Emerging role of liquid biopsy in rat sarcoma virus mutated metastatic colorectal cancer:A case report

BACKGROUND In patients with metastatic colorectal cancer(mCRC),the treatment options are limited and have been proved to be affected by rat sarcoma virus(ras)mutational *** ras wild-type(wt)patients,the combination of antiepidermal growth factor receptor(EGFR)monoclonal antibodies with chemotherapy(CT)is more effective than CT *** the other hand,ras-mutated patients are not eligible for treatment with anti-EGFR *** SUMMARY Eleven patients with initially ras-mutated mCRC were followed from diagnosis to May *** the time of cell-free DNA determination,five patients had undergone one CT line,five patients had undergone two CT lines,and one patient had undergone three CT lines(all in combination with bevacizumab).At the second and third treatment lines[second line(2L),third line(3L)],patients with neo-ras wt received a combination of CT and *** neo-ras wt patients treated with anti-EGFR,our findings indicated an increase in progression-free survival for both 2L and 3L(14.5 mo,P=0.119 and 3.9 mo,P=0.882,respectively).Regarding 2L overall survival,we registered a slight increase in neo-ras wt patients treated with anti-EGFR(33.6 mo vs 32.4 mo,P=0.385).At data cut-off,two patients were still alive:A ras-mutated patient undergoing 3L treatment and a neo-ras wt patient who received 2L treatment with anti-EGFR(ongoing).CONCLUSION Our case series demonstrated that monitoring ras mutations in mCRC by liquid biopsy may provide an additional treatment line for neo-ras wt patients.

Oscillating droplet tribometer for sensitive and reliable wetting characterization of superhydrophobic surfaces

Accurate wetting characterization is crucial for the development of next‐generation superhydrophobic ***,wetting properties are measured with a contact angle goniometer(CAG)suitable for a broad range of ***,due to optical errors and challenges in baseline positioning,the CAG method suffers from inaccuracies on superhydrophobic *** we present an improved version of the oscillating droplet tribometer(ODT),which can reliably assess wetting properties on superhydrophobic surfaces by measuring the frictional forces of a water‐based ferrofluid droplet oscillating in a magnetic *** demonstrate that ODT has superior accuracy compared to CAG by measuring the wetting properties of four different superhydrophobic surfaces(commercial Glaco and Hydrobead coatings,black silicon coated with fluoropolymer,and nanostructured copper modified with lauric acid).We show that ODT can detect the small but significant changes in wetting properties caused by the thermal restructuring of surfaces that are undetectable by *** more,unlike any other wetting characterization technique,ODT features an inverse sensitivity:the more repellent the surface,the lower the error of measurement,which was demonstrated by experiments and simulations.

Sums of Primes and Quadratic Linear Recurrence Sequences

Let u be a sequence of positive integers which grows essentially as a geometric progression. We give a criterion on u in terms of its distribution modulo d, d = 1, 2,..., under which the set of positive integers expressible by the sum of a prime number and an element of u has a positive lower density. This criterion is then checked for some second order linear recurrence sequences. It follows, for instance, that the set of positive integers of the form p ＋ [（2 ＋ √3）n], where p is a prime number and n is a positive integer, has a positive lower density. This generalizes a recent result of Enoch Lee. In passing, we show that the periods of linear recurrence sequences of order m modulo a prime number p cannot be ＂too small＂ for most prime numbers p.

The author constructed a transformer Scp4 of functional programs. The transformer uses the technology known as Turchin's supercompilation. Scp4 was implemented in a functional language Refal 5. The input language for Scp4 is also Refal 5. In the present paper we give an outline of the supercompiler and consider a number of tools of the transformer in details. The tools can be formally thought of as quasi distributive laws.

