Risk Factors Associated with Unsuccessful Vaginal Birth after One Cesarean (VBAC-1) in Puerto Rico

Background: Cesarean section (CS) has increased steadily over the last decade, with an estimated one-third of women delivering by cesarean section worldwide. Objective: Our study aimed to investigate the demographic and associated factors influencing vaginal birth after one cesarean (VBAC-1) success focusing on variables like pre-pregnancy BMI, diabetes, hypertension, education, and smoking. Study Design and Methods: In this retrospective study, we analyzed 285 cases (81 unsuccessful VBAC-1, 204 successful VBAC-1) from San Juan City Hospital (Puerto Rico) between January 1, 2019, and December 31, 2020. We used odds ratios and model selection comparison to assess the impact of variables on successful VBAC-1, using a significance threshold of 95% CI. Model selection assessed binomial model combinations using a generalized linear approach to identify key risk factors. Results: Unsuccessful VBAC-1 (a repeat cesarean), was associated with diabetes (OR: 0.376, p = 0.086), hypertension (OR: 0.23, p = 0.006), and university-educated women (OR: 1.372, p = 0.711). High school-educated women had an OR of 3.966 (p = 0.105), while overweight women were 0.481 times more likely to have unsuccessful VBAC-1 (p = 0.041). Significant associations were not found with obesity (OR: 0.574, p = 0.122), underweight/normal (OR: 1.01, p = 0.810), or smoking (OR: 1.227, p = 0.990). Conclusion: Results revealed women with higher education levels, hypertension, or diabetes are less likely to have a successful VBAC-1. Understanding the complex interactions affecting these outcomes is aimed at establishing guidelines for healthcare professionals to conduct systematic risk/benefit assessments. This study lays a foundation for evidence-based practices and policies, offering initial insights into VBAC-1 success factors in Puerto Rico.

Background: Intussusception is defined as a telescoping of a proximal gastrointestinal segment with its mesentery to a distal one, only 5% occur in adults and in colon the probability that it is caused by a malignant disease is up to 65%. Only 1% occurs in a retrograde manner, the rest occur in an anterograde manner. Aim: Describe the clinical presentation of an intussusception in the adult patient as well as its most frequent causes and possible complications that influence decision making for a definitive treatment. Case Presentation: A 66-year-old woman diagnosed with colon adenocarcinoma who underwent elective transverse colectomy and colonic anastomosis with an incidental finding of a transverse colon tumor in a retrograde intussusception was studied. Conclusion: In any adult patient with an intussusception especially in colon a neoplasia should be suspected and the affected segment should be resected without being reduced due to the risk of perforation and tumor dissemination.

Video learning based image classification method for object recognition

Automatic image classification is the first step toward semantic understanding of an object in the computer vision *** key challenge of problem for accurate object recognition is the ability to extract the robust features from various viewpoint images and rapidly calculate similarity between features in the image database or video *** order to solve these problems,an effective and rapid image classification method was presented for the object recognition based on the video learning *** optical-flow and RANSAC algorithm were used to acquire scene images from each video *** the selection of scene images,the local maximum points on comer of object around local area were found using the Harris comer detection algorithm and the several attributes from local block around each feature point were calculated by using scale invariant feature transform （SIFT） for extracting local ***,the extracted local descriptor was learned to the three-dimensional pyramid match *** results show that our method can extract features in various multi-viewpoint images from query video and calculate a similarity between a query image and images in the database.

Although intestinal microbiome have been established as an important biomarker and regulator of cancer development and therapeutic response, less is known about the role of microbiome at other body sites in cancer. Emerging evidence has revealed that the local microbiota make up an important part of the tumor microenvironment across many types of cancer, especially in cancers arising from mucosal sites, including the lung, skin and gastrointestinal tract. The populations of bacteria that reside specifically within tumors have been found to be tumor-type specific, and mechanistic studies have demonstrated that tumor-associated microbiota may directly regulate cancer initiation, progression and responses to chemo- or immuno-therapies. This review aims to provide a comprehensive review of the important literature on the microbiota in the cancerous tissue, and their function and mechanism of action in cancer development and treatment.

Effect of magnesium on the aluminothermic reduction rate of zinc oxide obtained from spent alkaline battery anodes for the preparation of Al–Zn–Mg alloys

The aluminothermic reduction of zinc oxide（ZnO） from alkaline battery anodes using molten Al may be a good option for the elaboration of secondary 7000-series alloys. This process is affected by the initial content of Mg within molten Al, which decreases the surface tension of the molten metal and conversely increases the wettability of ZnO particles. The effect of initial Mg concentration on the aluminothermic reduction rate of ZnO was analyzed at the following values： 0.90wt%, 1.20wt%, 4.00t%, 4.25wt%, and 4.40wt%. The ZnO particles were incorporated by mechanical agitation using a graphite paddle inside a bath of molten Al maintained at a constant temperature of 1123 K and at a constant agitation speed of 250 r/min, the treatment time was 240 min and the ZnO particle size was 450？500 mesh. The results show an increase in Zn concentration in the prepared alloys up to 5.43wt% for the highest initial concentration of Mg. The reaction products obtained were characterized by scanning electron microscopy and X-ray diffraction, and the efficiency of the reaction was measured on the basis of the different concentrations of Mg studied.

Increasing use on mountain summits has both social and ecological *** numbers of visitors climbing mountain summits can be a safety issue,particularly in areas where terrain or elevation leads to queueing that may cause time *** visitor use levels at site specific locations en route to summits is needed to understand the potential benefits and impacts of visitor use in these ***,it can be difficult to obtain reliable and robust data to estimate use and develop statistical relationships because of the remote and harsh climates on mountain summits,as well as the financial and personnel requirements involved to collect the data in remote *** 2015,data were collected on the higher stretches of the Keyhole route on Longs Peak in rocky Mountain National Park,USA(RMNP)to better understand use levels near the summit and to explore potential statistical relationships to trailhead use data that are relatively easy to *** statistical relationships from.robust regression analyses were found between trailhead use counts and daily and hourly use totals on the"Homestretch"which is a final section of the Keyhole ***,a strong statistical relationship was found between total daily use and maximum hourly use on the *** results suggest that trailhead counts are an accurate and reliable means from which to estimate use levels on upper portions of the Keyhole ***,this research demonstrates the usefulness of an approach using proxy variables to estimate visitor use along remote peaks where data collection can be *** types of data can elucidate various options and decisions for park management teams who are charged with deciding if and how to manage high use areas.
