Clinicopathological features of alpha-fetoprotein producing early gastric cancer with enteroblastic differentiation

AIM To investigate clinicopathological features of early stage gastric cancer with enteroblastic differentiation(GCED).METHODS We retrospectively investigated data on 6 cases of early stage GCED and 186 cases of early stage conventional gastric cancer(CGC: well or moderately differentiated adenocarcinoma) who underwent endoscopic submucosal dissection or endoscopic mucosal resection from September 2011 to February 2015 in our hospital.GCED was defined as a tumor having a primitive intestine-like structure composed of cuboidal or columnar cells with clear cytoplasm and immunohistochemical positivity for either alpha-fetoprotein, Glypican 3 or SALL4. The following were compared between GCED and CGC: age, gender, location and size of tumor, macroscopic type, ulceration, depth of invasion, lymphatic and venous invasion, positive horizontal and vertical margin, curative resection rate.RESULTS Six cases(5 males, 1 female; mean age 75.7 years; 6 lesions) of early gastric cancer with a GCED component and 186 cases(139 males, 47 females; mean age 72.7 years; 209 lesions) of early stage CGC were investigated. Mean tumor diameters were similar but rates of submucosal invasion, lymphatic invasion, venous invasion, and non-curative resection were higher in GCED than CGC(66.6% vs 11.4%, 33.3% vs 2.3%, 66.6% vs 0.4%, 83.3% vs 11% respectively, P < 0.01). Deep submucosal invasion was not revealed endoscopically or by preoperative biopsy. Histologically, in GCED the superficial mucosal layer was covered with a CGC component. The GCED component tended to exist in the deeper part of the mucosa to the submucosa by lymphatic and/or venous invasion, without severe stromal reaction. In addition, Glypican 3 was the most sensitive marker for GCED(positivity, 83.3%), immunohistochemically.CONCLUSION Even in the early stage GCED has high malignant potential, and preoperative diagnosis is considered difficult. Endoscopists and pathologists should know the clinicopathological features of this highly malignant type of cancer.

Mallory-Weiss tear during gastric endoscopic submucosal dissection

A 78-year-old woman was referred to our department for treatment of an early gastric cancer. Esophagogastroduodenoscopy (EGD) demonstrated a flat elevated lesion and a polypoid lesion on the greater curvature of the antrum. Histological analysis of, endoscopic biopsy samples taken from these lesions revealed an adenocarcinoma and a hyperplastic polyp, respectively. ESD was conducted for removal of the lesions. Carbon dioxide (CO2) in- stead of room air was used for insufflation, and the patient was adequately sedated without struggling or vomiting during the treatment. No significant bleeding from the lesion was observed during ESD, but fresh blood was identified endoscopically. Surprisingly, a Mallory- Weiss tear with active bleeding was detected on the lesser curvature of the gastric corpus. A total of eight hemoclips were applied for hemostasis. Both lesions were completely removed en bloc, and no bleeding or perforation developed after ESD. Histologically, the first lesion was apapillary carcinoma limited to the mucosal layer and without lymphovascular invasion or involvement of the surgical margins, while the second lesion was a benign hyperplastic polyp.

The heterogeneous loss of HO_(2)radicals onto ambient aerosols plays an important role in tropospheric chemistry.However,sparse investigation of the dominating parameters controlling the HO_(2)uptake coefficients onto ambient aerosols(γHO_(2))has largely hindered the application of the measuredγHO_(2)to the global spatial prediction.Here we induced an offine method using LFP-LIF technique to measure the kinetics of HO_(2)uptake onto ambient aerosols collected in summertime and wintertime in Yokohama city,a regional urban site near Tokyo,Japan.By controlling the dominating parameters which infuenceγHO_(2),we were able to investigate the detailed HO_(2)uptake mechanism.We characterized the chemical composition of the collected ambient aerosols,including organics,inorganics,transition metals ions,etc.and modeledγHO_(2)using different mechanisms.Results show thatγHO_(2)increased with the increase in RH,and the aerosol states(“dry”or wet/aqueous)have large effects onγHO_(2).With fixed RH and aerosol chemical composition,γHO_(2)was highly dependent on pH and inversely correlated with[HO_(2)]_0.By combing the measuredγHO_(2)values with the modeled ones,we found that both the HO_(2)self-reaction and transition metal-catalyzed reactions should be accounted for to yield a single parameterization to predictγHO_(2),and different chemical compositions may have collective effects onγHO_(2).Results may serve for extending theγHO_(2)values measured at one observation site to different environmental conditions,which will help us to achieve more accurate modeling results concerning secondary pollutant formation(i.e.,ozone).

