Introduction: Panorama studies of autoimmune and auto-inflammatory diseases are still very little carried out in Africa and particularly in Mali. The objective of this descriptive study with retrospective collection was to describe the epidemiological and clinical profile of all autoimmune and auto-inflammatory diseases in the department of internal medicine at the University Hospital Center of the Point G. Methods: This was a descriptive study with a retrospective survey of the records of patients hospitalized for autoimmune and auto-inflammatory diseases in the department of internal medicine at the CHU of Point G for a study period of 15 years from January 1, 2005 to December 31, 2019. We included in the study all patients hospitalized for autoimmune and auto-inflammatory diseases. Results: During the study period (January 31, 2005 to December 31, 2019), 6383 patients were hospitalized in internal medicine at the University Hospital Center of the Point G, of which 317 patients presented with autoimmune and/or auto-inflammatory disease with an average annual hospital recruitment rate of 21 ± 7.87 cases per year. The female sex accounted for 64.98% with a sex ratio of 0.54. The mean age of patients was 35.27 ± 16.27 years and the extreme ages were 07 and 79 years. Out of the 317 medical records included according to our inclusion criteria, there were 07 cases of association between autoimmune disease and autoinflammatory disease, i.e. 14 cases of autoimmune and autoinflammatory diseases. A total of 331 autoimmune diseases and/or auto-inflammatory diseases were collected, i.e. a frequency of 5.19%, including 291 cases of autoimmune diseases (221 cases of organ-specific autoimmune diseases and 70 cases of systemic autoimmune diseases) and 40 cases of autoinflammatory diseases (no case of monogenic forms, 08 cases of “systemic” polygenic forms and 32 cases of “organ-specific” polygenic forms). Introduction: Diabetes, a chronic pathology, is the source of many acute and chronic complications whose treatment requires many daily constraints that can have a serious impact on the quality of life of the patient and his family. The diabetic foot is a real public health problem which is still dominated by a very high lower limb amputation rate even in countries with high socioeconomic status. The diabetic foot classically results from the association of three entangled mechanisms which are neuropathy, arteriopathy and infection. Objectives: To determine the frequency of arterial disease, neuropathy, and other aggravating factors that influence the podiatry risk, and to proceed with the gradation of the podiatry risk. Methodology: This was a cross-sectional study concerning the period from July 1 to October 31, 2017, covering all diabetic patients received in consultation and hospitalization in the Internal Medicine Department of the G-University Hospital Center. Results: Our study included 50 patients (35 women for 15 men) out of 95 diabetic patients receiving a frequency of 52.6%, with a sex ratio of 0.42. The average age of our patients was 54.38 ± 13.98 years. Overweight/obesity found in 44% of patients. The average Body Mass Index (BMI) of our patients was 27.16 kg/m2. Patients with diabetes less than 5 years old represented 56% with an average duration of 5.39 ± 5.34 years. The mode of discovery of diabetes was polyuria-polydipsia syndrome in 56% of cases. Type 2 diabetes represented 74% of patients. Among the patients who had performed the HbA1C test, 80.77% were not well balanced. The antecedent of macrosomia in women accounted for 77.1%. Patients who did not know foot hygiene recommendations accounted for 82%. Among the identified podiatry risk factors, diabetic neuropathy accounted for 73.8%, and arterial disease was 32.3%. Management of Traumatic Brain Injuries at the Kara Regional Hospital

Introduction: Traumatic Brain Injury (TBI) is a major public health problem causing significant morbidity and mortality in young adults. This study aimed to describe the epidemiological, diagnostic, therapeutic, and evolutionary aspects of TBI. Materials and Methods: This was a prospective, descriptive study conducted from 1 April 2022 to 31 March 2023 on patients admitted to and treated for cranioencephalic trauma in the General Surgery department of Kara Regional Hospital. Results: Eighty-three (83) patients with cranioencephalic trauma were managed out of 773 patients admitted to the department during the study period. The mean age was 34 ± 14.98 years and the sex ratio was 3.6 in favour of men. Motorbike taxi drivers were the social group most affected (n = 33, 40%). The causes of trauma were dominated by public road accidents (n = 80;96%). TBI was mild (n = 40;48%), moderate (n = 35;42%) and severe (n = 8;10%). Cerebral CT scans were performed in 19 patients (23%). Cerebral contusion (n = 4) was the most frequent cerebral lesion. Six patients (7%) with severe head injuries were transferred to Kara University Hospital. Six deaths (7%) occurred in patients with severe head injuries. The main sequelae were intermittent headaches in all patients reviewed, and memory problems (6%). Conclusion: Traumatic brain injuries are common at Kara Regional Hospital. Severe cranial trauma is less frequent but leads to death because of financial difficulties and limited technical facilities.

