Prognostic factors and clinical features of rhino-orbitalmucormycosis cases:an update for patient and visual survivals

Comparison of the inhibitory effect of different doses of subconjunctival bevacizumab application in an experimental model of corneal neovascularization

AIM： To evaluate the inhibitory effect of subconjunctival bevacizumab as single-and multiple-dose application, and compare their effects on corneal neovascularization in a rat model. METHODS： Thirty adult Sprague-Dawley rats were used in this experimental study. The central cornea of the rats was cauterized chemically. The rats were randomly enrolled into three groups. All groups received subconjunctival injections. In Group 1（control group, n=10）, 0.05 m L 0.9% Na Cl solution was injected on the first day. In Group 2（single-dose group, n=10）, 0.05 m L bevacizumab（1.25 mg） was injected on the first day. In Group 3（multiple-dose group, n=10）, four doses of 0.05 m L bevacizumab（1.25 mg） were injected on the first, third, fifth and seventh day. Slit-lamp examination of all rats was performed at the third and ninth day. Digital images of the corneas were taken and analyzed using image analysis software to calculate corneal neovascularization area. All rats were sacrificed on the tenth day. In corneal sections, the number of blood vessels, state of inflammation and collagen formation was evaluated histopathologically. RESULTS： In Group 3, corneal edema grades were significantly lower than Group 1 and Group 2（P=0.02, and P=0.035, respectively）. The mean percentage of neovascularized corneal area in Group 3 was significantly lower than Group 2（P=0.005）. On histopathological examination, Group 2 and Group 3 showed significantly less number of blood vessels than Group 1（P=0.005, and P=0.001, respectively）. Additionally, Group 3 showed significantly less number of blood vessels compared to Group 2（P=0.019）. Inflammation and edema grades were significantly lower in Group 3 compared to Group 1（P=0.001）. CONCLUSION： Subconjunctival bevacizumab injection is effective in inhibition of newly formed corneal neovascularization. The multiple-dose bevacizumab treatment seems to be more effective compared to single-dose treatment.

Detecting treeline dynamics in response to climate warming using forest stand maps and Landsat data in a temperate forest

Background: Treeline dynamics have inevitable impacts on the forest treeline structure and composition. The present research sought to estimate treeline movement and structural shifts in response to recent warming in Cehennemdere, Turkey. After implementing an atmospheric correction, the geo-shifting of images was performed to match images together for a per pixel trend analysis. We developed a new approach based on the NDVI, LST(land surface temperature) data, air temperature data, and forest stand maps for a 43-year period. The forest treeline border was mapped on the forest stand maps for 1970, 1992, 2002, and 2013 to identify shifts in the treeline altitudes, and then profile statistics were calculated for each period. Twenty sample plots(10 × 10 pixels) were selected to estimatethe NDVI and LST shifts across the forest timberline using per-pixel trend analysis and non-parametric Spearman’s correlation analysis. In addition, the spatial and temporal shifts in treeline tree species were computed within the selected plots for four time periods on the forest stand maps to determine the pioneer tree ***: A statistically significant increasing trend in all climate variables was observed, with the highest slopein the monthly average mean July temperature(tau = 0.62, ρ < 0.00). The resultant forest stand maps showed a geographical expansion of the treeline in both the highest altitudes(22 m–45 m) and the lowest altitudes(20 m–105 m) from 1970 to 2013. The per pixel trend analysis indicated an increasing trend in the NDVI and LST values within the selected plots. Moreover, increases in the LST were highly correlated with increases in the NDVIbetween 1984 and 2017(r = 0.75, ρ < 0.05). Comparison of satellite images with different spatial resolutions to estimate stand structural diversity in urban forests

The structural diversity in urban forests is highly important to protect biodiversity. In particular, fruit trees and bush species, cavity-bearing trees and coarse, woody debris provide habitats for animals to feed, nest and hide. Improper silvicultural practices, intensive recreational use and illegal harvesting lead to a decline in the structural diversity in forests within larger metropolitan cities. It is important to monitor the structural diversity at definite time intervals using effective technologies with a view to instituting the necessary conservation measures. The use of satellite images seems to be appropriate to this end. Here we aimed to identify the associations between the textural features derived from the satellite images with different spatial resolutions and the structural diversity indices in urban forest stands (Shannon-Wiener index, complexity index, dominance index and density of wildlife trees). RapidEye images with a spatial resolution of 5 m × 5 m, ASTER images with a spatial resolution of 15 m × 15 m and Landsat-8 ETM satellite images with a spatial resolution of 30 m × 30 m were used in this study. The first-order (standard deviation of gray levels) and second order (GLCM entropy, GLCM contrast and GLCM correlation) textural features were calculated from the satellite images. When associations between textural features in the images and the structural diversity indices were assessed using the Pearson correlation coefficient, very high associations were found between the image textural features and the diversity indices. The highest association was found between the standard deviation of gray levels (SDGLRAP) derived from RVIRAP of RapidEye image and the Shannon-Wiener index (H h) calculated on the basis of tree height (R 2 = 0.64). Intensive care outcomes in adult hematopoietic stem cell transplantation patients

