Colorectal cancer in patients under 50 years of age:A retrospective analysis of two institutions' experience

AIM:To investigate the epidemiological characteristics of colorectal cancer(CRC)in patients under 50 years of age across two ***:Records of patients under age 50 years of age who had CRC surgery over a 16 year period were assessed at two *** following documents where reviewed:admission notes,operative notes,and discharge *** main study variables included:age,presenting symptoms,family history,tumor location,operation,stage/differentiation of disease,and post operative *** of disease was classified according to the American Joint Committee on Cancer TNM staging system:tumor depth;node status;and ***:CRC was found in 180 patients under age50 years(87 females,93 males;mean age 41.4±6.2years).Young patients accounted for 11.2%of cases during a 6 year period for which the full data set *** percent had a 1stdegree and 12%a 2nd degree family CRC *** all patients(94%)were symptomatic at diagnosis;common symptoms included:bleeding(59%),obstruction(9%),and abdominal/rectal pain(35%).Evaluation was often delayed and bleeding frequently attributed to *** stage CRC(Stage 3 or 4)was noted in 53%of *** tumors were distal to the splenic flexure(77%)and 39%involved the *** patients(95%)had segmental resections;6 patients had subtotal/total *** differentiated tumors were noted in 12%and mucinous lesions in 19%of patients of which most had Stage 3 or 4 ***-two patients(13%)developed recurrence and/or progression of disease to *** patients(ages 42,42and 49 years)went on to develop metachronous primary colon cancers within 3 to 4 years of their initial ***:CRC was common in young patients with no family *** patients with symptoms merit a timely evaluation to avoid presentation with late stage CRC.

Calling underwater is a costly signal： size-related differences in the call rates of Antarctic leopard seals

It is proposed that where sexually selected vocal communication is an honest signal, the call production rate is predicted to change throughout the breeding season. Male leopard seals call underwater for many hours each day over their three- to four-month breeding season, and it is hypothesized that a decrease in calling rate would be associated with the declining body condition of smaller males. The calling rates of leopard seals were measured （N= 49 recordings） and com- pared between seals of different size classes throughout the breeding season. Male leopard seals produce their calls at more stable rates as they become larger. In this study, larger male leopard seals adopted a strategy of consistent underwater calling throughout the breeding season, whereas there was a breakdown in the calling stereotypy of the smaller males at its height. Toward the end of the breeding season, the smaller seals produced fewer calls in shortened calling bouts, and they took more rest periods. Therefore, underwater calling may represent an honest signal in the leopard seal. For marine mammals that call underwater, the production of repetitive sequences advertises the breath-holding ability of the caller to the listeners, and this ability may be related to male stamina and endurance, thus representing an honest signal that could be widespread in other species.

Use of the stepwise progression return-to-play protocol following concussion among practicing athletic trainers

Purpose: The purpose of this study was to determine whether practicing athletic trainers(ATs) were using the stepwise progression to make return-to-play(RTP) decisions after concussion and to determine what factors influenced their decision to use the stepwise ***: A total of 166 ATs(response rate = 16.6%) completed a 21-item questionnaire that evaluated participant demographics, methods of concussion management, and RTP decision-making using the stepwise progression. Descriptive statistics and a logistic regression were completed to analyze ***: Factors such as education level(p = 0.05) and number of concussions treated(p = 0.05) predicted use of the stepwise progression,whereas sex(p = 0.17), employment setting(p = 0.17), state law(p = 0.86), and years practicing(p = 0.17) did not predict whether ATs were following the stepwise ***: The majority of the ATs from this study are employing the stepwise progression to safely return athletes to play after sustaining a concussion. This demonstrates that ATs are providing a standard of care for concussed athletes across various athletic training settings; however,having a graduate degree and treating more concussions per year are predictors of whether an AT follows all steps of the stepwise progression.

Efficacy of epothilones in central nervous system trauma treatment:what has age got to do with it?

