Rare combination of traumatic subarachnoid-pleural fistula and intracranial subdural hygromas:A case report

BACKGROUND Subarachnoid-pleural fistula(SPF)is a complex and rare condition characterized by a pathological shunt between the subarachnoid and pleural *** can lead to the accumulation of cerebrospinal fluid(CSF)in the pleural space,pneumocephalus,and the development of central nervous system *** or thoracic spinal surgery are common causes of SPF,with symptoms including postural headache,consciousness status changes,and *** combination of SPF and subdural hygroma is a severe and rare condition,with little existing literature on its clinical *** SUMMARY We report a case of an 83-year-old male patient with traumatic SPF and bilateral frontal subdural hygroma following a fall from *** patient initially presented with severe lower back and buttock *** admission,the patient developed worsening lower limb weakness and pleural *** investigation revealed the presence of subdural hygromas with mass effect,requiring emergency bilateral subdural drainage.A multidisciplinary approach was undertaken to manage this complex condition,including intervention for hypovolemic CSF status and subdural hygroma *** pleural effusion eventually resolved and the patient attained a higher level of con-sciousness after bilateral hygroma drainage *** also reviewed the present literature relating to this rare combination of medical *** Traumatic SPF with subsequent subdural hygroma is a rare but serious *** the optimal treatment strategy for this complex condition remains uncertain,our literature review suggested that a multidisciplinary approach,including intervention for hypovolemic CSF and management of the subdural hygroma,is the most beneficial.

Precision-based drug delivery via remote triggering is fast becoming an attractive therapeutic design and is highly useful in complicated clinical situations that may require accurate site-delivery of drug while reducing the risk of collateral damage to surrounding healthy *** the many strategies available to achieve these desirable effects,silica/gold nano-assemblies offers a practical means to achieving these ***,as a proof-of-concept,a silica nanocapsule passivated with a gold outer nanoshell had been fabricated to deliver Doxorubicin,and this nano-assembly can be remotely triggered via two-photon excitation(TPE),even under in vivo setting.A polyethylene glycol(PEG)layer as well as AS1411 DNA aptamer had also been grafted to the surface to improve homing specificity toward MDA-MB-231 breast cancer *** assembly of silica/gold nanocapsules was characterized via TEM,FTIR,and UVVis to validate the the *** TPE irradiation,a higher expression level of Annexin V and Caspase-3 was observed in both in vitro and in vivo animal models.A significant reduction in tumor size on mice model was noticed after 21 days,and these results had suggested a viable nano-sized design serving as remotely triggered drug release platform based on current well-established silica nanoparticulate methodologies.

In addition to the effect that the COVID-19 pandemic has had on the physical and mental health of individuals,it has also led to a change in the mental and emotional state of many *** among businesses and private companies,which faced many restrictions due to the special conditions of the ***,the present study aimed to design an artificial neural network with MLP technique to analyze the relationship between demographic variables,resilience,COVID-19 and burnout in start-ups in *** research method was *** and employees of start-ups formed the statistical population of the study,based on the statistical sample size of the unlimited community,384 of them were *** data gathering,standard questionnaires include of MBI-GS and BRCS and researcher-made questionnaire of stress caused by COVID-19 were *** validity of the questionnaires was confirmed by a panel of experts and their reliability was confirmed by Cronbach’s alpha *** number of neurons in the input layer was equal to 10,the number of neurons in the 1st hidden layer was equal to 7,the number of neurons in the output layer was equal to 1,and the number of epochs was equal to 500.70%of the data were used for training and 30%for *** the designed artificial neural network,all experiment data except one were correctly predicted and the obtained MAE error was less than 0.012%.Finally,he precision and correction of the presented model was confirmed by the obtained results.

CYP2C19 polymorphism has no influence on rabeprazole-based hybrid therapy for Helicobacter pylori eradication

AIM To evaluate the impact of cytochrome P450 2C19(CYP2C19) and interleukin-1β(IL-1β) polymorphisms on the efficacy of Helicobacter pylori(H. pylori) eradication by using rabeprazole-based hybrid *** A total of 88 H. pylori-infected patients were recruited to receive 14-d of hybrid therapy from March 2013 to May 2014. Three patients were excluded from analysis because of incomplete compliance. Either a follow-up endoscopy or 13 C-urea test was performed to determine the results of H. pylori eradication therapy. The genotypes of CYP2C19 and IL-1β were analyzed to investigate the impact on treatment effect. RESULTS The total eradication rate of H. pylori was 92.94%(79/85). According to the CYP2C19 genotypes, the rates of H. pylori eradication were 89.19% in extensive metabolizers(EM) and 95.83% in non-EM. The *** eradication rates regarding the IL-1β genotypes were 92.59% in the normal acid secretion group and 93.10% in the low acid secretion group. After multivariable logistic regression analysis, both the genotypes of CYP2C19 and IL-1β had no significant influences on the eradication rates of H. *** The CYP2C19 and IL-1β polymorphisms are not significantly independent factors of *** eradication using rabeprazole-based hybrid therapy.

Biliopleural fistula: A rare complication of percutaneous transhepatic gallbladder drainage

A 79-year-old previously healthy man presented with acute acalculous cholecystitis with obstruction of the biliary tract. He was successfully treated with antibiotics and percutaneous transhepatic gallbladder drainage, but returned to the hospital two days after discharge with a rare complication of this technique, biliopleural fistula. A thoracostomy tube was inserted to drain the pleural effusion, and the patient’s previous antibiotics reinstated. After two weeks of drainage and antibiotics, the fistula healed spontaneously without the need for further intervention.

