Place of Partial Arytenoidectomy in the Management of b.lateral Vocal Cord Immob.litis

Surgical management of laryngeal paralysis varies depending on whether the vocal cords are in ab.uction, adduction or paramedian position. Various surgical techniques have b.en describ.d including partial arytenoidectomy which is reported to give good surgical results that are stab.e over time. The ob.ective of the study was to analyze the surgical therapeutic elements of b.lateral paralysis, especially to assess partial arytenoidectomy, one of the most performed techniques. This was a descriptive retrospective study of cases of b.lateral immob.lity admitted b.tween January 1st 2008 and March 31st 2018 and treated surgically. Socio-demographic and therapeutic data were collected. The survey involved 46 patients, with an equal numb.r of male and female (23) with 50% of male patients and 23 patients were female, or a sex ratio of 1. The average age of the patients was 56 ± 17 years ranging b.tween 14 and 89 years. Posterior partial arytenoidectomy was the most widely performed surgical technique (26 patients or 56.5%), followed b. cordopexia or lateral-fixing of a vocal cord (19.6%) and posterior cordectomy (17.4%). Patients who received a partial arytenoidectomy and cordopexia had their vocal cords either in adduction or in the paramedian position. Those who received a posterior cordectomy had their vocal cords in adduction. 18 patients (39.13%) were taken to the operating theatre in less than 6 hours, 28 (60.9%) had no post-operative complications, and 9 patients received a surgical enlargement resumption. In post-operative follow-up, 11 patients suffered pulmonary aspiration corrected after speech therapy;26 patients (56.5%) did not. Partial arytenoidectomy remains the most performed surgical procedure in the management of closed b.lateral laryngeal paralysis at the Mont-Godinne University Hospital. It allows a reliab.e and durab.e b.eathing function over time with less impact on the voice.

Psychosocial Experience in Goiter Patients

Introduction: Goiter often poses aesthetic disgrace prob.em. The psychosocial impacts of goiter, in particular in woman may b. significant. Ob.ective: studying the psychological and social impacts of the disease in goiter patients within the African context and their consequences on the treatment. Methods: It was a transversal study that took place from May 1to July 31, 2011 in the Collines and Donga departments situated in the center and the north of b.nin. The study consisted of a survey conducted through an individual discussion b.sed on questionnaire sub.itted to goiter patients who accepted to provide their answers. Results: This survey involved 86 patients of which 83 women and 3 men. Their average age was 43.7 ± 13.41. Goiter was noticeab.e in the totality of the patients. 68.6% of patients declared that they experienced on daily b.sis the shame caused b. the disease. 66% thought that it was a natural disease;however, 27.9% b.lieved that it was caused b. b.witchment or sorcery. 46.5% and 37.2% resorted to modern and indigenous medicine respectively;16.3% made no therapeutic move. Despite the psychosocial impacts of the disease, 50% of the patients rejected surgical intervention possib.lity. The core motives they raised were lack of financial means (34.8%), disease recurrence fright (23.3%), anesthesia fright (16.3%), and surgical fright (14%). Conclusion: Despite the psychological impacts of the disease, strong resistance exists prob.b.y more on cultural aspect than the motives mentioned b. thyroidectomy patients.

A Rare Tumor of the Scalp: Papilliferous Cystadenoma

Introduction: Papilliferous cystadenoma is a rare adnexal tumor of early childhood. Case Report: We report a case of papilliferous cystadenoma of the scalp in a 22-year-old adult with no previous history of the disease. She had b.en presenting with a slowly progressing scalp mass for ab.ut 10 years. The mass was mildly pruritic and painless, b.t the patient reported several painful episodes treated with local herb.l applications and unspecified antib.otics. On inspection, the lesion was raised, granular, sessile and vegetated, greyish in color, with an irregular surface, nippled and hemispherical in shape. On palpation, the lesion was painless, fleshy and surrounded b. crusts that were easily removed b. applying a saline-moistened compress. The patient underwent excision with at least 4-mm margins. Pathological examination of the operative specimen revealed a papilliferous syringocystadenoma. There was no tumor recurrence after three years. Conclusion: Papilliferous cystadenoma is a rare b.nign tumor of the scalp requiring clinical analysis and surgical excision to confirm its histological nature. Post-operative follow-up is necessary due to frequent recurrences.

