Investigation of possible risk factors in the development of seasonal allergic conjunctivitis

AIM： To analyze the possible risk factors in the development of seasonal allergic conjunctivitis（SAC） through an evaluation of skin allergy tests and data obtained from ***： The study included a total of 75 SAC patients and 71 control subjects without SAC diagnosis who were admitted to the Abant Izzet Baysal University Medical Faculty Ophthalmology Clinic between March 2016 and December 2016. Skin prick tests were performed for all participants. Serum levels of total IgE and 25-OH vitamin D were also measured. In the tear, total IgE levels were measured. Moreover, possible risk factors for the onset of SAC（smoking, paracetamol exposure, vitamin D supplementation and environmental factors etc.） were examined for all patients by both prenatal and postnatal ***： The patients with SAC were found to have a history of maternal paracetamol exposure during the prenatal period. Likewise, in the same patient group, the duration of postnatal vitamin D supplementation was shorter（P〈0.001）. However, no significant correlation was found between SAC and maternal antibiotic exposure, maternal smoking, the mode of delivery and birth weight, as well as presence of pets. Moreover, patients with SAC were more likely to have asthma, allergic rhinitis and oral allergy syndrome. We have also found that SAC patients＇ mothers and siblings were more likely to have allergic conjunctivitis. Likewise, their fathers were more likely to have allergic rhinitis. CONCLUSION： Prenatal maternal paracetamol exposure and shorter duration of vitamin D supplementation in the postnatal period may play a role in development of SAC. Therefore prevention of unnecessary gestational paracetamol intake and vitamin D supplementation during infancy could potentially reduce the onset and development of SAC.

Alanine aminotransferase normalization at week 8 predicts viral response during hepatitis C treatment

AIM:To investigate alanine aminotransferase(ALT)and sustained virological response(SVR)in chronic hepatitis C(CHC)during peginterferon-ribavirin ***:One hundred and fifty-one genotype 1CHC patients underwent treatment for 48 wk with peginterferon and ribavirin,and were retrospectively divided into two groups as having a rapid virological response(RVR)(Group 1,n=52)and not having an RVR(Group 2,n=99).We also subdivided each group into two according to the initial ALT level being high(Group1h and Group 2h)or normal(Group 1n and Group 2n).HCV RNA and ALT levels were measured at baseline;at 4,12,24 and 48 wk during the treatment period;and at 24 wk *** levels were also obtained at 8 *** to the results of ALT,patients were enrolled in either the follow-up abnormal or follow-up normalized ALT groups at each *** with high and normal ALT levels were compared for each interval in terms of ***:The SVR rates were 83%vs 40%(P=0.000),82%vs 84%(P=0.830),and 37%vs 44%(P=0.466)when comparing Group 1 with 2,1h with1n,and 2h with 2n,*** Group 2h,the SVR rates were 34%vs 40%(P=0.701),11%vs 52%(P=0.004),12%vs 50%(P=0.007),7%vs 50%(P=0.003),6%vs 53%(P=0.001),and 0%vs 64%(P=0.000)when comparing patients with high and normalized ALT levels at week 4,8,12,24,48 and 72,*** multiple logistic regression analysis revealed that RVR(OR=7.05;95%CI:3.1-16.05,P=0.000),complete early virological response(cEVR)(OR=17.55;95%CI:6.32-48.76,P=0.000),normalization of ALT at8 wk(OR=3.04;95%CI:1.31-7.06,P=0.008),and at 12 wk(OR=4.21;95%CI:1.65-10.76,P=0.002)were identified as independent significant predictive factors for ***:Normalization of ALT at 8 wk may predict viral response during peginterferon-ribavirin treatment in genotype-1 CHC patients especially without RVR.

Effect of nepafenac 0.1% on retinal thickness after cataract surgery in patients without risk factors for cystoid macular edema

