Combined thoracoscopic and endoscopic surgery for a large esophageal schwannoma

This paper addresses photovoltaics（PV） power prediction and energy storage problem which are known to be a key technology in energy management systems（EMS）. Extending results of the point prediction of PV power, we first describe a prediction interval（PI） method using a copula, which can express the relation between a multivariable joint distribution and each marginal distribution. Then, resorting to the PI method, the energy storage optimization problem in a building is developed. A scenario robust（SR） optimization theorem, which calculates the robustness of the optimal solution, is applied to the proposed PI method,and hence we obtain an optimal energy storage solution taking the robustness of the solution into account. Additionally, we propose a method which combines a model predictive control（MPC） technique and SR to reduce the total electricity costs. The simulation results finally illustrate the cost reduction and robustness of the proposed method.

A 47-year-old woman presented to our hospital with complaints of dysphagia. Esophagogastroduodenoscopy identified a submucosal tumor in the left wall of the esophagus that was diagnosed as a benign schwannoma on biopsy. Computed tomography revealed a tumor of length 60 mm in the thoracic esophagus, with its cranial edge at the level of the aortic arch. On endoscopy, a submucosal tunnel was created 40 mm proximal to the cranial edge of the tumor, and its oral end was dissected from the mucosal and muscular layers. This was followed by the resection of the entire tumor by left-sided thoracoscopy. The esophageal defect was closed in layer by continuous suture from the thoracic side. Endoscopic closure was achieved by using clips. No postoperative complications were observed. Oral diet was resumed from postoperative day 7 and the patient was discharged on postoperative day 9. This combined approach has not been described for similar tumors. Our experience demonstrated that large esophageal tumors can be safely excised with minimally invasive surgery by using a combination of thoracoscopy and endoscopy.

The Effect of Nitrous Oxide on the Intraocular Pressure in Patients Undergoing Abdominal Surgery under Sevoflurane and Remifentanil Anesthesia

Introduction: Although inhalational anesthesia and nitrousoxide (N2O) are known to affect the intraocular pressure (IOP), little is known about the effect of nitrousoxide on the IOP during sevoflurane and remifentanil anesthesia. In the present study, we examined the effect of balanced anesthesia on the IOP. Materials and Methods: After obtaining informed consent, the patients undergoing abdominal surgery under general anesthesia were divided into two groups: N2O group (n = 10) and control group (n = 12). General anesthesia was maintained with remifentanil (0.05 - 0.3 μg/kg/min), 33% O2 and 1.2% sevoflurane to keep ETCO2 of 35 - 40 mmHg following tracheal intubation. After baseline measurement of IOP (T0, 20 minutes after injection of anesthesia), the patients in the N2O group received 67% nitrousoxide, and the patients in the control group received air, with O2 and 1.2% sevoflurane. Then, IOP was measured at 1 hour (T1), 2 hours (T2), and 3 hours (T3) after anesthesia induction in the supineposition. Blood pressure and heart rate were recorded at the same time interval. Results: There was no significant difference in the IOP at any period between the two groups. In both groups, the IOP at the T3 was significantly higher than that at T0. Conclusion: These results suggest that N2O does not affect the IOP in patients undergoing abdominal surgery under sevoflurane and remifentanil anesthesia.

We report here on a case of duodenal metastasis from primary lung adenocarcinoma. A 69-year old man was diagnosed with primary lung adenocarcinoma. Four courses of combined chemotherapy with carboplatin and paclitaxel associated with irradiation of 60 Gy shrunk the lung tumor. However, soon after,the para-aortic lymph node became swollen. Esophagogastroduodenoscopy revealed three duodenal tumors. Differential diagnosis between malignant lymphoma and metastatic duodenal cancer was endoscopically difficult. The histology of biopsied specimens was poorly differentiated adenocarcinoma. Immunohistochemical analysis revealed a positive reaction for thyroid transcription factor-1 (TTF-1). Thus, we concluded that these were metastatic duodenal tumors from lung adenocarcinoma. Two courses of gemcitabine led to a complete remission in this duodenal metastasisand para-aortic lymph node swelling with only scarring remaining in computed tomography. He is now on the continuous generalized chemotherapy. In conclusion, duodenal metastasis from primary lung adenocarcinoma is rare and hard to diagnose. In such an instance, TTF-1 immunostaining is crucial to obtain the correct diagnosis.

