Successful treatment of tubulointerstitial nephritis in immunoglobulin G4-related disease with rituximab: A case report

BACKGROUND Immunoglobulin G4-related disease (IgG4-RD) is an immune-mediated condition that consisted of disorders that share particular clinical,serologic and pathologic *** common presentation of disease includes tumor-like swelling of involved organs and the histopathological findings are a lymphoplasmacytic infiltrate enriched with IgG4-positive plasma cells,and a variable degree of fibrosis that has a characteristic "storiform" pattern in biopsy specimens of tumorlike *** presentations of this disease,which often affects more than one organ,include autoimmune pancreatitis,salivary gland disease (sialadenitis),orbital disease and retroperitoneal *** steroid treatment is essential for the treatment of the disease however,other immunosuppressive drugs including cyclophosphamide or rituximab could be an option in resistant *** SUMMARY Herein,we reported a 34-year-old woman whom previously had diagnosed with asthma,rheumatoid arthritis and Sj?gren’s syndrome (SS) referred our nephrology department due to acute kidney failure development at the last rheumatology *** kidney biopsy she has been diagnosed with IgG4-RD and tubuluointerstitial *** had been accepted resistant to steroid,mycophenolate mofetil,methotrexate and azathioprine therapies due to receiving in last two *** refused to receive cyclophosphamide due to potential gonadotoxicity of the ***,rituximab therapy was *** received 1000 mg infusion,15 d apart and 6 mo later it has been administered same *** one year from the last rituximab dose serum creatinine decreased from 4.4 mg/dL to 1.6 mg/dL,erythrocyte sedimentation rate decreased from 109 mm/h to 13 mm/h [reference range (RR) 0-20],and Creactive protein decreased from 55.6 mg/L to 5 mg/L (RR 0–6).All pathologic lymph nodes and masses were also *** Patients with IgG4-RD usually misdiagnosed with rheumatologic diseases including systemic lupus erythem

Determining of Employee Frauds in Forensic Accounting： A Research on Forensic Cases in the City of Kars in Turkey

Discovering and preventing the frauds which affect the business organizations negatively require a greater degree of specialism. Detecting a fraud in the organizations is very hard, because not only such a fraud is exercised by the people who have deep professional knowledge, but they also use some peculiar methods to hide their tricky activities. Therefore, it is obvious that it is necessary to have the fraud examiners and especially fraud auditors who should have deep professional knowledge and experience. The aim of this study is to give some general information about employee fraud, which targets the different functions of the companies, takes many forms, and reaches important levels in recent years, in qualitative point. In this study, firstly, forensic accounting is a highly dynamic area in nowadays which is related to fraud auditing and its profession, and its search area of frauds and employee frauds subjects have been reviewed. Finally, qualitative data were collected about fraud incidents which had occurred and been sent to the court in the province of Kars in Turkey. Actual case analysis method has been used in this study. The obtained data have been analyzed by using Statistical Package for the Social Sciences （SPSS） 17 statistics package program. Results of the study have been discussed and interpreted in details.

Primary glomerular diseases in the elderly

Primary glomerular diseases in the elderly population are a frustrating topic due to difficulties in both the diagnosis and decision making about treatment. The most frequent type of primary glomerular disease in elderly is membranous nephropathy; while its counterpart in younger population is Ig A nephropathy. The most frequent cause of nephrotic syndrome in the elderly is also membranous nephropathy. Pauci-immune crescentic glomerulonephritis(GN) rate increases both in elderly and very elderly population. Pauci-immune crescentic GNs should be regarded as urgencies in elderly patients as in their younger counterparts due to potential for causing end-stage renal disease in case of delayed diagnosis and treatment, and also causing mortality due to alveolar hemorrhage in patients with pulmonary involvement. Renal biopsy is the inevitable diagnostic method in the elderly as in all other age groups. Renal biopsy prevents unnecessary treatments and provides prognostic data. So advanced age should not be the sole contraindication for renal biopsy. The course of primary glomerular diseases may differ in the elderly population. Acute kidney injury is more frequent in the course and renal functions may be worse at presentation. These patients are more prone to be hypertensive. The decision about adding immune suppressive therapies to conservative methods should be made considering many factors like co-morbidities, drug side effects and potential drug interactions, risk of infection, patient preference, life expectancy and renal functions at the time of diagnosis.

