AIm:To analyse the possible association of various Helicobacter species and certain common gut bacteria in patients with meckel's diverticulum and ***:A nested-polymerase chain reaction (PCR),specific to 16S rRNA of the Helicobacter genus,was performed on paraffin embedded samples,50 with acute appendicitis,50 normal appendixes,and 33 meckel's diverticulum with gastric heterotopia and/or *** genus positive samples were sequenced for species *** samples were also analysed for certain gut bacteria by ***:Helicobacter pullorum DNA was found in one out of 33 cases and Enterobacteria in two cases of meckel's *** pylori (***) was found in three,Enterobacter in 18,and Bacteroides in 19 out of 100 appendix samples by *** was not found in any mD or appendix *** *** positive cases were from normal ***:Helicobacter is not an etiological agent in the pathogenesis of symptomatic meckel's diverticulum or in acute appendicitis.

AIm： To determine whether gastric and enteric Helicobacter species are associated with pancreatic cancer. mETHODS： Patients with exocrine pancreatic cancer （n = 40）, neuroendocrine cancer （n = 14）, multiple endocrine neoplasia type 1 （n = 8）, and chronic pan- creatitis （n = 5） were studied. Other benign pancreatic diseases （n = 10） and specimens of normal pancreas （n = 7） were included as controls. Pancreatic tissue specimens were analyzed by Helicobacter-specific PCR-assay and products were characterized by denaturing gradient electrophoresis and DNA-sequencing. From a subset of the pancreatic cancer patients, gastric and/or duodenal tissue as well as gallbladder and ductus choledochus tissue were analyzed. Gallbladder and choledochus samples were included as controls. Stomach and duodenum samples were investigated to analyze whether a gastric helicobacter might disseminate to the pancreas in pancreatic cancer patients. Pancreatic specimens were analyzed by Bacteroidesspecific PCR for detecting the translocation of indigenous gut microbes to the diseased pancreas. RESULTS： He/icobacter DNA was detected in pancreas （tumor and/or surrounding tissue） of 75% of patients with exocrine cancer, 57% of patients with neuroendocrine cancer, 38% of patients with multiple endocrine neoplasia, and 60% of patients with chronic pancreatitis. All samples from other benign pancreatic diseases and normal pancreas were negative. Thirtythree percent of the patients were helicobacterpositive in gastroduodenal specimens. Surprisingly, H. bilis was identified in 60% of the positive gastroduodenal samples. All gallbladder and ductus choledochus specimens were negative for helicobacter. Bacteroides PCR-assay was negative for all pancreatic samples. CONCLUSION： Helicobacter DNA commonly detected in pancreatic cancer suggests a possible role of the emerging pathogens in the development of chronic pancreatitis and pancreatic cancer.

AIm- To investigate if and how the proinflammatory cytokine interferon γ （IFNγ） affects ghrelin expression in mice. mETHODS： The plasma concentration of ghrelin, and gastric ghrelin and somatostatin expression, were ex- amined in wild-type mice and mice infected with Helico- bacter pylori （H. pylorO. Furthermore, ghrelin expression was examined in two achlorhydric mouse models with varying degrees of gastritis due to bacterial overgrowth. To study the effect of IFNγ, alone, mice were given a subcutaneous infusion of IFNγ, for 7 d. Finally, the influ- ence of IFNγ, and somatostatin on the ghrelin promoter was characterized. RESULTS： H. py/ori infection was associated with a 50% reduction in ghrelin expression and plasma concentration. Suppression of ghrelin expression was in- versely correlated with gastric inflammation in achlorh- dyric mouse models. Subcutaneous infusion of IFNγ, suppressed fundic ghrelin mRNA expression and plasma ghrelin concentrations. Finally, we showed that the ghrelin promoter operates under the control of soma- tostatin but not under that of IFNγ. CONCLUSION： Gastric infection and inflammation is associated with increased IFNγ, expression and reduced ghrelin expression. IFNγ, does not directly control ghre- lin expression but inhibits it indirectly via somatostatin.

