Hepatocellular carcinoma (HCC), commonly known as primary liver cancer, is a major cause of malignant tumors and cancer-related deaths in china, accounting for approximately 85% of all cancer cases in the country. Several guidelines have been used to diagnose and treat liver cancer. However, these guidelines provide a broad definition for classifying advanced liver cancer, with an emphasis on a singular approach, without considering treatment options for individual patients. Therefore, it is necessary to establish a comprehensive and practical expert consensus, specifically for china, to enhance the diagnosis and treatment of HCC using the Delphi method. The classification criteria were refined for Chinese patients with HCC, and the corresponding optimal treatment regimen recommendations were developed. These recommendations took into account various factors, including tumor characteristics, vascular tumor thrombus grade, distant metastasis, liver function status, portal hypertension, and the hepatitis B virus replication status of patients with primary HCC, along with treatment prognosis. The findings and recommendations provide detailed, scientific, and reasonable individualized diagnosis and treatment strategies for clinicians.

Endoscopic sphincterotomy associated cholangitis in patients receiving proximal biliary self-expanding metal stents

BACKGROUND: Endoscopic biliary stent placement during the procedure of endoscopic retrograde cholangiopancreatography (ERCP) is preferred to provide biliary drainage for unresectable malignant biliary obstruction. There is considerable controversy over the use of endoscopic sphincterotomy (ES) prior to stent placement. This study aimed to determine whether ES before intraductal self-expanding metal stent (SEMS) placement affects the clinical outcome and complications in patients with proximal malignant obstructive biliary diseases. METHODS: In a prospective randomized controlled trial, 82 patients with inoperable malignant biliary strictures were randomly assigned to biliary stenting groups with or without ES. Resolution of jaundice and the incidence of complications including acute cholangitis, pancreatitis and stent occlusion within 6 months were evaluated. RESULTS: SEMSs were successfully deployed in all patients, resulting in clinical and biochemical improvement of obstructive symptoms in both groups. The incidence of cholangitis was higher in the ES group than in the non-ES group (58.5% vs 31.7%, P=0.015). The interval between stent placement and the first acute cholangitis was much shorter in the ES group than in the non-ES group (P=0.024). The use of ES increased the incidence of cholangitis (P=0.004, risk ratio, 8.196). The rate of post-ERCP pancreatitis after stent placement was greater in the non-ES group than in the ES group (31.7% vs 9.8%, P=0.014). No significant differences were found in the rate of restenosis and the mortality rate between the two ***: ES prior to intraductal SEMS placement was associated with an increased incidence of acute cholangitis. ES should be carefully evaluated prior to its use in patients with proximal malignant obstructive biliary diseases.

Objectives:To evaluate the impact of TangHerb(a proprietary Chinese medicine for AIDS treatment) on the efavirenz plasma concentration and the safety and efficacy of efavirenz when be used in combined with TangHerb. Design:Fifty HIV-infected patients taking efavirenz containing antiretroviral regimens were *** of them were taking TangHerb(eight capsule 3 times per day) together with antiretroviral drugs while 30 of them were ***-state plasma concentrations of efavirenz were determined using HPLC-MS *** associated central nervous system side effects and viralogic failure was observed. Results:no significant difference of the efavirenz concentration between these two groups was found(2765 ng/ml vs 2325 ng/ml,P=0.25).The frequency of efavirenz associated central nervous system side effects and viralogic failure werealso comparable among these two populaitionspatients taking TangHerb. Conclusion:TangHerbdo not impact the EFV steady plasma concentration and its *** could be used safely together with EFV.

Sexual dimorphism definitely exists in the pathogenesis of a variety of cardiovascular,neurodegenerative and bone metabolism *** affects the healing of ischemic myocardium partially through paracrine growth hormone production by bone marrow mesenchymal stem cells(MSCs) and facilitation on mobilization of endothelial progenitors cells(EPCs) to the ischemic *** can also inhibit the proliferation of the cardiac ***,estrogen effectively enhances the neovascularization at the ischemic border zone and limits pathological myocardial ***,estrogen increases proliferation of embryonic neural stem cells and accelerates differentiation of neurons during neurogenesis,suggesting a possible role of estrogen in transplantation of neural stem cells as a therapeutic approach for neurodegenerative ***,estrogen can modulate osteogenic progenitors and osteoclasts,preventing the *** general,estrogen offers significant benefits on diverse stem/progenitor cell populations.A great understanding of estrogens on these cells and relevant intracellular mechanisms will allow modulation of the potent stem cells directly for the ultimate clinical applications.

Evaluation of Hysteroscopic-guided Removal of Intrauterine Device in shanghai: A Report of Four Dead Cases

Objective To analyze the possible cause leading to death during the procecture oy IUD removal under hysteroscopy and how it could be prevented. Methods All （11 110） cases of IUD removal under hysteroscopy performed at 87 hospitals in shanghai from 2001 to 2007 were reviewed. Four dead cases were reported. Results Air embolism was suspected as the cause of death. Associated risk factors included insufficient preoperative preparation, short duration between surgeries, non-standard procedure, careless observation and untimely resuscitation. Conclusion This emphasizes the necessity of early interventions taken such as prevention, early detection and management of the fatal complications.

Guidelines for treatment of severe acute pancreatitis

Severe acute pancreatitis is a disease of rap- id onset and progression with high mortality and morbidity. A new concept suggests that it is multifac- torially induced and involves multi-links and that therapy must be directed to several more important links at the same time. The regimen of integrated traditional Chinese and western medicine acts on the same or different sites and links to inhibit or block the inflammatory cascades. Concomitantly, preven- tive measures were taken at the start of the treatment for preventing the progression of the disease, protec- ting the function of vital organs, and avoiding the oc- currence of complications. No mortality or serious complications occurred in a series of 38 patients with Balthazar's CT grading of D and E. The treatment regimen and the mechanisms of actions of these drugs were delineated in detail in the tables and figure; the regimen is highly recommended for clinical use.

Asymptomatic process of hepatic artery thrombosis in a patient after orthotopic liver transplantation

BACKGROUND: Hepatic artery thrombosis is one of the serious complications after liver transplantation. It wil mostly cause a failure of the transplantation. This case o hepatic artery thrombosis showed a stable clinical course and minimal histological change, and now has been survi ving for 4 years with normal liver function. We investigated the possible causes for asymptomatic hepatic artery throm bosis in one patient after orthotopic liver transplantation ( OLT) and discussed the diagnosis of ischemia of OLT pathologically and clinically. METHODS: Liver function test, color Doppler ultrasono graphy, and hepatic arteriography were performed during the development of hepatic arteriothrombosis. Possible fac- tors for the asymptomatic process of the thrombosis were analyzed. RESULTS: On the 4th postoperative day, thrombosis form ed at the anastomotic stoma of the hepatic artery, and on the 11th postoperative day, the artery was completely oc- cluded. Serial liver biopsies revealed intrahepatic cholesta- sis, hydropic degeneration of hepatocytes, atrophy of the biliary epithelium, and fibrosis in the portal area. Monitor- ing of liver function showed nothing abnormal except ele- vation of γ-GT and ALP levels. On the 71st day after OLT, arteriography demonstrated that the hepatic artery re- mained completely occluded in addition to the establish- ment of collateral circulation and compensation of the por- tal vein. The patient didn' t show any symptoms of arterial thrombosis. CONCLUSION: Collateral circulation and compensation o the portal vein are beneficial to allograft survival and avoidence of retransplantation after thrombosis of the he- patic artery. Color Doppler ultrasonography within 2 weeks after OLT is helpful to the early diagnosis of hepatic arterio- thrombosis.
