A preliminary study on rectal dose reduction associated with hyaluronic acid implantation in brachytherapy for prostate cancer

Objectives: Hydrogel spacer (HS) was developed to reduce rectal toxicities caused by radiotherapy, but has been reported to cause major adverse events. Our institute has attempted to introduce a hyaluronic acid (HA) as an alternative spacer. This study aimed to compare rectal doses and geometric distributions between the HS and HA implantation in prostate ***: HS and HA were inserted in 20 and 18 patients undergoing high-dose brachytherapy, respectively. The rectum spacer volumes injected were 10 mL and 22 mL, respectively. In the treatment planning system, 13.5 Gy was administered with common catheter positions. The rectal dose indices were assessed between the spacer groups for dosimetry evaluation. Distances between the prostate and rectum and configurations of the spacers were ***: The mean doses irradiated to 0.1 and 2 mL of the rectum were 10.45 Gy and 6.71 Gy for HS, and 6.73 Gy and 4.90 Gy for HA (p<0.001). The mean minimum distances between the prostate and rectum were 1.23 cm and 1.79 cm for HS and HA, respectively (p<0.05). Geometrical configuration comparisons revealed that HA has a higher ability to expand the space than ***: The rectal dose reduction ability of HA is significantly greater than that of HS, suggesting its potential as a new spacer.

AIM:To explore the usage of choroidal thickness measured by swept-source optical coherence tomography(SS-OCT)to detect myopic macular degeneration(MMD)in high myopic ***:Participants with bilateral high myopia(≤−6 diopters)were recruited from a subset of the Guangzhou Zhongshan Ophthalmic Center-Brien Holden Vision Institute High Myopia Cohort ***-OCT was performed to determine the choroidal thickness,and myopic maculopathy was graded by the International Meta-Analysis for Pathologic Myopia(META-PM)Classiﬁ*** of MMD was defined as META-PM category 2 or ***:A total of 568 right eyes were included for *** with MMD(n=106,18.7%)were found to have older age,longer axial lengths(AL),higher myopic spherical equivalents(SE),and reduced choroidal thickness in each Early Treatment Diabetic Retinopathy Study(ETDRS)grid sector(P<0.001).The area under the receiver operating characteristic(ROC)curves(AUC)for subfoveal choroidal thickness(0.907)was greater than that of the model,including age,AL,and SE at 0.6249,0.8208,and 0.8205,*** choroidal thickness of the inner and outer nasal sectors was the most accurate indicator of MMD(AUC of 0.928 and 0.923,respectively).An outer nasal sector choroidal thickness of less than 74μm demonstrated the highest odds of predicting MMD(OR=33.8).CONCLUSION:Choroidal thickness detects the presence of MMD with high agreement,particularly of the inner and outer nasal sectors of the posterior pole,which appears to be a biometric parameter more precise than age,AL,or SE.

BACKGROUND Laparoscopic duodenojejunostomy(LDJ) has become the standard surgical procedure for superior mesenteric artery syndrome due to its sufficient outcome in terms of safety and symptom relief. However, there are only a few reports about LDJ for malignant stenosis and its indication remains *** SUMMARY A 77-year-old woman with a history of pancreatic cancer(PC) treated with distal pancreatectomy with en bloc resection of the transverse colon 7 mo ago was admitted for recurrent vomiting. Imaging upon admission revealed marked distention of the duodenum and a tumor around the duodenojejunal flexure. She was diagnosed with malignant stenosis caused by local recurrence of PC. LDJ was performed with an uneventful postoperative course, followed by chemotherapy which gave her 10 mo overall *** We think that LDJ is a valuable method for unresectable malignant stenosis around the duodenojejunal flexure as a part of multimodal therapy.

In this study, Bayesian parameter calibration is applied to Saplart-Allmaras(SA) turbulence model, and the prediction improvement by the calibrated model is demonstrated. The quantity of interest(QOI) is the pitch-wise distribution of Mach number in the corner separation flow region. The 10 model parameters included in the SA model with Rotation-Curvature correction are considered as random variables obeying uniform prior probability distributions. The order of generalized Polynomial Chaos(g PC) used for sensitivity analysis and surrogate model in calibration is incrementally increased during the calibration process. Posterior convergence is obtained at the 3 rd order expansion level in this study. At this final level, sensitivity analysis indicates 3 model parameters, cb1, κ and cr3 are the most influential random variables, and 3-parameter Bayesian calibration is conducted. The likelihood function in the Bayesian theorem is specified in the form of Gaussian distribution, including experimental uncertainty. The combination of prior and likelihood brings the posterior distribution of model parameters, and Maximum A Posterior(MAP) value is selected as a calibrated parameter set. The flow simulation with calibrated parameters shows a significant increase in the accuracy of the Mach number profile in the corner separation region. The increase in accuracy is attributed to enlarged turbulent viscosity due to the parameter modification of the turbulent viscosity source term. The calibrated parameter is also tested in the off-design flow field, not included in the calibration process. The calibrated CFD again shows improved accuracy for corner separation prediction, and the effectiveness of the parameter set outside of the calibration field is demonstrated.
