Direct cholangioscopy combined with doubleballoon enteroscope-assisted endoscopic retrograde cholangiopancreatography

Double-balloon enteroscope (DBE)-assisted endoscopic retrograde cholangiopancreatography (ERCP) is an effective endoscopic approach for pancreatobiliary disorders in patients with altered gastrointestinal anatomy. Endoscopic interventions via DBE in these postoperative settings remain difficult because of the lack of an elevator and the use of extra-long ERCP accessories. Here, we report the usefulness of direct cholangioscopy with an ultra-slim gastroscope during DBE-assisted ERCP. Three patients with choledocholithiasis in postoperative settings (two patients after Billroth Ⅱ gastrojejunostomy and one patient after Roux-en-Y gastrojejunostomy) were treated. DBE was used to gain access to the papilla under carbon dioxide insufflation, and endoscopic sphincterotomy was performed with a conventional sphincterotome. For direct cholangioscopy, the enteroscope was exchanged for an ultra-slim gastroscope through an incision in the overtube, which was inserted directly into the bile duct. Direct cholangioscopy was used to extract retained bile duct stones in two cases and to confirm the complete clearance of stones in one case. Bile duct stones were eliminated with a 5-Fr basket catheter under direct visual control. No adverse events were noted in any of the three cases. Direct cholangioscopy with an ultra-slim gastroscope facilitates subsequent treatment within the bile duct. This procedure represents another potential option during DBE-assisted ERCP.

Leptin transiently antagonizes ghrelin and long-lastingly orexin in regulation of Ca^(2+) signaling in neuropeptide Y neurons of the arcuate nucleus

AIM: To explore the mechanism for interactions of leptin with ghrelin and orexin in the arcuate nucleus (ARC) activating neuropeptide Y (NPY) neurons during physiological regulation of feeding. METHODS: Single neurons from ARC of adult rats with matured feeding function were isolated. [Ca2+]i was measured to monitore their activities. The time course of leptin effects on ghrelin-induced versus orexin-induced [Ca2+]i increases in NPY neurons was studied. RESULTS: Administration of ghrelin or orexin-A at 10-10 mol/L increased cytosolic Ca2+ concentration ([Ca2+]i) in NPY neurons isolated from the ARC of adult rats. Upon administration of leptin at 10-14-10-12 mol/L, ghrelin-induced [Ca2+]i increases were initially (< 10 min) inhibited but later restored, exhibiting a transient pattern of inhibition. In contrast, orexin-induced [Ca2+]i increases were inhibited by leptin in a longlasting manner. Furthermore, a prior administration of leptin inhibited orexin action but not ghrelin action to increase [Ca2+]i. CONCLUSION: Leptin counteracted ghrelin effects transiently and orexin effects long-lastingly in NPY neurons. The transient property with which leptin counteracts ghrelin action in NPY neurons may allow the fasting-associated increase in ghrelin levels to activate NPY neurons in the presence of physiological leptin and to stimulate feeding.

Interaction between hepatitis C virus and metabolic factors

Hepatitis C virus(HCV)infection disrupts the normal metabolism processes,but is also influenced by several of the host’s metabolic *** obvious and significantly detrimental pathophysiological feature of HCV infection is insulin resistance in hepatic and peripheral *** research efforts have been put forth recently to elucidate the molecular mechanism of HCV-induced insulin resistance,and several cytokines,such as tumor necrosis factor-α,have been identified as important contributors to the development of insulin resistance in the distant peripheral tissues of HCV-infected patients and animal *** demonstrated etiologies of HCV-induced whole-body insulin resistance include oxidative stress,lipid metabolism abnormalities,hepatic steatosis and iron *** addition,myriad effects of this condition have been characterized,including glucose intolerance,resistance to antiviral therapy,progression of hepatic fibrosis,development of hepatocellular carcinoma,and general decrease in quality of ***-related conditions and disorders,such as visceral obesity and diabetes mellitus,have been shown to synergistically enhance HCV-induced metabolic disturbance,and are associated with worse ***,the molecular interactions between HCV-induced metabolic disturbance and hostassociated metabolic factors remain largely *** diet and lifestyle recommendations for chronic hepatitis C are basically the same as those for obesity,diabetes,and metabolic ***,patients are suggested to restrict their dietary iron intake,abstain from alcohol and tobacco,and increase their intake of green tea and coffee(to attain the beneficial effects of caffeine and polyphenols).While successful clinical management of HCV-infected patients with metabolic disorders has also been achieved with some antidiabetic(i.e.,metformin)and anti-lipid(i.e.,statins)medications,it is recommended that sulfonylurea and insulin be avoided.

Inactivation of severe fever with thrombocytopenia syndrome virus for improved laboratory safety

Severe fever with thrombocytopenia syndrome(SFTS),a tick-borne infectious disease with high mortality,is diagnosed by the serological testing of serum samples from patients in the acute and convalescent phases and/or by direct detection of the SFTS virus(SFTSV).Conventionally,heat and UV treatments have been used to inactivate the viruses present in serum samples before performing the serological ***,we examined the inactivation conditions optimal for SFTSV-containing serum samples to ensure the safety of laboratory workers while maintaining the accuracy of serological assay results *** human serum samples spiked with SFTSV to 60C for 30 min or exposing them to UV irradiation for 30 min,reduced the infectious virus titer below the limit of *** in sera from patients in the acute phase of SFTS was completely inactivated by heating to 60C for 30 min,followed by UV irradiation for 10 *** inactivation procedure had minimal impact on the performance of SFTSV antibody detection *** data provided herein can serve as a guide for laboratory workers and researchers working with SFTS serological tests.

Ectopic hepatocellular carcinoma mimicking a retroperitoneal tumor:A case report

BACKGROUND An ectopic hepatocellular carcinoma(EHCC)arises from the ectopic liver which is defined as a hepatic organ or tissue not connected to surrounding *** is a rare disease and it is difficult to diagnose ***,the clinical features are not fully *** SUMMARY A retroperitoneal tumor(6 cm)was located at the dorsal side of the pancreas head on abdominal ultrasonography in an 81-year old woman positive for hepatitis C virus *** enhanced-computed tomography and gadolinium-ethoxybenzyl-diethylenetriamine pentaacetic acid-enhanced magnetic resonance imaging showed viable HCC patterns with early enhancement and delayed *** tumor markers-serum alphafetoprotein and alpha-fetoprotein-L3%-were increased to 30.1 ng/m L and83.1%,*** induced by vitamin K absence or antagonist-Ⅱwas within normal levels(17 m AU/m L).Positron emission tomography-computed tomography showed strong accumulation into the tumor(Standardized Uptake Value max:13.8),and the tumor cytology following endoscopic ultrasoundguided fine needle aspiration showed poorly differentiated *** extirpation was performed,and operative findings showed that the retroperitoneal tumor was disconnected from the pancreas and the *** lymph nodes near the tumor were histologically *** histological examination,the tumor was finally diagnosed as EHCC with Arginase-1 positive *** We report our experience of a rare EHCC which was difficult to diagnose,and we present a review of the literature.
