Robotic approach with neoadjuvant chemotherapy in adult Wilms’ tumor: A feasibility study report and a systematic review of the literature

Objective:The incidence of Wilms’tumor(WT)among adult individuals accounts for less than 1%of kidney cancer cases,with a prognosis usually less favorable when compared to younger individuals and an overall survival rate of 70%for the adult patients versus 90%for the pediatric cases.The diagnosis and treatment of WT are complex in the preoperative setting;neoadjuvant chemotherapy(NAC)or robotic surgery has rarely been described.This study aimed to review the literature of robotic surgery in WT and report the first adult WT management using both NAC and robotic strategy.Methods:We reported a case of WT managed in a multidisciplinary setting.Furthermore,according to Preferred Reporting Items for Systematic reviews and Meta-Analyses recommendations,a systematic review of the literature until August 2020 of WT treated with a robotic approach was carried out.Results:A 33-year-old female had a diagnosis of WT.She was scheduled to NAC,and according to the clinical and radiological response to a robotic radical nephrectomy with aortic lymph nodes dissection,she was managed with no intraoperative rupture,a favorable surgical outcome,and a follow-up of 25 months,which did not show any recurrence.The systematic review identified a total number of 230 cases of minimally invasive surgery reported in the literature for WT.Of these,approximately 15 patients were carried out using robotic surgery in adolescents while none in adults.Moreover,NAC has not been administered before minimally invasive surgery in adults up until now.Conclusion:WT is a rare condition in adults with only a few cases treated with either NAC or minimally invasive approach so far.The advantage of NAC followed by the robotic approach could lead to favorable outcomes in this complex scenario.Notwithstanding,additional cases of adult WT need to be identified and investigated to improve the oncological outcome.

BACKGROUND Non-clear cell(ncc)metastatic renal-cell carcinoma(RCC)has dismal results with standard systemic therapies and a generally worse prognosis when compared to its clear-cell counterpart.New systemic combination therapies have emerged for metastatic RCC(mRCC),but the pivotal phase III trials excluded patients with nccRCC,which constitute about 30%of metastatic RCC cases.AIM To provide a piece of real-life evidence on the use of pazopanib in this patient subgroup.METHODS The present study is a multicenter retrospective observational analysis aiming to assess the activity,efficacy,and safety of pazopanib as first-line therapy for advanced nccRCC patients treated in a real-life setting.RESULTS Overall,48 patients were included.At the median follow-up of 40.6 mo,the objective response rate was 27.1%,the disease control rate was 83.3%,and the median progression-free survival and overall survival were 12.3(95%confidence interval[CI]:3.6-20.9)and 27.7(95%CI:18.2-37.1)mo,respectively.Grade 3 adverse events occurred in 20%of patients,and no grade 4 or 5 toxicities were found.CONCLUSION Pazopanib should be considered as a good first-line option for metastatic RCC with variant histology.

The impact of heterogeneity and pore network characteristics on single and multi-phase fluid propagation in complex porous media:An X-ray computed tomography study

This study investigates the impact of pore network characteristics on fluid flow through complex and heterogeneous porous media, providing insights into the factors affecting fluid propagation in such systems. Specifically, high-resolution or micro X-ray computed tomography(CT) imaging techniques were utilized to examine outcrop stromatolite samples of the Lagoa Salgada, considered flow analogous to the Brazilian Pre-salt carbonate reservoirs. The petrophysical results comprised two distinct stromatolite depositional facies, the columnar and the fine-grained facies. By generating pore network model(PNM), the study quantified the relationship between key features of the porous system, including pore and throat radius, throat length, coordination number, shape factor, and pore volume. The study found that the less dense pore network of the columnar sample is typically characterized by larger pores and wider and longer throats but with a weaker connection of throats to pores. Both facies exhibited less variability in the radius of the pores and throats in comparison to throat length. Additionally, a series of core flooding experiments coupled with medical CT scanning was designed and conducted in the plug samples to assess flow propagation and saturation fields. The study revealed that the heterogeneity and presence of disconnected or dead-end pores significantly impacted the flow patterns and saturation.Two-phase flow patterns and oil saturation distribution reveal a preferential and heterogeneous displacement that mainly swept displaced fluid in some regions of plugs and bypassed it in others. The relation between saturation profiles, porosity profiles, and the number of fluid flow patterns for the samples was evident. Only for the columnar plug sample was the enhancement in recovery factor after shifting to lower salinity water injection(SB) observed.