Evaluation of the Use of Jadelle Contraceptive Implant b. Women in the City of Parakou in 2016

Introduction: Maternal mortality is still very high in b.nin. b.ing one of the methods of reducing this mortality, contraception is still very rarely used in b.nin, particularly in b.rgou district. The ob.ective of this work was to evaluate the experience of using Jadelle contraceptive implants b. women in the city of Parakou in the b.rgou department. Methodology: This was a descriptive cross-sectional study, with retrospective and prospective data collection over the period from March 1 to July 31, 2016. Results: A total of 320 women were involved in the study. The mean age was 28.29 years, with extremes of 16 and 44 years. Socio-professionally, housewives predominated (56.88%). Sixty-eight point forty-four percent of women were educated and only 111 (34.69%) women had reached secondary school;married women were mostly represented. Ob.tetrically, multi-gestures were the most affected (43.75%). b.rth spacing was the main reason for choosing Jadelle (48.13%). Majority of patients had their husb.nds consent to use Jadelle contraceptive method (85.31%). Sixty-eight point fifty-two percent of women had used this contraceptive method for 2 to 4 years. Jadelle had b.en chosen b. the majority (78.75%) of the women, for its long duration of action and its easy use. The dropout rate for Jadelle was 35/320 (10.94%) after 3 years of use. The failure rate for this method was 0.31% (1/320). The satisfaction rate was 87.5%. Conclusion: Jadelle contraceptive implants are an important part of contraceptive methods in Parakou.

Introduction: The use of oxytocic plants (OPs) to facilitate childb.rth is old and it is a common practice in the world, particularly in northern b.nin. Ob.ective: To study the effect of oxytocic plants on the progress of lab.ur. Materials and Method: This was a cross-sectional and analytical study with prospective data collection conducted in the pub.ic maternity hospitals of Parakou from 15 July 2022 to 15 August 2022. All the parturients admitted to the maternity units were included. Data were entered and processed using Epi Data 3.1fr, Epi info 7.***.3.3 and Excel 2010. The difference was statistically significant for a p-value of less than 0.05. Results: Of 297 parturients surveyed, 109 had used OPs, representing a frequency of use of 36.7%. The average age was 27.5 ± 8.9 years, with extremes of 17 and 45 years. They were in common-law unions (54.1%), housewives (32.1%), from Muslim religion (53.2%), from b.rib. ethnic group and related (30.3%) and from a rural area (60.5%). These parturients were pauciparous (37.6%) and primiparous (44.9%). Six OPs were used b. the parturients. These were Hyb.nthus enneaspermus (40.7%), Schrankia leptocarpa (12.0%), Ceratotheca sesamoides (35.2%), Cassytha filiformis (5.6%), Selaginella lepidophylla (5.6%) and Newb.uldia leavis (18.5%). The parturients used the leaves (90.6%) and decoction (97.2%). The administration was oral (99.0%) without precise dosage (85.3%) in the third trimester of pregnancy (84.41%) and facilitated delivery (99.1%). Fon and related ethnic groups (p = 0.014), Yorub. and related ethnic groups (p = 0.031), rural b.ckground (p = 0.032) and multiparity (p = 0.04) were the factors associated with OPs use. These parturients had a higher risk of caesarean delivery (p = 0.001;OR = 1.8 [1.23 - 2.65]), dynamic dystocia (p = 0.001;OR = 1.3 [1.12 - 1.49]), b.rnout syndrome (p = 0.002;OR = 1.1 [1.02 - 1.17]), uterine rupture (p = 0.032;OR = 1.1 [0.97 - 1.17]), rapid lab.ur (p = 0.001;OR = 1.6 [1.38 - 2.00]), and risk

Prognostic Factors of Induction of Lab.r with Misoprostol at CHUD/b.in Parakou, b.nin in 2016

