Diagnosis and pharmacologic management of neuropathic pain among patients with chronic low back pain

Chronic low back pain consists of both nociceptive and neuropathic mechanisms and can be classified as a mixed pain syndrome. Neuropathic component of chronic low back pain has often been under-recognized and under-treated by the physicians. Recent studies have demonstrated that approximately 20%-55% of chronic low back pain patients have neuropathic pain symptoms. An altered peripheral, spinal, and supraspinal processing of pain arising as a result of a lesion affecting the nerves system are the major contributor to neuropathic low back pain. The clinical evaluation is still the gold standard for assessment and diagnosis of neuropathic low back pain. Although diagnosis can be difficult due to the lack of reliable gold standard diagnostic test for neuropathic low back pain, screening tools may help non-specialists, in particular, to identify potential patients with neuropathic low back pain who require further diagnostic evaluation and pain management. Several screening tools for neuropathic pain have been developed and tested with different patient populations. Among the screening tools, the pain DETECT questionnaire and the Standardized Evaluation of Pain are validated in patients with low back pain. The Standardized Evaluation of Pain may lead to more effectivein discriminating between neuropathic and nociceptive pain in patients with low back pain according to the higher rate of sensitivity and its validity in patients with low back pain. However, the most appropriate approach is still to combine findings on physical and neurologic examinations and patient's report in distinguishing neuropathic pain from nociceptive pain. The clinical examination including bedside sensory tests is still the best available tool for assessment and diagnosis neuropathic pain among patients with chronic low back pain. Due to the fact that chronic low back pain consists of both nociceptive and neuropathic mechanisms, a multimodal treatment approach is more rational in the management

Effect of hydroxyapatite:zirconia volume fraction ratio on mechanical and corrosive properties of Ti-matrix composite scaffolds

Ti-based scaffolds reinforced with zirconia and hydroxyapatite were produced successfully by a hybrid method with an eco-friendliness and low cost to obtain low elastic modulus(E) with sufficient physical, electrochemical and biological properties. The effect of simultaneous modification of the volume fraction of hydroxyapatite(HA) and zirconia(ZrO_(2)) on scaffolds was investigated in terms of mechanical, corrosive, and antibacterial properties. Scanning electron microscopy with attached electron dispersive spectroscopy and X-ray diffraction were used for the characterization of scaffolds. Compression and electrochemical tests were performed to determine mechanical properties with detailed fracture mechanism and in-vitro corrosion susceptibility to simulated body fluid at 37 ℃,respectively. Antibacterial tests were carried out by comparing the inhibition areas of *** and *** bacteria. It was observed that the mechanical strength of the scaffolds decreased with increasing HA:ZrO_(2)volume fraction *** lowest E was achieved(6.61 GPa) in 6:4 HA:ZrO_(2)composite scaffolds. Corrosion current density(J_(corr)) values were calculated to be 21, 337, and 504 μ A/cm^(2) for unreinforced Ti, 3:2 and 6:4 HA:ZrO_(2)reinforced scaffolds,respectively. The inhibition capacity of the 6:4 reinforced composite scaffold was found to be more effective against *** bacteria than other scaffolds.

α-MgAgSb based thermoelectric(TE)device attracts much attention for its commercial application because it shows an extremely high conversion efficiency of ~8.5% under a temperature difference of 225 ***,the mechanical behavior of α-MgAgSb is another serious consideration for its engineering ***,we apply density functional theory(DFT)simulations to examine the intrinsic mechanical properties of all three MgAgSb phases,including elastic properties,shear-stress-shear-strain relationships,deformation and failure mechanism under ideal shear and biaxial shear *** find that the ideal shear strength of α-MgAgSb is 3.25 GPa along the most plausible(100)slip *** strength is higher than that of β-MgAgSb(0.80 GPa)and lower than that of γ-MgAgSb(3.43 GPa).The failure of α-MgAgSb arises from the stretching and breakage of MgeSb bond α-MgAgSb under pure shear load,while it arises from the softening of MgeAg bond and the breakage of AgeSb bond under biaxial shear *** suggests that the deformation mechanism changes significantly under different loading conditions.

