Gastric-and intestinal-type marker expression in invasive ductal adenocarcinoma of the pancreas

BACKGROUND:Although invasive ductal adenocarcinoma of the pancreas(PDAC) manifests as a relatively uniform histomorphological feature of the pancreatobiliary type,it may be complicated by metaplastic changes and heterogeneous gastric and intestinal elements.This study aimed to investigate the complication rate and clinicopathological significance of such heterogeneous elements.METHODS:Fifty-nine patients who underwent resection of PDAC were examined in this study.Immunohistochemically,tumors showing high expression(>25%) of the intestinal-type(INT) marker CDX2 were classified as PDAC with INT.Those with high expression(>25%) of the gastric-type(GAS) marker MUC5AC were classified as PDAC with GAS,while those with high expression of both markers were classified as PDAC with INT/GAS.These patients were compared with those with PDAC of the negative group in which neither markers was highly expressed to examine their clinicopathological significance.RESULTS:In the 59 patients,31(52.5%) showed high CDX2 or MUC5AC expression.Twenty-eight patients(47.5%) belonged to a negative group,11(18.6%) to a PDAC with INT group,15(25.4%) to a PDAC with GAS group,and 5(8.5%) to a PDAC with INT/GAS group.No significant differences were observed for age,gender,size,localization,T classification,or prognosis among the four groups.Although the PDAC with GAS group had well differentiated types significantly more than the other groups,the rate of lymph node metastasis in this group was significantly higher(PDAC with GAS:73%;other groups:36%).CONCLUSION:Complications with heterogeneous elements are not uncommon in PDAC,and this should be considered during the diagnosis and treatment of PDAC along with histogenesis of the disease.

Unsaturated polyester（UPR）/thermoplastic polyurethane（TPU）/organoclay nanocomposites were prepared by melt compounding of thermoplastic polyurethane and unsaturated polyester prepolymer,and then mixing with the hybrids of styrene monomers and organoclay at ambient temperature.The crosslinking reaction eventually occurred through the unsaturated polyester prepolymer and styrene monomer.The morphology of the composites was investigated by scanning electron microscopy（SEM） and transmission electron microscopy（TEM）.The results show that the impact strength of UPR/TPU/organoclay nanocomposites increases obviously;the cure shrinkage decreases markedly,the glass transition temperature is enhanced and an elastic response to the deformation is prominent at the temperature above 10℃.

Intussusception due to hematogenous metastasis of hepatocellular carcinoma to the small intestine:A case report

BACKGROUND The commonest sites of extrahepatic metastases from hepatocellular carcinoma(HCC)are the lungs,bones,adrenal glands,and regional lymph nodes.Hematogenous metastasis to the gastrointestinal(GI)tract is a rare condition in patients with HCC,and the prognosis is usually poor.We report,herein,an extremely rare case of a patient with intussusception due to hematogenous metastasis of HCC to the ileum and his long-term survival with multidisciplinary therapy.CASE SUMMARY The patient was a 71-year-old man with a history of chronic hepatitis B,who had undergone three surgeries for HCC.He was treated with sorafenib for peritoneal metastases of HCC.He was admitted to our hospital with chief complaints of abdominal pain and vomiting.Abdominal contrast-enhanced computed tomography imaging revealed a small intestinal tumor,presenting with intussusception and small bowel obstruction.Conservative treatment was started,but due to repeated exacerbation of symptoms,surgery was planned on the 28th d of hospitalization.Partial ileal resection without reducing the intussusception and end-to-end anastomosis was performed.On histological examination,tumor cells were not observed on the serosal surface,but intravascular invasion of tumor cells was seen.Immunohistochemistry was positive for immunohistochemical markers,and a diagnosis of hematogenous metastasis of HCC to the ileum was made.He remains alive 82 mo after the first surgery.CONCLUSION Prognosis of HCC patients with GI tract metastasis is usually poor,but in some cases,multidisciplinary therapy may prolong survival.

Short-and long-term results of endoscopic ultrasoundguided transmural drainage for pancreatic pseudocysts and walled-off necrosis

AIM To evaluate the short-and long-term results of endoscopic ultrasound-guided transmural drainage(EUS-GTD) for pancreatic fluid collection(PFC) and identify the predictive factors of treatment outcome for walled-off necrosis(WON) managed by EUS-GTD alone.METHODS We investigated 103 consecutive patients with PFC who underwent EUS-GTD between September 1999 and August 2015. Patients were divided into four groups as follows: WON(n = 40), pancreatic pseudocyst(PPC; n = 11), chronic pseudocyst(n = 33), and others(n = 19). We evaluated the short-and long-term outcomes of the treatment. In cases of WON, multiple logistic regression analyses were performed to identify the predictor variables associated with the treatment success. In addition, PFC recurrence was examined in patients followed up for more than 6 mo and internal stent removal after successful EUS-GTD was confirmed.RESULTS In this study, the total technical success rate was 96.1%. The treatment success rate of WON, PPC, chronic pseudocyst, and others was 57.5%, 90.9%, 91.0%, and 89.5%, respectively. Contrast-enhanced computed tomography using the multivariate logistic regression analysis revealed that the treatment success rate of WON was significantly lower in patients with more than 50% pancreatic parenchymal necrosis(OR = 17.0; 95%CI: 1.9-150.7; P = 0.011) and in patients with more than 150 mm of PFC(OR = 27.9; 95%CI: 3.4-227.7; P = 0.002).The recurrence of PFC in the long term was 13.3%(median observation time, 38.8 mo). Mean amylase level in the cavity was significantly higher in the recurrence group than in the no recurrence group(P = 0.02).CONCLUSION The reduction of WON by EUS-GTD alone was associated with the proportion of necrotic tissue and extent of the cavity. The amylase level in the cavity may be a predictive factor for recurrence of PFC.

