The Volume Effect and Safety of 6% Hydroxyethyl Starch 130/0.4 in Patients Undergoing Major Elective Surgery: An Uncontrolled, Open-Labeled, Multi-Center Study

Purpose: The primary aim of this study was to investigate volume effect and safety of up to 50 mL/kg BW 6% hydroxyethyl starch (HES) 130/0.4 in adult and pediatric patients undergoing major elective surgery. The need to infuse human albumin may be reduced or avoided in Japan if these large doses 6% HES 130/0.4 can be infused. Methods: The study was an uncontrolled, open-labeled, multi-center trial. Fifteen adult and 5 pediatric patients undergoing major elective surgery received 6% HES 130/0.4 (Voluven®) with a maximum dose of 50 mL/kg from the start of surgery until 2 hours after the end of surgery according to a treatment algorithm. The primary efficacy endpoint was the volume effect of 6% HES 130/0.4 determined by the volume of saved albumin during the investigational period and the time course of hemodynamic stability in adult and pediatric patients. Safety parameters were fluid balance, hemodynamic and laboratory parameters ECG, local and systemic tolerance and adverse events. Results: Adult patients received a mean of 32.0 mL/kg of 6% HES 130/0.4. For 12 out of 15 adult patients an average amount of 1033.8 mL (18.6 mL/kg) albumin could be saved. The other 3 adult patients did not receive more than 1000 mL of HES 130/0.4. All pediatric patients received approximately 50 mL/kg of HES 130/0.4;for these patients an average amount of 39.9 mL/kg body weight albumin could be saved. The majority of adult patients, and all pediatric patients were hemodynamically stable at all 3 time points. The observed changes of the assessed laboratory parameters including hematological and coagulation parameters or in any other safety parameter determined did not reveal any safety concern related to the administration of 6% HES 130/0.4 up to doses of 50 mL/kg body weight. Conclusion: The study results indicate that 6% HES 130/0.4 has a reliable volume effect, could contribute to significant human albumin savings and was safe and well tolerated up to a maximum dose of 50 mL/kg bo

Disastrous flood events in recent years include 2018 Japan floods（July 2018）,Typhoon 19（Hagibis, October 2019） and the following heavy rain event, and July 2020 heavy rain disaster. Such disastrous flood events are expected to occur more frequently as the climate change progresses, and it is indispensable to update information regarding water areas such as rivers, reservoirs, and coastal waters. This study demonstrates practical techniques to analyze underwater topography. Longitude and latitude components of ellipsoidal data recorded by a GPS receiver are projected to a rectangular coordinate system,and results are combined with vertical components including data recorded by an echo sounder unit so that tracks on an underwater floor are obtained. A mapping on a finite dimensional space of continuous functions over a triangular mesh is formulated, and its fixed point gives rise to an underwater floor that fits the tracks. Our techniques are illustrated with data obtained in measurement conducted in a reservoir called Kojima Lake located in Okayama Prefecture, Japan.

＜正＞Transanal Endoscopic Microsurgery（TEM） has been reported to be a better operating method for resecting a rectal mucosal lesion than any other conventional transanal or transsacral procedure. One of the greatest advantage of TEM is en bloc resection with safety margin even of large sized lesion. Nevertheless. according to our clinical experiences, an enough safety margin could not be secured in more than 30％ of cases in our series, including cases in which local recurrence occurred, requiring additional surgery. To perform en bloc resection completely with a safety margin, we modified TEM by using hemoclips and proved to be useful clinical practice.

