Intracavernous de Novo Aneurysms without Prior Manipulation of the Harboring Vessel: Report of Two Cases

Intracavernous aneurysms of the cranial base, though less frequent than intradural intracranial aneurysms, can pose considerable therapeutic problems. To date, little is known concerning their spontaneous development and eventual growth kinetics. We report the observation of two de novo aneurysms without prior therapeutic manipulation of the vessels. In our first case, the 33-year-old woman had suffered from an intracerebral hemorrhage at the age of 22. At this time, neurosurgical evacuation was performed, neither angiographically nor histologically was an underlying vascular malformation disclosed. During follow-up observation 11 years later, a cavernous aneurysm of the right internal carotid artery was detected, together with an arteriovenous malformation of the left pericallosal artery next to the former hemorrhage. In the second case, the 49-year-old woman had undergone magnetic resonance imaging (MRI) for headache, revealing two intracavernous aneurysms of the right internal carotid artery. During follow-up, 7 years later, angiography disclosed a de-novo aneurysm of the left internal carotid artery, and an enlargement of the more distal right-sided aneurysm. Most cases of de novo aneurysm formation are described intradurally and after manipulation of the harboring vessel. In the patients presented here, therapeutically induced hemodynamic changes or vessel wall manipulations are not involved, pointing to a cerebrovascular predilection, that—in combination with a set of risk factors—leads to the development of cavernous aneurysms.

MALDI-TOF Mass Spectrometric Analysis of Brain Tumor Cyst Fluid Reveals a Protein Peak Corresponding to ApoC1 and LuzP6

Objectives: SELDI-TOF and MALDI-TOF mass spectrometry (MS) are laser desorption technologies that allow for proteomic examination of molecular masses in small amounts of samples. In a precedent study, the feasibility of SELDI-TOF MS assessment of proteins in cerebrospinal fluid and tumor cyst fluid had been shown. In the present study, we analyzed whether MALDI-TOF MS examination of these fluids leads to comparable results. Methods: During neurosurgical intervention, cyst fluids from 24 glioblastomas and 15 metastases were collected. As control, cerebrospinal fluid samples from 23 patients were obtained. The samples were prepared using a protocol optimized for MALDI-TOF MS. Mass spectra were recorded and peaks were extracted, characterized by masses and relative intensities. These peaks were analyzed for statistically significant differences between the diagnosis groups and compared to SELDI-TOF MS data. Results: 41 protein peaks known from the SELDI-TOF MS analysis could be confirmed by MALDI-TOF MS, and the cellular expression of the proteins LuzP6 and ApoC1, corresponding to the protein peaks 6433 and 6632, was shown immunohistochemically in glioblas-toma tissue. The MALDI-TOF spectrometry extends the range of analysis down to 1.4 kDa, whereas the upper detection limit lies below 23 kDa. Discussion: The presented proteomic approach yields an inventory of protein masses, found in the tumor cyst at the time of puncture. It does not reveal pathophysiologic, metabolic or secretory pathways that lead to the presence of proteins in the cyst. These have to be assessed immunohistochemically or on mRNA level in the surrounding tumor cells. Conclusion: MALDI-TOF MS of tumor cyst fluid discloses protein sizes, overexpressed or lost in tumor tissue. A thorough proteomic work-up is needed to identify the underlying proteins and metabolic pathways.

Objective: Sinogenic subdural empyema remains a neurosurgical emergency, even in the antibiotic era. Sickle cell disease is one of the most frequent hereditary diseases, with an incidence of around 4 in 1000 newborns, which necessitates special considerations before a neurosurgical intervention. Case Description: The case of a 10-year-old boy with sickle cell disease type HbSC is reported, who presented with meningitis about ten days after a nasal sinusitis. CT and MRI showed subdural empyema and immediate intravenous antibiotic treatment was started. The patient received partial exchange transfusion. Craniotomy allowed evacuation of empyema and repairment of a frontobasal dural defect. Endoscopic evacuation of the paranasal sinuses was done in the same anesthesia. After initial good recovery, the patient developed right sided paresis with epileptic seizures a few days later. CT scan revealed three distant empyema recurrencies which were evacuated in a second neurosurgical intervention. The child recovered without neurologic deficit, and control MRI examinations three and twelve months after surgery found frontal and parietal dural thickening as sequelae. Conclusion: Urgent neurosurgical interventions in the presence of sickle cell anemia require special anesthesiologic preparations. An immediate, multidisciplinary approach can lead to a good outcome in these life-threatening situations.

Neonatal hydrocephalus can arise from a multitude of disturbances, among them congenital aqueductal stenosis, myelomeningocele or posthemorrhagic complications in preterm infants. Diagnostic work-up comprises transfontanellar ultrasonography, T2 weighted MRI and clinical assessment for rare inherited syndromes. Classification of hydrocephalus and treatment guidelines is based on detailed consensus statements. The recent evidence favors catheter-based cerebrospinal fluid diversion in children below 6 months, but emerging techniques such as neuroendoscopic lavage carry the potential to lower shunt insertion rates. More long-term study results will be needed to allow for individualized, multidisciplinary decision making. This article gives an overview regarding contemporary pathophysiological concepts, the latest consensus statements and most recent technical developments.

Purpose:This study aimed to quantify the dose-response association and the minimal effective dose of leisure-time physical activity(PA)to prevent mortality and cardiovascular disease in adults with type 2 ***:Cross-country comparison of 2 prospective cohort studies including 14,913 and 17,457 population-based adults with type 2 diabetes from the UK and *** leisure-time PA was self-reported and categorized by metabolic equivalent hours per week(MET-h/week)according to World Health Organization recommendations:none,below recommendation(>0-7.49 MET-h/week);at recommended level(7.5-14.9 MET-h/week);above recommendation(>15 MET-h/week).Mortality and cardiovascular disease data were obtained from national ***:During a median follow-up of 12.4 and 9.7 years,in the UK and China cohorts,repectively,higher levels of leisure-time PA were inversely associated with all-cause(1571 and 2351 events)and cardiovascular mortality(392 and 1060 events),mostly consistent with a linear dose-response *** below,at,and above recommendations,compared with no activity,yielded all-cause mortality hazard ratios of0.94(95%confidence interval(95%CI):0.79-1.12),0.90(95%CI:0.74-1.10),and 0.85(95%CI:0.70-1.02)in British adults and 0.87(95%CI:0.68-1.10),0.88(95%CI:0.74-1.03),and 0.77(95%CI:0.70-0.85)in Chinese *** with cardiovascular mortality were more pronounced in British adults(0.80(95%CI:0.58-1.11),0.75(95%CI:0.52-1.09),and 0.69(95%CI:0.48-0.97))but less pronounced in Chinese adults(1.06(95%CI:0.76-1.47),1.01(95%CI:0.80-1.28),and 0.79(95%CI:0.69-0.92)).PA at recommended levels was not associated with lower rates of major adverse cardiovascular events(2345 and 4458 events).Conclusion:Leisure-time PA at the recommended levels was not convincingly associated with lower mortality and had no association with risk of major adverse cardiovascular events in British or Chinese adults with type 2 ***-time PA above current recom
