Risk factors and mortality rates of carbapenem-resistant Gram-negative bacterial infections in intensive care units

Background The prevalence of hospital-acquired infections caused by carbapenem-resistant gram-negative bacteria(CRGNB)is increasing *** risk factors have been associated with such *** present study aimed to identify risk factors and determine the mortality rates associated with CRGNB infections in intensive care *** This retrospective case-control study was conducted at Erciyes University Hospital(Kayseri,Turkey)between January 2017 and December *** and laboratory data were obtained from the Infection Control Committee data and record *** who had CRGNB infection 48–72 h after hospitalization were assigned to the case group,while those who were not infected with CRGNB during hospitalization formed the control *** factors,comorbidity,demographic data,and mortality rates were compared between the two *** Approximately 1449 patients(8.97%)were monitored during the active follow-up period;of those,1171 patients were included in this *** infection developed in 14 patients(70.00%)who had CRGNB colonization at admission;in 162(78.26%)were colonized during hospitalization,whereas 515(54.56%)were not *** was no significant difference in age,sex(male/female)or *** total length of hospital stay was statistically significantly longer(P=0.001)in the case group(median:24[interquartile range:3–378]days)than the control group(median:16[interquartile range:3–135]days).The rates of colonization at admission(25.5%;vs.10.6%,P=0.001)and mortality(64.4%vs.45.8%,P=0.001)were also significantly higher in the cases than in the control group,*** the univariate analysis,prolonged hospitalization,the time from intensive care unit admission to the development of infection,presence of CRGNB colonization at admission,transfer from other hospitals,previous antibiotic use,enteral nutrition,transfusion,hemodialysis,mechanical ventilation,tracheostomy,reintubation,central veno

Services provided at the premarital counseling center and characteristics of the clients, in kayseri, Turkey

Objectives: The study was conducted retro-spectively to evaluate the services offered at the Premarital Counseling Center (PCC) and the characteristics of the individuals who obtained counseling at the center in Kayseri. Methods: A retrospective review is based on data from the Premarital Counseling Center (PCC) in Kayseri. Seven hundred fifty four (754) individuals who attended the Center were included in the study sample. Data were collected from the Center’s records. Records of all people were reviewed by the researchers. Differences in proportions were analyzed by Chi square test and t test. Results: The age-average of the individuals who consulted the PCC was 26.2 ± 8.3, 91.1% of the individuals had Rh (+), 99.5% of them had VDRL (-), 3.9% had Hbs Ag (+), 0.8% had HCV (+) while none of the individuals was HIV (+) affected. It’s shown that 90.6% of the individuals who were between the ages of 15 - 19 were women, 9.4% of them were men. The difference between the age and the gender of the individuals who came to the center was considered significant. It was determined that the individuals who received counseling services at the PCC were not given any kind of educational and consulting services before.

The existing systems sustained with the investments made require more automation and digital transformation with the continuous advancement of *** aviation industry is a sector that is open to more automation and digi-tal transformation,mainly because of the intense competition and the analysis of a large variety of *** long duration of operations in current airline processes and some processﬂows cause customer dissatisfaction and cost *** this study,the boarding process,which is one of the operational processes of airline transportation and is open to improvement,was *** classical boarding process has been redesigned using Internet of Things technology a model called Boarding 4.0 was *** Boarding 4.0,it is aimed to design a process where passengers can take their time before boarding more efﬁ*** the study,the sub-processes of the Boarding 4.0 model,other processes that the sub-processes interact with,their activities,and data exchange passenger move-ments during the activities are explained in *** to the classical boarding process and Boarding 4.0 with the fuzzy ahp technique,it has been shown that boarding 4.0 is more advantageous and passenger movement times can be reduced during *** a result of the evaluation made with the fuzzy ahp,it was determined that boarding 4.0 is more advantageous than the classical boarding *** addition,when the total time of the sub-activities in the board-ing process is calculated,boarding activities for a passenger take 50 min with the classic boarding process and 20 min with Boarding ***,when Boarding 4.0 is used,the passenger gains 30 ***,when the calculation is made concerning the airport’s current capacity,two passengers are hosted with the clas-sical boarding process,whileﬁve passengers are hosted with Boarding *** acquisition is signiﬁcant for airports in terms of efﬁcient use of resources.

Assessment of socio-demographic factors,health status and the knowledge on probiotic dairy products

The probiotic dairy products have become the fastest growing in functional foods market in response to the increasing numbers of consumers interested in improving their ***,data collected from 314 participants in the city of Bursa,Turkey,were analyzed with the aim to assess the socio-demographic characteristics and health status as well as the consumer’s knowledge and awareness of probiotic dairy products in order to gain a better understanding of consumers’*** was observed that sociodemographic characteristics of the participants were not significantly associated with the degree of knowledge or awareness and purchasing frequencies of probiotic dairy *** a higher educational level,higher income level and being a woman indicated an increased the awareness,knowledge and purchase probability of probiotic dairy *** study emphasized that consumers should be more informed about the positive relationship between a healthier life and the consumption of probiotic dairy products.

