Assessment and Visualization of Ki67 Heterogeneity in Breast Cancers through Digital Image Analysis

The Ki67 index (KI) is a standard clinical marker for tumor proliferation;however, its application is hindered by intratumoral heterogeneity. In this study, we used digital image analysis to comprehensively analyze Ki67 heterogeneity and distribution patterns in breast carcinoma. Using Smart Pathology software, we digitized and analyzed 42 excised breast carcinoma Ki67 slides. Boxplots, histograms, and heat maps were generated to illustrate the KI distribution. We found that 30% of cases (13/42) exhibited discrepancies between global and hotspot KI when using a 14% KI threshold for classification. Patients with higher global or hotspot KI values displayed greater heterogenicity. Ki67 distribution patterns were categorized as randomly distributed (52%, 22/42), peripheral (43%, 18/42), and centered (5%, 2/42). Our sampling simulator indicated analyzing more than 10 high-power fields was typically required to accurately estimate global KI, with sampling size being correlated with heterogeneity. In conclusion, using digital image analysis in whole-slide images allows for comprehensive Ki67 profile assessment, shedding light on heterogeneity and distribution patterns. This spatial information can facilitate KI surveys of breast cancer and other malignancies.

Gastric cancer is believed to result in part from the accumulation of multiple genetic alterations leading to oncogeneoverexpression and tumor suppressor loss. Epigenetic alterations as a distinct and crucial mechanism to silence a varietyof methylated tissue-specific and imprinted genes, have been extensively studied in gastric carcinoma and play impor-tant roles in gastric carcinogenesis. This review will briefly discuss the basic aspects of DNA methylation and CpGisland methylation, in particular the epigenetic alterations of certain critical genes implicated in gastric carcinogenesisand its relevance of clinical implications.

A steel dual-core self-centering brace （DC-SCB） is an innovative structural member that provides both energy dissipation and self-centering properties to structures, reducing maximum and residual drifts of structures in earthquakes. The axial deformation capacity of the DC-SCB is doubled by a parallel arrangement of two inner cores, one outer box and two sets of tensioning elements. This paper presents cyclic test results of a DC-SCB component and a full- scale one-story, one-bay steel frame with a DC-SCB. The DC-SCB that was near 8 m-long was tested to evaluate its cyclic behavior and durability. The DC-SCB performed well under a total of three increasing cyclic loading tests and 60 low- cycle fatigue loading tests without failure. The maximum axial load of the DC-SCB was near 1700 kN at an interstory drift of 2.5%. Moreover, a three-story dual-core self-centering braced frame （DC-SCBF） with a single-diagonal DC-SCB was designed and its first-story, one-bay DC-SCBF subassembly specimen was tested in multiple earthquake-type loadings. The one-story, one-bay subassembly frame specimen performed well up to an interstory drift of 2% with yielding at the column base and local buckling in the steel beam; no damage of the DC-SCB was found after all tests. The maximum residual drift of the DC-SCBF caused by beam local buckling was 0.5% in 2.0% drift cycles.

This paper presents an off-line handwritten signature verification system based on the Siamese network,where a hybrid architecture is *** Residual neural Network(ResNet)is used to realize a powerful feature extraction model such that Writer Independent(WI)features can be effectively learned.A single-layer Siamese Neural Network(NN)is used to realize a Writer Dependent(WD)classifier such that the storage space can be *** the purpose of reducing the impact of the high intraclass variability of the signature and ensuring that the Siamese network can learn more effectively,we propose a method of selecting a reference signature as one of the inputs for the Siamese *** take full advantage of the reference signature,we modify the conventional contrastive loss function to enhance the *** using the proposed techniques,the accuracy of the system can be increased by 5.9%.Based on the GPDS signature dataset,the proposed system is able to achieve an accuracy of 94.61%which is better than the accuracy achieved by the current state-of-the-art work.

Posterior reversible encephalopathy syndrome (PRES), a rare disease is characterized by multiple neurological complications. It has been reported to be associated with the use of angiogenesis inhibitors such as sorafenib, sunitinib, pazopanib, regorafenib, and lenvatinib. We reported a case of 76-year-old woman with history of stage III hepatocellular carcinoma (HCC), who developed adverse drug reactions related to pazopanib induced PRES. Pazopanib, an angiogenesis inhibitor which inhibits the vascular endothelial growth factor (VEGF) pathway may lead to vascular endothelial damage, and these pathophysiological changes may lead to vascular leaks and brain edema. Medical staff must be aware of the possible association between angiogenesis inhibitors and the development of PRES. In patients with retroperitoneal soft-tissue sarcoma undergoing treatment with pazopanib, regular monitoring of their blood pressure and following-up brain magnetic resonance imaging (MRI) should be encouraged.

Minimally invasive surgery has revolutionized the treatment of gastrointestinal tumors. Submucosal tumors of the stomach can be resected using laparoscopic techniques. We report here a case of ectopic pancreas tissue in the gastric wall that was removed using robotic-assisted laparoscopic resection. The patient was a 15-year-old female who presented with abdominal discomfort and tarry stools. Laboratory analysis showed iron deficiency anemia. Preoperative endoscopy revealed a submucosal lesion in the posterior wall of the gastric high body. Intraoperative upper endoscopy clearly located the lesion. A robotic-assisted laparoscopic wedge resection of the putative gastric submucosal tumor was performed. The pathology results showed an ectopic pancreas. The patient had an uneventful recovery and we believe that this is a valid treatment option for this benign condition.

Tumor necrosis factor (TNF)-related apoptosis-inducing ligand (TRAIL) has been implicated in the regulation ofinflammation in rheumatoid arthritis (RA), primarily due to its ability to promote apoptosis in synoviocytes andinfiltrating lymphocytes. The aim of this study was to investigate the immunomodulatory mechanism and role ofTRAIL in inflammatory arthritis. We created an animal model of inflammatory arthritis and demonstrated that TRAILsignificantly inhibited joint inflammation and reduced the severity of arthritis. The suppression of joint inflammationwas not due to the TRAIL-mediated induction of apoptosis in T cells, macrophages or synovial fibroblasts. Incontrast, TRAIL directly inhibited T-cell proliferation and suppressed the production of cytokines, which indicatedthat TRAIL exerted its anti-inflammatory effects by direct inhibition of T-cell activation. Moreover, TRAIL receptor(TRAIL-R)-knockout mice developed more severe disease, and the protective effects of TRAIL were abolished in theexperimental arthritis model in TRAIL-R knockout mice. From these results, we conclude that TRAIL suppressesjoint inflammation via an apoptosis-independent pathway and directly inhibits T-cell activation. Our results providea novel apoptosis-independent, immune regulatory role for TRAIL in suppressing inflammatory arthritis and shedlight on the development of effective new therapies for autoimmune inflammatory diseases.
