Narrow pelvic inlet plane area and obesity as risk factors for anastomotic leakage after intersphincteric resection

BACKGROUND Intersphincteric resection(ISR)has been increasingly used as the ultimate sphincter-preserving procedure in extremely low rectal *** most critical complication of this technique is anastomotic *** incidence rate of anastomotic leakage after ISR has been reported to range from 5.1%to 20%.AIM To investigate risk factors for anastomotic leakage after ISR based on clinicopathological variables and *** This study was conducted at Department of Colorectal Surgery,Japanese Red Cross Medical Center,Tokyo,Japan,with a total of 117 *** enrolled 117 patients with extremely low rectal cancer who underwent laparotomic and laparoscopic ISRs at our *** conducted retrospective univariate and multivariate regression analyses on 33 items to elucidate the risk factors for anastomotic leakage after *** dimensions were measured using threedimensional reconstruction of computed tomography *** optimal cutoff value of the pelvic inlet plane area that predicts anastomotic leakage was determined using a receiver operating characteristic(ROC)*** We observed anastomotic leakage in 10(8.5%)of the 117 *** the multivariate analysis,we identified high body mass index(odds ratio 1.674;95%confidence interval:1.087-2.58;P=0.019)and smaller pelvic inlet plane area(odds ratio 0.998;95%confidence interval:0.997-0.999;P=0.012)as statistically significant risk factors for anastomotic *** to the receiver operating characteristic curves,the optimal cutoff value of the pelvic inlet plane area was 10074 *** pelvic inlet plane area(≤10074 mm2)predicted anastomotic leakage with a sensitivity of 90%,a specificity of 85.9%,and an accuracy of 86.3%.CONCLUSION Narrow pelvic inlet and obesity were independent risk factors for anastomotic leakage after *** leakage after ISR may be predicted from a narrow pelvic inlet plane area(≤10074 mm2).

＜正＞It is well-known that severe deformation to metals causes a direct grain refinement of the matrix without special heat-treatments due to the mechanism of dynamic continuous recrystallization （DCR）. However, the microstructural revolution during severe deformation is seemed to be different depending on the deformation mode, namely the direction of deformation. In general, multi-directional deformation is thought to be effective for the grain refinement caused through DCR. For instance, ultra-grain-refinement to 10nm has already achieved by mechanical milling treatment in a steel powder and hardness of the mechanically milled steel powder is increased to around HV12GPa by such a marked grain refinement. On the other hand, hardness of iron never exceeds HV4GPa by the mode of uni-directional deformation such a conventional cold rolling. In this paper, a limit of dislocation density is discussed for iron which is severely strained by the mode of uni-directional deformation, and also the importance of multi-directional deformation on DCR will be mentioned in association with a significant work hardening behavior in mechanicallv milled iron powder.

The Mechanism of Flow Field around a Rotating Cylinder with Fins for High Performance Magnus Wind Turbine

Spiral Magnus is a unique wind turbine system that rotates with cylinders which have spiral-shaped fins coiled around them （instead of using the more common propeller-type blades）. In the present study, three models （cylinder with no fins, cylinder with straight fins and cylinder with spiral fins） were installed, and fluid force measurements were performed by a strain gauge force balance. A PIV （particle image velocimetry） system was used to better understand the flow fields around the cylinder. Considering the results of the experiment, it was confirmed that, the aerodynamic performance of the rotating cylinder can be improved by the fin. However, the straight fin makes the flow close to the cylinder surface ineffective. The rotary cylinder with the spiral fins was able to generate the greatest lift among three models, because the spiral fin effectively influences the vicinity of the cylinder surface.

Undiagnosed Anterior Cranial Fossa Dural Arteriovenous Fistula with Intracranial Hematoma: Case Report and Review of the Literature about Its Natural History

Dural arteriovenous fistulas (dAVFs) of an anterior cranial fossa are rare. Because of the high risk of intracranial hemorrhage and relatively easy access for direct surgery, aggressive treatment has been recommended. The natural history of anterior cranial fossa dAVFs (ACF dAVFs) is unclear in spite of many reports for the natural history of general dAVFs. To treat ACF dAVFs, direct surgery has traditionally been performed and endovascular surgery has recently been introduced. A 74-year-old man was transferred with severe consciousness disturbance and presented with devastating intracerebral hemorrhage on the CT scan. Digital subtraction angiography revealed the ACF dAVFs with a large venous pouch. The patient received direct surgery, nevertheless he became vegetative state. Later on, a smaller venous pouch was recognized on the CT scan when he had suffered from the thalamic hemorrhage sixteen months before. There are twelve cases including our case which was treated for a certain period and documented in detail. Eleven of twelve cases were asymptomatic. Three of the six cases with a venous pouch had some events possibly related to the disease, though none of the six cases without a venous pouch had any events during observation. In conclusion, an ACF dAVF with a venous pouch should be treated by direct surgery or endovascular surgery even if it is incidentally found. By contrast, careful observation might be a possible therapeutic option for an ACF dAVF without a venous pouch if there is mild reflux flow.

Combined-Type Continuous Variable Transmission with Quadric Crank Chains and One-Way Clutches

The conventional Continuously Variable Transmission （CVT） has been classified as belt-type or toroidal CVTs. Each CVT is fundamentally composed of several components to transmit the frictional force or torque generated by the driving force. Since the conventional CVTs use friction force, their energy （force or torque） transfer efficiency might be inferior due to slippage and pressure between the transmission components. Consequently, we propose a new type of structural CVT. The CVT we proposed in this paper means a combined-type transmission with quadric crank chains and one-way clutches.