Better outcomes with desmopressin melt than enuretic alarm therapy in children with nocturnal enuresis during coronavirus disease 2019(COVID-19)

Objectives:This study aimed to investigate the effect of the coronavirus disease 2019(COVID-19)pandemic on the treatment of children with primary monosymptomatic nocturnal enuresis(MNE)with desmopressin melt versus an enuresis *** and methods:This study included 56 children with primary MNE who were taking desmopressin melt or using an *** anxiety levels were evaluated using the Social Anxiety Scale for *** both treatment methods,data from a 3-month bedwetting diary between the third and sixth months of the pre-pandemic treatment were compared with those assessed during the same period during the ***:Prior to the COVID-19 pandemic,the median 3-month mean frequency of MNE was 1(0-7.67)in children using desmopressin melt versus 1.33(0-6)in those using alarm treatment(p=0.095).During the COVID-19 pandemic period,the median monthly mean frequency of MNE was 1.33(0-7.33)in children using desmopressin melt versus 6(1.33-13)in those using alarm treatment(p<0.001).Conclusions:The COVID-19 pandemic and its accompanying psychological effects did not affect the treatment efficacy of desmopressin melt in children with primary MNE but did adversely affect that of enuresis alarms.

Choroidal thickness measurements with optical coherence tomography in branch retinal vein occlusion

AIM： To evaluate central macular thickness（CMT） and mean choroidal thickness（MCT） in eyes with branch retinal vein occlusion（BRVO）, before and after ranibizumab treatment using spectral domain-optical coherence tomography（SD-OCT）.·METHODS： Forty-two patients with unilateral BRVO and macular edema were included in this study. There were 25 men and 17 women. Using SD-OCT, choroidal thickness was measured at 500 μm intervals up to 1500μm temporal and nasal to the fovea. MCT was calculated based on the average of the 7 locations. All the eyes with BRVO were treated with intravitreal ranibizumab（0.5 mg/0.05 m L）. Comparisons between the BRVO and fellow eyes were analyzed using Mann-Whitney U test. Preinjection and post-injection measurements were analyzed using Wilcoxon test and repeated measure analysis.· RESULTS： At baseline, there was a significant difference between the BRVO and fellow eyes in MCT[BRVO eyes 245（165-330） μm, fellow eyes 229（157-327） μm]and CMT [BRVO eyes 463（266-899） μm, fellow eyes 235（148-378） μm（P =0.041, 0.0001, respectively）]. Following treatment, CMT [295（141-558） μm] and MCT [229（157-329） μm] decreased significantly compared to the baseline measurements（P =0.001, 0.006, respectively）.Also BCVA（log MAR） improved significantly（P=0.0001） in the BRVO eyes following treatment. After treatment CMT[BRVO eyes 295（141-558） μm, fellow eyes 234（157-351） μm]and MCT [BRVO eyes 229（157-329） μm, fellow eyes 233（162-286） μm] values did not reveal any significant difference in BRVO eyes and fellow eyes（P=0.051, 0.824,respectively）.· CONCLUSION： In eyes with BRVO, CMT and MCT values are greater than the fellow eyes, and decrease significantly following ranibizumab injection.

High-frequency resistance(HFR)is a critical quantity strongly related to a fuel cell system’s *** is beneficial to estimate the fuel cell system’s HFR from the measurable operating conditions without resorting to costly HFR measurement *** this study,we propose a data-driven approach for a real-time prediction of ***,we use a long short-term memory(LSTM)based machine learning model that takes into account both the current and past states of the fuel cell,as characterized through a set of *** sensor signals form the input to the *** data is experimentally collected from a vehicle lab that operates a 100 kW automotive fuel cell stack running on an automotive-scale test *** current results indicate that our prediction model achieves high accuracy HFR predictions and outperforms other frequently used regression *** also study the effect of the extracted features generated by our LSTM *** study finds that only very few dimensions of the extracted feature are influential in HFR *** study highlights the potential to monitor HFR condition accurately and timely on a car.