Harvesting body heat using thermoelectricity provides a promising path to realizing self-powered,wearable electronics that can achieve continuous,long-term,uninterrupted health *** paper reports a flexible thermoelectric generator(TEG)that provides efficient conversion of body heat to electrical *** device relies on a low thermal conductivity aerogel–silicone composite that secures and thermally isolates the individual semiconductor elements that are connected in series using stretchable eutectic gallium-indium(EGaIn)liquid metal *** composite consists of aerogel particulates mixed into polydimethylsiloxane(PDMS)providing as much as 50%reduction in the thermal conductivity of the silicone *** on the wrist,the flexible TEGs present output power density figures approaching 35μWcm^(−2)at an air velocity of 1.2 ms^(−1),equivalent to walking *** results suggest that these flexible TEGs can serve as the main energy source for low-power wearable electronics.

One-year experience in the retinopathy of prematurity: frequency and risk factors, short-term results and follow-up

· AIM: As a result of the increase in premature births and the advances in neonatal intensive care, retinopathy of prematurity (ROP) remains one of the most important causes of childhood blindness worldwide. The main factors in the development of ROP are gestational age, birth weight and oxygen therapy. ROP continues to gain importance due to the increasing survival rates of more immature babies. · METHODS: Between January 2007 and October 2008, 203 premature infants treated at the Neonatal Intensive Care Unit (NNICU) were prospectively enrolled and the relationship between known risk factors and the occurance of ROP was studied. · RESULTS: ROP in various stages developed in 86 cases (42.4%). Statistically significant correlations were found between the development of ROP and birth weight (P < 0.0001) gestational age (P <0.0001), oxygen treatment and its duration (P <0.0001 and P =0.002), mechanical ventilation (MV) and its duration (P =0,0001 and P =0.0001), apnea(P = 0.001), intraventricular hemorrhage (IVH) (P =0.046), sepsis (P =0.0001), use of erythropoietin (EPO) (P =0.003), the number of blood transfusions and frequency (P =0.0001 and P =0.0001), surfactant application (P =0.0001), the presence of patent ductus arteriosus (PDA) (P =0.001) or bronchopu- lmonary dysplasia (BPD) (P =0.0001). No significant correlations were found between the occurance of ROP and maternal pre-eclampsia (P =0.293), multiple pregnancy (P = 0.218), or hyperbilirubinemia (P =0.494). Severity of ROP was related significantly with birth weight (P =0.0001), but no significant correlation between severity of ROP and gestational age was present. · CONCLUSION: Early description and reduction of the risk factors related with the occurance of ROP with the help of routine screening programs may warrant the prevention of visual loss, however early ophthalmic diagnosis and treatment are still mandatory to provide better visual rehabilitation. ·

Effects of daily telephone-based re-education before taking medicine on Helicobacter pylori eradication

We read the article "Effects of daily telephone-based re-education before taking medicine on Helicobacter pylori(H. pylori) eradication: A prospective singlecenter study from China" written by Wang et al with great interest. It is reported in American and European guidelines that there is no sufficient test for the diagnosis of H. pylori except culture and that using at least two different tests for diagnosis of H. pylori is recommended. Patients who used antibiotics or bismuth salts in the previous 2 wk were excluded from study. But patients who used probiotics and antioxidant vitamins such as vitamins C and E were not excluded.

Validation of the PAGE-B score to predict hepatocellular carcinoma risk in caucasian chronic hepatitis B patients on treatment

BACKGROUND Several risk scores have been developed to predict hepatocellular carcinoma(HCC)risk in chronic hepatitis B(CHB)*** majority of risk scores are based on pretreatment variables that are no longer considered risk factors for HCC development due to the suppression of hepatitis B virus replication early in the course of potent antiviral treatment in most *** PAGE-B score,which is based on platelet levels,age and sex,has been shown to accurately predict HCC risk in CHB patients on antiviral treatment in various *** We aimed to evaluate the PAGE-B score in predicting HCC risk in Turkish CHB patients on antiviral *** In this study,we recruited 742 CHB patients who had been treated with tenofovir disoproxil fumarate or entecavir for≥1 *** groups were determined according to the PAGE-B scores as follows:≤9,low;10-17,moderate and≥18,*** cumulative HCC incidences in each risk group were computed using Kaplan-Meier analysis and were compared using the log-rank *** accuracy of the PAGE-B score in predicting HCC risk was evaluated using a timedependent area under the receiver operating characteristic(AUROC)curve at all study time *** and multivariate logistic regression analyses were used to assess the risk factors for HCC *** The mean follow-up time was 54.7±1.2 *** was diagnosed in 26 patients(3.5%).The cumulative HCC incidences at 1,3,5 and 10 years were 0%,0%,0%and 0.4%in the PAGE-B low-risk group;0%,1.2%,1.5%and 2.1%in the PAGE-B moderate-risk group;and 5%,11.7%,12.5%,and 15%in the PAGE-B high-risk group,respectively(log-rank P<0.001).The AUROCs of the PAGE-B score in the prediction of HCC development at 1,3,5 and 10 years were 0.977,0.903,0.903 and 0.865,*** the multivariable analysis,older age,male sex,lower platelet levels,presence of cirrhosis,and absence of alanine aminotransferase normalization at month 6 were associated with HCC development(all P<0.05).C

