treatment. AIM： To analyze the impact of the GNAS1 T393C polymorphism on prognosis and histopathology of gastric cancer. METHODS： Genomic DNA was extracted from paraffinembedded tissues of 122 patients with primary gastric carcinoma and from the blood of 820 healthy white individuals. Allelic discrimination was performed by quantitative real-time polymerase chain reaction. Genotyping was correlated with histopathologic parameters and with overall survival according to the Kaplan-Meier approach and with multivariate analysis by multiple stepwise regression. RESULTS： Thirty-nine （32%） patients displayed a CC genotype, 57 （46.7%） a CT genotype and 26 （21.3%） a TT genotype. The frequency of the C allele （fC） in the patient group was 0.55, which was not significantly different from that of healthy blood donors. The distribution was compatible with the Hardy-Weinberg equilibrium. Analysis of clinicopathological parameters did not show any significant correlation of the T393C genotype with gender （P = 0.50）, differentiation （P = 0.29）, pT-category （P = 0.19）, pN-category （P = 0.30）, pM-category （P = 0.25）, R-category （P = 0.95）, the classifications according to WHO （P = 0.34）, Lauren （P = 0.16）, Goseki （P = 1.00） and Ming （P =0.74）. Dichotomization between C＋（CC＋CT） and C-genotypes （FI）, however, revealed significantly more advanced tumor stages （P = 0.023） and lower survival rates （P = 0.043） for C allele carriers. CONCLUSION： The present study provides strong evidence to suggest that the GNAS1 T393C allele carrier status influences tumor progression and survival in gastric cancer with higher tumor stages and a worse outcome for C allele carriers.

We present a study of star-forming galaxies (SFGs) with pseudobulges (bulges with Sérsic index n2σ) mean of specific star formation rate (sSFR) enhancement (sSFR_(enh)=0.33±0.07 versus sSFR_(enh)=0.12±0.06) and broader scatter (by~1 dex).The eight SFGs that have the highest sSFR_(enh)in the sample all have pseudobulges.A majority (69%) of paired SFGs with strong enhancement (having sSFR more than5 times the median of the control galaxies) have *** Spitzer data show that the pseudobulges in these galaxies are tightly linked to nuclear/circum-nuclear *** SFGs in S+S and in S+E pairs have significantly (>3σ) different sSFR enhancement,with the means of sSFR_(enh)=0.45±0.08 and-0.04±*** find a decrease in the sSFR enhancements with the density of the environment for SFGs with *** a high fraction (5/11) of pseudobulge SFGs in S+E pairs are in rich groups/clusters (loca density N_(1Mpc)≥7),the dense environment might be the cause for their low s SFR_(enh).

Sodium-carbon dioxide(Na-CO_(2))batteries are regarded as promising energy storage technologies because of their impressive theoretical energy density and CO_(2)reutilization,but their practical applications are restricted by uncontrollable sodium dendrite growth and poor electrochemical kinetics of CO_(2)*** suitable multifunctional electrodes for dendritefree anodes and kinetics-enhanced CO_(2)cathodes is considered one of the most important ways to advance the practical application of Na-CO_(2)***,RuO2 nanoparticles encapsulated in carbon paper(RuCP)are rationally designed and employed as both Na anode host and CO_(2)cathode in Na-CO_(2)*** outstanding sodiophilicity and high catalytic activity of RuCP electrodes can simultaneously contribute to homogenous Na+distribution and dendrite-free sodium structure at the anode,as well as strengthen discharge and charge kinetics at the *** morphological evolution confirmed the uniform deposition of Na on RuCP anode with dense and flat interfaces,delivering enhanced Coulombic efficiency of 99.5%and cycling stability near 1500 ***,Na-CO_(2)batteries with RuCP cathode demonstrated excellent cycling stability(>350 cycles).Significantly,implementation of a dendrite-free RuCP@Na anode and catalytic-site-rich RuCP cathode allowed for the construction of a symmetric Na-CO_(2)battery with long-duration cyclability,offering inspiration for extensive practical uses of Na-CO_(2)batteries.