研究目的:疼痛为运动损伤中骨骼肌肉系统损伤的常见症状,疼痛的性质与程度可影响局部组织结构与功能、远端组织结构与功能及其他的系统性症状。当出现局部疼痛时,患者的整体姿势、稳定性、呼吸模式及运动表现都会受到影响,会出现局部不稳定和其他部位代偿的情况,从而进一步增加运动损伤风险,延迟康复进度。因此对疼痛症状的关注和管理在运动损伤的骨骼肌肉系统疾病中尤为重要。疼痛阈(Pain Threshold, PT)为疼痛管理中一重要概念,当前国内外许多研究皆通过局部疼痛强度和疼痛阈的变化作为某治疗方法对疼痛管理的评判标准。当前研究指出,指压手法可通过调节下行疼痛通路起到缓解症状的效果,另外,拮抗肌等长收缩可以通过交互抑制原理对肌肉产生放松作用,或可抑制疼痛。本研究旨在探讨两者结合的综合治疗方法对疼痛管理的效果。探讨指压结合拮抗肌等长收缩对腓肠肌疼痛阈的影响效果,探究骨骼肌肉疼痛调节的更有效干预手段。研究方法:18名健康大学生随机分为对照组和实验组。对照组方案为进使用指压手法按压腓肠肌外侧头肌腹进行治疗,实验组方案为使用指压手法按压腓肠肌外侧头肌腹同时进行50%最大背屈肌力的足背屈。实验在Biodex仪器上进行,起始姿势为受试者右下肢固定于髋关节屈曲90°,膝关节屈曲120°,踝关节屈曲90°位。受试者利用Biodex system 3进行最大等长跖屈力量(Maximal Plantarflexion Force, MPF)测试,随后利用压力测试仪进行疼痛阈(Pain Threshold,PT)测试,疼痛阈定义为受试者主观疼痛在数字疼痛量表(Numeric Pain Rating Scale, NRS)上达到5级(5/10NRS)。随后进行实验干预。对照组:实验者向受试者右下肢腓肠肌外侧头施加指压治疗手法,起始力量为疼痛阈(5/10NRS)力度,随后保持指压力量两分钟。受试者在实验30s,1min,1min30s,2min时报告NRS疼痛值。实验组:在对照组的基础上,受试者行10s踝关节50%1RM等长背屈,随后休息10s,等长收缩与休息交替进行直到指压治疗结束。试验后再次进行最大等长跖屈力量测试和疼痛阈测试。实验前后疼痛阈测试分别施行三次,结果取平均值;最大等长跖屈力量测试分别测试三次,结果取最大值。利用协方差分析进行干预前后疼痛阈组间结果分析,利用配对样本t检验进行干预前后组内疼痛阈组间结果分析,利用重复测量的方差分析对实验中NRS值进行数据分析,数据统计结果标准为:p0.50。研究结果:(1)与实验前相比,实验组与对照组的疼痛阈无显著变化(p>0.05,r=0.026),最大等长跖屈力量未见显著变化(p>0.05,r=0.296);(2)在实验中,对照组与实验组的NRS值明显下降(p<0.05,ηp2=0.952),下降效果随着干预时间的延长而增强,在2min时达到最大(实验组NRS2min=1.722±0.364,95%CI:0.951-0.493;对照组NRS2min=2.667±0.364,95%CI:1.896-3.438),分组对NRS结果存在主效应影响(p<0.05,ηp2=0.315),且分组与干预时间存在明显交互作用(p=0.021,ηp2=0.563)。讨论:结果显示,指压法与拮抗肌等长收缩的交互抑制法均可对疼痛起到改善作用。其中,指压法的作用机制主要在于对局部循环和疼痛通路的影响。首先,按压可导致局部血液循环改变,继而引起短暂局部缺血情况,但在按压结束后,局部血流会增加,继而提高局部代谢水平,降低局部炎性产物,从而降低疼痛感受器传入信号,起到缓解症状的作用。另外,按压可刺激疼痛感受器和机械感受器,机械感受器突触可通过"门控理论(GateTheory)"于脊髓后角抑制疼痛感受器信号的传递,指压法产生对疼痛感受器产生的持续刺激可进一步激活和强化室周灰质(PeriventricularGrayMatter,PVG)、水管周围灰质(PeriaueductalGrayMatter,PAG)、蓝斑(LocusCoeruleus)、网状结构(ReticularFormation)及大脊核(RapheNucleusMagnus)的下行抑制通路信号,使以上中枢系统产生更多5-羟色胺、去甲肾上腺素至脊髓后角,降低疼痛感受器信号、抑制信号传递,并激活脊髓后角的中间神经元,从而进一步抑制疼痛。拮抗肌(足背屈方向)的等长收缩可以通过γ系统调控(GammaLoopSystem)引起交互抑制,从而引起腓肠肌肌张力的下降。拮抗肌的等长收缩可降低H反射(H-reflex),证明交互抑制可引起肌张力和肌肉刚度(MuscleStiffness)的降低,因此,拮抗肌的等长收缩引起的放松作用或为在治疗中疼痛程度的原因。另外,疼痛阈结

在过去的几年里,电催化氮还原反应(eNRR)吸引了大量的研究兴趣.尽管如此,NH3的产量和选择性仍然没有达到实际应用的标准.本论文报道了成分为Ba_(x)(FeCoNiZrY)_(0.2)O_(3−δ)(B_(x)(FCNZY)_(0.2)(x=0.9,1))的高熵钙钛矿作为eNRR催化剂的新材料研究平台.通过改变A位金属元素的非化学计量比,使材料产生更高密度的氧缺陷,进而提升氮气还原性能.B_(0.9)(FCNZY)_(0.2)的NH_(3)产率和法拉第效率是B(FCNZY)_(0.2)的1.51和1.95倍.理论上,利用d-带中心理论预测了B-位点的催化活性中心,并确定了镍元素为催化位点.最佳远端反应途径中的中间状态的自由能值表明,第三个质子化步骤(*NNH_(2)→*NNH_(3))是决定速率的步骤,高熵钙钛矿氧化物中氧空位的增加对氮的吸附和还原都有贡献.这项工作为具有多个活性位点的高熵结构应用于电催化固氮提供了一个新的研究框架.

Fuzzy Controller for Dual Sensors Cardiac Pacemaker System in Patients with Bradycardias at Rest

Cardiovascular disease is defined as a heart rate that is less than 60 bpm. Implantable cardiac devices such as pacemakers are widely used nowadays. In this paper, design and implementation of the heart model can be controlled to be the heart of a patient suffering from a decrease in heart rate (Bradycardia). A system is designed to sense and calculate the heart rate per minute and it is considered as an input to the controller. The design and implementation of Mamdani fuzzy controller to generate electric pulses that mimic the natural pacing system of the heart maintains an adequate heart rate by delivering controlled, rhythmic electrical stimuli to the chambers of the patient heart. The proposed controller is tested by using Matlab/Simulink program.
