hepatitis C is the most common indication for orthotopic liver transplantation in the United States. Unfortunately, hepatitis C recurs universally in the transplanted liver and is the major cause of decreased graft and patient survival. The combination therapy of interferon and ribavirin has been shown to be the most effective therapy for recurrent hepatitis C. however, pre-and post-transplant hypersplenism often precludes patients from receiving the antiviral therapy. Splenectomy and partial splenic embolization are the two invasive modalities that can correct the cytopenia associated with hypersplenism. In this report we review the two treatment options, their associated outcomes and complications.

AIM: To examine whether the fasting levels of serum gastrin-17 (G-17) are lower in Barrett's esophagus (BE)patients than in non-Barrett ***: Nineteen patients with BE (presenting with a tubular segment ≥2 cm long in lower esophagus and intestinal metaplasia of incomplete type ('specialized columnar epithelium') in endoscopic biopsies from the tubular segment below the squamocolumnar junction were collected prospectively from outpatients referred to diagnostic gastroscopy. The controls comprised 199 prospectively collected dyspeptic outpatients without BE or any endoscopically visible lesions in the upper GI *** levels of serum G-17 (G-17fast) were assayed with an EIA test using a Mab highly specific to amidated G-17. None of the patients and controls received therapy with PPIs or other antisecretory ***: The mean and median levels of G-17fast in serum were significantly lower (P = 0.001) in BE patients than in controls. The positive likelihood ratios (LR+) of low G-17fast to predict BE in the whole study population at G-17fast levels ＜0.5, ＜1, or ＜1.5 pmol/L were 3.5, 3.0,and 2.8, respectively. Among patients and controls with healthy stomach mucosa, the LR+ were 5.6, 3.8, and 2.6,respectively. In the whole study population, serum G-17 was below 2 pmol/L in 15 of 19 BE patients (79%). The corresponding prevalence was 66 of 199 (33%) in controls (P＜0.001). The G-17fast was 5 pmol/L or more in only one of the 19 BE patients (5%). In controls, 76 of the 199 patients (38%) had such high serum G-17fast levels (P＜0.01).CONCLUSION: Serum levels of G-17fast tend to be lower in native patients with BE than in healthy controls.

The intramolecular photochedcal interaction of a series of bis-coumarinyl compounds with various alkyl chain lengths (An) has been studied in/without the presence of β-cyclodextrin (β-CD) by fluorescence spectroscopy. The results show that both photedimerization andexcimer formation can occur in the respective favourable microenvironments.

Two-dimensional structure of density fluctuations is examined during the current jump phase, indicating a change from the open magnetic fields to the closed ones. During the smooth current ramp-up phase the two-dimensional contour of the LiI intensity shows vertically alignment, consistent with the magnetic surfaces. At the inflection point in Ip ramp-up the LiI intensity contour becomes flat in the observation regime and then suddenly a steep gradient and higher intensity regime are formed in the vertical direction. This higher intensity corresponds to a burst of LiI waveform. According to these changes in the contour, it is found that, within ~1 ms around the burst of LiI, a low frequency coherent wave with a long wavelength rapidly grows. The relations with other signals （magnetic flux and microwave stray power） are discussed with respect to the topological change in the magnetic configuration and mode conversion of the incident electromagnetic waves.

Excited states of125Sb have been studied using in-beam γ spectroscopy techniques via the124Sn(7Li, α2n) reaction at a beam energy of 32 MeV. A high-spin level scheme including 21 new γ-transitions and 14 new excited states have been established. Three isomers have been identified at 1970, 2110 and 2471 keV and the ranges of their half-lives have been estimated from the delayed coincidence data. The level structure of125Sb is discussed in terms of particle-core excitation coupling. With the help of empirical shell model calculations the three isomers are proposed to have three-quasiparticle πg7/2?v(h 11/2 s 1/2)5?, πg7/2?v(h 11/2 d 3/2)7? and πg7/2?v(h 11 2/2 )10 + configurations, respectively.

Objective To determine the diagnostic yield of repeated screening for atrial fibrillation(AF)among geriatric *** A pragmatic prospective cohort study into applying opportunistic screening for AF with a handheld single lead ECG device(SLD)in a geriatric *** patients of 65 years old and older visiting the geriatric outpatient clinic were eligible for inclusion.A 12 lead ECG was performed,followed by measurements with the SLD during every visit to the geriatric outpatient clinic.A frailty index was based on the accumulation of deficits *** 478 patients were *** were excluded if they did not give informed consent(17 patients),had a pacemaker or implantable cardioverter defibrillator(20 patients),or had incomplete medical files(two patients).After exclusion,439 patients participated in this *** mean age was 78 years(range 65 to 100 years),54%were *** was known in 89 patients(20%),first detected on the baseline ECG in four patients(1%)and first detected with the SLD in 20 patients(5%)during follow up *** of the SLD was 90.0%,specificity 99.0%,negative predictive value 99.7%,and positive predictive value 73.5%.Most patients(82%)with AF were frail and 53%were severely *** Repeated screening in geriatric patients has a five times higher diagnostic yield than usual *** was easily combined with usual *** of the positive predictive value of 73.5%,it remains necessary to confirm AF with a 12 lead ECG or 24-h holter monitoring.

For the success of PAL-XFEL, two critical systems, namely a low emittance injector and a variable gap out-vacuum undulator, are under development. In order to realize the target emittance of the PAL-XFEL injector we carried out an optimization study of various parameters, such as the laser beam transverse profile, the laser pulse length, the laser phase,and the gun energy. The transverse emittance measured at the Injector Test Facility(ITF) is εx=0.48±0.01 mm *** undulator prototype based on the EU-XFEL design and modified for PAL-XFEL was built and tested. A local-K pole tuning procedure was developed and tested. A significant reduction(90%) of the local-K fluctuation was observed. The requirement of undulator field reproducibility better than 2×10-4and the undulator gap setting accuracy below 1 μm were achieved for the prototype. The optical phase jitter after the pole height tuning at the tuning gap was calculated to be 2.6? rms, which satisfies the requirement of 5.0?.

Effects of oxygen and nitrogen on MC carbide in nickel-base superalloy K465 were investigated, because neither of the two gas elements was found to decide MC carbide morphology monotonically. The morphology of MC carbide under alloy solidified in conventional investment casting condition was observed to transform from script-type to block-type with the total oxygen and nitrogen content (gas content) in samples rising up from 1.2×10-6 mol/g to 2.6×10-6 mol/g. The area fraction and composition of the MC carbide also varied with increase of gas content. however, the morphology of MC carbide kept to be script-type even when gas content was up to 3.4×10-6 mol/g in samples solidified with a much higher cooling rate. These results indicate that effects of gas content on formation of MC carbide can be observed in specimens solidified with low cooling rate, but such changes in MC carbide cannot be found when the cooling rate is high.