Natural rubber/nitrile butadiene rubber/hindered phenol composites with high-damping properties

New natural rubber(NR)/nitrile butadiene rubber(NBR)/hindered phenol(AO-80)composites with high-damping properties were prepared in this study.The morpholo-gical,structural,and mechanical properties were characterized by atomic force micro-scopy(AFM),polarized Fourier transform infrared spectrometer(FTIR),dynamic mechanical thermal analyzer(DMTA),and a tensile tester.Each composite consisted of two phases:the NR phase and the NBR/AO-80 phase.There was partial compat-ibility between the NR phase and the NBR/AO-80 phase,and the NR/NBR/AO-80(50/50/20)composite exhibited a co-continuous morphology.Strain-induced crystal-lization occurred in the NR phase at strains higher than 200%,and strain-induced orientation appeared in the NBR/AO-80 phase with the increase of strain from 100%to 500%.The composites had a special stress–strain behavior and mechanical properties because of the simultaneous strain-induced orientation and strain-induced crystalliza-tion.In the working temperature range of a seismic isolation bearing,the composites(especially the NR/NBR/AO-80(50/50/20)composite)presented a high loss factor,high area of loss peak(TA),and high hysteresis energy.Therefore,the NR/NBR/AO-80 rubber composites are expected to have important application as a high-perfor-mance damping material for rubber bearing.

River management system development in Asia based on Data Integration and Analysis System(DIAS) under GEOSS

This paper introduces the process of development and practical use implementation of an advanced river management system for supporting integrated water resources management practices in Asian river basins under the framework of GEOSS Asia water cycle initiative(AWCI).The system is based on integration of data from earth observation satellites and in-situ networks with other types of data,including numerical weather prediction model outputs,climate model outputs,geographical information,and socio-economic data.The system builds on the water and energy budget distributed hydrological model(WEB-DHM)that was adapted for specific conditions of studied basins,in particular snow and glacier phenomena and equipped with other functions such as dam operation optimization scheme and a set of tools for climate change impact assessment to be able to generate relevant information for policy and decision makers.In situ data were archived for 18 selected basins at the Data Integration and Analysis System(DIAS)of Japan and demonstration projects were carried out showing potential of the new system.It included climate change impact assessment on hydrological regimes,which is presently a critical step for sound management decisions.Results of such three case studies in Pakistan,Philippines,and Vietnam are provided here.

We conducted a near-infrared imaging survey of 11 young dwarfs in the Pleiades cluster using the Subaru Telescope and the near-infrared coronagraph imager. We found ten faint point sources,with magnitudes as faint as 20 mag in the K-band,with around seven dwarfs.Comparison with the Spitzer archive images revealed that a pair of the faint sources around V 1171 Tau is very red in infrared wavelengths,which indicates very low-mass young stellar objects.However,the results of our follow-up proper motion measurements implied that the central star and the faint sources do not share common proper motions,suggesting that they are not physically associated.

法藏PT1249号残片,是用藏文音写的汉文文献,其内容难以解读,以往并无人涉及。笔者通过对该残片的研究认为:其内容是式叉摩那受大戒时所使用的问答的一部分,除了起首的导言外,作白的语句均是抄录自《四分律·比丘尼揵度》。目前为止,从未发现有使用藏文音写的汉文律藏文献。而与该文献完全一致的汉字文本,似也未能留存下来。不过,受戒时此类对白是必须的,因此必定为僧尼所记诵。从其对音的特点来看,该写本时代约在吐蕃统治时期。本文提出藏汉文的对照文本,不见于《四分律》的部分语句由笔者按照对音补缺。据此文献可知,在吐蕃时期的敦煌,不识汉字的式叉摩那也能够受大戒,成为正式的尼师。而这一残片除了丰富我们对敦煌佛教的实际情况的认识之外,也加深了我们对敦煌语言文字的理解。

Effect of thermal drilling condition on burring of 0.2C-(1.0-2.5)Si-(1.0-2.0)Mn (mass%) ultrahigh strength TRIP-aided sheet steels with polygonal ferrite matrix (TDP steels) was investigated for automotive applications.The combined rotational and downward force of the thermal drilling tool bit created friction heat.The height of the bushing was roughly 3 to 4 times the initial sheet thickness.The bushings are ideal for thread applications,as the strength of threads was significantly increased.We found that the burring and tapping contributed to the improvement of the tensile strength of 980 MPa class TRIP steel.

Influence of proton pump inhibitor treatment on Helicobacter pylori stool antigen test

AIM: To investigate the effects of proton pump inhibitor (PPI) treatment on stool antigen test using the TestMate pylori enzyme immunoassay. METHODS: This study assessed 28 patients [16 men and 12 women; mean age (63.1 ± 5.9) years; range, 25-84 years] who underwent stool antigen test and urea breath test (UBT) before and after PPI administration. RESULTS: Using the UBT as the standard, the sensitivity, specif icity and agreement of the stool antigen test in all 28 patients were 95.2%, 71.4%, and 89.3%, respectively, before PPI administration, and 88.9%, 90.9%, and 89.3%, respectively, after PPI treatment. Mean UBT values were 23.98% ± 5.33% before and 16.19% ± 4.75% after PPI treatment and, in 15 patients treated for ≥ 4 wk, were signif icantly lower after than before 4 wk of PPI treatment (12.58% ± 4.49% vs 24.53% ± 8.53%, P = 0.048). The mean optical density (A450/630) ratios on the stool antigen test were 1.16 ± 0.20 before and 1.17 ± 0.24 after PPI treatment (P = 0.989), and were 1.02 ± 0.26 and 0.69 ± 0.28, respectively, in the group treated for > 4 wk (P = 0.099).