Neuroendocrine tumor incidentally detected during living donor hepatectomy: A case report and review of literature

To our best knowledge, no case of a tumor that was incidentally detected during living donor hepatectomy(LDH) has been reported in the English language medical literature. We present two cases in which grade Ⅰ neuroendocrine tumors(NET) were incidentally detected during our twelve-year LDH experience. First Case: A 26-yearold male underwent LDH for his brother suffering from HBV-related chronic liver disease(CLD). After right lobe LDH, intestinal length was measured as part of a study concerning the relationship between small intestinal lengths and surgical procedure. At this stage, a mass lesion with a size of 10 mm × 10 mm was detected on the antimesenteric surface, approximately 90 cm proximal to the ileocecal valve. A wedge resection with primary intestinal anastomosis was performed. Second Case: A 29-year-old male underwent right lobe LDH for his father with hepatitis B virus(HBV)-related CLD. An abdominal exploration immediately prior to the closure of the incision revealed that the appendix vermiformis was edematous and had firmness with a size of 8-10 mm at its tip. An appendectomy was performed. The pathological examinations of the specimens of both patients revealedgrade 1 NET. In conclusion, even if patients undergoing LDH are healthy individuals, whole abdominal cavity should be gently palpated and all findings recorded after completing laparotomy. Suspected masses or lesions should be confirmed by frozen section examination. Such an approach would avert potential medicolegal issues.

Cu2Se based thermoelectric materials are of great potential for high-temperature energy harvesting due to their high-temperature figure-of-merit(zT).For further development of Cu2Se,both engineering and mid-temperature figure-of-merit need to be *** this work,we report that carbon-coated boron(C/B)nanoparticles incorporation can significantly improve both mid-and high-temperature zT in *** nanoparticle inclusions can result in a homogeneous distribution of Cu:C:B interfaces responsible for both improvement of the Seebeck coefficient and significantly reduction in thermal *** mid-and high temperature thermoelectric performance with zT=1.7 at 700 K and 2.23 at 1000 K as well as significantly improved engineering zT are achieved in the C/B incorporated Cu2Se with desirable mechanical properties and cycling *** findings will stimulate further study and exploration for the Cu2Se based thermoelectric materials for broad applications in converting waste heat to electricity with competitive energy conversion efficiency.

A Simple Jerky Dynamics,Genesio System

The third order explicit autonomous differential equations named as jerk equations represent an interesting subclass of dynamical systems that can exhibit many major features of the regular and chaotic motion. In this paper, we show that an algebraically simple system, the Genesio system can be recast into a jerky dynamics and its jerk equation can be derived from one-dimensional Newtonian equation. We also investigate the global dynamical properties of the corresponding jerk system.

Soil-Structure Interaction under the Effect of High Groundwater Table

This paper gives an account of a study performed for the raft foundation of a commercial building of considerable height and area. A raft 175?m long was designed without due consideration to the buoyancy effect due to high groundwater table as the building is near the sea. Although the raft was designed as an uninterrupted system, the designer used different, and insufficient, thicknesses for the foundation in order to lower costs. A 3D study was subsequently undertaken to analyze the settlements of the raft using finite elements. There was reasonable agreement between the computed and the measured settlements. However, the front block of the raft was observed to float as soon as pumping for?lowering of the groundwater table was halted. This instigated the analyzers to tie this portion of the raft to the surrounding piled curtain that was used for excavation of the foundation pit, by means of reinforced concrete beams. The computations show that?the?heave of the floor was restrained at acceptable levels. It is planned to stop pumping in the near future and compare the computed and measured vertical movements.