Although outcomes of intensive care for patients undergoing hematopoietic stem cell transplantation(HSCT)have improved in the last two decades, the short-term mortality still remains above 50% among allogeneic HSCT patients. Better selection of HSCT patients for intensive care, and consequently reduction of nonbeneficial care, may reduce financial costs and alleviate patient suffering. We reviewed the studies on intensive care outcomes of patients undergoing HSCT published since 2000. The risk factors for intensive care unit(ICU) admission identified in this report were primarily patient and transplant related: HSCT type(autologous vs allogeneic), conditioning intensity, HLA mismatch, and graft-versus-host disease(GVHD). At the same time, most of the factors associated with ICU outcomes reported were related to the patients' functional status upon development of critical illness and interventions in ICU. Among the many possible interventions, the initiation of mechanical ventilation was the most consistently reported factor affecting ICU survival. As a consequence, our current ability to assess the benefit or futility of intensive care is limited. Until better ICU or hospital mortality prediction models are available, based on the available evidence, we recommend practitioners to base their ICU admission decisions on: Patient pretransplant comorbidities, underlying disease status, GVHD diagnosis/grade, and patients' functional status at the time of critical illness.

Effect of nepafenac 0.1% on retinal thickness after cataract surgery in patients without risk factors for cystoid macular edema

AIM:To evaluate the effect of topical preoperative nepafenac 0.1%treatment on postoperative macular edema using optical coherence tomography(OCT)after uncomplicated cataract ***:Ninety eyes of 90 patients without any risk factors were included in the *** patients were assigned to three groups:group 1,treated with topical prednisolone acetate 1%;group 2,treated with topical nepafenac 0.1%in addition to prednisolone acetate(1%);and group 3,those who started receiving nepafenac 0.1%treatment 3 d prior to surgery and continued the treatment postoperatively in addition to prednisolone acetate(1%).Central retinal thickness(CRT)and macular volume values were recorded using OCT at weeks 3 and ***:The increases in macular volume in the central 1 mm area after 3 and 6 wk were significantly lower in patients who used prophylactic topical nepafenac preoperatively(group 3)compared with those in group 1(P=0.028 and 0.008,respectively).No significant differences in the increase in macular volume and CRT were noted between groups 2 and 3(P>0.05).In group 1,the increases in macular volume in the central 3 mm area at weeks 3 and 6 were significantly higher than that in group 2 and 3(3rd week,P=0.004;6th week,P=0.005).CONCLUSION:Nepafenac 0.1%treatment in addition to topical steroids after uncomplicated cataract surgery reduce the increase in macular volume in the early postoperative period.

Treatment of iron deficiency anemia associated with gastrointestinal tract diseases

The gastrointestinal (GI) tract is a common site of bleeding that may lead to iron deficiency anemia (IDA). Treatment of IDA depends on severity and acuity of patients’ signs and symptoms. While red blood cell transfusions may be required in hemodynamically unstable patients, transfusions should be avoided in chronically anemic patients due to their potential side effects and cost. Iron studies need to be performed after episodes of GI bleeding and stores need to be replenished before anemia develops. Oral iron preparations are efficacious but poorly tolerated due to non-absorbed iron-mediated GI side effects. However, oral iron dose may be reduced with no effect on its efficacy while decreasing side effects and patient discontinuation rates. Parenteral iron therapy replenishes iron stores quicker and is better tolerated than oral therapy. Serious hypersensitive reactions are very rare with new intravenous preparations. While data on worsening of inflammatory bowel disease (IBD) activity by oral iron therapy are not conclusive, parenteral iron therapy still seems to be advantageous in the treatment of IDA in patients with IBD, because oral iron may not be sufficient to overcome the chronic blood loss and GI side effects of oral iron which may mimic IBD exacerbation. Finally, we believe the choice of oral vs parenteral iron therapy in patients with IBD should primarily depend on acuity and severity of patients’ signs and symptoms.

Effect of diluted povidone iodine in adenoviral keratoconjunctivitis on the rate of subepithelial corneal infiltrates

AIM: To evaluate the clinical characteristics of adenoviral keratoconjunctivitis, the management modalities, as well as the incidence of subepithelial corneal infiltrates(SEI).METHODS: Patients with characteristic clinical symptoms and signs, who presented to our clinic within the first week of symptoms and received the diagnosis of adenoviral keratoconjunctivitis between January 2013 and April 2016, were included in the study. A total of 211 patients were included in the study. Patients were evaluated for the incidence of clinical signs, late complications, management preferences, and the effect of diluted povidone-iodine(d-PVP-I) 2%. RESULTS: Patients’ mean age was 33.03±14.76 y. We observed an increase in the number of cases according to the years. At presentation and/or early follow-up, the clinical signs were conjunctival hyperemia(100%), conjunctival follicules(79.1%), edema of the eyelids(39.3%), chemosis(16.1%), pseudomembrane formation(16.6%), and corneal epitheliopathy(29.9%). During late follow-up 13.3% patients developed conjunctival subepithelial fibrosis, and 39.8% developed SEI. A significant decrease in the incidence of SEI development was observed in patients who used d-PVP-I 2%(P=0.032;33.3% vs 45.9%, respectively in patients who received d-PVP-I 2% and who did not).CONCLUSION: Adenoviral keratoconjunctivitis has a tremendous effect on patient’s comfort and abilities in short-term. Additionally, almost half of the patients develop visual problems related to SEI. According to our clinical experience, using d-PVP-I 2% in the first days of adenoviral keratoconjunctivitis might be helpful in reducing the risk of SEI as a complication.
