Locally Advanced Colon Cancer with Abdominal Wall Abscess: A Challenging Case Treated by an Innovative Approach

Colon cancer rarely combines with abscess of the abdominal wall. We here describe a case treated by extensive surgery, biological mesh abdominal wall repair and negative pressure therapy. A 58-year-old woman presented with a locally advanced right colon cancer with abdominal wall abscess and no evidence of distant metastasis. Extended right hemicolectomy was performed with en-bloc excision of the bladder dome, the right annex and full thickness removal of the anterior abdominal wall including the abscess. Abdominal wall repair was perfomed by a biological mesh (PermacolTMBiologic Implant) and to facilitate healing the patient was then treated with Vacuum-Assisted Closure (V.A.C.?) Therapy. Histology showed a mucinous moderately differentiated adenocarcinoma without nodal metastases (n = 57). Surgical margins including the abdominal wall was tumor free. The postoperative clinical course was uneventful. VA.C.? Therapy treatment reported excellent results in terms of active promotion of the granulation tissue, this allowing for a subsequent placement of a skin graft. Patient is alive and disease-free one year after surgery. The present case shows some peculiar characteristics such as the size of the initial lesion, the abdominal wall abscess and the use of innovative devices such as biological mesh and V.A.C.? Therapy. We demonstrate that extensive surgery for locally advanced colon cancer, in high-volume centers, provides favorable results in terms of survival and quality of life.

Influence of specific speed on hydraulic performances and pressure fluctuations in mixed-flow pumps

A series of steady and unsteady numerical calculations of the internal flow in mixed-flow pumps with three different specific speeds were carried out based on the N-S equation coupled with the standard k-εturbulence model under different operating conditions to investigate the relationship between the impeller specific speed and the pump performance as well as pressure ***,the pump performance and pressure pulsations inside the mixed-flow pump with three different specific speeds were also analyzed and compared with the corresponding test *** the results,the averaged deviations between the predicted and tested head among different impellers are below 5%,and with respect to the equivalent impeller specific speeds of 280 and 260,the values are 4.30%and 3.69%,*** all the impeller schemes,the best efficiency point of the mixed-flow pump is found at the flow rate of 1.2 Q_(d) and the higher head deviation occurs at lower flow ***,it can be found that the specific speed has a slight effect on the pressure fluctuation in the ***,it is determined that the pump performance curves calculated by numerical simu-lations have good agreement with the relevant experimental results,which verifies that the numerical methods used in the present study are accurate to a certain ***,the results also provide some references to the pressure pulsation analysis and the performance improvement of the mixed-flow pump design.

Carpal tunnel syndrome(CTS)is a multifactorial compression *** is reported to be very common and rising ***’s treatment varies from conservative measures to surgical *** has shown to be an effective method for more severe *** few unclear aspects and room for further research and improvements still *** performed a narrative literature review on the most up to date progress and innovation in terms of surgical treatments for *** simple algorithm of leaving the choice of the surgical method to surgeons’preference and experience(together with consideration of patients’related factors)seem to be the best available option,which is supported by the most recent metanalysis and systematic *** suggest that surgeons(unless in presence of precise indications towards endoscopic release)should tend to perform a minimally invasive open approach release,favoring the advantage of a better neurovascular structures visualization(and a consequent higher chance to perform a complete release with long term relief of symptoms)instead of favoring an early reduction(in the first postoperative days)of immobilization and *** towards a universally accepted standardization should be aimed for by the researchers,who have failed to date to sufficiently limit bias and limitations.

Pathogenetic factors affecting gastroesophageal refluxin patients with esophagitis and concomitant duodenal ulcer:a multivariate analysis

AIM To assess the relationship between gastric acid output (GAO) and both pattern of gastroesophageal reflux (GER) and esophageal lesions, and to evaluate the role of GAO and other potential pathogenetic factors in the development of esophagitis. METHODS Gastric acid secretory testing and 24 h intraesophageal pH monitoring were performed in 31 patients with esophagitis and concomitant duodenal ulcer (E+DU) and compared with those of 72 patients with esophagitis (E) alone. RESULTS The GAO in patients with E+DU was significantly higher than in patients with E ( P 0 05). A multiple regression analysis with stepwise deletion showed that the pre sence of hiatal hernia (HH), GER in upright position and age appeared to correlate significantly with the presence of esophagitis. CONCLUSIONS No parallel relationship between GAO and severity of GER or esophageal lesions exists in patients with E+DU, and that GAO is not a major pathogenetic factor in GER disease.

Gastroesophageal reflux: the features in elderlypatients

AIM To compare the features ofgastroesophageal reflux disease between elderlyand younger *** Twenty-four hour pH-monitoring andendoscopy were P6rformed for the 66 elderlypatients with typical gastroesophageaI refluxsymptoms, and the results were compared with112 symptomatic younger *** The results of 24-h pH-monitoringand endoscopy showed that the elderly patientshad pathological reflux and reflux esophagitismore frequently than the younger patients.P6rcentage time with pH< 4 in elderly patientswith reflux esophagitis was 32.5% in 24 hours,as compared with 12.9% in the younger patientswith reflux esophagitis (P < 0 .05 ). The elderlypatients with reflux esophagitis have longerP6riOds of acid reflux in both upright and Supinepositions than the younger patients. Endoscopyshowed that 20 .8% of elderIy patients had grade Ⅲ / Ⅳ esophagitis, whereas only 3.4% ofyounger patients had grade Ⅲ/ Ⅳ esophagitis(P < 0.002 ). Percentages of grades Ⅰ /Ⅱesophagitis in the two groups were 12.5% and26. 5%, respectiveIy (P< 0. 002 ).CONCLUSION Elderly patients, as comparedwith younger patients, have more severegastroesophageal reflux and esophageal *** incompetence of lower esophageal sphincter and the presence of hiataI hernia may beimportant factors leading to the difference in incidence and severity of reflux esophagitisbetween elderly and younger patients.

Despite the bi-directional interaction between gut microbiota and the brain not being fully understood,there is increasing evidence arising from animal and human studies that show how this intricate relationship may facilitate inflammatory bowel disease(IBD)pathogenesis,with consequent important implications on the possibility to improve the clinical outcomes of the diseases themselves,by acting on the different components of this system,mainly by modifying the *** the emergence of precision medicine,strategies in which patients with IBD might be categorized other than for standard gut symptom complexes could offer the opportunity to tailor therapies to individual *** aim of this narrative review is to elaborate on the concept of the gutbrain-microbiota axis and its clinical significance regarding IBD on the basis of recent scientific literature,and finally to focus on pharmacological therapies that could allow us to favorably modify the function of this complex system.