Adsorption and Mutual Separation of Noble Metals Using Chitosan Modified by Propane Sultone

Chitosan modified by propane sultone (hereafter PSC) is prepared and the adsorption characteristics of noble metal ions are examined by using the chitosan resin. Gold, platinum and palladium are tried to separate in each other by using a glass column packed with PSC. Firstly, the chitosan derivative PSC having sulfonic group was prepared and crosslinked with ethylene glycol diglycidyl ether. IR spectra and elemental analyses were measured in order to evaluate the characteristics of PSC. Secondly, the adsorption and desorption experiments of the noble metals were carried out by a batch method and a continuous flow method using PSC. From the metal adsorption experiment in a batch method, it was found that the noble metals were adsorbed onto PSC through both sulfonic and amino groups. And thiourea dissolved in hydrochloric acid solution was found to be very effective to desorb the metals from PSC. In the separation of gold, platinum and palladium with PSC packed in the column, they were effectively separated by operating the column at pH 2.5. In the desorption step, 0.2 mol/dm3 thiourea prepared using 0.1 mol/dm3 hydrochloric acid made the adsorbed metals to desorb perfectly.

A novel selenium-containing compound,selenoneine,has been isolated as the major form of organic selenium in the blood and tissues of *** harbors a selenium atom in the imidazole ring,2-selenylNα,Nα,Nα-trimethyl-L-histidine,and is a selenium analog of *** selenium compound has strong antioxidant capacity and binds to heme proteins,such as hemoglobin and myoglobin,to protect them from iron auto-oxidation,and it reacts with radicals and methylmercury(MeHg) .The organic cations/carnitine transporter OCTN1 transports selenoneine and MeHg,regulates Se-enhanced antioxidant activity,and decreases MeHg ***,the dietary intake of selenoneine,by consuming fish,might decrease the formation of reactive oxygen radicals that could oxidize nucleotides in DNA,and thereby inhibit carcinogenesis,chronic diseases,and aging.

Background: Posttraumatic Stress Disorder (PTSD) is a psychiatric disorder found in individuals afflicted by a traumatic event including the natural disaster. “Tsunami” occurred in Andaman coast of Thailand on December 26, 2004, in which 33.6% of survivors were diagnosed as PTSD. This study aimed to explore the single nucleotide polymorphism (SNP). rs267943 genotype is located on chromosome 5 in the intron of the death-associated protein 1 (DAP1) gene and psychosocial factors for PTSD. Methods: Participants (N = 1970) were recruited from volunteers who have complete data both of DAP1 gene and psychosocial factor. Results: Using a binary logistic regression model, significant gene-environment interactions were found for the single nucleotide polymorphism (SNP) rs267943 and psychosocial factors including depression (adj. OR = 6.0, 95% CI = 4.29 - 8.39), neurotic personality (adj. OR = 2.73, 95% CI = 2.18 - 3.42), planning (adj. OR = 1.52, 95% CI = 1.20 - 1.93), use of emotional support (adj. OR = 1.32, 95% CI = 1.21 - 1.94) with statistical significant p Conclusion: This study demonstrated that GxE studies can be utilized to shed light on the origins of PTSD.

Posttraumatic stress disorder (PTSD) is a psychiatric disorder found in individuals afflicted by a traumatic event. Multiple environmental and genetic factors can contribute to PTSD susceptibility. Since it is rare to find members of the same family afflicted by the same catastrophic event, it is not practical to determine PTSD susceptibility genes by a gene linkage analysis. A natural disaster, such as the 2004 Tsunami, provided us with a rare chance for a genetic analysis of PTSD. To identify SNPs associated with PTSD susceptibility, we conducted a genome-association study (GWAS) in Thai-Tsunami survivors. Initial phase of the study with 396 chronic PTSD patients and 457 controls, we identified top ninety SNPs (P -4), which were further assessed in the second phase with 395 chronic PTSD patients and 798 controls. Two SNPs (rs267950 and rs954406), were identified in the second phase, and subjected to fine mapping using a data set from both phases. SNP rs267943 showed the strongest association with PTSD susceptibility and was in complete linkage disequilibrium with SNP rs267950 with P = 6.15 × 10-8, OR = 1.46 and 95% CI = 1.19 - 1.79, reaching genome-wide significance. SNP rs267943 is located on chromosome 5 in the intron of the death-associated protein 1 (DAP1) gene and, when linked to a synthetic promoter, could regulate transcription. To our knowledge, this is the first GWAS for PTSD susceptibility in an Asian population which could provide an important insight into the genetic contribution of PTSD and may lead to new treatment strategies for PTSD.
