macrophage plasticity is critical for normal tissue repair following *** pathologic states such as diabetes,macrophage plasticity is impaired,and macrophages remain in a persistent proinflammatory state;however,the reasons for this are ***,using single-cell RNA sequencing of human diabetic wounds,we identified increased JmJD3 in diabetic wound macrophages,resulting in increased inflammatory gene ***,we report that in wound healing,JmJD3 directs early macrophage-mediated inflammation via JAK1,3/STAT3 ***,in the diabetic state,we found that IL-6,a cytokine increased in diabetic wound tissue at later time points post-injury,regulates JmJD3 expression in diabetic wound macrophages via the JAK1,3/STAT3 pathway and that this late increase in JmJD3 induces NFκB-mediated inflammatory gene transcription in wound macrophages via an H3K27me3 ***,RNA sequencing of wound macrophages isolated from mice with JmJD3-deficient myeloid cells(Jmjd3f/fLyz2Cre+)identified that the STING gene(Tmem173)is regulated by JmJD3 in wound *** limits inflammatory cytokine production by wound macrophages during ***,in diabetic mice,its role changes to limit wound repair and enhance *** finding is important since STING is associated with chronic inflammation,and we found STING to be elevated in human and murine diabetic wound macrophages at late time ***,we demonstrate that macrophage-specific,nanoparticle inhibition of JmJD3 in diabetic wounds significantly improves diabetic wound repair by decreasing inflammatory cytokines and *** together,this work highlights the central role of JmJD3 in tissue repair and identifies cell-specific targeting as a viable therapeutic strategy for nonhealing diabetic wounds.

This review presents a conceptual framework and supporting evidence that links impaired motor control after sport-related concussion(SRC)to increased risk for musculoskeletal *** studies have found that athletes who are post-SRC have higher risk for musculoskeletal injury compared to their counterparts.A small body of research suggests that impairments in motor control are associated with musculoskeletal injury *** control involves the perception and processing of sensory information and subsequent coordination of motor output within the central nervous system to perform a motor *** control is inclusive of motor planning and motor *** sensory information is not accurately perceived or there is interference with sensory information processing and cognition,motor function will be altered,and an athlete may become vulnerable to injury during sport *** with SRC show neuroanatomic and neurophysiological changes relevant to motor control even after meeting return to sport criteria,including a normal neurological examination,resolution of symptoms,and return to baseline function on traditional concussion *** conjunction,altered motor function is demonstrated after SRC in muscle activation and force production,movement patterns,balance/postural stability,and motor task performance,especially performance of a motor task paired with a cognitive task(i.e.,dual-task condition).The clinical implications of this conceptual framework include a need to intentionally address motor control impairments after SRC to mitigate musculoskeletal injury risk and to monitor motor control as the athlete progresses through the return to sport continuum.

A novel deadline and budget constrained scheduling heuristics for computational grids

The conventional deadline and budget constrained （DBC） scheduling heuristics for economic-based computational grids does not take the inconsistency of grid heterogeneity into account, which can lead to decline of application completion ratios. motivated by this fact, a novel DI3C scheduling heuristics was proposed to deal with sequential workflow applications. In order to valuate the inconsistency, the relative cost （RC） metric was introduced, which was used to indicate the task-starving degree for resources. The new algorithm assigns tasks to resources, considering completion time, budget and RC together. The GridSim toolkit and the benchmark suites of the standard performance evaluation corporation （SPEC） were used to simulate the heterogeneous grid environment and applications. The experimental results show that the task and workflow completion ratios of the new heuristics are higher than those of the conventional heuristics.

Post-endoscopic retrograde cholangiopancreatography pancreatitis

Acute pancreatitis remains the most common complication of endoscopic retrograde cholangiopancreatography(ERCP).It is reported to occur in 2–10%of unselected patient samples and up to 40%of high-risk *** purpose of this article is to review the evidence behind the known risk factors for post-ERCP pancreatitis,as well as the technical and medical approaches developed to prevent *** have been many advances in identifying the causes of this *** on this knowledge,a variety of preventive strategies have been developed and *** approach to prevention begins with careful patient selection and performing ERCP for specific indications,while considering alternative diagnostic modalities when *** should also be classified by high-risk factors such as young age,female sex,suspected sphincter of Oddi dysfunction,a history of post-ERCP pancreatitis,and normal serum bilirubin,all of which have been identified in numerous research *** pathways of injury that are believed to cause post-ERCP pancreatitis eventually lead to the common endpoint of inflammation,and these individual steps can be targeted for preventive therapies through procedural techniques and medical *** includes the use of a guide wire for cannulation,minimizing the number of cannulation attempts,avoiding contrast injections or trauma to the pancreatic duct,and placement of a temporary pancreatic duct stent in high-risk *** of rectal non-steroidal anti-inflammatory agents(NSAIDs)in high-risk patients is the proven pharmacological measure for prevention of post-ERCP *** evidence for or against numerous other attempted therapies is still unclear,and ongoing investigation is required.

Retrospective analysis of hepatitis C infected patients treated through an integrated care model

AIm To determine if our health system's integrated model reflects sustained virologic response(SVR) outcomes similar to those in clinical trial data, maximizes adherence, and averts drug *** Subjects with chronic hepatitis C had their medical records reviewed from November 1st, 2014 through march 1st, 2016. Patients eligible for treatment were entered into an integrated care model therapy algorithm. The primary outcome was SVR12 based on intention to treat(ITT) analysis. Inclusion criteria consisted of both treatment na?ve and experienced patients over the age of 18 who were at least twelve weeks post-therapy completion with any genotype(GT) or mETAVIR score. Secondary outcomes included adherence, adverse events, and number of drug interaction *** At the time of analysis, 133 patients had reached twelve weeks post therapy with ITT. In the ITT analysis 70 patients were GT 1a, 26 GT 1b, 23 could not be differentiated between GT 1a or 1b, 8 GT 2, 4 GT 3, and 2 patients with multiple genotypes. The ITT treatment regimens consisted of 97 sofosbuvir(SOF)/ledipasvir(LDV), 8 SOF/LDV and ribavirin(RBV), 7 SOFand Simeprevir(SmV), 6 3D and RBV, 1 3D, 11 SOF and RBV, and 1 SOF, peg interferon alpha, and RBV. The overall SVR12 rate was 93% in the ITT analysis with a total of 6 patients relapsing. In patients with cirrhosis, 89% obtained SVR12. All 33 patients who were previous treatment failures achieved SVR12. Drug-drug interactions were identified in 56.4% of our patient population, 69 of which required interventions made by the pharmacist. The most common side effects were fatigue(41.4%), headache(28.6%), nausea(18.1%), and diarrhea(8.3%).