Chronic Subdural Hematoma Associated with an Arachnoid Cyst in Elderly, an Intraoperative Finding after Re-Bleeding: Case Report

Intracranial arachnoid cysts (AC) are believed to be congenital and chronic subdural hematomas tend to occur in elderly patients with a history of mild head injury. The association between these two entities sporadically occur in relatively young patients but rare in elderly patients. We report a 65-year-old man who presented with headache and dizziness of 2 months’ duration with a history of head injury. Brain computed tomography (CT) a CSDH in right side and a hygroma in left side. After first operation with burr holes in both sides, the patient underwent an early recurrence of acute subdural hematoma in the right side. The evacuation of this hematoma by a craniotomy allowed seeing an AC that we resected partially with complete recovery of the patient. The association CSDH/AC is rare and possible in elderly patients and there is no consensus on treatment.

Lipid Production,Oxidative Status,Antioxidant Enzymes and Photosynthetic Efficiency of Coccomyxa chodatii SAG 216-2 in Response to Calcium Oxide Nanoparticles

The extensive use of nanoparticles(NPs)in diverse applications causes their localization to aquatic habitats,affecting the metabolic products of primary producers in aquatic ecosystems,such as algae.Synthesized calcium oxide nanoparticles(CaO NPs)are of the scarcely studied NPs.Thus,the current work proposed that the exposure to CaO NPs may instigate metabolic pathway to be higher than that of normally growing algae,and positively stimulate algal biomass.In this respect,this research was undertaken to study the exposure effect of CaO NPs(0,20,40,60,80,and 100μg mL^(−1))on the growth,photosynthesis,respiration,oxidative stress,antioxidants,and lipid production of the microalga Coccomyxa chodatii SAG 216-2.The results showed that the algal growth concomitant with chlorophyll content,photosynthesis,and calcium content increased in response to CaO NPs.The contents of biomolecules such as proteins,amino acids,and carbohydrates were also promoted by CaO NPs with variant degrees.Furthermore,lipid production was enhanced by the applied nanoparticles.CaO NPs induced the accumulation of hydrogen peroxide,while lipid peroxidation was reduced,revealing no oxidative behavior of the applied nanoparticles on alga.Also,CaO NPs have a triggering effect on the antioxidant enzymes such as superoxide dismutase,catalase,ascorbate peroxidase,and guaiacol peroxidase.The results recommended the importance of the level of 60μg mL^(−1) CaO NPs on lipid production(with increasing percentage of 65%compared to control)and the highest dry matter acquisition of C.chodatii.This study recommended the feasibility of an integrated treatment strategy of CaO NPs in augmenting biomass,metabolic up-regulations,and lipid accumulation in C.chodatii.

First record of Lepidoderma carestianum and Arthrinium bambusicola from Egypt

This study introduces two new protozoan and fungal species for the first time to the Egyptian environment.The first one is myxomycetous,Lepidoderma carestianum and the second is the ascomycetous;Arthrinium bambusicola.The sporocarp of L.carestianum was discovered attached to the soil surface while,A.bambusicola was found growing saprophytically on the dead branch of a tree.Both specimens were transferred to the laboratory to be cultivated as pure cultures and subjected to identification process;morphologically and genetically.

Geochemistry and spectrometric prospection of younger granites and granitic pegmatites bearing uranium mineralization at G.Kab El Rakeb area, Central Eastern Desert, Egypt