Introduction: Misoprostol is a molecule used for inducing lab.r these days in order to reduce perinatal morb.dity and mortality. This study aimed at appreciating maternal and fetal prognosis after induction of lab.r on indication for delivery with misoprostol on live fetus at CHUD/b.in 2016. Patients and Method: It was an ob.ervational case-control type study with prospective data collection carried out in the gynecology and ob.tetrics department of CHUD/b. Were included, 37 WA healthy pregnant women whose b.shop score was less than 6. Their fetal heart rate and their pelvis were normal. Fisher chi-2 test was used and the significance threshold of different associations was 5%. Results: In the study, 99 women involved were divided into 33 cases and 66 case-controls. The average age of cases was 29.70 ± 5.03 years old with extremes of 20 and 40 years and that of case-controls was 29.44 ± 4.96 years with extremes of 21 and 40 years. Out of the 1195 deliveries which took place in the gynecology and ob.tetrics department of CHUD/b. 33 inductions of lab.r on full-term live fetuses with misoprostol were performed that is to say a frequency of 2.76%. The factors associated with maternal and fetal prognosis were profession, mode of admission, b.shop score with respectively p-values of 0.049, 0.00005, 0.00001. Conclusion: The use of misoprostol seems b.neficial as regards maternal and fetal prognoses in our practice at a low cost.

Epidemiological and Therapeutic Aspects of Ob.tetric Fistula in 2021: A Review of 97 Cases at the Departmental University Hospital Centre of b.rgou and Alib.ri in b.nin

b.ckground: Ob.tetric Fistula (OF) is a b.each of the b.rth canal due to ob.tructed lab.r. It is a major pub.ic health prob.em in developing countries. Ob.ective: Study the epidemiology and treatment of ob.tetric fistulas at the Departmental University Hospital Centre of b.rgou and Alib.ri (CHUD-b.A) in Parakou, b.nin. Study Method: This was a descriptive and analytical cross-sectional study with prospective data collection on women treated during OF care missions from Feb.uary 2020 to April 2021 in CHUD-b.A in Parakou, b.nin. The data concerned their socio-demographic characteristics, ob.tetric history, the clinic and therapeutic characteristics of their ob.tetric fistulas. Results: In total 97 patients with OF were treated during the 14-month period. The average age of the patients was 36.42 ± 11.75 years old. The patients were married (51.54%), without professional occupation (54.64%) and had no formal education (73.20%). The vesico-vaginal variety was predominating (54.64%). The fistulas were of Type I (46.39%), Type II (20.62%) and Type III (32.99%) according to the classification of Waaldjik Kees. The patients were operated on under spinal anesthesia (94.79%), and vaginally (55.67%) according to the principle of Chassar Moir (68, 60%). The associated procedures were lengthening urethroplasty (17.53%), and interposition of the Martius flap (1.03%). The overall success rate without Stress Urinary Incontinence was 71.13%. The factors associated with the failure of the fistula repair were: the nutritional status of the patients (p = 0.004), the previous repair failure (p = 0.001), the high size of the fistula (p = 0.007), the fistula severity (p and a urethral reconstruction (p Conclusion: OF is a relatively frequent pathology in b.nin. Vesico-vaginal fistula is the most common form. The success rate of the surgical treatment is satisfactory, b.t depends on some factors.

Factors Associated with Hypertensive Disorders of Pregnancy in Pub.ic Maternities of the City of Parakou In 2019

The purpose of this study was to evaluate the prevalence of Hypertensive Disorders in Pregnancy (HDP) and to determine the factors associated with them in the city of Parakou in b.nin, a West African country. Method: It has b.en a descriptive cross-sectional study with a prospective collection of data using a questionnaire in prenatal consultation in the health center of Parakou’s commune for 4 months (May 1 to August 31, 2019). Result: A proportion of 8.2% (55/671) of the pregnancy had the hypertensive disorders of pregnancy. The Gestational HyperTension was the most common type of arterial hypertension (AHT) with a proportion of 49.1%. The factors associated with HDP were the age of 30 to 34 (OR: 11.9;95% CI: 1.54 - 92.29;p = 0.0346), the family history of Arterial HyperTension (AHT) (OR: 1.5;95% CI: 1.03 - 4.66;p = 0.0481), the BMI > 30 (OR:14.2;95% IC: 7.02 - 28.69;p = 0.0235), being married or in a common-law relationship (OR = 2.51;95% CI: 1.30 - 4.86;p = 0.0412), the stress (OR: 2.0;95% CI:1.09 - 3.32;p = 0.0209), the history of HDP (OR: 9.0;95% CI: 2.53 - 15.23;p are common in Parakou.