Design of Weigh Feeder Control System for Energy Saving Using Performance-Adaptive Method

Cu precipitation and hardening behaviour of Cu added 0.19%C-1.5%Mn steels were investigated to elucidate the effectiveness of nano size Cu particle dispersion in tempered martensite matrix as the alloy design of ultra-high strength steels with superior formability. In this work, hardness measurement, TEM observation, lattice parameter measurement and tensile testing were conducted for the 0.19%C-1.5%Mn steels with addition of up to 4%Cu those were water-quenched followed by aging at 250 through 550 for 20 seconds through 360 *** added steels exhibited higher hardness than a Cu free steel in each aging condition and the hardenability was proportional to the Cu content. TEM observation revealed that the strengthening in Cu added steels was due to precipitation hardening by Cu particles. From the lattice parameter measurement, it is observed that Cu precipitation started at the aging time of 20 seconds and abruptly increased in the early stage of aging at 450℃. At the aging time of 60 minutes at maximum precipitation hardening, precipitated Cu content was estimated to be 2.5%. Smaller estimated activation energy for Cu precipitation than that of Cu diffusion in bcc-Fe matrix also suggested that high dislocation density introduced by martensitic transformation acted as the nucleation site or accelerated the diffusion of *** the tensile properties of the aged Cu free and 4%Cu steels, 4%Cu steel exhibited superior balance of tensile strength and elongation with tensile strength level of 1300MPa. This result indicates that dispersing nano Cu particles in tempered martensite matrix could give new design of ultra-high strength steels with superior formability.

This paper discusses a design method for the control system of a weigh feeder that supplies powder and granular material at a constant rate. Most weigh feeders employed in industry are controlled by proportional and integral （PI） compensation, and the control performance is decided by the selection of parameters. To attain advanced control performance by PI control, the PI parameters are designed on the basis of generalized minimum variance control （GMVC）. In this study, to achieve user-specified control performance by GMVC-based PI control, the design parameters of GMVC are automatically adjusted using a performance-adaptive method. The control performance discussed in this study consists of the variance of the control error and that of the difference in the control input. In a conventional performance-adaptive method, the variance of the control error is reduced. In this study, to reduce energy consumption and to achieve user-specified control performance, the variance of the difference in the control input is specified and the design parameter is determined. To demonstrate its effectiveness, the proposed method is applied to an actual weigh feeder.

We propose a learning architecture for integrating multi-modal information e.g., vision, audio information. In recent years, artificial intelligence (AI) is making major progress in key tasks like a language, vision, voice recognition tasks. Most studies focus on how AI could achieve human-like abilities. Especially, in human-robot interaction research field, some researchers attempt to make robots talk with human in daily life. The key challenges for making robots talk naturally in conversation are to need to consider multi-modal non-verbal information same as human, and to learn with small amount of labeled multi-modal data. Previous multi-modal learning needs a large amount of labeled data while labeled multi-modal data are shortage and difficult to be collected. In this research, we address these challenges by integrating single-modal classifiers which trained each modal information respectively. Our architecture utilized knowledge by using bi-directional associative memory. Furthermore, we conducted the conversation experiment for collecting multi-modal non-verbal information. We verify our approach by comparing accuracies between our system and conventional system which trained multi-modal information.

In recent years, water jet instruments have been used in the field of gastrointestinal endoscopy, mainly in two clinical situations: Investigation and treatment under endoscopic view. Injecting water jet into the gastrointestinal lumen is helpful for maintaining a clear endoscopic view, washing away blood or mucous in the lumen or on the surface of the tip of the endoscope. This contributes to reducing time and discomfort of examination. Water jet technology is an alternative method for dissecting soft tissue; this method does not harm the small vessels or cause mechanical or thermal damage. However, its use in clinical settings has been limited to the transmucosal injection of water into the submucosal layer that elevates the mucosa to prepare for endoscopic mucosal resection or endoscopic submucosal dissection, instead of tissue dissection, which may occur because of the continuous water jet. A preclinical study has been conducted using a pulsed water jet system as an alternative method for submucosal dissection by reducing intraoperative water consumption and maintenance of dissection capability. This review introduces recent studies pertaining to using a water jet in gastrointestinal endoscopy and discusses future prospects.