AIM:To evaluate the effect of topical preoperative nepafenac 0.1%treatment on postoperative macular edema using optical coherence tomography(OCT)after uncomplicated cataract ***:Ninety eyes of 90 patients without any risk factors were included in the *** patients were assigned to three groups:group 1,treated with topical prednisolone acetate 1%;group 2,treated with topical nepafenac 0.1%in addition to prednisolone acetate(1%);and group 3,those who started receiving nepafenac 0.1%treatment 3 d prior to surgery and continued the treatment postoperatively in addition to prednisolone acetate(1%).Central retinal thickness(CRT)and macular volume values were recorded using OCT at weeks 3 and ***:The increases in macular volume in the central 1 mm area after 3 and 6 wk were significantly lower in patients who used prophylactic topical nepafenac preoperatively(group 3)compared with those in group 1(P=0.028 and 0.008,respectively).No significant differences in the increase in macular volume and CRT were noted between groups 2 and 3(P>0.05).In group 1,the increases in macular volume in the central 3 mm area at weeks 3 and 6 were significantly higher than that in group 2 and 3(3rd week,P=0.004;6th week,P=0.005).CONCLUSION:Nepafenac 0.1%treatment in addition to topical steroids after uncomplicated cataract surgery reduce the increase in macular volume in the early postoperative period.

Comparison of the effect of intravitreal bevacizumab and intravitreal fasudil on retinal VEGF,TNFα,and caspase 3 levels in an experimental diabetes model

AIM: To evaluate the influence of an intravitreal injection of bevacizumab and fasudil on the retinal vascular endothelial growth factor(VEGF),tumor necrosis factor alpha(TNFα),and caspase 3 levels in a diabetic rabbit model. · METHODS: The study included 6 healthy rabbits(Group 1),6 rabbits with experimentally induced diabetes mellitus(DM)(Group 2),7 rabbits with experimentally induced DM to which intravitreal bevacizumab was administered(Group 3),and 7 rabbits with experimentally induced DM to which intravitreal fasudil was administered(Group 4). An intravitreal injection of 1.25mg/50μL bevacizumab in the right eye of rabbits in Group 3 and an intravitreal injection of 0.0064mg/50μL fasudil in the right eye of rabbits in Group 4 were administered on day 21 after the induction of DM. The studied eyes of the rabbits were enucleated three days after the intravitreal injection. The TNFα,VEGF,and caspase 3 levels were determined using the ELISA method. · RESULTS: There was a statistically significant difference in the VEGF and caspase 3 levels between groups(P =0.005 and P =0.013,respectively),but the TNF α level did not differ significantly between groups(P = 0.792). It was found that VEGF levels were significantly lower in Group 1 and Group 3 than in Group 2 using the Mann-Whitney U test with the Bonferroni correction(P = 0.004 for both comparison). There was no statistically significant difference between other groups with regard to VEGF levels(the P value ranged between 0.015 and 0.886). Although the P values of the caspase 3 levels were 0.015 for Group 1 and Group 4,0.038 for Group 2and Group 3,and 0.018 for Group 3 and Group 4,these P values remained above the threshold P value of 0.0083,which was the statistically significant level for post hoc tests. ·CONCLUSION: An intravitreal injection of bevacizumab decreased both the VEGF level,which plays a role in angiogenesis,and the caspase 3 level,which plays a role in apoptosis. Streamlined CFD simulation framework to generate wind-pressure coefficients on building facades for airflow network simulations

Building energy modeling software generally comes with capable airflow network solvers for natural ventilation evaluation in multi-zone building energy models. These approaches rely on arrays of pressure coefficients representing different wind directions derived from simple box-shaped buildings without contextual obstructions. For urban or obstructed sites, or more complex building shapes, however, further evaluation is needed to avoid geometric oversimplification. In this study, we present an automated and easy-to-use simulation workflow for OpenFOAM-based exterior airflow simulations to generate arrays of pressure coefficients for arbitrary building shapes and contextual situations. The workflow is compared to other methods commonly used to obtain pressure coefficients for natural ventilation analysis. Finally, we assess for which climate zones and building types modelers should rely on more accurate CFD-based pressure coefficients and where it may be justifiable to rely on easier and readily available analytical approaches to determine pressure coefficients. Results suggest that existing workflows lead to significant error in predicted comfort hours for climates in the Global South and modelers should consider CFD-based facade pressure coeficients.