Evaluation of choroidal thickness using spectral-domain optical coherence tomography in patients with severe obstructive sleep apnea syndrome: a comparative study

AIM: To assess choroidal thickness in patients with severe obstructive sleep apnea syndrome(OSAS) and compare them with healthy controls, using spectral domain optical coherence tomography(OCT).METHODS: In this observational, cross-sectional study,choroidal thicknesses of 23 newly severe OSAS patients and 23 body mass index- age- and sex-matched healthy subjects were measured using a high-speed, high-resolution frequency domain- OCT device( λ = 840 nm,26 000 A-scans/s, 5 μm axial resolution). All patients underwent a complete ophthalmic examination before the measurements. OCT measurements were taken at the same time of day(9:00 a.m.), in order to minimize the effects of diurnal ***: There was a statistically significant difference in median choroidal thickness between the OSAS patients(201 μm; range 145-237 μm) and the controls(324 μm; range 296-383 μm;P <0.001). There were significant differences at all measurement points(P <0.001 for all). The apnea-hypopnea index(AHI)values were more than 30 in all OSAS patients and the mean AHI was 48.57 ±6.54. The interexaminer intraclass correlation coefficient(ICC) for the mean choroidal thickness was 0.938(95% CI, 0.908-0.985) and ICC was greater than 0.90 for all measurement ***: The decreased choroidal thickness of patients with severe OSAS might be related to the the autonomic disregulation associated with this *** studies are needed to evaluate the etiopathologic relationship between choroidal thickness and OSAS.

Can the Prediction of Intrauterine Insemination Results by Used Aniline Blue Stain (ABS) and Sperm Chromatin Dispersion (SCD) Levels?

Introduction: This study aimed to perform routine seminal fluid analysis, sperm DNA fragmentation, and sperm function tests at the chromatin maturation level and evaluate pregnancy in the patients passing intrauterine insemination before starting Intrauterine Insemination (IUI) method. Materials and Methods: In this prospective study, 111 couples who underwent Intrauterine Insemination (IUI) in unexplained infertility patients were admitted to Al-Farah IVF and assisted reproductive center in Baghdad, Iraq between November 2020 and February 2021 were evaluated. Semen fluid analysis was performed based on (WHO 4th) guiding rules. In addition, Sperm Chromatin Dispersion (halo test) and sperm maturation were performed with Aniline Blue Stain (ABS). Results: Sperm Chromatin Dispersion (SCD) groups were compared in terms of pregnancy outcome;the positive pregnancy rate was found to be above in the normal SCD groups (p = 0.0005). In addition, Aniline Blue Stain (ABS) groups were compared in the terms of pregnancy outcome;the positive pregnancy rate was found to be higher in the normal ABS group (p = 0.017). Conclusion: Our study showed that the use of DNA fragmentation (SCD) and sperm maturation tests (ABS) together with routine semen analysis in intrauterine insemination cases will make a significant contribution to the prediction of Intrauterine Insemination (IUI) increased results. So, these results indicate a defect in the effect of DNA fragmentation on the outcome of intrauterine insemination.

Ultrasound biomicroscopy in patients with unilateral pseudoexfoliation

·AIM: To compare the anterior segment morphology evaluated using ultrasound biomicroscopy(UBM) in patients with clinical pseudoexfoliation syndrome(XFS)in one eye and no clinical XFS in the fellow eye.·METHODS: Thirty patients with unilateral XFS were included in the study. All patients underwent evaluation of their anterior segment using UBM with and without dilatation with 1% cyclopentolate. The anterior chamber depth(ACD), lens thickness(LT), anterior chamber angle(ACA), ciliary body thickness(CBT), scleral thickness(ST), trabeculae-ciliary processes distance(T-CPD), and iris-ciliary processes distance(I-CPD) were measured using UBM scans. All results between the eyes with clinical XFS and their fellow eyes without clinical XFS were then compared.·RESULTS: Before dilatation the eyes with XFS(4.350±0.531 mm) were found to have a significantly thicker lens(P =0.002) than the eyes without XFS(4.238 ±0.540 mm).In addition after dilatation, the eyes with XFS(4.310 ±0.500 mm) were found to have a significantly thicker lens than the eyes without XFS(4.160±0.480 mm)(P=0.019).The average ACD, for the group with XFS, comparing pre-dilatation(2.616 ±0.349 mm) and post-dilatation measurements(2.714±0.413) was found to be statistically increased(P =0.014). The average ACD, comparing pre-dilatation to post-dilatation measurements in patients without XFS(2.680±0.360),(2.720±0.500) was found to be statistically unchanged(P =0.450).·CONCLUSION: Crystalline lenses tended to be thicker in the eyes with clinical pseudoexfoliation than their fellow eyes without pseudoexfoliation.

Obestatin in male reproduction and infertility

Obestatin is a 23-amino acid peptide hormone secreted by the stomach and is found in several tissues all over the body, such as the gastric mucosa, spleen, mammary gland, plasma as well as in the testicular Leydig cells. Obestatin seems to operate as part of the integrated gut-brain network acting as an anorectic hormone, reducing food intake and reversing body weight gain. Besides the expressions of obestatin in male reproductive tissues, it is also shown to increase testosterone secretions, thus ameliorating testicular functions. In the present scenario where the increasing prevalence in obesity is considered as one of the major causatives of worldwide declining trend of semen quality, molecules like obestatin playing roles in both metabolic and reproductive functions find importance in management of obesity-induced male infertility or subfertility. The present review article aims to provide updated concepts on obestatin and its mode of actions, and its role in modulation of male reproductive functions.