Usefulness of serum des-γ-carboxy prothrombin in detection of hepatocellular carcinoma

AIM: To evaluate whether DCP is better than AFP for differentiating HCC from nonmalignant liver disease and further evaluate the usefulness of DCP in early diagnosis of small ***: Serum DCP and AFP levels were determined in 127 patients. Among these patients, 32 were with noncirrhotic chronic hepatitis, 34 were with compensated cirrhosis, and 61 were with HCC. The cut-off value for the DCP and AFP were set as 40 mAU/mL and 20 ng/mL,respectively. To compare the diagnostic value of DCP and AFP in distinguishing HCC from nonmalignant chronic liver disease, receiver operating characteristic (ROC) curves were constructed for each ***: The accuracy, sensitivity, and specificity of DCP were higher than AFP in detecting HCC (81.9%, 77%,and 86.4% vs 68.5%, 59%, and 77.3%, respectively).The area under the ROC (AUROC) curves revealed that DCP had a better accuracy than AFP in diagnosis of HCC (0.85 [95%CT, 0.78-0.91] vs 0.73 [95%CI, 0.65-0.81],P = 0.013). In 39 patients with solitary HCC, the positive rates of DCP were 100% in patients with tumor size larger than 3 cm, 66.7% in patients with tumor size 2-3 cm and50% in patients with tumor size less than 2 cm. The positive rates of AFP in patients with tumor size larger than 3 cm, 2-3 cm and less than 2 cm were 55.6%, 50%,and 33.3%, respectively. The median level of DCP in HCC patients with tumor size larger than 3 cm was significantly higher than those with tumor size 2-3 cm and those with the size of less than 2 ***: Our study indicates that DCP has a better diagnostic value than AFP in differentiating HCC from nonmalignant chronic liver disease. DCP has not only a stronger correlation with HCC than AFP in tumor size but also more effectiveness than AFP in detecting small size of HCC.

Hybrid vs sequential therapy for eradication of Helicobacter pylori in Taiwan: a prospective randomized trial

AIM: To evaluate the efficacy of sequential vs hybrid therapy in patients with Helicobacter pylori(H. pylori) ***: From March 2013 to May 2014,one hundred and seventy-five H. pylori infected patients who had not been treated for H. pylori before wererandomized to receive either sequential therapy(rabeprazole 20 mg and amoxicillin 1 g twice daily for 5 d,followed by rabeprazole 20 mg,clarithromycin 500 mg and metronidazole 500 mg twice daily for 5 d) or hybrid therapy(rabeprazole 20 mg and amoxicillin 1 g for 7 d,followed by rabeprazole 20 mg,amoxicillin 1 g,clarithromycin 500 mg and metronidazole 500 mg twice daily for 7 d). H. pylori status was confirmed by positive results of both rapid urease test and histology examination or a positive result of culture. Eradication efficacy was assessed by follow-up endoscopy with rapid urease test and histological examination 8 wk after the end of anti-H. pylori therapy,or 13C-urea breath test at least 4 wk after completion of treatment. The primary outcome was H. pylori eradication by intension-to-treat(ITT) and per-protocol(PP) ***: One hundred and sixty-seven patients(83 patients in the sequential group and 84 patients in the hybrid group) completed the study. The compliance rates were 97.6% and 97.7% for the two groups,respectively. The eradication rate was 78.2% for the sequential group and 92% for the hybrid group by ITT analysis(P = 0.01). The eradication rate was 81.9% for the sequential group and 96.4% for the hybrid group by PP analysis(P = 0.01). Univariate analysis for the clinical and bacterial factors did not identify any risk factors associated with treatment failure. Severe adverse events were observed in 2.3% of patients in the sequential group and 2.4% of those in the hybrid ***: Due to a grade A(> 95%) success rate for H. pylori eradication by PP analysis,similar compliance and adverse events,hybrid therapy seems to be an appropriate eradication regimen in Taiwan.

政治竞争与年金改革:台湾地区经验的检视

本论文检视台湾地区过去30年来年金改革的政治过程,比较国民党与民进党这两个主要政党在年金改革过程中的不同主张,并分析两党的政治竞争对于年金改革的影响。台湾地区自20世纪90年代初期至2017年间经历了数次重大的年金改革。在1993—2008年间,改革的重点在完善养老体系的老年经济保障功能,属于'权益扩张'的改革,两党的政治竞争是促成台湾地区年金制度扩张的主要因素。2012—2017年间的改革,则是试图以提高提拨率、降低给付以及延后请领年龄来提升长期的财务永续,属于'权益紧缩'的改革;两党在此阶段采取不同的改革策略,并获致不同的改革结果。尽管改革的方向与目标不同,但各阶段的改革过程中,两党的政治竞争是影响改革过程与结果的重要因素。在权益扩张的阶段,两党分别以不同的年金扩张策略作为选举中争取选票的要求;在权益紧缩的阶段,两党都因迫切的年金财务压力而不得不承担年金改革的责任。就此而言,台湾地区的年金政治与皮尔森(Pierson,1996)所提出的'福利国家新政治'(new politics of the welfare state)的论述并不全然相符。尽管国民党与民进党的福利意识形态相左,且对于年金改革的主张殊异,但两党主政下所推动的年金改革,大体上均是在既有的制度逻辑下做制度调整,符合西方文献所强调的'路径依赖'(path-dependency)的特征。