Comparative New Insight into the Tectonic Origin of Folds and Thrust Faults of an Extensional Basin: Soke-Kusadasi Basin, Aegean, Western Turkey

The Aegean area of the western Anatolian region of Turkey,controlled by the low-angle detachment normal fault system,forms an extensional province,the West Anatolian Extensional Province(WAEP).The tectonic deformation which occurred in the Miocene Period,including the Plio–Quaternary Period has created different structures in both the basement rocks and intra-basin deposits of the *** of these structures,high-angle normal faults,controls the supradetachment Soke-Kusadasi Basin(SKB).Within this basin,there are folds with different axes and thrust faults with a north-northwestnortheast(N,NW,NE)*** folds and thrust faults in the SKB deformed the sedimentary structures of intra-basin *** folds and thrust faults,which caused the rotation of beddings and imbrications in the SKB,are mainly associated with the tectonic process of the low angle detachment normal fault,which affected the SKB and the Aegean part of western *** the SKB,during the process of extensional deformation associated with primary low angle detachment normal faulting,the ramp-flat and inversion geometry observed in the basement rocks and basin deposits of the crust caused folds and thrust faults in only intra-basin *** the WAEP,it is determined for the first time that the folds and thrust faults causing limited shortening deformed the Plio–Quaternary sediments.

Axial gas mixing in conical spouted beds with high density particles

Conical spouted beds operating with high-density particles(ρp>2500 kg/m^(3))have recently gained attention because of their potential use as nuclear fuel coaters for next-generation nuclear *** the literature,the number of axial gas mixing studies in conical and conical-cylindrical spouted beds is very limited and all axial mixing studies were carried out with relatively light particles(ρp≤2500 kg/m^(3)).Therefore,the objective of this study was to generate experimental data that can be used to explain the gas axial mixing behavior in conical spouted beds operating with both low-and high-density *** were conducted in two(γ=30°,60°)conical spouted beds with three different types of particles:zirconia(ρp=6050 kg/m^(3)),zirconia toughened alumina(ρp=3700 kg/m^(3))and glass beads(ρp=2460 kg/m^(3)).In order to be able to compare experimental data obtained at different conditions,a 1-D convection-diffusion gas mixing model originally developed by San Joséet al.(1995)was implemented to determine the axial dispersion *** results show that the axial dispersion coefficients range between m^(2)/s and m^(2)/s,increase with superficial gas velocity and are higher than the corresponding dispersion coefficients of fixed beds,lower than the dispersion coefficients of fluidized beds and in the same range with the cylindrical spouted beds reported in the *** corresponding Peclet numbers were in the range of 0.6–7.8 for all operating conditions and slightly higher Peclet numbers were obtained with glass beads indicating the relative importance of gas convective transport over gas dispersion for light particles compared to heavy particles.

Effect of a semi electro-mechanical engine valve on performance and emissions in a single cylinder spark ignited engine

In this study, an electro-mechanical valve （EMV） system for the intake valve ofa fottr stroke, single cylinder, overhead valve and spark ignition （SI） engine was designed and constructed. An engine with the EMV system and a standard engine were tested to observe the effects of the EMV on engine performance and emissions at different speeds under full load. The EMV engine showed improved engine power, engine torque and break specific fuel consumption （BSFC）. A 66% decrease in CO emissions was also obtained with the EMV system, but hydrocarbons （HC） and NOx emissions increased by 12% and 13% respectively.

Assesment of Radial Artery by Means of Doppler Ultrasonography and Soft Tissue Roentgenogram in End-Stage Renal Disease Patients

Background: Although the radial artery (RA) is mostly selected for the creating of arterio-venous fistula (AVF) in end-stage renal disease patients (ESRDP), it still represents a suitable site to simply assess systemic calcification. Our goal of this study was to research the RA before the arteriovenous fistula creation in ESRDP and evaluate the risk factor determinants. Material and Methods: The RA has been determined to estimate vascular calcification in 67 ESRDPs on dialysis by doppler ultrasonography (USG). If there was any pathologic conditions, direct examination of the forehand soft tissue roentgenograms as a simply method was made. Preoperative spectral Doppler sonography measured radial artery peak systolic velocity during tight fist clenching for 3 minutes and after fist relaxation. Vessel diameters and peak systolic velocity were assessed for predictive value based on subsequent fistula adequacy. RA calcification was scored from Grade 0 which was defined as no calcification to Grade 4 which was described as severe calcification from proximal to distal end of RA. Results: Doppler USG has shown that the major or minor RA calcification was detected in 11 patients (16.5%). In these patients, direct roentgenogram also demonstrated RA calcific disease. Total and LDL cholesterol levels, gender and smoking status have no influence on calcification scores. When the dialysis periods are concerned, there was no correlation with the RA calcification. Serum calcium and phosphate levels, rather than calcium-phosphate products, and age were correlated with high calcification scores. The highest prevalence of RA calcification was also found in diabetic group. Conclusion: Our data indicate that RA calcification is highly prevalent among ESRDP, with the highest prevalence among diabetics.
