Therapeutic efficacy of the Qing Dai in patients with intractable ulcerative colitis

Ulcerative colitis (UC) is a chronic inflammatory bowel disease that may become intractable when treated with conventional medications such as aminosalicylates, corticosteroids, and azathioprine. The herbal medicine Qing Dai has traditionally been used in Chinese medicine to treat UC patients, but there is a lack of published data on the efficacy of Qing Dai in UC treatment. We report several cases of patients with intractable UC who take Qing Dai in a retrospective observational study. Furthermore, we explore the mechanisms of action of Qing Dai. Nine patients with active UC who received conventional medications but wished to receive Qing Dai as an alternative medication were included in our analysis. The UC severity level was determined based on the clinical activity index (CAI). Additionally, 5 of the 9 patients were endoscopically evaluated according to the Matts grading system. Each patient received 2 g/d of Qing Dai orally and continued taking other medications for UC as prescribed. Electron spin resonance was applied to explore the mechanisms of action of Qing Dai. After 4 mo of treatment with Qing Dai, the CAI score decreased from 8.3 ± 2.4 to 2.4 ± 3.4 (mean ± SD; P < 0.001). Similarly, the endoscopic Matts grade decreased from 3.4 ± 0.5 to 2.2 ± 0.8 (P = 0.02). Six of 7 patients who were on prednisolone upon enrollment in the study were able to discontinue this corticosteroid. Electron spin resonance revealed that Qing Dai possesses strong hydroxyl radical scavenging activity. Qing Dai showed significant clinical and endoscopic efficacy in patients who failed to respond to conventional medications. Scavenging of hydroxyl radicals appears to be a potential mechanism through which Qing Dai acts, but the significance of the scavenging ability of Qing Dai with respect to the anti-inflammatory effect in UC patients warrants further investigation.

Reoperative cardiac surgery is becoming more common surgery although it carries significant risk due to possible injury to vital structures under the sternum. Three-dimensional multidetector computed tomographic angiography (3-D MDCTA) allows identifying the relationship between the sternum and the mediastinal structures. Evidence shows that 3-D MDCTA guides surgical strategy and enables to perform this challenging surgery safely.

Malnutrition and Its Association with the Mortality of Patients with Femoral Intertrochanteric Fractures: A Retrospective Analysis of Japanese Patients

Background: The incidence of femoral intertrochanteric fractures in older adults is higher than that of femoral neck fractures;however, both conditions are often analyzed together as proximal femoral fractures. Considering the difference in treatment, postoperative complication, and mortality risk, these two fractures should be analyzed separately. This study aimed to analyze 1-year mortality and its risk factors in patients with surgically treated femoral intertrochanteric fractures. Methods: Consecutive patients with intertrochanteric fractures who underwent surgical interventions at our institution between January 2017 and December 2021 were retrospectively reviewed. A total of 238 patients were eligible for inclusion in this study. Patients’ demographic and clinical information were retrospectively collected. Patients were divided into the 1-year mortality (n = 16) and survival (n = 222) groups. The incidence of 1-year mortality and its independent risk factors were investigated using univariate and multivariate logistic regression analyses. Results: The mean age of patients was 85.6 ± 8.5 years. The 1-year mortality rate was 6.7% (16/238). Preoperative albumin level, the Geriatric Nutritional Risk Index (GNRI), and malnutrition status (GNRI p = 0.02, p = 0.02, and p = 0.0011, respectively). Multivariate analysis showed that malnutrition status (GNRI p = 0.035) was an independent risk factor for 1-year mortality. Conclusion: Malnutrition status assessed using GNRI (GNRI < 92) was an independent risk factor for 1-year mortality. Our findings suggest that GNRI may be an effective screening tool for predicting postoperative 1-year mortality of patients with surgically treated femoral intertrochanteric fractures.

Photocatalytic Activity of Anodized Titanium Sheets under Ultra-Violet and Visible Light Irradiation

A commercially pure titanium sheet precipitated TiC in the surface layer was fabricated by anodic oxidation in NH4NO3 solution and heat treatment in air. The fabricated sheet showed relatively high photocatalytic activity in 0.1 M KI solution, which was close to the activity level of the P-25 particle made by Degussa Corporation. It exhibited photocatalytic activity in antifungal and antivirus tests under black light irradiation. The better photocatalytic activity under black light irradiation is considered to be related to the formation of anatase and rutile type titanium dioxides and rough surface. It also showed photocatalytic activity under visible light irradiation, which is considered to be attributable to carbon and nitrogen doping in titanium dioxide.