Goblet cell carcinoid of the appendix:Six case reports

BACKGROUND Goblet cell carcinoid(GCC)of the appendix is a rare tumor characterized by neuroendocrine and adenocarcinoma features.Accurate preoperative diagnosis is very difficult,with most patients complaining mainly of abdominal pain.Computed tomography shows swelling of the appendix,so diagnosis is usually made incidentally after appendectomy based on a preoperative diagnosis of appendicitis.Even if a patient undergoes preoperative colonoscopy,accurate endoscopic diagnosis is very difficult because GCC shows a submucosal growth pattern with invasion of the appendiceal wall.CASE SUMMARY Between 2017 and 2022,6 patients with GCC were treated in our hospital.The presenting complaint for 5 of these 6 patients was abdominal pain.All 5 patients underwent appendectomy,including 4 for a preoperative diagnosis of appendicitis and the other for diagnosis and treatment of an appendiceal tumor.The sixth patient presented with vomiting and underwent ileocecal resection for GCC diagnosed from preoperative biopsy.Although 2 patients with GCC underwent colonoscopy,no neoplastic changes were identified.Two of the six patients showed lymph node metastasis on pathological examination.As of the last followup(median:15 mo),all cases remained alive without recurrence.CONCLUSION As preoperative diagnosis of GCC is difficult,this possibility must be considered during surgical treatments for presumptive appendicitis.

Risk factors and diagnostic biomarkers for nonalcoholic fatty liver disease-associated hepatocellular carcinoma:Current evidence and future perspectives

High rates of excessive calorie intake diets and sedentary lifestyles have led to a global increase in nonalcoholic fatty liver disease(NAFLD).As a result,this condition has recently become one of the leading causes of hepatocellular carcinoma(HCC).Furthermore,the incidence of NAFLD-associated HCC(NAFLD-HCC)is expected to increase in the near future.Advanced liver fibrosis is the most common risk factor for NAFLD-HCC.However,up to 50%of NAFLDHCC cases develop without underlying liver cirrhosis.Epidemiological studies have revealed many other risk factors for this condition;including diabetes,other metabolic traits,obesity,old age,male sex,Hispanic ethnicity,mild alcohol intake,and elevated liver enzymes.Specific gene variants,such as single-nucleotide polymorphisms of patatin-like phospholipase domain 3,transmembrane 6 superfamily member 2,and membrane-bound O-acyl-transferase domaincontaining 7,are also associated with an increased risk of HCC in patients with NAFLD.This clinical and genetic information should be interpreted together for accurate risk prediction.Alpha-fetoprotein(AFP)is the only biomarker currently recommended for HCC screening.However,it is not sufficiently sensitive in addressing this diagnostic challenge.The GALAD score can be calculated based on sex,age,lectin-bound AFP,AFP,and des-carboxyprothrombin and is reported to show better diagnostic performance for HCC.In addition,emerging studies on genetic and epigenetic biomarkers have also yielded promising diagnostic potential.However,further research is needed to establish an effective surveillance program for the early diagnosis of NAFLD-HCC.

FDNet:A Deep Learning Approach with Two Parallel Cross Encoding Pathways for Precipitation Nowcasting

With the goal of predicting the future rainfall intensity in a local region over a relatively short period time,precipitation nowcasting has been a long-time scientific challenge with great social and economic impact.The radar echo extrapolation approaches for precipitation nowcasting take radar echo images as input,aiming to generate future radar echo images by learning from the historical images.To effectively handle complex and high non-stationary evolution of radar echoes,we propose to decompose the movement into optical flow field motion and morphologic deformation.Following this idea,we introduce Flow-Deformation Network(FDNet),a neural network that models flow and deformation in two parallel cross pathways.The flow encoder captures the optical flow field motion between consecutive images and the deformation encoder distinguishes the change of shape from the translational motion of radar echoes.We evaluate the proposed network architecture on two real-world radar echo datasets.Our model achieves state-of-the-art prediction results compared with recent approaches.To the best of our knowledge,this is the first network architecture with flow and deformation separation to model the evolution of radar echoes for precipitation nowcasting.We believe that the general idea of this work could not only inspire much more effective approaches but also be applied to other similar spatio-temporal prediction tasks.