Nicotinamide treatment of plants and plant cell cultures has been shown to promote defense and decrease levels of DNA *** the present study,we used RNA-seq technology to study overall changes in gene expression induced in roots of 3-month-old spruce(Picea abies)seedlings grown from nicotinamide-treated seeds to examine the molecular mechanisms underlying the defense *** 350 genes were identified as differentially expressed in roots after the seed *** response genes,including transcription factors mYB77 and LHY and two chitinase enzymes,were generally upregulated,whereas genes thought to be involved in epigenetic regulation such as DDm1,known to promote DNA methylation,were present at high frequency among the downregulated *** all samples,the expression of downregulated epigenetic-related genes was highly correlated with the nicotinamide treatment,indicating a common *** results support an earlier hypothesis regarding a potential role of nicotinamide as a defense-signal mediator.

AIm： To present the rationale, design, methodology, and the baseline data of the Beijing Desheng Diabetic Eye Study（BDDES）, and to determine the prevalence of diabetic retinopathy（DR） and possible risk factors in patients with type 2 diabetes mellitus（T2Dm） in an urban community of Beijing, ***： Community-based prospective cohort study of persons diagnosed with T2Dm aged 30 y or older. The main variables of interest are the presence and progression of DR as determined by the standardized ETDRS grading of seven fields fundus photographs. The presence and severity of DR were analyzed for possible correlations to non-genetic and genetic ***： A total of 1438 participants with data available for analysis, the prevalence of any DR was 35.4%. The prevalence of mild non-proliferative diabetic retinopathy（NPDR）, moderate NPDR, severe NPDR, and proliferative diabetic retinopathy was 27.7%, 2.6%, 0.5% and 4.5%, respectively. By multiple logistic regression analysis, risk factors for the presence of any DR included male（P=0.031）, lower income level（P=0.011）, lower education background（P=0.022）, longer duration of diabetes（P=0.001）, younger age at diabetic onset（P=0.001）, higher systolic blood pressure（P=0.007）, higher glycosylated hemoglobin A1 c levels（P=0.001）, high albuminuria（P=0.03）, and use of insulin（P〈0.001）. For vision-threatening DR, four factors were significant： younger age at diabetic onset（P〈0.001）,higher systolic blood pressure（P=0.042）, high albuminuria（P〈0.001）, and use of insulin（P〈0.001）. CONCLUSION： The BDDES is the first large-scale ongoing cohort study of a Chinese urban population of persons with type 2 diabetes. Using standardized grading system comparable to large cohort studies from western populations, our baseline data shows that the prevalence of DR and major risk factors in this Chinese ethnic population are comparable to that found in the western population studies.

AIm： To investigate the relationship between insulin resistance （IR）/β-cell dysfunction and diabetic retinopathy （DR） in Chinese patients with type 2 diabetes mellitus （T2Dm）, and to explore further whether there were differences in the relationship among diabetic patients with higher and lower body mass index （BmI）. mETHODS： Cross-sectional study. A total of 1466 subjects with T2Dm were recruited in a local Desheng Community of urban Beijing from November 2009 to June 2012 for the cohort of Beijing Desheng Diabetic Eye Study. Standardized evaluation was carried out for each participant, including questionnaire, ocular and anthropometric examinations, and laboratory tests. Seven fields 30° color fundus photographs were used for DR grading according to the Early Treatment Diabetic Retinopathy Study protocols. Homeostatis model Assessment （HOmA） method was employed for IR and β-cell function assessment. RESULTS： After excluding those participants who were treated with insulin （n=352） or had missing data of fasting insulin （n=96）, and further excluding those with poor quality of retinal photographs （n=10）, a total of 1008 subjects were included for the final analysis, 406 （40.3%） were men and 602 （59.7%） were women, age ranging fiom 34 to 86 （64.87±8.28）y. Any DR （levels 14 and above） was present in 278 （27.6%） subjects. After adjusting for possible covariates, the presence of any DR did not correlate with HOmA IR [odds ratio （OR） 1.51, 95% confidence interval （Cl） 0.87-2.61, P=0.14] or HOmA β-cell （OR 0.71, 95%CI 0.40-1.26, P=0.25）. After stratification by BmI, the presence of any DR was associated positively with HOmA IR （OR 2.46, 95%CI： 1.18-5.12, P=0.016）, and negatively with HOmA β-cell （OR 0.40, 95%CI： 0.19-0.87, P=0.021） in the group of patients with higher BmI （225 kg/m2）. In the group of patients with lower BmI （〈25 kg/m2）, the presence of any DR was not associated with HOmA IR （OR 1.00, 95%C1： 0.43-2.33, P=I.00） or HOmA β-cell （OR 1.41, 95%CI： 0.60-3.32, P=