Improving the water productivity of paddy rice (Oryza sativa L.) cultivation through water saving irrigation treatments

Rice grows well under certain condition and environment including soil, water and nutrients. Some researches have shown that traditional method with continues flooding need tremendeous amount of water for rice cultivation and gives low water productivity. To increase the water productivity, number of water saving irigation techniques have been studied and applied. Study on effect of number of water irrigation treatments on water productivity of rice was carried out. Eight irrigation treatments were conducted for growing rice in pot experiment i.e. shallow intermittent irrigation (SII), alternate wetting and drying (AWD1, AWD2, AWD3 and AWD4), shallow water depth with wetting and drying (SWD1 and SWD2), and semi-dry cultivation (SDC). The performance of those treatments in terms of agronomic and water parameters was compared to the shallow intermittent irrigation as a control method. The study reveals that the shallow intermittent irrigation needs the highest amount of water compare with other treatments. The lowest amount of water was achieved under the semi-dry cultivation. It could save water up to 18.4% compare to the control treatment. By using the alternate wetting and drying and the shallow water depth with wetting and drying treatments, irrigated water can be reduced up to 13.1% and 5.4%, respectively. The highest grain was obtained by alternate wetting and drying (AWD4) and the semi-dry cultivation yielded the smalest grain. On average the alternate wetting and drying and shallow water depth with wetting and drying increased the grain yield by 22.9% and 17.9%, whereas the semi-dry cultivation reduced the yield up to 14% compare to the shallow water depth treatment. The alternate wetting and drying treatments have significantly improved the water productivity by 41.6%, shallow water depth with wetting and drying increased by 24.2% relative to the shallow intermittent irrigation treatment, whereas the most saving water treatment i.e. the semi-dry cultivation p

Applicability of Wireless Activity Sensor Network to Avian Influenza Monitoring System in Poultry Farms

Since there are cases that chickens infected with highly pathogenic avian influenza (HPAI) viruses die with almost no fever, body-temperature sensing cannot be effective for the early detection of avian influenza (AI) infection in these cases. In addition, sensors directly attached to the body surface are easily affected by their surroundings, therefore it is not easy to measure the body-temperature of chickens. This paper proposes a new method to detect abnormal states of chickens with only their activity data obtained by wearable wireless sensor nodes. The method is that if its state that the present average activity of a chicken in one hour is smaller than 0.6 times minimum average daytime activity in its life continues for 3 hours, a judgment that the chicken is abnormal state is made. This method can detect the abnormal states twice as early as that with body-temperature sensing does. Since the activity sensor nodes are easily attached to chickens and hardly affected by their surroundings, this system can be reliable.

Improvement in symptoms after H_2-receptor antagonist-based therapy for eradication of H pylori infection

AIM: To investigate the therapeutic effects of triple therapy combining lafutidine with clarithromycin and amoxicillin on H pylori infection and the resolution of gastroesophageal symptoms after eradication. METHODS: We conducted a randomized, multicenter, open-label controlled trial to compare the effective-ness of a triple therapy of lafutidine, clarithromycin, and amoxicillin (lafutidine group) with that of a triple therapy of lansoprazole, clarithromycin, and amoxicillin (lansopra- zole group) in patients with H pylori infection. The study group comprised 22 patients with gastric ulcers and 18 patients with duodenal ulcers who had H pylori infection. RESULTS: H pylori eradication rates were similar in the lafutidine group (14/20, 70%) and the lansoprazole group (14/20, 70%). Gastroesophageal reflux and ab-dominal symptoms improved after eradication therapy in both groups, whereas abdominal discomfort, diarrhea, and constipation were unchanged. H pylori status had no apparent effect on improvement of gastroesophageal reflux or abdominal symptoms after treatment. Adverse events were similar in both groups. CONCLUSION: The triple therapy including lafutidine is equivalent to triple therapy including lansoprazole in terms of H pylori eradication rates and improvement in gastroesophageal reflux and abdominal *** results are attributed to the fact that lafutidine has strong, continuous antisecretory activity, unaffected by CYP2C19 polymorphisms.

AIM:To analyze the efficacy and safety of a combination therapy of pegylated interferon(PEG-IFN) α-2b plus ribavirin(RBV) in older Japanese patients(65 years or older) infected with hepatitis C virus(HCV).METHODS:This multicenter study included 938 patients with HCV genotype 1 who received 1.5 μg/kg per week PEG-IFN α-2b plus RBV 600-1000 mg/d for 48 wk and 313 HCV genotype 2 patients who received this treatment for 24 ***:At 24 wk after the end of combination therapy,the overall sustained virological response(SVR) for genotypes 1 and 2 were 40.7% and 79.6%,*** SVR rate decreased signif icantly with age in each genotype,and was markedly reduced in genotype 1(P<0.001).Moreover,the SVR was significantly higher in patients with genotype 1 who were less than 65 years(47.3% of 685) than in those 65 years or older(22.9% of 253)(P<0.001) and was higher in patients with genotype 2 who were less than 65 years(82.9% of 252) than in those 65 years or older(65.6% of 61)(P=0.004).When patients received a dosage at least 80% or more of the target dosage of PEG-IFN α-2b and 60% or more of the target dosage of RBV,the SVR rate significantly increased to 66.5% in patients less than 65 years and to 45.2% in those 65 years or older(P<0.001).Adverse effects resulted in treatment discontinuation more often in patients with genotype 1(14.4%) than in patients with genotype 2(7.3%),especially by patients 65 years or older(24.1%).CONCLUSION:PEG-IFN α-2b plus RBV treatment was effective in chronic hepatitis C patients 65 years or older who completed treatment with at least the minimum acceptable treatment dosage.