Common telomerase reverse transcriptase promoter mutations in hepatocellular carcinomas from different geographical locations

AIM: To determine the mutation status of human telomerase reverse transcriptase gene(TERT) promoter region in hepatocellular carcinoma(HCC) from different geographical ***: We analyzed the genomic DNA sequences of 59 HCC samples comprising 15 cell lines and 44 primary tumors,collected from patients living in Asia,Europe and *** amplified a 474 bp DNA fragment of the promoter region of TERT gene including the 1295228 and 1295250 sequence of chromosome 5 by using *** were then sequenced by Sanger technique and the sequence data were analyzed with by using DNADynamo software in comparison with wild type TERT gene sequence as a ***: The TERT mutations were found highly frequent in *** of the fifteen tested cell lines displayed C228 T mutation,and one had C250 T mutation with a mutation frequency up to 60%.All of the mutations were heterozygous and mutually *** out of forty-four tumors displayed C228 T mutation,and additional five tumors had C250 T mutation providing evidence for mutation frequency of 34% in primary *** the geographic origins of HCC tumors tested,TERT promoter mutation frequencies were higher in African(53%),when compared to non-African(24%) tumors(P = 0.056).There was also a weak inverse correlation between TERT promoter mutations and murine double minute 2 single nucleotide polymorphism 309 TG polymorphism(P = 0.058).Mutation frequency was nearly two times higher in established HCC celllines(60%) compared to the primary tumors(34%).CONCLUSION: TERT promoter is one of most frequent mutational targets in liver cancer,and hepatocellular carcinogenesis is highly associated with the loss of telomere-dependent cellular senescence control.

Transient Exergy Analysis of the Condenser and Evaporator of an Air Source Heat Pump Water Heater

In this study the performance of an ASHPWH （air source heat pump water heater） is assessed from exergy point of view in component wise. In order to investigate the work potential of energy, the destruction on the exergy is analyzed and results are summarized for the components individually. The exergy destruction of the system is studied by considering real paths of the pressure and temperature data which are collected during the experiments of the ASHPWH under varying environmental conditions. In the following step, the evolution of the exergy destruction of the system is calculated by a code which is compiled on MATLAB along these temperature and pressure paths. The obtained results reveal the importance of the transient exergy analysis by providing detailed information about exergy destruction of the system such as where it drives up and reaches up to its max and where it drops down and evolves on a smooth path.

The Effect of the Grinding Time on the Mechanical Activation of MnO2 Ore and Tea Plant Waste Carbonization Product

In this study, pyrolusiteore （MnO2） was subjected to mechanical milling with a high-energy mill with carbonized tea plant wastes and the effect of grinding time on the crystal structure of the material was investigated. The ratio of Mn/Fe was 8/1, the ratio of C/（MnO2 ＋ Fe3O4） was 2 and the ratio of ball to ore was 10/1. The samples were mechanically ground at 10, 15, 20, 30, 60, 90 and 120 hours. In the processes performed on the attritor, the rotation speed of the mill shaft was determined to be 350 rpm. The results were characterized by TG-DTA, SEM and XRD analyzes. As a result of the experimental studies, it was observed that the samples subjected to mechanical grinding for 120 hours were gradually reduced due to the increasing grinding time at all the diffraction peaks when the XRD peaks were compared with the grinding times. In the thermogravimetric analysis, the sample milled for 120 hours, 50% weight loss was observed at 470 ℃, weight loss of up to 56% was observed at progressive temperatures.