Drug-induced entero-colitis due to interleukin-17 inhibitor use;capsule endoscopic findings and pathological characteristics:A case report

BACKGROUND Interleukin-17(IL-17)inhibitors are known to cause exacerbation or new onset of inflammatory bowel disease upon administration.However,few reports have described characteristic endoscopic and histopathologic findings,and no small intestinal lesions have been reported so far.CASE SUMMARY A woman in her 60s with psoriasis was administered ixekizumab(IXE),an anti-IL-17A antibody,for the treatment of psoriasis.Twenty months after commencing treatment,the patient visited our hospital because of persistent diarrhea.Blood tests performed at the time of the visit revealed severe inflammation,and colonoscopy revealed multiple round ulcers throughout the colon.A tissue biopsy of the ulcer revealed infiltration of inflammatory cells and granuloma-like findings in the submucosal layer.Capsule endoscopy revealed multiple jejunal erosions.After the withdrawal of IXE,the symptoms gradually improved,and ulcer reduction and scarring of the colon were endoscopically confirmed.CONCLUSION To the best of our knowledge,17 reports have documented IL-17 inhibitorinduced entero-colitis with endoscopic images,endoscopic findings,and pathological characteristics,including the present case.Nine of these cases showed diffuse loss of vascular pattern,coarse mucosa/ulcer formation in the left colon,and endoscopic findings similar to those of ulcerative colitis.In the remaining eight cases,discontinuous erosions and ulcerations from the terminal ileum to the rectum were seen,with endoscopic findings similar to those of Crohn’s disease.In this case,the findings were confirmed by capsule endoscopy,which has not been previously reported.

Analysis for Effects of Temperature Rise of PV Modules upon Driving Distance of Vehicle Integrated Photovoltaic Electric Vehicles

The development of vehicle integrated photovoltaics-powered electric vehicles (VIPV-EV) significantly reduces CO2 emissions from the transport sector to realize a decarbonized society. Although long-distance driving of VIPV-EV without electricity charging is expected in sunny regions, driving distance of VIPV-EV is affected by climate conditions such as solar irradiation and temperature rise of PV modules. In this paper, detailed analytical results for effects of climate conditions such as solar irradiation and temperature rise of PV modules upon driving distance of the VIPV-EV were presented by using test data for Toyota Prius and Nissan Van demonstration cars installed with high-efficiency InGaP/GaAs/InGaAs 3-junction solar cell modules with a module efficiency of more than 30%. The temperature rise of some PV modules studied in this study was shown to be expressed by some coefficients related to solar irradiation, wind speed and radiative cooling. The potential of VIPV-EV to be deployed in 10 major cities was also analyzed. Although sunshine cities such as Phoenix show the high reduction ratio of driving range with 17% due to temperature rise of VIPV modules, populous cities such as Tokyo show low reduction ratio of 9%. It was also shown in this paper that the difference between the driving distance of VIPV-EV driving in the morning and the afternoon is due to PV modules’ radiative cooling. In addition, the importance of heat dissipation of PV modules and the development of high-efficiency PV modules with better temperature coefficients was suggested in order to expand driving range of VIPV-EV. The effects of air-conditioner usage and partial shading in addition to the effects of temperature rise of VIPV modules were suggested as the other power losses of VIPV-EV.

A Study of Hie and LBP: The First Step to Improve Subjective Well-Being

Backgrounds: Health does not only physical health, therefore, we need to study it from various viewpoints. Many Japanese female complain of a Hie or a low back pain (LBP), which they reduce their subjective well-being. We analyze. Those patients often have acupuncture therapy. In this study, we analyze the characteristics of Hie and LBP, the satisfaction level of alternative therapy and we pursue a tip to improve subjective well-being. Methods: Of 1000 women, Hie (+)/Hie (−) or LBP (+)/LBP (−), we compared their body temperature (BT) (axilla) and body mass index (BMI). Furthermore, the Chi test identified ten factors of “body” and seven “mind” information. Results: In the result of BT (axilla) while LBP indicated a significant difference. Both Hie and LBP showed difference in the opposite direction. Hie did now show such clear differences in “body” information. However, interestingly, all seven questions in the “mind” information showed statistical difference. Discussion and conclusion: One reason why those patients have acupuncture therapy may acupuncture therapy traditionally has not separate “mind” and “body” and it has the concept of “mind-body unity”. To improve subjective well-being, first we need to focus on “Mind” as well as “mind-body unity”. Mental-health support is important for patients with Hie or LBP to reduce physiological stress.

Imaging through random media continues to be a challenging problem of crucial importance in a wide range of fields of science and technology,ranging from telescopic imaging through atmospheric turbulence in astronomy to microscopic imaging through scattering tissues in biology.To meet the scope of this anniversary issue in holography,this review places a special focus on holographic techniques and their unique functionality,which play a pivotal role in imaging through random media.This review comprises two parts.The first part is intended to be a mini tutorial in which we first identify the true nature of the problems encountered in imaging through random media.We then explain through a methodological analysis how unique functions of holography can be exploited to provide practical solutions to problems.The second part introduces specific examples of experimental implementations for different principles of holographic techniques,along with their performance results,which were taken from some of our recent work.