AIm： To investigate the pattern of diurnal variations of choroidal thickness of macular region of healthy individuals. ~ mETHODS： A prospective study of 32 healthy female subjects was conducted. Each subject underwent 1） a questionnaire on daily schedule, 2） the Pittsburgh Sleep Quality Index questionnaire （PSQI）, and 3） ocular examinations including an eye dominance test, fundus photography, and sequential optical coherence tomography （OCT） imaging, on two separate days at five fixed 3h time intervals. Choroidal thickness was measured by two masked graders. RESULTS： A significant diurnal variation of choriodal thickness at fovea （P〈0.001）, at 500 μm nasal （P〈0.001）, temporal to fovea （P=0.01） or 1500 μm nasal to fovea （P= 0.001） was observed. The median choroidal thickness peaked at 11：00 at fovea （P=0.01）, at 500μm nasal （P = 0.009） and temporal （P=0.03） to fovea. The median amplitude of foveal choroidal thickness was 20.5 pm （13, 31） and 20.0 μm （12.5, 28.2） for the first and second series of measurements, respectively. The greater amplitude of foveal choroidal thickness was associated with thickner initial foveal choroidal thickness [ 0.05 （0.03, 0.08）, P= 0.01], dominant eye 10.51 （4.02, 14.60）, P=0.04] in the multivariate linear regression. CONCLUSION： Our data show a significant diurnal variation of the choroidal thickness at fovea, at 500 μm nasal and temporal to fovea and 1500 μm nasal to fovea. Thicker initial foveal choroidal thickness and being dominant eye may influence the amplitude of foveal choroidal thickness.

Predictors for attending annual eye screening for diabetic retinopathy amongst patients with diabetes in an urban community of Beijing

AIm： To gain a better understanding of possible factors that may influence the decision of diabetes persons to participate in annual eye screening in an urban community setting of ***： A structured interview including questions on attendance of eye screening, knowledge and awareness of diabetic retinopathy was conducted. The presence and degree of retinopathy were assessed using two field nonmydriatic retinal photography. RESULTS： Totally 720 diabetes persons were recruited and 519 were enrolled in this cross-sectional study. In this urban setting of Beijing, among diabetes patients of average of 10 y duration, 77% confirmed having undergone at least one eye examination and 61% reported having at least one eye examination with dilated pupil. As for the last 12 mo, the number decreased to 210（47%） and 131（30%） separately. most of the participants（95%） were aware that diabetes could affect their vision and that regular eye examination was necessary. Very few of them（12%） however were aware that the early stages of diabetic retinopathy presented without symptoms of vision loss. Having attended patient education on diabetes was effective in building awareness about diabetic eye disease and was a significant positive predictor for attending eye screening [education in a year, Adj. OR=0.47（0.29-0.74）, P〈0.001, education years ago, Adj. OR=0.56（0.33-0.96）, P=0.036]. The duration of disease also increasedthe likelihood of having undergone eye screening（Adj. OR=0.96, P〈0.05）. CONCLUSION： Being exposed to education about the complications of diabetes increases the probability of attending diabetic eye screening. An appropriate patient knowledge building strategy should be made available to patients from the time of diagnosis.