Photocatalytic Activity of Titanium Dioxide Powder Fabricated from an Anodized Titanium Sheet under Ultra-Violet and Visible Light Irradiation

A commercially pure titanium sheet with titanium carbide (TiC) precipitated in its surface layer was anodized in NH4NO3 aqueous solution and heat treated in air. The photocatalytic activity of titanium dioxide powder collected from the surface of the anodized titanium sheet was evaluated under ultra-violet and visible light irradiation. It showed relatively high photocatalytic activity in 0.1 mol/l potassium iodide solution, which was almost equal to the activity level of TiO2 powder (P-25) manufactured by Degussa Corporation. The better photocatalytic activity under ultra-violet irradiation is considered to be related to the formation of anatase type titanium dioxide. Photocatalytic activity under visible light irradiation was also observed, which was considered to be attributable to impurity doping, (carbon), in the titanium dioxide powder.

Characterization of Surface Oxide Layers on Black-Colored Titanium

Black-colored titanium was obtained by anodic oxidation of a commercially pure grade-1 titanium sheet in a 0.06 M NH4NO3 solution, followed by heat treatment at 773 K for 1 h in a vacuum furnace. The resulting oxide layer on the titanium substrate was examined by X-ray photoelectron spectroscopy, X-ray diffraction, glow discharge spectroscopy, and scanning electron microscopy. It was found that the oxide layer on the black-colored titanium sheet was several micrometers thick and mainly consisted of rutile TiO2 exhibiting a sponge like nanoporous structure. It is considered that the black-colored appearance of the titanium sheet is due to the sponge like nanoporous structure of the oxide layer absorbing the incident light. The photocatalytic activity of the black-colored titanium sheet was examined by observing the decomposition of a methylene blue (MB, C16H18N3SCl) solution under ultraviolet irradiation due to the existence of rutile TiO2. The sheet also exhibited photocatalytic activity under visible light irradiation. It is believed that the photocatalytic response upon irradiation with white light is due to carbon doping of the titanium oxide layer on the titanium substrate.

Fabrication of Tungsten Carbide Films by Filtered Pulse Arc Deposition with Cemented Tungsten Carbide Cathodes

Tungsten carbide films (W-C films) were fabricated on silicon substrates by using the filtered pulse arc deposition (FPAD) method. Two types of cemented tungsten carbide (WC) were used as cathode, one containing Co and the other Ti, which were used as binders for forming the cathode shape. The films were fabricated by varying the pulse arc current and substrate bias voltage. The discharge, deposition and film properties were investigated under these deposition conditions. The cathode wear amount when using WC-Co (WC cathode containing Co) was found to be smaller than that measured when WC-Ti (WC cathode containing Ti) was used. The W-C film thickness was approximately 30 - 40 nm under all conditions, except when the pulse arc current was 50 A and the film thickness, was approximately 10 nm. Compared to the WC-Ti, the consumption of cathode material is suppressed in the WC-Co, indicating that the efficiency for film preparation of the latter is good. From the X-ray diffraction analysis, the crystalline phase of W-C films fabricated using WC-Co and WC-Ti were observed as W2C and WC1-x, respectively, indicating that different crystalline phases could be fabricated using different cathodes. From the X-ray photoelectron spectroscopy analysis, the oxidation layer formed by air exposure was observed to exclusively exist on the W-C film surface. Moreover, almost all oxygen in the oxidation layer bonded with tungsten.

Artificial joint replacement surgery is the orthopedic procedure of choice to relieve pain, correct joint deformities, and help patients resume everyday activities. However, the detailed mechanisms regulating peri-implant bone remodeling at the bone-prosthesis interface remain elusive. To address this gap in knowledge, we evaluated the natural postoperative course of bone metabolism by using [18F] NaF positron emission tomography (PET)/computed tomography in 57 patients (104 joints) who underwent asymptomatic total knee arthroplasty (TKA). First, we measured total tracer uptake around TKA to determine the total bone metabolism (TBM) and used the ischial tuberosity as the reference tissue for normalization of PET images. Second, we calculated the TBM ratio (TBMR) by dividing the TBM by tracer uptake in the reference tissue. A moderate increase in TBMR was observed 4 - 5 days after implantation, and its intensity reached the maximum on the seventh postoperative day. The TBMR showed no differences until 12 weeks, after which it decreased slowly and returned to the basal levels. The new parameter TBMR and the unique pattern of postoperative metabolic changes in the bone around the prosthesis may help set accurate interpretation criteria to diagnose complications such as loosening or infections.