The present study deals with the detailed field geology,mineralogy,geochemistry,and spectrometric prospection of rare metals and radioactive minerals associated with granitic pegmatites in the Gabal Um LassafAbu Dob-Um Atelia-Kadabora-Kab El Rakeb area(LDAKK),which are situated in the central Eastern Desert of Egypt.The study area is composed of granodiorites and monzogranites.The latter is dissected by granitic pegmatites with a discriminatory distribution of economically important mineralization(rare-metals and U-Th minerals).The granitic pegmatites display a zoning structure composed of feldspar-rich zones enveloping pockets of mica(mostly muscovite) and all zoning internal quartz core.Microscopically,the monzogranite and granitic pegmatites are composed of potash-feldspar,albite,and quartz with a small amount of biotite and muscovite.The mineralogical investigation of the highly radioactive zones in the granitic pegmatite shows enrichment in kasolite,uranothorite,thorite,xenotime,columbite,zircon,bismuth,and galena.Geochemically,the granitic pegmatites are enriched with high field strength elements(HFSE;Zr,Nb,Th,and U) and ion lithophilic elements(LILE;Rb,and Pb) and depleted in Ba,Sr,and Eu.They are strongly peralkaline to metalumainous except for two samples,which are peraluminous.They are formed by partial melting of metagreywackes at depth.The studied granitic pegmatites could belong to the NYF family(Niobium-Yttrium-Fluorine family) according to the geochemistry of the trace elements.The studied granitic pegmatites show deep negative Eu anomaly and display a strong M-type tetrad effect of rare earth elements.The field spectroscopy measurements showed the localization of the radiometric anomalous associated with the granitic pegmatites,which are intruded in the granitic host rocks.Their eU and eTh contents reached up to 316 and 1367 ppm,respectively.Uranium and thorium occurrences in the investigated granitic pegmatites are related to magmatic(syngenetic)origin with hydrothermal(epigenetic) input.The magm

Application of a Multi-Criteria Approach for the Assessment of Groundwater Potential in the Basement Zone: A Case of Tamou (South West Niger)

The commune of Tamou, located in the Department of Say in Niger, occupies the southwestern part of the Liptako crystallophyllian basement domain. In this area, the problem of the drinking water supply of the populations is acute, because of the low flow rates of the drillings capturing the crystallophyllian formations and the Voltaian sandstones, the failure rates of the drillings are very high there. Therefore, the main objective of this study is to improve the knowledge of the areas potentially favorable for the implantation of drillings likely to give more satisfactory flow rates. The methodological approach, based on the collection of data (Landsat 7 ETM+ satellite imagery, borehole data, geological and topographical maps) and their processing by a combination of remote sensing and GIS tools and a field check, allowed the elaboration of maps of availability, accessibility and exploitability of the groundwater resources in the study area. The maps developed were analyzed with a Spatial Reference Hydrogeological Information System following the technique of aggregation by weighting to generate the map of productive drilling sites. The results show that the area is moderately rich in groundwater (58%) and that only 31% of the potential is exploitable. The groundwater potential map shows that 46% of the study area is suitable for drilling.

Progressive Characteristics of HIV Infection in the Elderly in the Cohort of the Internal Medicine Department of the Points G University Hospital, Bamako. Mali

Few data are available on HIV infection in the elderly and they are an important population in our different cohorts in view of the improvement in the quality of care over the last ten years. Objective: to determine the clinical and immunovirological characteristics as well as the acceptance of antiretroviral drugs in this patient population. This is a cohort study of the files of PLHIV (People living with HIV) aged 50 years or more, followed in the Internal Medicine Department of the Point G University Hospital between January 2007 and December 2012. Results: Out of 161 HIV-infected patients in the cohort, 38 were aged 50 years or older, of whom 11 met the criteria and were included. The median age was 55 years, 90.9% of whom were in the [50 - 59 years] age group, with 63.6% being women (sex ratio = 0.57). At admission, 3 patients (27.3%) had prurigo and 2 (18.2%) had oral-pharyngeal candidiasis. At inclusion, 45.5% of patients were classified as WHO stage 2 and one as WHO stage 4. All were on ARVs, including 10 on 1st line HIV-1 and 1 on HIV-2. On triple therapy, the average weight gain was 5.1 kg at D15 and 6.84 kg at M6. However, at M12 there was a weight loss of 1.04 kg. The mean CD4 T cell gain was +102/mm3 at M6 and +188/mm3 at 12 months. At D0, mean viral load = 565024.75 copies/mm3 [99 - 1100000] in 4/11. At M12, two patients had undetectable viral loads. Conclusion: The prevalence of HIV in elderly subjects is certainly underestimated. Thanks to triple antiretroviral therapy, PLHIV are aging with HIV but screening is not systematically proposed during consultations in elderly subjects. It is necessary to reinforce communication about HIV at all ages.