Central nervous system injury,specifically traumatic brain and spinal cord injury,can have significant long lasting *** are no comprehensive treatments to combat the injury and sequalae of events that occurring following a central nervous system *** we discuss the potential for the epothilone family of microtubule stabilizing agents to improve outcomes following experimentally induced *** drugs,which are able to cross the blood-brain barrier,may hold great promise for the treatment of central nervous system trauma and the current literature presents the extensive range of beneficial effects these drugs may have following trauma in animal ***,the effect of the epothilones can vary and our most recent contributions to this field indicate that the efficacy of epothilones following traumatic brain injury is dependent upon the age of the ***,we present a case for a greater emphasis to be placed upon age when using an intervention aimed at neural regeneration and highlight the importance of tailoring the therapeutic regime in the clinic to the age of the patient to promote improved patient outcomes.

Competition and body size in plants:the between-species tradeoff for maximum potential versus minimum reproductive threshold size

Aims According to traditional theory,superior competitive ability in plants generally requires relatively large plant body *** even within the most crowded vegetation,most resident species are relatively small;species size distributions are right-skewed at virtually every *** examine a potential explanation for this paradox:small species coexist with and outnumber large species because they have greater‘reproductive economy’,*** are better equipped—and hence more likely—to produce offspring despite severe size suppression from intense *** Randomly placed plots within old-field vegetation were surveyed across the growing *** each plot,the largest(MAX)and smallest(MIN)reproductive individuals of each resident species were collected for above-ground dry mass *** tested three hypotheses:(i)smaller resident species(with smaller MAX size)have generally smaller reproductive threshold sizes;(ii)smaller resident species have greater‘reproductive economy’,i.e.a smaller MIN relative to MAX reproductive plant size;and(iii)MIN size predicts plot occupancy(species abundance within the community)better than MAX *** Findings The results supported the first and third,but not the second ***,we could not reject the hypothesis that smaller species have greater reproductive economy,as it was not possible to record data for the largest potential plant size for each species—since even the largest(MAX)plants collected from our sampled plots were subjected to competition from neighbours under these natural field ***,contrary to conventional competition theory,more successful species(in terms of greater plot occupancy)had smaller minimum not larger(or smaller)maximum reproductive *** results suggest that a small reproductive threshold size,commonly associated with relatively small potential body size,is generally more effective in transmitting genes into future generations when selection

Informal Caregiving and Disaster Risk Reduction:A Scoping Review

Informal caregivers are a population currently in the shadows of disaster risk reduction(DRR),and yet essential to the provision of healthcare *** scoping review explored the literature to understand issues related to informal caregiving and promising practices to support resilience for *** guidelines for scoping review as outlined by Tricco et al.(2016),relevant publications were identified from five major databases—Medline,Embase,PubMed,Web of Science,and *** studies referenced informal caregiving and disasters for a variety of population groups including children,people with disabilities or chronic illnesses,and older *** were excluded if they discussed formal caregiving services(for example,nursing),lacked relevance to disasters,or had insufficient discussion of informal ***,21 articles met the inclusion criteria and were fully *** themes were identified:(1)the need for education and training in DRR;(2)stressors around medication and supply issues;(3)factors affecting the decision-making process in a disaster;(4)barriers leading to disaster-related problems;and(5)factors promoting *** areas of strategic action and knowledge gaps are *** informal caregivers do not feel adequately prepared for *** the important role of informal caregivers in healthcare provision,preparedness strategies are essential to support community resilience for those requiring personal care *** understanding and mobilizing assets to support the resilience of informal caregivers,we also support the resilience of the greater healthcare system and the community,in disaster contexts.

Targeting inflammation to reduce brain injury in growth restricted newborns:a potential treatment?

Introduction： Intrauterine growth restriction （IUGR） is commonly caused by placental insufficiency, resulting in a chronic hypoxic environment and subsequent abnormal fetal development. The developing brain is particularly vulnerable to IUGR conditions. Multiple causal factors associated with brain injury in fetal growth restriction include the timing of placental insufficiency, onset and subsequent severity of fetal compromise, fetal cerebrovascular response and the redistribution of brain blood flow. Although a significant proportion of IUGR infants exhibit adverse long-term neu- rological outcomes, relatively few studies have focused on the mechanisms of brain injury in the IUGR neonate. Clini- cal imaging studies of IUGR infants demonstrate alterations in grey matter and white matter volume and structure （Tolsa et al., 2004; Esteban et al., 2010; Padilla et al., 2015）. Cortical grey matter volume is reduced by up to 28% compared with control infants （Tolsa et al., 2004） and both white and grey matter show structural changes （Esteban et al., 2010）.