Coronal magnetic fields evolve quasi-statically over long timescales and dynamically over short *** of now there exist no regular measurements of coronal magnetic fields,and therefore generating the coronal magnetic field evolution using observations of the magnetic field at the photosphere is a fundamental requirement to understanding the origin of transient phenomena from solar active regions(ARs).Using the magneto-friction(MF)approach,we aim to simulate the coronal field evolution in the solar AR *** MF method is implemented in the open source PENCIL CODE along with a driver module to drive the initial field with different boundary conditions prescribed from observed vector magnetic fields at the *** order to work with vector potential and the observations,we prescribe three types of bottom boundary drivers with varying free-magnetic *** MF simulation reproduces the magnetic structure,which better matches the sigmoidal morphology exhibited by Atmospheric Imaging Assembly(AIA)images at the pre-eruptive *** found that the already sheared field further driven by the sheared magnetic field will maintain and further build the highly sheared coronal magnetic configuration,as seen in AR ***-driven MF simulation is a viable tool to generate the coronal magnetic field evolution,capturing the formation of the twisted flux rope and its eruption.

Common psychiatric disorders(CPDs)and depression contribute significantly to the global epidemic of type 2 diabetes(T2D).We postulated a possible pathophysiological mechanism that through Bridge-Symptoms present in depression and CPDs,promotes the establishment of emotional eating,activation of the reward system,onset of overweight and obesity and,ultimately the increased risk of developing *** plausibility of the proposed pathophysiological mechanism is supported by the mechanism of action of drugs such as naltrexonebupropion currently approved for the treatment of both obesity/overweight with T2D and as separate active pharmaceutical ingredients in drug addiction,but also from initial evidence that is emerging regarding glucagon-like peptide 1 receptor agonists that appear to be effective in the treatment of drug *** hope that our hypothesis may be useful in interpreting the higher prevalence of CPDs and depression in patients with T2D compared with the general population and may help refine the integrated psychiatric-diabetic therapy approach to improve the treatment and or remission of T2D.

HIGH RESOLUTION SCHEMES FOR CONSERVATION LAWS AND CONVECTION-DIFFUSION EQUATIONS WITH VARYING TIME AND SPACE GRIDS

This paper presents a class of high resolution local time step schemes for nonlinear hyperbolic conservation laws and the closely related convection-diffusion equations, by projecting the solution increments of the underlying partial differential equations （PDE） at each local time step. The main advantages are that they are of good consistency, and it is convenient to implement them. The schemes are L^∞ stable, satisfy a cell entropy inequality, and may be extended to the initial boundary value problem of general unsteady PDEs with higher-order spatial derivatives. The high resolution schemes are given by combining the reconstruction technique with a second order TVD Runge-Kutta scheme or a Lax-Wendroff type method, respectively. The schemes are used to solve a linear convection-diffusion equation, the nonlinear inviscid Burgers＇ equation, the one- and two-dimensional compressible Euler equations, and the two-dimensional incompressible Navier-Stokes equations. The numerical results show that the schemes are of higher-order accuracy, and efficient in saving computational cost, especially, for the case of combining the present schemes with the adaptive mesh method [15]. The correct locations of the slow moving or stronger discontinuities are also obtained, although the schemes are slightly nonconservative.

Study on a Cross Diffusion Parabolic System

This paper considers a kind of strongly coupled cross diffusion parabolic system, which can be used as the multi-dimensional Lyurnkis energy transport model in semiconductor science. The global existence and large time behavior are obtained for smooth solution to the initial boundary value problem. When the initial data are a small perturbation of an isothermal stationary solution, the smooth solution of the problem under the insulating boundary condition, converges to that stationary solution exponentially fast as time goes to infinity.

A Space-Time Conservative Method for Hyperbolic Systems with Stiff and Non Stiff Source Terms

In this article we propose a higher-order space-time conservative method for hyperbolic systems with stiff and non stiff source terms as well as relaxation *** call the scheme a slope propagation(SP)*** is an extension of our scheme derived for homogeneous hyperbolic systems[1].In the present inhomogeneous systems the relaxation time may vary from order of one to a very small *** small values make the relaxation term stronger and highly *** such situations underresolved numerical schemes may produce spurious numerical ***,our present scheme has the capability to correctly capture the behavior of the physical phenomena with high order accuracy even if the initial layer and the small relaxation time are not numerically *** scheme treats the space and time in a unified *** flow variables and their slopes are the basic unknowns in the *** source term is treated by its volumetric integration over the space-time control volume and is a direct part of the overall space-time flux *** use two approaches for the slope calculations of the flow variables,the first one results directly from the flux balance over the control volumes,while in the second one we use a finite difference *** main features of the scheme are its simplicity,its Jacobian-free and Riemann solver-free recipe,as well as its efficiency and high of order *** particular we show that the scheme has a discrete analog of the continuous asymptotic *** have implemented our scheme for various test models available in the literature such as the Broadwell model,the extended thermodynamics equations,the shallow water equations,traffic flow and the Euler equations with heat *** numerical results validate the accuracy,versatility and robustness of the present scheme.