BACKGROUND Prevalence of hepatocellular carcinoma(HCC)is increasing,especially in patients with metabolic dysfunctionassociated steatotic liver disease(MASLD).AIM To investigate rifaximin(RIF)effects on epigenetic/autophagy markers in animals.METHODS Adult Sprague-Dawley rats were randomly assigned(n=8,each)and treated from 5-16 wk:Control[standard diet,water plus gavage with vehicle(Veh)],HCC[high-fat choline deficient diet(HFCD),diethylnitrosamine(DEN)in drinking water and Veh gavage],and RIF[HFCD,DEN and RIF(50 mg/kg/d)gavage].Gene expression of epigenetic/autophagy markers and circulating miRNAs were obtained.RESULTS All HCC and RIF animals developed metabolic-dysfunction associated steatohepatitis fibrosis,and cirrhosis,but three RIF-group did not develop HCC.Comparing animals who developed HCC with those who did not,miR-122,miR-34a,tubulin alpha-1c(Tuba-1c),metalloproteinases-2(Mmp2),and metalloproteinases-9(Mmp9)were significantly higher in the HCC-group.The opposite occurred with Becn1,coactivator associated arginine methyltransferase-1(Carm1),enhancer of zeste homolog-2(Ezh2),autophagy-related factor LC3A/B(Map1 Lc3b),and p62/sequestosome-1(p62/SQSTM1)-protein.Comparing with controls,Map1 Lc3b,Becn1 and Ezh2 were lower in HCC and RIF-groups(P0.05).miR-122 was higher in HCC,and miR-34a in RIF compared to controls(P0.05).CONCLUSION RIF might have a possible beneficial effect on preventing/delaying liver carcinogenesis through epigenetic modulation in a rat model of MASLD

Tumor budding as a potential histopathological biomarker in colorectal cancer:Hype or hope?

Colorectal cancer(CRC),the third most commonly diagnosed type of cancer in men and women worldwide is recognized as a complex multi-pathway disease,an observation sustained by the fact that histologically identical tumors may have different outcome,including various response to therapy.Therefore,particularly in early and intermediate stage(stages Ⅱ and Ⅲ,respectively) CRC,there is a compelling need for biomarkers helpful of selecting patients with aggressive disease that might benefit from adjuvant and targeted therapy.Histopathological examination shows that likely other solid tumors the development and progression of human CRC is not only determined by genetically abnormal cells,but also by intricate interactions between malignant cells and the surrounding microenvironment.This has led to reconsider the features of tumor microenvironment as potential predictive and prognostic biomarkers.Among the histopathological biomarkers,tumor budding(i.e.,the presence of individual cells and small clusters of tumor cells at the tumor invasive front) has received much recent attention,particularly in the setting of CRC.Although its acceptance as a reportable factor has been held back by a lack of uniformity with respect to qualitative and quantitative aspects,tumor budding is now considered as an independent adverse prognostic factor in CRC that may allow for stratification of patients into risk categories more meaningful than those defined by tumor-node-metastasis staging alone,and also potentially guide treatment decisions,especially in T2-T3 N0(stage Ⅱ) CRCs.

Chemical speciation of copper and manganese in solution of a copper-contaminated soil and young grapevine growth with amendment application

Copper(Cu)-based fungicide application to control foliar diseases in grapevine(Vitis vinifera L.)can increase soil Cu availability.Brazilian soils have high natural Cu and manganese(Mn)concentrations,increasing the potential for toxicity to grapevine plants.Application of amendment substances can reduce the concentrations of toxic chemical species of Cu and Mn in soil solution,especially for the soils grown with young plants.We evaluated the chemical speciation of Cu and Mn in soil solution and young grapevine growth using a contaminated soil with amendment application.Grapevine seedlings of Paulsen 1103rootstock were grown for 12 months on an old vineyard(>30 years of cultivation)soil without(control)and with vermicompost or limestone application.Grapevine dry matter,root morphological characteristics,and plant tissue Cu and Mn concentrations were evaluated.The cation and anion concentrations,dissolved organic carbon,p H,and chemical speciation of Cu and Mn in soil solution from different soil layers were analyzed.Vermicompost application did not reduce Cu and Mn phytotoxicity,and grapevine plants died after winter pruning.High soluble Mn concentration in soil(5.56 mg L-1in 0–45 cm soil layer)and Mn concentration in plant tissue at pruning(380.8 mg kg-1)might be the primary cause of plant death.Limestone application reduced Cu2+and Mn2+chemical species in soil solution,thereby increasing plant growth.The Cu2+and Mn2+species were negatively correlated(P<0.05)with grapevine shoot production at pruning(r=-0.92 and-0.97,respectively)and at the end of the trial(r=-0.73 and-0.51,respectively).Limestone application increased grapevine shoot and root dry matter production by 36%and 41%,respectively,while also presenting the highest root length and volume in the 5–10 cm soil layer.Limestone application is a viable and profitable alternative for reducing soil Cu and Mn availability and their phytotoxic effects.

