Vascular contractile response and signal transduction in endothelium-denuded aorta from cirrhotic rats

AIM: The mechanism of decreased vascular reactivity to vasoconstrictors in portal hypertension is still unclear. In addition to nitric oxide, defects in post-receptor signal transduction pathway have been suggested to play a ***, substantial evidences observed equivocal changes of vascular reactivity following different agonists that challenged the hypothesis of the post-receptor defect. The current study was to evaluate the vascular reactivity to different agonists and the inositol trisphosphate (IP3)changes in signal transduction cascade from cirrhotic rats with portal ***: The endothelial denuded aortic rings from cirrhotic and sham-operated rats were obtained for ex vivo tension study and measurement of the corresponding [3H] IP3 formation following different receptor and nonreceptor-mediated agonists' stimulation. Additionally,iNOS protein expression was measured in thoracic *** contractile response curves to phenylephrine were performed in endothelial denuded aortic rings with and without preincubation with a specific iNOS inhibitor (L-N (6)-(1-iminoethyl)-lysine, L-NIL).RESULTS: In endothelial denuded aortic rings of cirrhotic rats, the vascular responses were reduced with phenylephrine and arginine vasopressin (AVP) stimulation but were normal with U-46619, NaF/AlCl3, and phorbol esterdibutyrate (PdBU) stimulation. Compared to the corresponding control groups, the degree of the increment of [3H] IP3 formation from basal level was also decreased with phenylephrine and AVP stimulation, but was normal with U-46619 and NaF/AlCl3 stimulation. The preincubation with L-NIL did not modify the hyporesponsiveness to phenylephrine. Additionally, the iNOS protein expression in thoracic aorta was not different in cirrhotic and shamoperated ***: Without the influence of nitric oxide, vascular hyporeactivity to vasoconstrictors persisted in cirrhotic rats with portal hypertension. FULLY NONLINEAR WAVE COMPUTATIONS FOR ARBITRARY FLOATING BODIES USING THE DELTA METHOD

Fully nonlinear water wave problems are solved using Eulerian-Lagrangian timestepping methods in conjunction with a desingularized approach to solve the mixed boundary valueproblem that arises at each time step. In the desingularized approach, the singularities generatingthe flow field are outside the fluid domain. This allows the singularity distribution to be replacedby isolated Rankine sources with the corresponding reduction in computational complexity andcomputer time. A moving boundary technique is applied to e-liminate the reflection waves fromlimited computational boundaries. Examples of the use of the method in three-dimensions are givenfor the exciting forces acting on a modified wigley hull and Series 60 are presented. The numericalresults show good agreements with those of experiments.

A new protocol of wide use for e-mail with perfect forward secrecy

Recently, Sun et al. (2005) highlighted the essential property of perfect forward secrecy (PFS) for e-mail protocols when a higher security level is desirable. Furthermore, Sun et al. (2005)’s protocols take only a single e-mail server into account. Actually, it is much more common that the sender and the recipient register at different e-mail servers. Compared to existing protocols, the protocol proposed in this paper takes into account the scenario that the sender and the recipient register at different servers. The proposed protocol is skillfully designed to achieve PFS and end-to-end security as well as to satisfy the requirements of confidentiality, origin, integrity and easy key management. The comparison in terms of functionality and computational efficiency demonstrates the superiority of the present scheme.

Hepatocellular carcinoma in patients with renal dysfunction: Pathophysiology, prognosis, and treatment challenges

Air-Sea Exchange of Volatile Organic Compounds: A New Model with Microlayer Effects

The authors propose a new "three-layer" conceptual model for the air-sea exchange of organic gases, which includes a dynamic surface microlayer with photochemical and biological processes. A parameterization of this three-layer model is presented, which was used to calculate the air-sea fluxes of acetone over the Pacific Ocean. The air-sea fluxes of acetone calculated by the three-layer model are in the same direction but possess half the magnitude of the fluxes calculated by the traditional two-layer model in the absence of photochemical and biological processes. However, photochemical and biological processes impacting acetone in the microlayer can greatly vary the calculated fluxes in the three-layer model, even reversing their direction under favorable conditions. Our model may help explain the discrepancies between measured and calculated acetone fluxes in previous studies. More measurements are needed to validate our conceptual model and provide constraints on the model parameters.

Consensus and inconsistency between different consensus documents on renal denervation worldwide:the way forward

Given the unsatisfactory hypertension control rates and high rates of non-adherence to antihypertensive medications worldwide,device therapy which can safely provide durable blood pressure-lowering effects can fulfill the unmet need.A series of second-generation randomized sham-controlled renal denervation(RDN)trials have demonstrated the efficacy and safety of RDN in a wide range of hypertensive *** four representative consensus documents on RDN(from the Chinese Taiwan Hypertension Society and Taiwan Society of Cardiology[THS/TSOC 2019],Asia Renal Denervation Consortium 2019,European Society of Hypertension[ESH 2021],and Society for Cardiovascular Angiography&Intervention and National Kidney Foundation[SCAI/NKF 2021])consistently recommend RDN as an alternative or complementary treatment strategy for patients with uncontrolled *** addition,both documents from Asia further recommend that RDN can be considered as an initial treatment strategy for drug-naïve hypertensive *** is still inconsistency regarding whether ambulatory blood pressure monitoring should be used routinely both before and after RDN,and whether patients with a secondary cause of hypertension could be treated with RDN if their blood pressure remains uncontrolled after definitive treatment(treatment-resistant secondary hypertension).The THS/TSOC consensus provides acronyms to summarize key aspects of patient selection(RDNi2)and pre-RDN assessments(RAS).The ESH and SCAI/NKF documents recommend establishing structured pathways for clinical practice and issues regarding *** documents identify knowledge gaps in RDN,from identifying predictors of super-responders to demonstrating effects on cardiovascular *** gaps should be urgently filled to facilitate the wider application of this device therapy for patients with hypertension.