Risk Seeking Behavior in Anticipation to Volatility Mean Reversal:A Case Study Using Bitcoin and SKEW

The traditional finance approach to decision analysis,based on neo-classical economics,assumes self-interested,utility maximizing approach,and risk aversion.This essay points to a situation that investment in a risky asset(Bitcoin)is directly related to an increase in market risk,measured by SKEW index.This contradicts the traditional approach and aligns to several findings of behavioral finance.More specifically,it shows that investors may be risk seeking actors in anticipation to a belief that volatility will return to normal levels.

Role of blood AFP mRNA and tumor grade in the preoperative prognostic evaluation of patients with hepatoceiluiar carcinoma

AIM: To explore the potential prognostic role of preoperative tumor grade and blood AFP mRNA in a cohort of patients with hepatocellular carcinoma (HCC)eligible for radical therapies according to a well-defined treatment algorithm not including nodule size and number as absolute selection criteria.METHODS: Fifty patients with a diagnosis of HCC were prospectively enrolled in the study. Inclusion criteria were: (1) histological assessment of tumor grade by means of percutaneous biopsies; (2) determination of AFP mRNA status in the blood; (3) patient's eligibility for radical therapies.RESULTS: At preoperative evaluation, 54% of the study group had a well-differentiated HCC, 42% had AFP mRNA in the blood, 40% had a tumor larger than 5 cm and 56% had more than one nodule. Surgery (resection or liver transplantation) was performed in 29 patients,while 21 had percutaneous ablation procedures. After a median follow-up of 28 mo, 12-, 24-, and 36-mo survival rates were 78%, 58%, and 51%, respectively. Surgical therapy, performance status and three tumor-related variables (AFP mRNA, HCC grade and gross vascular invasion) resulted as significant survival predictors at univariate analysis. Nodule size and number did not perform as significant prognosticators. Multivariate study selected only surgical therapy and a biologically early HCC profile (AFP mRNA negative and well-differentiated tumor without gross vascular invasion) as independent survival variables.CONCLUSION: The preoperative determination of tumor grade and blood AFP mRNA status may potentially refine the prognostic evaluation of HCC patients and improve the selection process for radical therapies.

The Impact of Credit Ratings on Financial Performance (ROA) and Value Creation (Tobin’s Q)

This study employs a bibliometric and systematic approach to examine the impact of credit ratings as a measure of financial performance for companies listed in the S&P 500 index.The study identified a knowledge gap as only two researches were found,one suggesting and another using credit ratings to measure financial performance.Most researches use leverage,profitability,liquidity,and Share Return measures to explain financial performance.The empirical analysis uses the data of 2,398 observations of 240 companies rated by S&P Global Ratings for the period 2009-2013,applying a Generalized Method of Moments(GMM)methodology to estimate the models due to its ability to address potential endogeneity issues.The study considers Return on Assets(ROA)and Tobin’s Q as dependent variables.It incorporates credit ratings(CRWLTA)along with variables such as Total Debt to Total Assets(TDTA),Total Shareholder Return(TSR),EBITDA Interest coverage(EBITDAICOV),Quick Ratio(QR),Altman’s Z-Score(AZS),as well as macroeconomic factors like Gross Domestic Product(GDP)growth,inflation(Consumer Price Index-CPI),and the Federal Reserve Interest Rate(FDRI)as independent variables.The study argues that credit ratings,which incorporate historical data and confidential information about companies’strategies,provide reliable forward-looking creditworthiness assessments to the market.It is supported by specialized rating agencies that employ their methodologies.However,the findings suggested that CRWLTA,had a negative relationship with Q Tobin,although it was not statistically significant,and a negative relationship with ROA that was on the verge of significance.

Sudden cardiac death (SCD) remains a leading mode of death in western countries.Since SCD can be the first and last clinical presentation of the underlying disease,autopsy could be the only medical examination available for early diagnosis and it should be performed according to the guidelines of the Association for European Cardiovascular Pathology.Although the vast majority of SCD are due to coronary artery disease,non-ischemic causes of SCD do exist and are prevalent in young people with structural (i.***.arrhythmogenic,hypertrophic and inflammatory cardiomyopathy) and non-structural (ion channel diseases)cardiomyopathies,accounting for up to one half of cases.A standardized autopsy protocol,in combination with blood sampling to ensure feasibility of postmortem molecular testing if needed,is mandatory.The pathologist is called to provide the correct diagnosis and to advice the relatives on the need of a cascade clinical and genetic screening in the presence of a heredo-familial disease.
